CASE PRESENTATION A 24-year-old woman, a baby-sitter with no known comorbidities, presented to th... more CASE PRESENTATION A 24-year-old woman, a baby-sitter with no known comorbidities, presented to the outpatient department with complaints of modified Medical Research Council grade IV breathlessness for 3 months, chest pain, and dry cough for 2 weeks. There was no known disease history, including respiratory, flu-like illness, or connective tissue disorder. There was no use of chemotherapeutic, oral contraceptive drugs, exposure to toxic substances, or smoking. A review of systems was negative for fever, arthralgia, myalgia, Raynaud phenomenon, skin thickening, rash, or leg swelling. The patient had no family history suggestive of a genetic syndrome.

Objective: Obesity, hypertension and diabetes, which increase the risk of developing severe Coron... more Objective: Obesity, hypertension and diabetes, which increase the risk of developing severe Coronavirus disease-2019 (COVID-19) pneumonia, are also the most common comorbidities of obstructive sleep apnoea syndrome (OSAS). The in-hospital evaluation and testing of patients with OSAS were delayed due to the pandemic. We aimed to investigate the changes in Epworth sleepiness score (ESS), use of positive airway pressure (PAP) devices, sleep quality, weight change, COVID-19 related anxiety, anxiety at hospital admission, domestic contact and contraction of SARS-CoV-2 in OSA patients during the pandemic. Materials and Methods: In our sleep centre, patients (n=202) who were diagnosed with sleep apnoea were called by telephone and a survey related to COVID-19, anxiety levels, PAP use, weight change, sleep quality and OSA was conducted during the pandemic. Results: In the study, hypertension and diabetes were the most common comorbidities. Overall, some of the patients with OSA gained weight, stopped PAP treatment, and ESS scores increased compared to the prepandemic period. The anxiety of hospitalization due to COVID-19 was found to be higher than the anxiety for COVID-19. There was a positive correlation between the level of anxiety at admission to hospital due to the risk of COVID-19 and the ESS measured at diagnosis (r=0.203 p=0.004). Better sleep quality was seen in 77% of the participants during the pandemic. Conclusion: OSA patients with high ESS at diagnosis, PAP uses, COVID-19 positive or with domestic contact should be monitored closely by telemedicine. In future research should examine sleep quality and the effects of working from home in OSAS individuals.

The therapies proposed for women with endometriosis consist of medical treatments and surgery. Bronchial artery embolization has become a well-established and minimally invasive treatment modality for hemoptysis, and to the best of our knowledge, it has not been reported in pulmonary endometriosis. We report a case of pulmonary parenchymal endometriosis treated with embolotheraphy for hemoptysis.

Community-acquired pneumonia (CAP) is a common condition in healthy people, causing morbidity and... more Community-acquired pneumonia (CAP) is a common condition in healthy people, causing morbidity and mortality worldwide despite latest advances in therapy and immunization procedures. Causative agents cannot be detected in approximately 50% of CAP episodes and therapy is initiated empirically. We aimed to determine the spectrum and frequency of the causative agents in patients with CAP in a university hospital. Materials and methods: Seventy seven adult patients hospitalized with CAP from November 2007 to March 2008 were included. CAP was diagnosed with clinical, radiological, and laboratory signs. Results: Sputum and blood cultures grew Streptococcus pneumoniae in 15 specimens; Haemophilus influenzae in 4, Klebsiella pneumoniae in 4, Staphylococcus aureus in 3, and Escherichia coli in 2. Mycoplasma pneumoniae DNA was detected in serum from 10 patients with RT-PCR. Legionella pneumophila urinary antigen was detected in 5 patients. Serological IgM antibodies to Chlamydia pneumoniae in 7 patients and Respiratory Syncytial Virus in 2 patients were observed. Etiology was not determined in 32.5% of patients. The most frequently identified pathogens causing CAP were S. pneumoniae, M. pneumoniae, and C. pneumoniae in descending order in our hospital. Conclusion: Although determination of causative agents in all CAP patients has not been accomplished, knowledge of the spectrum and frequency of local causative agents are valuable for targeted therapy.

GOLD 2019 proposed a novel treatment decision tool for follow-up based on the predominant trait (... more GOLD 2019 proposed a novel treatment decision tool for follow-up based on the predominant trait (exacerbation or dyspnea) of patients, alongside treatment escalation and de-escalation strategies. This study was designed to provide an up-to-date snapshot of patient and disease characteristics, treatment pathways, and healthcare resource use (HRU) in COPD in real life, and comprehensively examine patients considering GOLD 2019 recommendations. Patients and Methods: This mixed design, observational, multicenter (14 pulmonology clinics) study included all patients with a documented COPD diagnosis (excluding asthma-COPD overlap [ACO]) for ≥12 months, aged ≥40 years at diagnosis who had a COPD-related hospital visit, spirometry test and blood eosinophil count (BEC) measurement under stable conditions within the 12 months before enrollment between February and December 2020. Data were collected cross-sectionally from patients and retrospectively from hospital medical records. Results: This study included 522 patients (GOLD group A: 17.2%, B: 46.4%, C: 3.3%, D: 33.1%), of whom 79.5% were highly symptomatic and 36.2% had high risk of exacerbation. Exacerbations (n = 832; 46.6% moderate, 25.5% severe) were experienced by 57.5% of patients in the previous 12 months. Inter-rater agreement between investigators and patients regarding the reason for visit was low (κ coefficient: 0.338, p = 0.001). Inhaled treatment was modified in 88 patients at index, mainly due to symptomatic state (31.8%) and exacerbations (27.3%); treatment was escalated (57.9%, mainly switched to LABA+LAMA+ICS), inhaler device and/or active ingredient was changed (36.4%) or treatment was de-escalated (5.7%). 27% had ≥1 hospital overnight stay over 12 months. Emergency department visits and days with limitation of daily activities were higher in group D (p < 0.001). Conclusion: Despite being on-treatment, many patients with COPD experience persistent symptoms and exacerbations requiring hospital-related HRU. A treatable trait approach and holistic disease management may improve outcomes by deciding the right treatment for the right patient at the right time.

Introduction: The aim of our study is to investigate the etiological distribution of ILD in Turke... more Introduction: The aim of our study is to investigate the etiological distribution of ILD in Turkey by stratifying the epidemiological characteristics of ILD cases, and the direct cost of initial diagnosis of the diagnosed patients. Material-Method: The study was conducted as a multicenter, prospective, cross-sectional, clinical observation study. Patients over the age of 18 and who accepted to participate to the study were included and evaluated as considered to be ILD. The findings of diagnosis, examination and treatment carried out by the centers in accordance with routine diagnostic procedures were recorded observationally. Results: In total,1070 patients were included in this study. 567 (53%) of the patients were male and 503 (47%) were female. The most frequently diagnosed disease was IPF (30.5%). Dyspnea (75.9%) was the highest incidence among the presenting symptoms. Physical examination found bibasilar inspiratory crackles in 56.2 % and radiological findings included reticular opacities and interlobular septal thickenings in 55.9 % of the cases. It was observed that clinical and radiological findings were used most frequently (74.9%) as a diagnostic tool. While the most common treatment approaches were the use of systemic steroids and antifibrotic drugs with a rate of 30.7% and 85.6%, respectively. The total median cost from the patient’s admission to diagnosis was 540 Turkish Lira. Conclusion: We believe that our findings compared with data from other countries will be useful in showing the current situation of ILD in our country to discuss this problem and making plans for a solution.

Background Although some adverse health effects of exposure to polyvinyl chloride (PVC) are well ... more Background Although some adverse health effects of exposure to polyvinyl chloride (PVC) are well known, there is limited evidence of its effects on the respiratory system. Aims To assess the pulmonary effects of exposure to PVC with high-resolution computed tomography (HRCT). Methods Workers and administrative staff of two PVC production plants completed questionnaires and went through pulmonary function testing and HRCT. Analysis of PVC dust in the work environment was performed by the Directorate of Occupational Health and Safety. Results In total, 104 PVC-exposed workers and 43 administrative controls participated. HRCT revealed pleural and/or parenchymal changes in 55% of the exposed subjects. Pleural thickening was detected in 14 subjects, 13 of whom were in the exposed group (P < 0.05). Isolated pleural thickening without parenchymal involvement was present in seven workers, who were all in the exposed group (P < 0.05). Pleural thickening was frequently bilateral and localized to the parietal and visceral pleura. Round opacities, heterogeneous attenuation and ground-glass opacities were only detected in the exposed group (P < 0.05). Exposure to dust increased the risk of findings on HRCT (odds ratio (OR) 4.2, P < 0.05). There were no correlations between pulmonary function tests or respiratory symptoms and HRCT findings. HRCT changes were more common in subjects with forced mid-expiratory flow (FEF 25-75) < 50% (P < 0.001). Conclusions This study found that exposure to PVC dust, at levels below the legal limit for respirable particulate matter, was associated with parenchymal changes and pleural thickening on HRCT.

Psychiatric disorders are common in obstructive sleep apnea syndrome (OSAS); however, interrelati... more Psychiatric disorders are common in obstructive sleep apnea syndrome (OSAS); however, interrelating factors influencing psychiatric comorbidity (PC) in OSAS are unclear. The aim of this study is to investigate gender related differences with PC in OSAS. Data of patients diagnosed as OSAS in University of Gaziantep from January 2006 to January 2010 were retrospectively evaluated. Polysomnographic data were recorded with Viasys Sleep Screen (Viasys Healthcare, Germany). Patients younger than 18 years old were excluded. PC was present in 53.1% of OSAS patients. The rate of male subjects with PC was 42.6%; however, 76.26% of females had PC (P = 0.00). Age (P = 0.00) and body mass index (BMI) (P = 0.00) were higher in patients with PC. Ferritin levels were lower in patients with PC (P = 0.00). Male subjects with PC were older and had lower sleep efficiency and longer rapid eye movement latency than males without PC. BMI was the only contributory factor to PC in female subjects. PC in OSAS is common, especially in females. Apnea hypopnea index does not seem to influence probability of PC.

The aim of this study was to determine the spectrum of aeroallergen sensitivity of patients. The skin prick test results of patients with symptoms compatible with allergic diseases between March 2002 and December 2004 were retrospectively evaluated in Gaziantep University Pulmonary outpatient clinic. Of 1627 patients in whom skin prick tests (Stallergenes, France) were performed, 528 (32.5%) patients had at least one positive result among 8 allergens. The mean age of patients who had positive allergic skin test was 33.03 ± 11.80 (16-69), and 335 (63.4%) of those were female. Two hundred twenty five (52%) patients had positive skin prick test for one allergen tested, while 253 (48%) patients had positive skin prick test for multiple allergens. Phleum pratensis was the most common allergen found to be positive in 221 (41.8%) patients. The prevalence of positive tests were as follows; cockroach (32.9%), Dermatophagoides pteronyssinus (32.7%), Olea europea (27%), cat dander (14.9%), Parieteria officinalis (11.7%), Cladosporium (9.8%) and Alternaria (8.9%).Phleum pratensis and cockroach were the most common allergens causing a sensitivity reaction detected in our clinic.

İki hafta önce mevcut tedavisine inhaler steroid dozu azaltılmadan montelukast eklendiğini belirtti. Hastanın serum eozinofil sayısı 1460/mm 3 (%15) ve immünglobulin E düzeyi 547 IU/mL idi. Toraks bilgisayarlı tomografisinde yamalı infiltrasyonlar saptanan hastanın bronkoalveoler lavaj sıvısında ve transbronşiyal biyopside eozinofilik inflamasyon tespit edildi. Churg-Strauss sendromu tanısıyla prednizolon tedavisi başlanan hastada üç aylık tedavi sonrasında tam yanıt elde edildi. Hasta halen flutikazon propionat ve salmeterol tedavisi altında sorunsuz izlenmektedir.

The therapies proposed for women with endometriosis consist of medical treatments and surgery. Bronchial artery embolization has become a well-established and minimally invasive treatment modality for hemoptysis, and to the best of our knowledge, it has not been reported in pulmonary endometriosis. We report a case of pulmonary parenchymal endometriosis treated with embolotheraphy for hemoptysis.

Causative agents cannot be detected in approximately 50% of CAP episodes and therapy is initiated empirically. We aimed to determine the spectrum and frequency of the causative agents in patients with CAP in a university hospital. Materials and methods: Seventy seven adult patients hospitalized with CAP from November 2007 to March 2008 were included. CAP was diagnosed with clinical, radiological, and laboratory signs. Results: Sputum and blood cultures grew Streptococcus pneumoniae in 15 specimens; Haemophilus influenzae in 4, Klebsiella pneumoniae in 4, Staphylococcus aureus in 3, and Escherichia coli in 2. Mycoplasma pneumoniae DNA was detected in serum from 10 patients with RT-PCR. Legionella pneumophila urinary antigen was detected in 5 patients. Serological IgM antibodies to Chlamydia pneumoniae in 7 patients and Respiratory Syncytial Virus in 2 patients were observed. Etiology was not determined in 32.5% of patients. The most frequently identified pathogens causing CAP were S. pneumoniae, M. pneumoniae, and C. pneumoniae in descending order in our hospital. Conclusion: Although determination of causative agents in all CAP patients has not been accomplished, knowledge of the spectrum and frequency of local causative agents are valuable for targeted therapy.

This study was designed to provide an up-to-date snapshot of patient and disease characteristics, treatment pathways, and healthcare resource use (HRU) in COPD in real life, and comprehensively examine patients considering GOLD 2019 recommendations. Patients and Methods: This mixed design, observational, multicenter (14 pulmonology clinics) study included all patients with a documented COPD diagnosis (excluding asthma-COPD overlap [ACO]) for ≥12 months, aged ≥40 years at diagnosis who had a COPD-related hospital visit, spirometry test and blood eosinophil count (BEC) measurement under stable conditions within the 12 months before enrollment between February and December 2020. Data were collected cross-sectionally from patients and retrospectively from hospital medical records. Results: This study included 522 patients (GOLD group A: 17.2%, B: 46.4%, C: 3.3%, D: 33.1%), of whom 79.5% were highly symptomatic and 36.2% had high risk of exacerbation. Exacerbations (n = 832; 46.6% moderate, 25.5% severe) were experienced by 57.5% of patients in the previous 12 months. Inter-rater agreement between investigators and patients regarding the reason for visit was low (κ coefficient: 0.338, p = 0.001). Inhaled treatment was modified in 88 patients at index, mainly due to symptomatic state (31.8%) and exacerbations (27.3%); treatment was escalated (57.9%, mainly switched to LABA+LAMA+ICS), inhaler device and/or active ingredient was changed (36.4%) or treatment was de-escalated (5.7%). 27% had ≥1 hospital overnight stay over 12 months. Emergency department visits and days with limitation of daily activities were higher in group D (p < 0.001). Conclusion: Despite being on-treatment, many patients with COPD experience persistent symptoms and exacerbations requiring hospital-related HRU. A treatable trait approach and holistic disease management may improve outcomes by deciding the right treatment for the right patient at the right time.

Introduction: The aim of our study is to investigate the etiological distribution of ILD in Turke... more Introduction: The aim of our study is to investigate the etiological distribution of ILD in Turkey by stratifying the epidemiological characteristics of ILD cases, and the direct cost of initial diagnosis of the diagnosed patients. Material-Method: The study was conducted as a multicenter, prospective, cross-sectional, clinical observation study. Patients over the age of 18 and who accepted to participate to the study were included and evaluated as considered to be ILD. The findings of diagnosis, examination and treatment carried out by the centers in accordance with routine diagnostic procedures were recorded observationally. Results: In total,1070 patients were included in this study. 567 (53%) of the patients were male and 503 (47%) were female. The most frequently diagnosed disease was IPF (30.5%). Dyspnea (75.9%) was the highest incidence among the presenting symptoms. Physical examination found bibasilar inspiratory crackles in 56.2 % and radiological findings included reticular opacities and interlobular septal thickenings in 55.9 % of the cases. It was observed that clinical and radiological findings were used most frequently (74.9%) as a diagnostic tool. While the most common treatment approaches were the use of systemic steroids and antifibrotic drugs with a rate of 30.7% and 85.6%, respectively. The total median cost from the patient’s admission to diagnosis was 540 Turkish Lira. Conclusion: We believe that our findings compared with data from other countries will be useful in showing the current situation of ILD in our country to discuss this problem and making plans for a solution.

Aims To assess the pulmonary effects of exposure to PVC with high-resolution computed tomography (HRCT). Methods Workers and administrative staff of two PVC production plants completed questionnaires and went through pulmonary function testing and HRCT. Analysis of PVC dust in the work environment was performed by the Directorate of Occupational Health and Safety. Results In total, 104 PVC-exposed workers and 43 administrative controls participated. HRCT revealed pleural and/or parenchymal changes in 55% of the exposed subjects. Pleural thickening was detected in 14 subjects, 13 of whom were in the exposed group (P < 0.05). Isolated pleural thickening without parenchymal involvement was present in seven workers, who were all in the exposed group (P < 0.05). Pleural thickening was frequently bilateral and localized to the parietal and visceral pleura. Round opacities, heterogeneous attenuation and ground-glass opacities were only detected in the exposed group (P < 0.05). Exposure to dust increased the risk of findings on HRCT (odds ratio (OR) 4.2, P < 0.05). There were no correlations between pulmonary function tests or respiratory symptoms and HRCT findings. HRCT changes were more common in subjects with forced mid-expiratory flow (FEF 25-75) < 50% (P < 0.001). Conclusions This study found that exposure to PVC dust, at levels below the legal limit for respirable particulate matter, was associated with parenchymal changes and pleural thickening on HRCT.

The aim of this study is to investigate gender related differences with PC in OSAS. Data of patients diagnosed as OSAS in University of Gaziantep from January 2006 to January 2010 were retrospectively evaluated. Polysomnographic data were recorded with Viasys Sleep Screen (Viasys Healthcare, Germany). Patients younger than 18 years old were excluded. PC was present in 53.1% of OSAS patients. The rate of male subjects with PC was 42.6%; however, 76.26% of females had PC (P = 0.00). Age (P = 0.00) and body mass index (BMI) (P = 0.00) were higher in patients with PC. Ferritin levels were lower in patients with PC (P = 0.00). Male subjects with PC were older and had lower sleep efficiency and longer rapid eye movement latency than males without PC. BMI was the only contributory factor to PC in female subjects. PC in OSAS is common, especially in females. Apnea hypopnea index does not seem to influence probability of PC.

There was no known disease history, including respiratory, flu-like illness, or connective tissue disorder. There was no use of chemotherapeutic, oral contraceptive drugs, exposure to toxic substances, or smoking. A review of systems was negative for fever, arthralgia, myalgia, Raynaud phenomenon, skin thickening, rash, or leg swelling. The patient had no family history suggestive of a genetic syndrome.

Objective: Obesity, hypertension and diabetes, which increase the risk of developing severe Coron... more Objective: Obesity, hypertension and diabetes, which increase the risk of developing severe Coronavirus disease-2019 (COVID-19) pneumonia, are also the most common comorbidities of obstructive sleep apnoea syndrome (OSAS). The in-hospital evaluation and testing of patients with OSAS were delayed due to the pandemic. We aimed to investigate the changes in Epworth sleepiness score (ESS), use of positive airway pressure (PAP) devices, sleep quality, weight change, COVID-19 related anxiety, anxiety at hospital admission, domestic contact and contraction of SARS-CoV-2 in OSA patients during the pandemic. Materials and Methods: In our sleep centre, patients (n=202) who were diagnosed with sleep apnoea were called by telephone and a survey related to COVID-19, anxiety levels, PAP use, weight change, sleep quality and OSA was conducted during the pandemic. Results: In the study, hypertension and diabetes were the most common comorbidities. Overall, some of the patients with OSA gained weight, stopped PAP treatment, and ESS scores increased compared to the prepandemic period. The anxiety of hospitalization due to COVID-19 was found to be higher than the anxiety for COVID-19. There was a positive correlation between the level of anxiety at admission to hospital due to the risk of COVID-19 and the ESS measured at diagnosis (r=0.203 p=0.004). Better sleep quality was seen in 77% of the participants during the pandemic. Conclusion: OSA patients with high ESS at diagnosis, PAP uses, COVID-19 positive or with domestic contact should be monitored closely by telemedicine. In future research should examine sleep quality and the effects of working from home in OSAS individuals.

The aim of this study was to determine the spectrum of aeroallergen sensitivity of patients. The skin prick test results of patients with symptoms compatible with allergic diseases between March 2002 and December 2004 were retrospectively evaluated in Gaziantep University Pulmonary outpatient clinic. Of 1627 patients in whom skin prick tests (Stallergenes, France) were performed, 528 (32.5%) patients had at least one positive result among 8 allergens. The mean age of patients who had positive allergic skin test was 33.03 ± 11.80 (16-69), and 335 (63.4%) of those were female. Two hundred twenty five (52%) patients had positive skin prick test for one allergen tested, while 253 (48%) patients had positive skin prick test for multiple allergens. Phleum pratensis was the most common allergen found to be positive in 221 (41.8%) patients. The prevalence of positive tests were as follows; cockroach (32.9%), Dermatophagoides pteronyssinus (32.7%), Olea europea (27%), cat dander (14.9%), Parieteria officinalis (11.7%), Cladosporium (9.8%) and Alternaria (8.9%).Phleum pratensis and cockroach were the most common allergens causing a sensitivity reaction detected in our clinic.

İki hafta önce mevcut tedavisine inhaler steroid dozu azaltılmadan montelukast eklendiğini belirtti. Hastanın serum eozinofil sayısı 1460/mm 3 (%15) ve immünglobulin E düzeyi 547 IU/mL idi. Toraks bilgisayarlı tomografisinde yamalı infiltrasyonlar saptanan hastanın bronkoalveoler lavaj sıvısında ve transbronşiyal biyopside eozinofilik inflamasyon tespit edildi. Churg-Strauss sendromu tanısıyla prednizolon tedavisi başlanan hastada üç aylık tedavi sonrasında tam yanıt elde edildi. Hasta halen flutikazon propionat ve salmeterol tedavisi altında sorunsuz izlenmektedir.
