OBJECTIVE This article attests the association between self-rated health and health care utilizat... more OBJECTIVE This article attests the association between self-rated health and health care utilization in a retired population by a narrative review. METHOD A systematic review was carried out from 1967 to 2007 to identify studies on self-rated health, in the databases MEDLINE, LILACS e SCIELO. The OVID software was used to Medline research. Contacts with experts were used. The key words retirement, self-rated health, health care utilization and narrative review were used in Portuguese, Spanish and English versions. Al the abstracts were analyzed, but only trials that associated self-rated health in retirement and health care utilization were fully analyzed. RESULTS Thirty one out of 443 abstracts were selected to be fully analyzed. Current data indicates that health deterioration, poor self-rated health and increased health care utilization are associated with early retirement, due to disability, but not with individuals who worked until retirement by age. Methods We used a multistep process, beginning with a Delphi method involving global experts and subsequently involving non-NF experts, patients, and foundations/patient advocacy groups. Results We reached consensus on the minimal clinical and genetic criteria for diagnosing and differentiating NF1 and LGSS, which have phenotypic overlap in young patients with pigmentary findings. Criteria for the mosaic forms of these conditions are also recommended. Conclusion The revised criteria for NF1 incorporate new clinical features and genetic testing, whereas the criteria for LGSS were created to differentiate the two conditions. A simpler, non-invasive and cost-effective method for measuring adiposity would be useful in the care of individuals with NF1. Dual energy X-ray absorptiometry (DXA) is the standard method for this assessment but it is not widely accessible in daily clinical practice. This study aimed to compare body compositions measured using bioelectrical impedance analysis (BIA) and skinfold thickness (ST) predictive equations to those measured with DXA in twenty-six individuals with NF1. Body fat percentage (BF%) was predicted using DXA, five ST-equations and four BIA-equations. The BF% measured by DXA was 26.6 ± 7.3 and 37.4 ± 7.2 % for men and women, respectively. The best predictor of BF% was the Sun et al. BIA equation with a smaller difference compared to DXA (P=0.664), better median of adequacy (101.0%) and accuracy of 46.2%. For males, the Kyle et al. and Lohman BIA equations were the best predictors (accuracy: 78.6 and 64.3%, respectively). For females, all nine equations showed lower differences compared to DXA (P<0.001 for all equations). Among ST equations, Durnin and Womersley showed a smaller difference, greater median of adequacy and percentage of adequacy compared to DXA, even when stratified by sex. This study showed that BIA equations present better adequacy and accuracy compared to SK equations. Nevertheless, these equations should be used with caution in this population due to the variations observed in comparison to DXA.

Subjects and methods Forty patients with NF1 were matched by sex, age, and body mass index (BMI) to 40 controls from the community. Blood samples were collected for biochemical assessment. Homeostasis model assessment adiponectin (HOMA-AD), Homeostasis model assessment insulin resistance (HOMA-IR), and adiponectin/leptin ratio (ALR) were used to identify IR. Results The median HOMA-IR values were similar between the groups. However, the HOMA-AD value was significantly lower and the ALR significantly higher in the NF1 group. Fasting blood glucose (FBG), leptin, and visfatin levels of patients with NF1 were significantly lower, although adiponectin levels were significantly higher than those in the controls. To investigate the sociodemographic and morbidity profile of advanced dementia patients and socio... more To investigate the sociodemographic and morbidity profile of advanced dementia patients and sociodemographic data of their primary caregivers. Data was obtained from 67 elderly recruited for an observational prospective study, through interviews performed with primary caregivers. For statistical analysis, the Statistical Package for the Social Sciences (SPSS(r)) for Windowsr, version 19.0. was employed. Advanced dementia patients were mostly women, widows, and non-institutionalized, with low-income. An elevated rate of stroke, sarcopenia, and pressure ulcers in this population is noted. Caregivers were mostly women, married, children of the studied patients, and unemployed. Only one third of caregivers were hired for the task. Patients with advanced dementia present a high morbidity profile, low income, and depend on the care given by family members, mostly unemployed daughters.

A cross-sectional study of 60 NF1 patients (29 men, 31 women) aged ≥ 18 years who were evaluated from September 2012 to September 2013 in a Neurofibromatosis Outpatient Reference Center. Patients underwent nutritional assessment including measurements of weight, stature, waist circumference (WC), upper-arm circumference (UAC), and skinfolds (biceps, triceps, subscapular, suprailiac). Body mass index (BMI), upper-arm total area (UATA), upper-arm muscle area (UAMA), upper-arm fat area (UAFA), body fat percentage (BFP), fat mass, fat-free mass, fat mass index, and fat-free mass index were also calculated. The mean age of the study population was 34.48±10.33 years. The prevalence of short stature was 28.3%. Low weight was present in 10% of the sample and 31.7% of patients had a BMI ≥ 25 kg/m2. Fasting blood glucose (FBG) level is the main criterion used to diagnose DM and glucose intolerance. Therefore, this study compared FBG level between adults with NF1 and non-NF1 controls. We selected clinical records of 57 out of 701 individuals attending the Neurofibromatosis Outpatient Reference Center (CRNF) of the Clinics Hospital of the Federal University of Minas Gerais in Brazil. The selected patients with NF1 were matched to non-NF1 controls selected from the Brazilian Longitudinal Study of Adult Health according to sex, age (range, 35-74 years), and body mass index at a ratio of 1:3. In both groups, individuals with DM were excluded. Sessenta e três pacientes com insuficiência renal aguda secundária a acidente ofídico foram trata... more Sessenta e três pacientes com insuficiência renal aguda secundária a acidente ofídico foram tratados no CTI do Hospital das Clínicas da UFMG. Em 32 pacientes (51%) o acidente foi produzido por serpentes do gênero Bothrops (grupo bio-trópico) e em 32 pacientes (49%) pela cascavel sul-americana (grupo crotálico). As principais complicações apresentadas pelos pacientes foram a uremia (100% dos casos), hiperpotassemia (89% dos casos), anemia (78% dos casos), infecção urinária (37% dos casos), hiper-hidratação (17% dos casos), parada cardíaca (14% dos casos) e edema agudo dos pulmões (11% dos casos). Cinco pacientes do grupo crotálico (16%) tiveram insuficiência respiratória aguda atribuída à ação neurotóxica do veneno, quatro dos quais se recuperaram completamente. O diagnóstico da rabdomiólise baseou-se na mialgia intensa e generalizada apresentada pelos pacientes e na constatação de níveis séricos elevados de CPK, TGO e DHL. A confirmação do diagnóstico foi obtida no caso n.° 2 pela detecção de mioglobina no soro através de imunoeletroforese contra soro antimioglobina humana e por biópsia muscular. Esse paciente desenvolveu também, como complicação do envenenamento ofídico, quadro clínico e laboratorial de insuficiência renal aguda. Objective: To evaluate the occurrence of complications, as well as the survival rates, in elderly... more Objective: To evaluate the occurrence of complications, as well as the survival rates, in elderly people having neurological diseases and undergoing enteral nutrition therapy (ENT). Methods: Patients aged over 60 years, assisted by a home medical service from a healthcare plan in the city of Belo Horizonte, MG, Brazil, were thoroughly evaluated. The mentioned evaluation occurred at their homes after hospital discharge with enteral nutrition (EN) after a three-month period, a six-month period, and at the end of the study. A nutritional assessment was performed along with data collection performed on the patients' electronic medical records, and interviews performed with patients' family members and caregivers. Results: Seventy-nine patients aged 82.9 ± 10.4 years old were evaluated; of these, 49.4% presented dementia, and 50.6% presented other neurological diagnoses. 100% of patients presented a high dependence level, assessed by the Katz index. The majority of patients (91.2%) presented some complications such as: pneumonia, catheter loss, diarrhea, constipation, vomiting, fluid leakage, periostotomy, tube obstruction, reflux, and myiasis. Pneumonia was the most frequent complication, occurring in 55.9% of cases. The mortality rates were 15.2% at a three-month period, 22.8% at a six-month period, and 43% at the end of study. The median survival after starting EN was 364 days. Differences among the mortality rate and neurological diagnosis, EN routes of access, and complications were not observed. The survival rate was lower in patients having inadequate nutritional status and albumin levels < 3.5 mg/dL. Conclusion: The population followed presented a high rate of complications and death at the end of the study. Diagnosis of dementia, EN routes of access, as well as complications, did not influence the survival rates. However, inadequate nutritional status according to the clinical assessment and albumin levels lower than 3.5 mg/dL significantly influenced the survival rates.

Seventeen patients stung by Tityus serrulatus scorpion were classi®ed as mild (pain at the site o... more Seventeen patients stung by Tityus serrulatus scorpion were classi®ed as mild (pain at the site of the sting, n = 6), moderate (local pain and one of the following manifestations: vomiting, psychomotor agitation, prostration, sweating, tachypnea, tachycardia and mild arterial hypertension, n = 10) and severe cases (equal moderate cases plus cardiac failure, pulmonary edema and shock, n = 1). Venous blood was sampled for biochemical and hematological analysis and for IL-1a, IL-6, IL-10, TNF-a, IFN-g and GM-CSF ELISAs at the time of hospital admission, 6 h (moderate and severe cases), and 12, 18, 36 and 72 h (severe case) later. Ten age-matched healthy volunteers were used as control. Increased serum levels of IL-1a was noticed in all patients, high levels of IL-6, IFN-g and GM-CSF were observed only in a patient with severe envenomation. Our data suggest that a systemic in¯ammatory response-like syndrome is triggered during severe envenomation caused by T. serrulatus sting and that release of cytokines may be involved in this response.

A cross-sectional study of 60 NF1 patients (29 men, 31 women) aged ≥ 18 years who were evaluated from September 2012 to September 2013 in a Neurofibromatosis Outpatient Reference Center. Patients underwent nutritional assessment including measurements of weight, stature, waist circumference (WC), upper-arm circumference (UAC), and skinfolds (biceps, triceps, subscapular, suprailiac). Body mass index (BMI), upper-arm total area (UATA), upper-arm muscle area (UAMA), upper-arm fat area (UAFA), body fat percentage (BFP), fat mass, fat-free mass, fat mass index, and fat-free mass index were also calculated. The mean age of the study population was 34.48±10.33 years. The prevalence of short stature was 28.3%. Low weight was present in 10% of the sample and 31.7% of patients had a BMI ≥ 25 kg/m2. Fasting blood glucose (FBG) level is the main criterion used to diagnose DM and glucose intolerance. Therefore, this study compared FBG level between adults with NF1 and non-NF1 controls. We selected clinical records of 57 out of 701 individuals attending the Neurofibromatosis Outpatient Reference Center (CRNF) of the Clinics Hospital of the Federal University of Minas Gerais in Brazil. The selected patients with NF1 were matched to non-NF1 controls selected from the Brazilian Longitudinal Study of Adult Health according to sex, age (range, 35-74 years), and body mass index at a ratio of 1:3. In both groups, individuals with DM were excluded. Em 32 pacientes (51%) o acidente foi produzido por serpentes do gênero Bothrops (grupo bio-trópico) e em 32 pacientes (49%) pela cascavel sul-americana (grupo crotálico). As principais complicações apresentadas pelos pacientes foram a uremia (100% dos casos), hiperpotassemia (89% dos casos), anemia (78% dos casos), infecção urinária (37% dos casos), hiper-hidratação (17% dos casos), parada cardíaca (14% dos casos) e edema agudo dos pulmões (11% dos casos). Cinco pacientes do grupo crotálico (16%) tiveram insuficiência respiratória aguda atribuída à ação neurotóxica do veneno, quatro dos quais se recuperaram completamente. O diagnóstico da rabdomiólise baseou-se na mialgia intensa e generalizada apresentada pelos pacientes e na constatação de níveis séricos elevados de CPK, TGO e DHL. A confirmação do diagnóstico foi obtida no caso n.° 2 pela detecção de mioglobina no soro através de imunoeletroforese contra soro antimioglobina humana e por biópsia muscular. Esse paciente desenvolveu também, como complicação do envenenamento ofídico, quadro clínico e laboratorial de insuficiência renal aguda. Essa complicação foi atribuída à ação nefrotóxica e hemolítica do veneno crotálico e à hipotensão arterial apresentada pelo paciente, não se afastando a possibilidade de que a rabdomiólise tenha sido um fator contribuinte para a sua instalação. Objective: To evaluate the occurrence of complications, as well as the survival rates, in elderly... more Objective: To evaluate the occurrence of complications, as well as the survival rates, in elderly people having neurological diseases and undergoing enteral nutrition therapy (ENT). Methods: Patients aged over 60 years, assisted by a home medical service from a healthcare plan in the city of Belo Horizonte, MG, Brazil, were thoroughly evaluated. The mentioned evaluation occurred at their homes after hospital discharge with enteral nutrition (EN) after a three-month period, a six-month period, and at the end of the study. A nutritional assessment was performed along with data collection performed on the patients' electronic medical records, and interviews performed with patients' family members and caregivers. Results: Seventy-nine patients aged 82.9 ± 10.4 years old were evaluated; of these, 49.4% presented dementia, and 50.6% presented other neurological diagnoses. 100% of patients presented a high dependence level, assessed by the Katz index. The majority of patients (91.2%) presented some complications such as: pneumonia, catheter loss, diarrhea, constipation, vomiting, fluid leakage, periostotomy, tube obstruction, reflux, and myiasis. Pneumonia was the most frequent complication, occurring in 55.9% of cases. The mortality rates were 15.2% at a three-month period, 22.8% at a six-month period, and 43% at the end of study. The median survival after starting EN was 364 days. Differences among the mortality rate and neurological diagnosis, EN routes of access, and complications were not observed. The survival rate was lower in patients having inadequate nutritional status and albumin levels < 3.5 mg/dL. Conclusion: The population followed presented a high rate of complications and death at the end of the study. Diagnosis of dementia, EN routes of access, as well as complications, did not influence the survival rates. However, inadequate nutritional status according to the clinical assessment and albumin levels lower than 3.5 mg/dL significantly influenced the survival rates.

Seventeen patients stung by Tityus serrulatus scorpion were classi®ed as mild (pain at the site o... more Seventeen patients stung by Tityus serrulatus scorpion were classi®ed as mild (pain at the site of the sting, n = 6), moderate (local pain and one of the following manifestations: vomiting, psychomotor agitation, prostration, sweating, tachypnea, tachycardia and mild arterial hypertension, n = 10) and severe cases (equal moderate cases plus cardiac failure, pulmonary edema and shock, n = 1). Venous blood was sampled for biochemical and hematological analysis and for IL-1a, IL-6, IL-10, TNF-a, IFN-g and GM-CSF ELISAs at the time of hospital admission, 6 h (moderate and severe cases), and 12, 18, 36 and 72 h (severe case) later. Ten age-matched healthy volunteers were used as control. Increased serum levels of IL-1a was noticed in all patients, high levels of IL-6, IFN-g and GM-CSF were observed only in a patient with severe envenomation. Our data suggest that a systemic in¯ammatory response-like syndrome is triggered during severe envenomation caused by T. serrulatus sting and that release of cytokines may be involved in this response.
