Objective: This study aimed to investigate the smoking prevalence and associated factors among th... more Objective: This study aimed to investigate the smoking prevalence and associated factors among the students of Balıkesir University and determine the level of nicotine dependence in smokers. Methods: Six faculties and two graduate schools located at the Balıkesir University campus were enrolled. The cluster method was used as the sampling method. To observe the rates of smoking initiation while studying at the university, first-year and final year students were compared. Results: A total of 1096 students were enrolled, with a mean age of 20.4±2.2 years. The regular smoking prevalence rate was 25.5%, occasional smoking rate was 9.5%, alcohol consumption rate was 28.0%, and drug abuse rate was 5.0%. The mean age for attempting to smoke was 16.6±2.2 years. The leading causes of smoking were curiosity, spiritual space, affection, and to comply with friends. According to the Fagerström questionnaire, the level of nicotine dependence was low in 53.0% of the participants, moderate in 8.7%, and high in 28.4%. The average smoking prevalence rate was 23.6% in first-grade students, whereas it was 44.8% in last-grade students. The factors that influence smoking were as follows: being in the last grade, mother, siblings, and friends who smoke, higher allowance, use of alcohol and drugs. Smoking was found to be more prevalent among males. Conclusion: Last-grade students had a higher smoking prevalence rate than the first-grade students. Family, social environment, and smoking behavior of friends have an influence on smoking. All risk factors, including past history and university period, should be considered together in tobacco control.

Swyer-James-MacLeod Syndrome (SJMS) is a rare disease characterized by a hyperlucent lung appeara... more Swyer-James-MacLeod Syndrome (SJMS) is a rare disease characterized by a hyperlucent lung appearance due to hypoplasia of the pulmonary artery on the affected side. It is thought to develop secondary to viral bronchiolitis and pneumonia in childhood. A 59-year-old female patient presented with the complaints of dyspnea with exertion, a feeling of pressure in the chest, and sputum. On physical examination, respiratory sounds were normal. Posteroanterior (PA) chest X-ray showed marked hyperlucency and volume loss in the left hemithorax. On thorax computed tomography, ipsilateral hyperlucency with a significant decrease in the calibration of the left pulmonary artery and its branches compared to the right, significant volume loss in the lower lobe of the left lung and areas of bronchiectasis were observed. While large segmental perfusion defects were observed in almost the entire lower lobe of the left lung and in the lingular segment in perfusion scintigraphy, ventilation defects were observed in these areas in ventilation scintigraphy. The patient was diagnosed with SJMS based on the present findings. Treatment of the disease is conservative in most cases. It is recommended to protect patients against infections and to have flu and pneumococcal vaccines. We present our case to emphasize that this syndrome should be among the differential diagnoses when unilateral hyperlucent lung appearance is detected.

Although isolated cases have been reported, it is usually due to a hepatic lesion invading the hepatic veins or inferior vena cava, or a direct embolism from right cardiac lesions. Patients may present with hemoptysis, dyspnea, chest pain, acute cor pulmonale, or sudden death. The diagnosis is made from a medical history, clinical findings and computed tomography imaging. The computed tomography shows single or multiple cystic lesions leading to a filling defect or obstruction in the pulmonary arteries. In this article we present a 76-year-old woman with multiple lung lesions and a bilateral pulmonary artery embolism. In this case the pulmonary artery embolism was considered to be secondary to the invasion of the inferior vena cava by the liver lesion.

Görüntüleme rehberliğinde yapılan transtorasik biyopsi işlemleri, lokal anestezi ile yapılabilmes... more Görüntüleme rehberliğinde yapılan transtorasik biyopsi işlemleri, lokal anestezi ile yapılabilmesi ve düşük komplikasyon riski nedeni ile klinisyenler tarafından tercih edilmektedir. Çalışmamızda bilgisayarlı tomografi (BT) ve ultrasonografi (US) rehberliğinde yapılan perkütan akciğer biyopsilerinin teknik ve tanısal başarı oranlarını, komplikasyonlarını ve komplikasyonu arttıran olası sebepleri araştırmayı ve literatürdeki çalışmalar ile karşılaştırmayı amaçladık. Gereç ve Yöntem: Seksenyedi hastaya US, 31 hastaya BT rehberliğinde perkütan akciğer biyopsisi yapılmış olup bu hastaların dosyaları ve radyolojik görüntüleri retrospektif olarak taranmıştır. Lezyonların boyutu, yerleşimi ve işlem sırasında geçilen transparankimal mesafe, iğne tipi, gelişen komplikasyonlar, histopatolojik sonuçlar, işlem süreleri ve maliyetleri not edilmiştir. Bulgular: US ve BT rehberliğinde yapılan biyopsilerde tanısal başarı oranları sırası ile %91,9 ve %77,4 olarak hesaplanmıştır. US rehberliğinde ince iğne biyopsisi (İİAB) (n:33) ve kesici iğne biyopsisi (Tru-Cut) (n:54) yapılan 87 hastanın 85'inde komplikasyon gelişmemiştir. Bir hastada pnömotoraks, 1 hastada hematoraks gözlenmiştir. BT rehberliğinde biyopsi yapılan hastaların 14'ünde pnömotoraks, 2' sinde parankimal kanama saptanmıştır. Yüz dört (%88,1) lezyona spesifik tanı koyulabilmiştir. US rehberliğinde yapılan biyopsi işlemlerinde, BT rehberliğinde yapılan işlemlere göre belirgin olarak düşük komplikasyon oranları saptanmıştır (p<0,001). Komplikasyon gelişimi ile hastaların demografik özellikleri, lezyon boyutu, lokalizasyonu transparankimal mesafe arasında korelasyon saptanmamıştır (p>0,05). Plevral tabanlı lezyonlarda, parankimal yerleşimli lezyonlara göre komplikasyon gelişme olasılığı daha düşük bulunmuştur (p<0,001). Sonuç: Akciğer lezyonlarının US ve BT rehberliğinde iğne biyopsileri teknik ve tanısal başarısı yüksek, majör komplikasyon oranı düşük işlemlerdir. Uygun lezyon yerleşimi durumunda US rehberliğinde yapılan biyopsi işlemleri tanısal başarısı yüksek, komplikasyon riski düşük, BT rehberliğinde yapılan biyopsi işlemlerine göre daha kısa süreli ve düşük maliyetli uygulamalardır. OBJECTIVE: We aimed to determine the preoperative parameters that may predict postoperative pulmo... more OBJECTIVE: We aimed to determine the preoperative parameters that may predict postoperative pulmonary complications (POPCs) and the value of some current practical indexes in predicting POPCs. MATERIALS AND METHODS: Our study is a retrospective cohort study carried out in 9 different centers. Patients admitted to the chest diseases outpatient clinic for preoperative evaluation were followed up during the 6-month study period. Patients with or without postoperative complications were evaluated retrospectively, and the effect of some parameters and indexes recorded during the preoperative evaluation of chest diseases on POPC development was investigated statistically. RESULTS: A total of 307 patients were included in the study. POPCs were observed in 100 patients (32.6%). About 13% of these complications were respiratory tract infections, 59% were respiratory failure, 45% were pleural effusion, and 42% were atelectasis, which were the most common pulmonary complications. The probability of experiencing POPCs by patients with chronic obstructive pulmonary disease (COPD) is 2.5 (1.18-5.67) times more than those without COPD. We determined that patients with the history of upper respiratory tract infection during the preoperative period are 5.3 times more likely to have POPCs; similarly, the number was 4.7 for patients undergoing cardiac operation and 3.3 for patients with interstitial infiltration. CONCLUSION: The risk of pulmonary complications was higher for those with the history of upper respiratory tract infection during the preoperative period, those undergoing cardiac surgery, those with the shortness of breath, those with the history of COPD, and those with the reticular/interstitial infiltrations in the chest X-ray. These parameters should be examined carefully in the preoperative period and should be careful in terms of pulmonary complications that may develop during the postoperative period.

Spontaneous pneumomediastinum and subcutaneous emphysema secondary to pulmonary alveolar microlit... more Spontaneous pneumomediastinum and subcutaneous emphysema secondary to pulmonary alveolar microlithiasis Pulmonary alveolar microlithiasis (PAM) is a rare autosomal recessive disease characterized by intraalveolar accumulation of microliths. PAM is described as the formation and accumulation of calcium phosphate microliths in the alveoli after mutations in the SLC34A2 gene. The patients may be asymptomatic or present with heterogeneous signs such as dyspnea, cough, sputum, weakness, chest pain and hemoptysis. Recurrent pneumothoraces may occur over the course of the disease in advanced cases. Her, it was aimed to report a case of a 40-year-old female presenting with chest pain, sore throat, cough and green sputum for the past two months. Chest radiography and computed tomography showed pneumomediastinum and subcutaneous emphysema which have not been reported in previous studies due to PAM. Bronchoscopy with bronchoalveolar lavage (BAL) and mucosal biopsy were done after pneumomediastinum had cleared. BAL result was concordant with pulmonary alveolar microlithiasis. Diagnosis can be made with computed tomography but may necessitate histopathological verification for definitive diagnosis.

There are limited data about the coexistence of asthma and the Coronavirus disease-2019 (COVID-19). The COVID-19 pandemic is a new disease for the whole world. In this study, we aimed to examine the published case series with the COVID-19 and asthma coexistence, by reviewing existing studies and other publications. We discussed the suggestions of guidelines such as The Global Initiative for Asthma (GINA), The National Institute for Health and Care Excellence (NICE), and the Canadian Respiratory Guideline about the management of asthma during the COVID-19 pandemic and the experiences of different countries. We analyzed the publications, reports, and expert opinions about asthma and COVID-19, that were released and expressed from the onset of the disease in Wuhan. In this review, we aimed to summarize the approach to patients with asthma during the pandemic and to make recommendations concerning it.

Introduction: The aim of our study is to investigate the etiological distribution of ILD in Turke... more Introduction: The aim of our study is to investigate the etiological distribution of ILD in Turkey by stratifying the epidemiological characteristics of ILD cases, and the direct cost of initial diagnosis of the diagnosed patients. Material-Method: The study was conducted as a multicenter, prospective, cross-sectional, clinical observation study. Patients over the age of 18 and who accepted to participate to the study were included and evaluated as considered to be ILD. The findings of diagnosis, examination and treatment carried out by the centers in accordance with routine diagnostic procedures were recorded observationally. Results: In total,1070 patients were included in this study. 567 (53%) of the patients were male and 503 (47%) were female. The most frequently diagnosed disease was IPF (30.5%). Dyspnea (75.9%) was the highest incidence among the presenting symptoms. Physical examination found bibasilar inspiratory crackles in 56.2 % and radiological findings included reticular opacities and interlobular septal thickenings in 55.9 % of the cases. It was observed that clinical and radiological findings were used most frequently (74.9%) as a diagnostic tool. While the most common treatment approaches were the use of systemic steroids and antifibrotic drugs with a rate of 30.7% and 85.6%, respectively. The total median cost from the patient’s admission to diagnosis was 540 Turkish Lira. Conclusion: We believe that our findings compared with data from other countries will be useful in showing the current situation of ILD in our country to discuss this problem and making plans for a solution.

Görüntüleme rehberliğinde yapılan transtorasik biyopsi işlemleri, lokal anestezi ile yapılabilmes... more Görüntüleme rehberliğinde yapılan transtorasik biyopsi işlemleri, lokal anestezi ile yapılabilmesi ve düşük komplikasyon riski nedeni ile klinisyenler tarafından tercih edilmektedir. Çalışmamızda bilgisayarlı tomografi (BT) ve ultrasonografi (US) rehberliğinde yapılan perkütan akciğer biyopsilerinin teknik ve tanısal başarı oranlarını, komplikasyonlarını ve komplikasyonu arttıran olası sebepleri araştırmayı ve literatürdeki çalışmalar ile karşılaştırmayı amaçladık. Gereç ve Yöntem: Seksenyedi hastaya US, 31 hastaya BT rehberliğinde perkütan akciğer biyopsisi yapılmış olup bu hastaların dosyaları ve radyolojik görüntüleri retrospektif olarak taranmıştır. Lezyonların boyutu, yerleşimi ve işlem sırasında geçilen transparankimal mesafe, iğne tipi, gelişen komplikasyonlar, histopatolojik sonuçlar, işlem süreleri ve maliyetleri not edilmiştir. Bulgular: US ve BT rehberliğinde yapılan biyopsilerde tanısal başarı oranları sırası ile %91,9 ve %77,4 olarak hesaplanmıştır. US rehberliğinde ince iğne biyopsisi (İİAB) (n:33) ve kesici iğne biyopsisi (Tru-Cut) (n:54) yapılan 87 hastanın 85'inde komplikasyon gelişmemiştir. Bir hastada pnömotoraks, 1 hastada hematoraks gözlenmiştir. BT rehberliğinde biyopsi yapılan hastaların 14'ünde pnömotoraks, 2' sinde parankimal kanama saptanmıştır. Yüz dört (%88,1) lezyona spesifik tanı koyulabilmiştir. US rehberliğinde yapılan biyopsi işlemlerinde, BT rehberliğinde yapılan işlemlere göre belirgin olarak düşük komplikasyon oranları saptanmıştır (p<0,001). Komplikasyon gelişimi ile hastaların demografik özellikleri, lezyon boyutu, lokalizasyonu transparankimal mesafe arasında korelasyon saptanmamıştır (p>0,05). Plevral tabanlı lezyonlarda, parankimal yerleşimli lezyonlara göre komplikasyon gelişme olasılığı daha düşük bulunmuştur (p<0,001). Sonuç: Akciğer lezyonlarının US ve BT rehberliğinde iğne biyopsileri teknik ve tanısal başarısı yüksek, majör komplikasyon oranı düşük işlemlerdir. Uygun lezyon yerleşimi durumunda US rehberliğinde yapılan biyopsi işlemleri tanısal başarısı yüksek, komplikasyon riski düşük, BT rehberliğinde yapılan biyopsi işlemlerine göre daha kısa süreli ve düşük maliyetli uygulamalardır. OBJECTIVE: We aimed to determine the preoperative parameters that may predict postoperative pulmo... more OBJECTIVE: We aimed to determine the preoperative parameters that may predict postoperative pulmonary complications (POPCs) and the value of some current practical indexes in predicting POPCs. MATERIALS AND METHODS: Our study is a retrospective cohort study carried out in 9 different centers. Patients admitted to the chest diseases outpatient clinic for preoperative evaluation were followed up during the 6-month study period. Patients with or without postoperative complications were evaluated retrospectively, and the effect of some parameters and indexes recorded during the preoperative evaluation of chest diseases on POPC development was investigated statistically. RESULTS: A total of 307 patients were included in the study. POPCs were observed in 100 patients (32.6%). About 13% of these complications were respiratory tract infections, 59% were respiratory failure, 45% were pleural effusion, and 42% were atelectasis, which were the most common pulmonary complications. The probability of experiencing POPCs by patients with chronic obstructive pulmonary disease (COPD) is 2.5 (1.18-5.67) times more than those without COPD. We determined that patients with the history of upper respiratory tract infection during the preoperative period are 5.3 times more likely to have POPCs; similarly, the number was 4.7 for patients undergoing cardiac operation and 3.3 for patients with interstitial infiltration. CONCLUSION: The risk of pulmonary complications was higher for those with the history of upper respiratory tract infection during the preoperative period, those undergoing cardiac surgery, those with the shortness of breath, those with the history of COPD, and those with the reticular/interstitial infiltrations in the chest X-ray. These parameters should be examined carefully in the preoperative period and should be careful in terms of pulmonary complications that may develop during the postoperative period.

Spontaneous pneumomediastinum and subcutaneous emphysema secondary to pulmonary alveolar microlit... more Spontaneous pneumomediastinum and subcutaneous emphysema secondary to pulmonary alveolar microlithiasis Pulmonary alveolar microlithiasis (PAM) is a rare autosomal recessive disease characterized by intraalveolar accumulation of microliths. PAM is described as the formation and accumulation of calcium phosphate microliths in the alveoli after mutations in the SLC34A2 gene. The patients may be asymptomatic or present with heterogeneous signs such as dyspnea, cough, sputum, weakness, chest pain and hemoptysis. Recurrent pneumothoraces may occur over the course of the disease in advanced cases. Her, it was aimed to report a case of a 40-year-old female presenting with chest pain, sore throat, cough and green sputum for the past two months. Chest radiography and computed tomography showed pneumomediastinum and subcutaneous emphysema which have not been reported in previous studies due to PAM. Bronchoscopy with bronchoalveolar lavage (BAL) and mucosal biopsy were done after pneumomediastinum had cleared. BAL result was concordant with pulmonary alveolar microlithiasis. Diagnosis can be made with computed tomography but may necessitate histopathological verification for definitive diagnosis.

There are limited data about the coexistence of asthma and the Coronavirus disease-2019 (COVID-19). The COVID-19 pandemic is a new disease for the whole world. In this study, we aimed to examine the published case series with the COVID-19 and asthma coexistence, by reviewing existing studies and other publications. We discussed the suggestions of guidelines such as The Global Initiative for Asthma (GINA), The National Institute for Health and Care Excellence (NICE), and the Canadian Respiratory Guideline about the management of asthma during the COVID-19 pandemic and the experiences of different countries. We analyzed the publications, reports, and expert opinions about asthma and COVID-19, that were released and expressed from the onset of the disease in Wuhan. In this review, we aimed to summarize the approach to patients with asthma during the pandemic and to make recommendations concerning it.

Introduction: The aim of our study is to investigate the etiological distribution of ILD in Turke... more Introduction: The aim of our study is to investigate the etiological distribution of ILD in Turkey by stratifying the epidemiological characteristics of ILD cases, and the direct cost of initial diagnosis of the diagnosed patients. Material-Method: The study was conducted as a multicenter, prospective, cross-sectional, clinical observation study. Patients over the age of 18 and who accepted to participate to the study were included and evaluated as considered to be ILD. The findings of diagnosis, examination and treatment carried out by the centers in accordance with routine diagnostic procedures were recorded observationally. Results: In total,1070 patients were included in this study. 567 (53%) of the patients were male and 503 (47%) were female. The most frequently diagnosed disease was IPF (30.5%). Dyspnea (75.9%) was the highest incidence among the presenting symptoms. Physical examination found bibasilar inspiratory crackles in 56.2 % and radiological findings included reticular opacities and interlobular septal thickenings in 55.9 % of the cases. It was observed that clinical and radiological findings were used most frequently (74.9%) as a diagnostic tool. While the most common treatment approaches were the use of systemic steroids and antifibrotic drugs with a rate of 30.7% and 85.6%, respectively. The total median cost from the patient’s admission to diagnosis was 540 Turkish Lira. Conclusion: We believe that our findings compared with data from other countries will be useful in showing the current situation of ILD in our country to discuss this problem and making plans for a solution.
