Patients and methods: Sixty infertile women with PCOS and 30 age-matched women with unexplained infertility (UEI), were included. Patients in PCOS and control groups were divided into three subgroups according to their BMI values as normal, overweight and obese. Each BMI group was divided into three subgroups according to vitamin D levels. While AMH and vitamin D levels were determined at first admission, AFC was measured on the third day of cycle. Results: BMI, AFC, and AMH levels of women with PCOS were significantly higher than the UEI group. AMH values of women with PCOS with normal BMI were found to be significantly higher than UEI controls with normal BMI. AMH values of overweight and obese PCOS patients and controls were similar. As BMI values of the PCOS group increased, vitamin D levels decreased significantly. Vitamin D levels of the patients in the PCOS group were found to be significantly lower than the control group. When evaluated according to BMI, the vitamin D levels of normal, overweight and obese women with PCOS were significantly lower than the UEI. Conclusions: Rising BMI in PCOS leads to a significant decrease in vitamin D and AMH. Deficiency, insufficiency or normality of vitamin D do not affect the main markers of ovarian reserve.

This study was planned to investigate whether endometrial injury alters the expression of both transcripts in women with implantation failure. Methods: A total of 54 women with implantation failure were divided into two equal groups as experimental (scratching) and sham (no scratching). Participants in the scratching group were exposed to endometrial injury in the mid-luteal phase, and those in the sham group were exposed to endometrial flushing. The scratching group, but not the sham group, underwent prior endometrial sampling. A second endometrial sampling was performed on the scratching group in the mid-luteal phase of the following cycle. The mRNA and protein levels of the HOXA10 and 11 transcripts were determined in endometrial samples collected before and after injury/flushing. Participants in each group underwent IVF/ET in ...

Leukemia inhibitory factor (LIF) is an important cytokine involved in implantation. This study was planned to investigate the effect of HSG on LIF mRNA expression in endometrial flushing samples. Methods: Forty infertile patients scheduled for HSG were included in the study. Before HSG, endometrial flushing was performed in the mid-luteal phase, followed by contrast-medium infusion into the cavity. Second flushing was done in the mid-luteal phase of the next cycle. Ten patients who underwent diagnostic hysteroscopy were taken as the second control group. LIF mRNA levels were measured by RT-PCR in endometrial flushing samples collected from each group. Results: The pre-HSG LIF mRNA levels of the infertile group were significantly lower than those of the fertile group. In infertile patients, LIF mRNA expression increased 3.2 times after HSG compared to baseline leve...

PATIENTS AND METHODS Fifty-eight men diagnosed with unexplained infertility and 50 age and BMI matched fertile men were included in the study. A participant whose semen parameter is normal but pregnancy is not achieved was accepted as unexplained male infertility. Blood samples were taken from all participants following three-day abstinence for measurement of vitamin D. Sperm DNA damage was assessed by Aniline Blue staining of the collected samples. RESULTS Compared with the fertile men, male patients with unexplained infertility had significantly lower vit D levels (27.00 ng/mL (12.63-39.30) vs. 23.66 ng/mL (7.50-55.00), p<0.004). While the number of patients with vitamin D levels lower than 20 ng/mL was 26 (44.8%) in the infertile group, it was recorded as 5 (10.0%) in the fertile group (p<0....

The present study aimed to evaluate the hormonal alterations through body mass index (BMI) on the relationship between insulin, insulin resistance (IR), and thyroid hormone levels in PCOS. Materials and Methods: This study is a retrospective clinical analysis study conducted at the department of gynecology and obstetrics between January 2019 and April 2020. PCOS participants were divided into two groups: 59 obese PCOS patients whose BMI value is equal and higher than 27kg/m2; 51 normal or overweight PCOS with BMI less than 27kg/m2. Results: Age factor did not differ between groups (29.8±6 vs 31±7.2; p=0.345). In hormone tests, insulin (14.2±6.8 vs 14.8±8; p=0.688), Homeostatic Model Assessment for Insulin Resistance (HOMA-IR) (3.1±1.4 vs 3.4±1.8; p=0.344), thyroxine (T4)(1.2±0.5 vs 1.2±0.4) and thyroid-stimulating hormone (TSH)(1.9±1.2 vs 1.8±0.9; p=0.848) levels did not differ for BMI groups (p>0.05). Because there was no difference in laboratory comparison for any parameter we analyzed, we could not perform multivariate analysis for further investigations in terms of BMI factor. In the hematological analysis, we did not see any significant alteration for groups. Conclusion: We did not observe any difference between thyroid and IR in the patients who were grouped according to the BMI. PCOS and obesity may be directly related to hormonal interaction rather than adipose tissue.

Sperm DNA fragmentation (SDF) testing was recently added to the sixth edition of the World Health... more Sperm DNA fragmentation (SDF) testing was recently added to the sixth edition of the World Health Organization laboratory manual for the examination and processing of human semen. Many conditions and risk factors have been associated with elevated SDF; therefore, it is important to identify the population of infertile men who might benefit from this test. The purpose of this study was to investigate global practices related to indications for SDF testing, compare the relevant professional society guideline recommendations, and provide expert recommendations. Materials and Methods: Materials and Methods: Clinicians managing male infertility were invited to take part in a global online survey on SDF clinical practices. This was conducted following the CHERRIES checklist criteria. The responses were compared to professional society guideline recommendations related to SDF and the appropriate available evidence. Expert recommendations on indications for SDF testing were then formulated, and the Delphi method was used to reach consensus. Results: Results: The survey was completed by 436 experts from 55 countries. Almost 75% of respondents test for SDF in all or some men with unexplained or idiopathic infertility, 39% order it routinely in the work-up of recurrent pregnancy loss (RPL), and 62.2% investigate SDF in smokers. While 47% of reproductive urologists test SDF to support the decision for varicocele repair surgery when conventional semen parameters are normal, significantly fewer general urologists (23%; p=0.008) do the same. Nearly 70% would assess SDF before assisted reproductive technologies (ART), either always or for certain conditions. Recurrent ART failure is a common indication for SDF testing. Very few society recommendations were found regarding SDF testing. Conclusions: Conclusions: This article presents the largest global survey on the indications for SDF testing in infertile men, and demonstrates diverse practices. Furthermore, it highlights the paucity of professional society guideline recommendations. Expert recommendations are proposed to help guide clinicians.
