In chronic pancreatitis, recurrent episodes of inflammation lead to the replacement of pancreatic parenchyma with fibrotic connective tissue. Chronic pancreatitis pain, which may initially mimic acute pancreatitis, is severe, frequent, and continual and has a major impact on the quality of life and social functioning of patients. The standard treatments for this disease are endoscopy, surgery, splanchnic nerve denervation, thoracoscopic splanchnicectomy (TS), and video-assisted thoracoscopic surgery (VATS). Considering the advantages of the single-port method, we attempted to describe the post-treatment conditions of the patients undergoing this therapeutic approach.Materials & Methods: Ten chronic pancreatitis patients with severe resistant pain volunteered to enter the study. We recorded the data on patients’ age, gender, pre-operative pain level, surgical complications, and post-operati...

We aimed to examine the diagnostic accuracy of imaging modalities in pediatric thoracic masses. Materials and Methods This prospective cross-sectional study was conducted from 2017 to 2018 in Dr. Sheikh hospital, Mashhad, Iran. A total of 130 patients with a confirmed pathology report of thoracic masses were recruited in this study. A pediatric radiologist independently evaluated the existing chest X-ray (CXR), ultrasound (US) and CT and reported the probable diagnosis. Imaging reports of CXR, US, and CT were compared with the pathology results. Results 83 (63.8%) of the patients were boys with the mean age of 72.15 + 46 months. The most prevalent site of the thoracic masses was the lung parenchyma with the frequency of 81 (62.3%), and the most frequent mass was hydatid cyst with the frequency of 57 (43.8%). Thoracic CT had the overall ...

The aim of this study was to determine the relationship between early risk of coronary artery disease(CAD) with renin angiotensin(RAS) genes and endothelial nitric oxide synthase(eNOS) gene in a sample of the Iranian population. Methods & Materials: In this cross-sectional study, 63 patients with premature CAD and 72 healthy samples were enrolled. Polymorphism of the promotor region of eNOS- and ACE-I/D(Angiotensin Converting Enzyme-I/D) polymorphism were evaluated. Polymerase chain reaction(PCR) test and PCR-RFLP(Restriction Fragment Length Polymorphism) was performed for ACE and eNOS-786 gene, respectively. Results: The frequency of deletion(D) for ACE gene was 96% for patients and 61% for healthy controls and was significantly higher in patients(P<0.001). Conversely, the number of defective C allele for eNOS gene was similar in both groups(p>0.9). Conclusion:...

The following data was obtained:1) NCDs disability-adjusted life years (DALYs), health-adjusted life expectancy, and the health access and quality index from the Global Burden of Disease study; 2) the number of hospital beds, physicians, nurses and midwives per population, and out-of-pocket payments from the WHO website. Using the multilevel generalized linear model, these variables’ independent associations with COVID-19 mortality rate ratio (MRR) was examined. Results: Hospital beds were associated with reduced COVID-19 mortality (MRR=0.47; 95% CI: 0.44 to 0.5) globally. During COVID-19 peak periods, despite a decreasing trend in COVID-19 MRR with increasing beds in high-income countries, the odds of mortality remai...

Drug-Induced Sleep Endoscopy (DISE) in sedated patients with obstructive sleep apnea is the technique of choice for revealing anatomic and dynamic collapsible areas. Materials and Methods: In a prospective cross-sectional study adult patients with OSA documented by polysomnography were evaluated by sleep endoscopy. DISE had been done by an otolaryngologist in the setting of operating room during infusion of propofol and after the start of snoring. Endoscopic findings were recorded and evaluated from the aspect of obstruction level, severity, and multiplicity. Results: Twenty OSA patients (60% men) with mean±SD age of 38.9±9.26 years and mean Body Mass Index (BMI) of 26.57 kg/m2 were included in the study. OSA was severe in 11(55%) and moderate in 5(25%) subjects. Unilevel airway collapse was observed as re...

The aim of this study was to evaluate the perception to problem identification among nursing students in a newly-founded Nursing school in Torbat, Iran. Materials & Methods: This cross-sectional study was conducted in a newly-founded Nursing School in Torbat. 120 students completed the questionnaire. The Dundee Ready Education Environment Measure (DREEM) was used to evaluate the educational environment. Results: Mean age of the study population was 20.81±1.55 yrs with a female/male ratio of 1.22. The mean total score of the DREEM scale was 115.67±12.4 out of 200. The mean score of all subscales of the DREEM questionnaire were "positive perception" except for the students' self-perception domain that had many negative aspects. Conclusion: In this study the students' self-perception was not favorable according to the DREEM scale and was identified as "need to be changed and improved" in our newly-founded nursing school. Having a schedule to train nursing students and providing friendly, supportive and comfortable atmosphere can improve the educational environment.

This study is designed to assess the associations between pre-stroke physical activity and age of first-ever stroke occurrence and long-term outcomes. Six hundred twenty-four cases with first-ever stroke were recruited from the Mashhad Stroke Incidence Study a prospective population-based cohort in Iran. Data on Physical Activity Level (PAL) were collected retrospectively and were available in 395 cases. According to the PAL values, subjects were classified as inactive (PAL < 1.70) and active (PAL ≥ 1.70). Age at onset of stroke was compared between active and inactive groups. Using logistic model, we assessed association between pre-stroke physical activity and long-term (5-year) mortality, recurrence, disability, and functional dependency rates. We used multiple imputation to analyze missing data. Inactive patients (PAL < 1.70) were more than 6 years younger at their age of first-ever-stroke occurrence (60.7 ± 15.5) than active patients (67.0 ± 13.2; p < 0.001). Patients with PAL< 1.7 also had a greater risk of mortality at 1 year [adjusted odds ratio (aOR) = 2.31; 95%CI: 1.14–4.67, p = 0.02] and 5 years after stroke (aOR = 1.81; 95%CI: 1.05–3.14, p = 0.03) than patients who were more physically active. Recurrence rate, disability, and functional dependency were not statistically different between two groups. Missing data analysis also showed a higher odds of death at one and 5 years for inactive patients. In our cohort, we observed a younger age of stroke and a higher odds of 1- and 5-year mortality among those with less physical activity. This is an important health promotion strategy to encourage people to remain physically active.
