Introduction The non-motor symptoms have a major impact on quality of life in patients with Parki... more Introduction The non-motor symptoms have a major impact on quality of life in patients with Parkinson Disease (PD). We present results of the study on the impact of subthalamic deep brain stimulation (DBS-STN) on sleep and other non-motor symptoms in PD patients. Materials and Methods Thirty-six patients with advanced PD were included into the study. Twenty four were evaluated with two-night polysomnography (PSG) before surgery and at 6 months after DBS programming. The whole group (n=36) was assessed using motor, non-motor symptoms (sleep disturbances in particular) and quality of life measures (QoL), before surgery, 6 and 12 months after DBS programming. Results DBS-STN resulted in the significant deterioration of objective sleep parameters, as assessed by PSG, mostly in terms of total sleep time, sleep efficiency, duration of N1 and N2 sleep, wakefulness after sleep onset and sleep latency. At the same time, improvement in the subjective sleep measures, other non-motor symptoms (particularly fatigue, cardiovascular, gastrointestinal, and sexual symptoms) and QoL was identified. The subjective improvement of sleep, other non-motor symptoms and QoL was most prominent in the first 6 months after DBS-STN, diminished slightly (being still better than before surgery) after 12 months, in parallel to mood deterioration. Conclusion DBS-STN resulted in the subjective sleep quality improvement with worsening of objective (PSG) sleep parameters after 6 months. After 12 months all sleep clinical outcome measures were still better than before surgery, albeit worse when compared to the first follow-up visit. Subjective sleep quality correlated positively with mood.

BackgroundClinical and ethical implications of personality and mood changes in Parkinson's di... more BackgroundClinical and ethical implications of personality and mood changes in Parkinson's disease (PD) patients treated with subthalamic deep brain stimulation (STN-DBS) are under debate. Although subjectively perceived personality changes are often mentioned by patients and caregivers, few empirical studies concerning these changes exist. Therefore, we analysed subjectively perceived personality and mood changes in STN-DBS PD patients.MethodIn this prospective study of the ELSA-DBS group, 27 PD patients were assessed preoperatively and 1 year after STN-DBS surgery. Two categories, personality and mood changes, were analysed with semi-structured interviews. Patients were grouped into personality change yes/no, as well as positive/negative mood change groups. Caregivers were additionally interviewed about patients’ personality changes. Characteristics of each group were assessed with standard neurological and psychiatric measurements. Predictors for changes were analysed.ResultsPersonality changes were perceived by six of 27 (22%) patients and by 10 of 23 caregivers (44%). The preoperative hypomania trait was a significant predictor for personality change perceived by patients. Of 21 patients, 12 (57%) perceived mood as positively changed. Higher apathy and anxiety ratings were found in the negative change group.ConclusionsOur results show that a high proportion of PD patients and caregivers perceived personality changes under STN-DBS, emphasizing the relevance of this topic. Mood changed in positive and negative directions. Standard measurement scales failed to adequately reflect personality or mood changes subjectively perceived by patients. A more individualized preoperative screening and preparation for patients and caregivers, as well as postoperative support, could therefore be useful.

ZusammenfassungIm Jahr 2010 wurden für den Fall eines auffälligen pränataldiagnostischen Befundes... more ZusammenfassungIm Jahr 2010 wurden für den Fall eines auffälligen pränataldiagnostischen Befundes des Ungeborenen neue ärztliche Beratungs- und Informationspflichten bestimmt, um die werdenden Eltern in dieser für sie sehr belastenden Situation zu unterstützen. Zu den ärztlichen Pflichten zählt auch, der Schwangeren mit ihrem Einverständnis einen Kontakt zu einer psychosozialen Schwangerschaftsberatungsstelle zu vermitteln. Die vorgestellte Studie hat u. a. untersucht, wie Ärzte und Beraterinnen diese Vermittlungspflicht interpretieren und umsetzen. Dabei zeigte sich, dass die meisten Gynäkologen ohne Schwerpunkt Pränataldiagnostik unter Vermittlung bereits den Hinweis auf den Beratungsanspruch und das Aushändigen von Kontaktdaten verstehen. Die meisten Pränataldiagnostiker und Beraterinnen hingegen verstehen darunter auch das aktive Herstellen des Kontakts durch den Arzt oder eine medizinische Fachangestellte. Das Vermittlungsverhalten der Pränataldiagnostiker ist interindividuell recht unterschiedlich ausgeprägt, die Anzahl der an Beratungsstellen vermittelten Schwangeren ist seit 2010 leicht gestiegen. Besonders erfolgreich funktioniert die Vermittlung, wenn etablierte Kooperationen zwischen Ärzten und psychosozialen Beraterinnen bestehen.AbstractIn 2010 new information and consultation obligations for physicians were defined in order to support parents in the stressful situation when a prenatal diagnosis (PND) indicates impairment to the child’s health. Among the medical obligations is to establish contact to a psychosocial counselling service subject to the consent of the pregnant woman. This study examined how physicians and psychosocial counsellors understand this obligation and how they put in into practice. The results show that most gynecologists without specialization in PND think of contact establishing in terms of informing the pregnant woman of her entitlement to psychosocial counselling and of making contact addresses of counselling services available. In contrast most psychosocial counsellors and gynecologists with specialization in PND also attribute a more active role to the physician or medical personnel in this task. While the total number of women for whom physicians have established the contact to psychosocial counsellors has risen slightly since 2010, the number varies considerably among individual physicians. This functions particularly successfully when physicians and counsellors cooperate closely.

To study the caregivers' perception of their own well-being 1 year after subthalamic deep bra... more To study the caregivers' perception of their own well-being 1 year after subthalamic deep brain stimulation (STN-DBS) surgery in Parkinson's disease (PD) patients, using a qualitative and quantitative approach. 25 patients and caregivers, living together in partnerships, were examined before and at 3-month and 1-year follow-up (FU) after STN-DBS surgery. Semi-structured FU interviews concerning caregivers' own well-being under STN-DBS were conducted and analyzed: caregivers were accordingly assigned to positive or negative outcome groups. Quality of life (QoL), depression, apathy and anxiety of caregivers and patients were measured. These quantitative data were compared to the 1-year FU interview outcomes. Semi-structured FU interviews concerning caregivers' own well-being under STN-DBS were conducted and analyzed: caregivers were accordingly assigned to positive or negative outcome groups. Quality of life (QoL), depression, apathy and anxiety of caregivers and patients were measured. These quantitative data were compared to the 1-year FU interview outcomes.
