Background Various international guidelines recommend the use of non‐endoscopic tests [such as u... more Background Various international guidelines recommend the use of non‐endoscopic tests [such as urea breath test (UBT)] for the evaluation of dyspeptic patients, unless endoscopy is clinically indicated. Aim To assess adherence with guidelines for UBT referrals among primary care doctors in Israel. Methods Patients referred by primary care doctors to an open‐access UBT service were included in the study. Prior to the test, all patients were administered with a short questionnaire regarding their symptoms, previous and concomitant medications including previous Helicobacter pylori eradication. Results The study sample consisted of 209 patients, aged 18–94 years, M/F = 74/135. The UBT was judged to be appropriate in 94 (45%) subjects, inappropriate in 93 (44.5%) subjects and appropriate but avoidable in 22 (10.5%) subjects, most of them asymptomatic patients following anti H. pylori treatment. AimThe effect of restorative proctocolectomy (RPC) on fertility and pregnancy in women with ulcer... more AimThe effect of restorative proctocolectomy (RPC) on fertility and pregnancy in women with ulcerative colitis (UC) was evaluated.MethodPost‐RPC female patients with UC who were attempting to become pregnant filled out questionnaires on fertility and pregnancy. Demographic and pouch data of pregnancies ending with delivery were collected from a prospective database.ResultsForty‐one women, 44 ± 10 years of age, completed the questionnaires. The median follow‐up period post‐RPC was 167 (range, 20–352) months. Before RPC, 26 women had 70 pregnancies and 62 deliveries. After RPC, 17 women had 32 pregnancies and 26 deliveries (P = 0.0035). Post‐RPC, 10 (37%) of 27 patients failed to conceive compared with 26/26 successful attempts before RPC (P = 0.0006). The number of offspring per patient was 2.38 ± 1.27 before, and 0.68 ± 0.93 after, RPC (P < 0.0001). In the absence of alarm symptoms, the current recommended policy in young dyspeptic patients is a ‘test and treat’ strategy for Helicobacter pylori; in GERD patients, a therapeutic trial with proton pump inhibitors is the treatment of choice.Aim To create a short and simple clinical algorithm, for the diagnosis and treatment of patients with upper gastrointestinal complaints.Methods The clinical usefulness and cost‐effectiveness of the new algorithm were evaluated in a controlled clinical trial, held in primary‐care clinics in Israel. Clinical and economical treatment outcomes were evaluated after 1, 3 and 6 months comparing doctors who used the algorithm (cases) vs. those who did not (controls).Results 78 cases and 54 controls completed the 6 months of follow up. To verify whether there is a gender difference in the (13)C-urea breath test results in a large c... more To verify whether there is a gender difference in the (13)C-urea breath test results in a large cohort. The test results of dyspeptic patients referred for (13)C-urea breath testing between January and December, 2007 were evaluated. Testing was carried out at the health insurance organization branches and evaluated at a central laboratory in Israel. Of a total of 28 746 test results, 18 122 (63.04%) were from females and 10 624 (36.95%) from males. Overall, 10 188 (35.4%) results [expressed as delta over baseline (DOB)] were positive (DOB (13)C > 5), 18,326 (63.7%) were negative (DOB (13)C < 3.5) and 232 (0.8%) were borderline (DOB (13)C 3.5-5). There was a significant difference between the total positive rate among females and males (34.8% vs 37.2%, respectively, P = 0.0003). The mean test value was increased by approximately 10 units for females compared to males (P < 0.01) and this difference was consistent for all age groups (i.e., between 10-80 years of age, P < 0....

Babies born before RPC weighed more than those born after RPC (3.16 ± 0.61 kg vs 2.79 ± 0.68 kg, respectively; P = 0.0327). RPC is associated with an increased risk of infertility, similar duration of gestation and lower birthweight. Female candidates for RPC who have not finished family planning should be counselled accordingly.
