Although the move to smoke-free mental health inpatient settings is an internationally common and... more Although the move to smoke-free mental health inpatient settings is an internationally common and popular trend, these policies are neither implemented nor supported by any national program in Iran. This study investigates the attitude of mental health staff and psychiatric patients toward smoking cessation in 2 psychiatric inpatient units (psychosomatic and adult general psychiatry) in the Taleghani general hospital in Tehran. One hundred and twenty participants of this cross-sectional study consist of 30 mental health staff and 90 psychiatric patients. An eight-item questionnaire was used for collecting information. Both staff and patients expressed a positive attitude towards smoking cessation. Patients favoured the implementation of these policies and expressed a more positive attitude towards the feasibility. Background: Febrile seizures are the most common convulsive events in childhood, occurring in 2-5... more Background: Febrile seizures are the most common convulsive events in childhood, occurring in 2-5% of children. About one third of these children will have a recurrence during a subsequent febrile infection. Methods: This is a randomized clinical trial in 85 healthy children, aged 6 months to 5 years, who were not treated before. These children received randomly either oral diazepam (0.33 mg/kg/tds for two days during febrile illness) or continuous oral Phenobarbital (3-5mg/kg /24 h). Results: Ultimately 64 patients completed the study and were followed up for an average of 13 months (12-18 months). The rate of recurrence of febrile seizure was 18.2% in diazepam group and 32.3% in phenobarbital group; the difference is not statistically significant (p=0.16). Conclusion: There was no significant difference between intermittent oral diazepam or continuous oral phenobarbital.

This study was conducted to determine the time given to patients to explain their problem before being interrupted and redirected by clinical faculties in outpatient clinics of Mashhad University of Medical Sciences. Methods: The cross-sectional observational descriptive study was carried out in year 2011-2012, by observing and audio recording 330 clinical consultations from 30 faculty member physicians. The sample size was determined by doing a pilot study and the faculties were selected randomly using stratified sampling. The time initially given to patients to explain their problem before being interrupted and redirected by physicians was measured in seconds using a chronometer. An MP3 recorder was used for audio recording. Data were collected by non-participatory observation method. Although alcohol use is against cultural norms and religious law in Iran, anecdotal reports from substance abuse treatment providers show that there is demand for alcohol treatment in the country. A recent national report also documented alcohol use disorders in Iran. Development and implementation of evidence-based and culturally appropriate alcohol treatment services is one of Islamic Republic of Iran Ministry of Health national multi- pronged approach to address burden of alcohol use disorders. During development of national alcohol use disorders treatment guideline, guideline development team reviewed international evidences and guidelines for alcohol and drug treatment and designed a draft model for alcohol treatment. A genetic approach is gaining increasing importance in this disorder. The dysbindin gene, located at 6p22.3 is considered a susceptibility gene for schizophrenia. Certain genotypes of dysbindin are thought to be associated with other psychoses such as bipolar disorders. This study intends to assess the association in previously implicated dysbindin genotypes and haplotypes with bipolar I disorder in an Iranian population. Methods: We genotyped four previously reported SNPs: rs2619522, rs1018381, 2743852 and rs2619538. Their haplotypes were analyzed in a population of 124 patients that consisted of 44 confirmed bipolar I disorder patients and 80 control subjects. We used multiple allele-specific PCR method for genotyping, which was verified by direct sequencing. Pre-transplantation depression is known to exert negative effects on clinical outcomes after hema... more Pre-transplantation depression is known to exert negative effects on clinical outcomes after hematopoietic stem cell transplantation (HSCT). Accumulating evidence shows a bidirectional association between inflammation and depression. Systemic inflammation can affect clinical outcomes after transplantation, such as time to engraftment. This study evaluated the effect of pre-transplantation depression and serum level of proinflammatory and anti-inflammatory cytokines on clinical outcomes such as neutrophil recovery time and mortality in patients undergoing HSCT. Patients and methods: In this cross-sectional study, we recruited 73 patients who were autologous or allogeneic HSCT candidates. Hospital Anxiety and Depression Scale questionnaire was used to assess depression in the first 2 days after their hospital admission. Serum levels of IL-10, IL-6, and hs-CRP were measured at the same time. Neutrophil engraftment time, length of hospitalization, rate of mortality, relapses, readmissions, and occurrence of major complications following HSCT in a 1-year follow-up period were recorded as comparative outcomes. Results: Thirty-five patients with depression and 38 patients without depression participated in the study. Among the related outcomes, time of engraftment (P=0.020) and mortality rate (P=0.059) were statistically different between the two groups. Depressed patients had longer neutrophil engraftment time and higher mortality rate 1 year following transplantation (14.14±3.95 days and 31.43%) in comparison to non-depressed patients (12.21±2.81 days and 13.16%). Depressed patients showed significantly higher serum levels of IL-6 and IL-6-toIL -10 ratio compared to non-depressed participants (P<0.001 and P=0.004). Conclusion: According to the results, pre-transplant depression can negatively impact neutrophil recovery time and mortality rate following HSCT. Higher levels of inflammatory factors in depressed patients might be one of the mechanisms that negatively affect clinical outcomes after HSCT.

Background: Despite the considerable efforts made in Iran to treat people with addiction, relapse... more Background: Despite the considerable efforts made in Iran to treat people with addiction, relapse is still common. This study was designed to determine retention and relapse rates and related factors at Yasin Methadone Maintenance Treatment Center. Patients and Methods: In this study, retention rate, relapse rate, and related factors were examined in 74 cases using a descriptive-analytical method. Information about 20 variables were gathered from patient medical records using an information form and a checklist and data was analyzed by the chi-square, Fisher's exact, and Mann-Whitney tests using SPSS software. Findings: The retention and relapse rates were 24.3% and 75.7%, respectively. Only three factors showed a significant difference (p<0.50) between the retention and relapse groups: (1) financial status, (2) motivation for treatment, and (3) cooperation during counseling (p=0.04, p=0.001, p=0.026, respectively). No significant differences were observed between groups for the other factors. Conclusion: Because it is clear that financial status, motivation, and counseling can affect retention rate, it is recommended to study the effect of these factors on retention using methods such as a randomized clinical trial.

Background: In the last decades cancer has become one of the important causes of death in Iran .T... more Background: In the last decades cancer has become one of the important causes of death in Iran .This study examined perspective of a group of Iranian health professionals, patients and patients' family members regarding their view on disclosure of cancer information at a university hospital in Tehran, Iran. Methods: The method of study was qualitative semi-structured focused group content analysis. Two group leaders (psychologist and psychiatrist) run the focus groups. Oncologists, nurses, patients and family members participated in separate focus groups. Five group sessions were held to sum up the participants views in four major topics related to disclosure of cancer information to patients and families. Results: Most of physicians and nurses believed that disclosure of cancer diagnosis is a mistake. Family members think that it should be delivered gradually during stages of therapy based on patient's psychological state, but most of the patients consider truth telling as a patient right. All physicians, most of nurses and all the patients see the physician as a person responsible to break the diagnostic disclosure. All patients wanted the physicians to take the total control of decisionmaking process for their treatment. Conclusion: Iranian physicians and nurses hesitate to disclose cancer diagnosis compared to patients, who want to know the truth. Patients, nurses and physicians consider the physician to be the person responsible for delivering the information of cancer diagnosis .Development and implementation of a protocol based on Iranian culture is a necessity.

In developing countries the problem is more complicated. Based on the estimation by UNICEF, rate of RTA in Iran is 20 times more than world's average. In Iran among all unintentional fatal injuries inflicted on children under five, traffic related fatalities are the leading cause of death. By the year 2020 road crashes will rank ahead of cancer and behind only part disease and depression in times of life years. Globally RTA is the second leading cause of death for young people aged 5-25 years. There is an international consensus that a combination of public education, law enforcement and changing infrastructures are necessary to reduce the rate of RTA and its detrimental effects.
