Primary psoas abscess is very rare in childhood; its diagnosis and treatment can be improved by t... more Primary psoas abscess is very rare in childhood; its diagnosis and treatment can be improved by the use of recent imaging techniques. Six children, aged 11 months to 10 years (mean: 51 months) were admitted from January 1987 to 15 December 1993 because they suffered mainly from fever, lumbar pain and/or homolateral flexed hip. Examination showed a painful inguinal mass in five patients and hyperleucocytosis in all. X-rays showed disappearance of the external limit of psoas in two patients; ultrasonography showed enlarged psoas in all, associated with hypoechogenic mass in two and without echo in four patients. CT scan showed the abscess in all cases, permitting a needle aspiration for bacteriological studies: Staphycococcus aureus was present in five cases. Drainage of the abscess by CT-guided percutaneous catheter was performed in two patients. Surgical drainage was performed in three including one for whom percutaneous catheter drainage did not succeed. The two other patients were only given antibiotics. CT-guided needle aspiration to establish presence and nature of fluid collection is a well established technique that may be extended to treatment of psoas abscesses.

A series of four cases observed in the department of pediatric surgery in Monastir between 1990 a... more A series of four cases observed in the department of pediatric surgery in Monastir between 1990 and 1998 are reported. The diagnosis was made in the two first days of life. The clinical features associated paleness, shock and abdominal distension. Ultrasonography revealed that the hemoperitoneum was associated with liver hematoma in three cases and with retroperitoneal hematoma in two cases. Three infants required emergency hemostatic laparotomy. The fourth was stabilized by initial resuscitation. One infant died at the age of six months following subarachnoid hemorrhage and subdural hematoma. No complication occurred in the three remaining patients. The pathogeny, clinical features and treatment of the hemoperitoneum in the newborn are discussed. The authors stress preventive measures, the only way to decrease the incidence of this neonatal complaint.

The authors report two cases in children aged 3 years and 5 years with left ectopic testis. Surgical exploration demonstrated two testes on the left side with a third testis on the right. Descent of the two testes into the left hemi-scrotum was performed. The authors review the management of polyorchidism in the light of these two cases.

Treatment of this tumour is not well defined and its prognosis remains poor despite progress in paediatric oncology. The authors report a new case of neonatal rhabdoid tumour of the kidney, with a rapidly fatal outcome and present a review of the literature.

Up until the beginning of the 1990s, surgery was the only available modality for the treatment of... more Up until the beginning of the 1990s, surgery was the only available modality for the treatment of renal and ureteric stones in infants. From 1990 onwards, two new minimally invasive techniques were developed, leading to a real revolution in the management of these patients. The first and most spectacular breakthrough was extension of the application of ESWL to this age-group and the second breakthrough was ureteroscopy following the enormous technological progress in the manufacture of endoscopes. This technique is increasingly useful when lithotripsy is impossible or insufficient. Surgery is still indicated in the case of contraindications to minimally invasive methods or after failure of these methods. It is the method of choice for bladder stones. Medical treatment or watchful waiting have valuable indications, at the cost of regular clinical and radiological follow-up of patients. The choice of treatment modality depends on the size of the stone, its site, its composition, the anatomy of the urinary tract and the availability of the various techniques.

We evaluated the metabolic and the nutritional aspects of 134 urolithiasis children in order to o... more We evaluated the metabolic and the nutritional aspects of 134 urolithiasis children in order to outline the characteristics of idiopathic urolithiasis in children. This prospective study group of 134 children (56 females, 78 males) with renal calculi was evaluated. The age range of the patients was six months to 16 years. A dietary survey was performed on every child. All patients were investigated with respect to stone localization and serum and urine risk factors. Statistical analysis of data was carried out using software SPSS 11.0 for Windows. Hypercalciuria was the most common risk factor detected in this group (28.3%). A decrease of water intake was noted in all age groups, especially in the rural area (549.6 mL/day vs. 1150.6 mL/day), and there was an increase in animal protein intake in 17 cases (mean: 1.9 g/kg). In addition, increased intake of starchy foods and food with high oxalate content (sorgum) were detected in the ten to 16 years age group (51%) of our study. Calciu...

This retrospective study defines the current status of urolithiasis among children and adolescents in Tunisia. We retrospectively reviewed the records of 310 children and adolescents (age: 3 months - 19 years) between 2003 and 2010, holding urolithiasis. A first-line metabolic, urine and plasma work-up was performed in all patients. Physical and chemical analysis of the stones was performed respectively by stereomicroscopy and infrared spectroscopy. Statistical analysis of the results was performed with SPSS 11.0 software. The Chi-square test was used for comparison of percentages. Our study shows a male predominance of urolithiasis with a sex ratio of 1.5. Stones were located in the upper urinary tract in 70.7% of cases. The aim of this study was to identify the stone composition in Tunisian patients and to highlight their modification according to patients' sex and age. We studied 1200 urolithiasic patients, from the urologic and the pediatric surgery departments, ranging from six months to 92years old and known as having urinary stones (729 males and 471 females). Stone analysis was performed respectively using a stereomicroscope and infrared spectroscopy to determine, respectively, the morphological type and the molecular composition of each. Kidney stones were encountered in 48.6% of calculi. Children and old men were more affected by bladder stone. One infant survived and 2 died. The aetiology of neonatal gastric rupture is discussed, as well as some diagnostic features. The importance of early diagnosis and prompt surgical intervention is stressed.
Inflammatory myofibroblastic tumors or inflammatory pseudotumors are uncommon solid tumors that a... more Inflammatory myofibroblastic tumors or inflammatory pseudotumors are uncommon solid tumors that are highly vascularized, have an intermediate prognosis, and are associated with local recurrence, and rare metastasis. Inflammatory myofibroblastic tumors (IMT) most often are seen in the lung of young adults, but there have been reports of these tumors occurring in children, in various sites. The authors report a case of an ovarian IMT arising in a six-year-old girl (third case reported in the literature), treated by a right adnexectomy with uneventful postoperative course, and discuss recent reports.

Pyelogenic cyst is an intraparenchymal renal cavity lined by transitional cell epithelium which c... more Pyelogenic cyst is an intraparenchymal renal cavity lined by transitional cell epithelium which communicates with a calyx via a neck. It is a rare disease, often discovered incidentally. The diagnosis is based on imaging. Asymptomatic forms require simple surveillance and surgical management is only indicated in forms complicated by urinary tract infections or stones. In the light of a case of pyelogenic cyst revealed by upper urinary tract infection in a 12-year-old girl, the authors recall the clinical, histological and therapeutic features of this rare disease.
