■ ABSTRACT Background: Traditionally, cleft of the hard palate is repaired in two layers, with a ... more ■ ABSTRACT Background: Traditionally, cleft of the hard palate is repaired in two layers, with a nasal mucosal layer and an oral mucoperiosteal layer. The aim of this study was to evaluate the results of one layer closure of hard palate cleft compared to the traditional two layers closure. Methods: The charts of 101 consecutive cases of repair of hard palate cleft performed by the authors from 1981 to 2012 at a tertiary care clinic/hospital in central Wisconsin were reviewed. The cases utilized the single layer closure and were followed in the Cleft Palate Clinic on a yearly basis. Cases included unilateral and bilateral cleft lip and palate as well as isolated cleft palate. Results: All cases healed satisfactorily except for two cases that later required small fistulae repair. Conclusion: Single layer closure of the hard palate cleft is as effective as traditional two-layer closure, with minimal complications.

Ressecções do dorso nasal mal-sucedidas podem gerar problemas funcionais ou estéticos. Realizamos uma revisão de literatura acerca das técnicas essenciais para uma ressecção segura do dorso nasal. Dentre as mais publicadas, ressecções compostas ou por componentes foram as duas grandes vertentes, cada uma apoiada por nomes de peso da rinoplastia mundial. A rotina padronizada em cinco passos pareceu ser ideal dentro de um ambiente de ensino devido à ressecção óssea gradual realizada por raspas e pela reprodutibilidade do método de separação dos componentes do dorso nasal com uma abordagem individualizada de cada estrutura, sendo efetiva e segura ao mesmo tempo. ■ RESUMO

RESULTS/COMPLICATIONS: Nineteen patients matched inclusion criteria, with a mean age of 39 years ... more RESULTS/COMPLICATIONS: Nineteen patients matched inclusion criteria, with a mean age of 39 years old. Six patients (32%) had a history of a prior rhinoplasty procedure. In addition to resection of the medial crura, thirteen patients (68%) also had resection of their lateral crura. All patients underwent a septal resection and 18 (95%) received a columellar strut. Three (16%) patients had spreader grafts inserted and three (16%) patients had a tip graft placed. There were no complications in this series. Postoperative cosmesis was considered excellent by both patient and surgeon in all cases at mean follow-up of 11 months.
