Цель исследования: оценка состояния регионарной гемодинамики глаза с помощью современных методов ... more Цель исследования: оценка состояния регионарной гемодинамики глаза с помощью современных методов визуализации сосудов для выявления ранних признаков атеросклеротического процесса. Материал и методы. Обследовано 45 человек в воз расте от 40 до 60 лет. Проведено стандартное офтальмологическое обследование, исследование ли пи до граммы крови, дуплексное сканирование брахио цефальных артерий для верификации субклинического атеросклероза. Состояние кровотока в ретробульбарных сосудах оценивали с помощью цветового допплеровского картирования и импульсной допплерографии. Для оценки ретинальной циркуляции применяли фоторегистрацию глазного дна с последующим калиброметрическим анализом. Результаты. У 30 (66,7%) из 45 человек определены признаки субклинического атеросклероза. Исследование кровотока в ретробульбарных сосудах показало увеличение вазорезистентности в центральной артерии сетчатки и задних коротких цилиарных артериях, калиброметрический анализ сосудов глазного дна выявил достоверное изменение диаметра ретинальных венул при субклиническом атеросклерозе. Выводы. С помощью ультразвуковых допплеровских методов исследования ретробульбарных сосудов и калиброметрического анализа сосудов глазного дна установлены изменения ретинальной и хориоидальной циркуляции у пациентов с субклиническим атеросклерозом, что свидетельствует о необходимости мониторинга состояния глазного кровотока наряду с дуплексным сканированием брахиоцефальных артерий у этих больных. Ключевые слова: субклинический атеросклероз, гемодинамика глаза, цветовое допплеровское картирование, калиброметрия. *** Purpose: to evaluate parameters of ocular blood flow in subclinical atherosclerosis. Materials and methods. We observed 45 patients aged from 40 to 65 years. The methods of investigation of the blood flow in orbital vessels and carotid arteries included duplex ultrasonography, Color Doppler Imaging and spectral Doppler analysis. Retinal vascular caliber was measured by using digital retinal photographs. Results. Color Doppler Imaging and spectral Doppler analysis of ocular blood flow showed significant increase of vascular resistance in central retinal artery and short posterior ciliary arteries, dilatation of retinal venular caliber in cases of subclinical atherosclerosis. Retinal venular caliber was significantly larger in individuals with subclinical atherosclerosis. Conclusion. Ultrasound methods of investigation of ocular blood flow and measurement of retinal vessel caliber showed the changes of choroidal and retinal circulation in patients with subclinical atherosclerosis.

Aim: Previously individuals suspected of having familial hypercholesterolaemia (FH) would undergo... more Aim: Previously individuals suspected of having familial hypercholesterolaemia (FH) would undergo analysis of serum lipids followed by exercise ECG testing and eventually cardiac catheterisation. Frequently, results of invasive coronary angiography would illustrate stable coronary plaque disease that could be managed with medications only. The aim of this study is to show the benefit of CT coronary angiography in identifying stable plaque disease to initiate early primary prevention strategies and avoid unnecessary invasive procedures. Method: A database of patients known to have LDL receptor mutations predisposing to dyslipidaemia was compiled between 2012-2014. 10 patients were identified suitable for CTCA scanning and underwent noninvasive imaging. A Consultant Cardiologist and Radiologist reported the findings jointly. Recommendations were made based on severity of plaque disease and subsequently patients were followed up to correlate the findings of subsequent imaging with that of CTCA. Results: Out of the 10 patients who underwent CTCA, 6 patients were found to have single vessel or non-significant CAD disease requiring only primary prevention with no subsequent investigations. 2 patients were found to have mixed plaque disease with an area of tight stenosis requiring functional imaging. Furthermore, 2 patients were found to have significant multi-vessel disease requiring urgent invasive angiography. Conclusion: We propose that CTCA offers clinicians valuable information on the anatomical location and severity of coronary stenosis without the need for invasive cardiac catheterisation. This small, but timely, prospective study highlights a novel use of CTCA in the primary prevention of CAD in a potentially high risk population.

Purpose: To determine the association between tear film thickness (TFT) as measured with ultra-hi... more Purpose: To determine the association between tear film thickness (TFT) as measured with ultra-high resolution optical coherence tomography (UHR-OCT) and signs and symptoms of dry eye disease (DED). Methods: A total of 450 eyes from 225 patients with DED from six different randomized clinical trials were included in this pooled analysis. In all subjects, TFT was measured with a custom-built UHR-OCT system. Symptoms of DED were quantified using a standardized Ocular Surface Disease Index (OSD)I questionnaire and clinical signs including tear film break up time (TFBUT) and Schirmer I test were assessed. Associations of the average TFT with OSDI, TFBUT, and Schirmer I test were calculated using a linear regression analysis. Results: The average TFT of the included sample (mean age, 45.0 ± 13.3 years; 65% female) was 4.2 ± 0.5 µm and the OSDI 36.2 ± 10.4.
