In 2003, The National Academy of Clinical Biochemistry published a consensus monograph on thyroid... more In 2003, The National Academy of Clinical Biochemistry published a consensus monograph on thyroid tests (available via Internet from 2002). The document, a result of international collaboration, describes the pathophysiological background and clinical application of the measurements as well as principles for their technical performance. Of value, and of definite clinical relevance, is the detailed description of factors affecting the results in health and disease. However, we lack a description of the sometimes large differences in values obtained with different generally available methods, as well as a presentation of the occurrence and evaluation of analytical interference and related technical factors affecting results. Readers with little experience from quantitative measurement in the clinical laboratory, therefore, are not stimulated to obtain a better understanding of causes and evaluation of unexpected results.

When communicating results from laboratory investigations from the laboratory to the requesters a... more When communicating results from laboratory investigations from the laboratory to the requesters and further between different information systems, it is important that the value as well as the unique identity and name of the laboratory investigation are correctly cited. A committee under the International Federation for Clinical Chemistry and Laboratory Medicine (IFCC) and International Union for Pure and Applied Chemistry (IUPAC) has developed a systematic nomenclature for the correct classification of laboratory investigations. Each generic laboratory investigation is provided with a unique NPU code. The system is in use among approximately 30 different clinical laboratories in Sweden, and has capacity to be the common denominator of all laboratory investigations, and to be used as the identifier in various information systems. The purpose of this investigation was to improve a rankit ordinal model for evaluating and valida... more The purpose of this investigation was to improve a rankit ordinal model for evaluating and validating dichotomized tests in a prospective Nordic project. The model is based on the assumption that the S-shaped curve of fractions of positive for increasing concentrations can be de-convoluted to a histogram and thereby used to calculate the parameters for a ln-Gaussian distribution. In a Nordic survey, four urine samples with known concentrations of hCG (human chorionic gonadotrophin) and nitrites were distributed to more than 2500 practitioners' offices. The results are presented as parameters (geometric mean and CV) for the components urine-hCG and urine-nitrites, together with fractions of positive for clinical critical values (5 and 40 IU/L for hCG), for which fractions should be below 0.01 and above 0.99, respectively, and 7 micromol/L for nitrites. Furthermore, the concentration intervals of varying fractions of positive from 0.01 to 0.99 are estimated as grey zones. The parameters and grey zones for different kits are compared. No urine-hCG kit fulfilled the low clinical criterion, whereas all fulfilled the high criterion. Seven of the eight nitrites kits had fractions of positive above 0.9 for the company confirmation limit, but varying fractions for the clinically important limit of 7 micromol/L (fractions from 0.06 to 0.83). The present model makes it easy to estimate parameters for the kits, and also to estimate the fractions of measured positives for specified concentrations. It is thus suited for external quality assessment as well as for manufacturers' method validation.

The reliability of interpretations of findings from dip-slide devices for culturing urine was inv... more The reliability of interpretations of findings from dip-slide devices for culturing urine was investigated in a national Swedish external quality assessment (EQA) programme. Also investigated was the extent of improvement in the examination procedure achieved through personnel training programmes and information. According to Swedish national recommendations, dip-slide should only be used in primary health care (PHC) in cases of uncomplicated urinary tract infection (UTI) in females of childbearing age. The recommendations also define six possible outcomes of a dip-slide examination, outcomes that have formed the basis for the EQA programme since 2001. No improvement in ability to classify readings correctly into the six categories was noted for the period 2001 to 2006. Preparations containing 'mixed flora' presented participants with the greatest difficulty, with only 28 % correct reports. The EQA programme, with educational components and voluntary participation, has not improved quality. The disappointing results might be a reflection of the limited effort and resources allocated by clinical microbiology laboratories for training and for sustaining proficiency in the evaluation of dip-slides. For these reasons, we cannot at present recommend the dip-slide technique for use in PHC settings.

Aims: To assess general practitioners (GPs) knowledge of guideline recommendations on diagnosing ... more Aims: To assess general practitioners (GPs) knowledge of guideline recommendations on diagnosing microalbuminuria (MA) and to evaluate how this diagnosis influences drug treatment of diabetes patients. Methods: A postal case-history based questionnaire describing a male patient (previously not tested for MA) with type 2 diabetes who had several risk markers for cardiovascular disease. Results: 2078 GPs from nine European countries were included, with response rates varying from 7% to 43%. Almost all GPs recommended annual testing for MA. Scientists of disciplines in clinical laboratory sciences have long recognized the need for a com... more Scientists of disciplines in clinical laboratory sciences have long recognized the need for a common language for efficient and safe request of investigations, reporting of results, and communication of experience and scientific achievements. Widening the scope, most scientific disciplines, not only clinical laboratory sciences, rely to some extent on various nominal examinations, in addition to measurements. The ‘International vocabulary of metrology – Basic and general concepts and associated terms’ (VIM) is designed for metrology, science of measurement. The aim of the proposed vocabulary is to suggest definitions and explanations of concepts and terms related to nominal properties, i.e., properties that can be compared for identity with other properties of the same kind-of-property, but that have no magnitude. The first perspective, from an accreditation body, is based on the experience of Australia's national laboratory accreditation body (NATA), which has accredited fourteen proficiency testing (PT) providers and is currently processing an application from another. The second perspective is based on the experience of EQUALIS, the national quality assurance organisation for laboratory medicine in Sweden, which was granted accreditation in 2002.
