Spatial health inequalities have often been analyzed in terms of socioeconomic and environmental ... more Spatial health inequalities have often been analyzed in terms of socioeconomic and environmental factors. The present study aimed to evaluate spatial relationships between spatial data collected at different spatial scales. The approach was illustrated using health outcomes (mortality attributable to cancer) initially aggregated to the county level, district socioeconomic covariates, and exposure data modeled on a regular grid. Geographically weighted regression (GWR) was used to quantify spatial relationships. The strongest associations were found when low deprivation was associated with lower lip, oral cavity and pharynx cancer mortality and when low environmental pollution was associated with low pleural cancer mortality. However, applying this approach to other areas or to other causes of death or with other indicators requires continuous exploratory analysis to assess the role of the modifiable areal unit problem (MAUP) and downscaling the health data on the study of the relationship, which will allow decision-makers to develop interventions where they are most needed.

ABSTRACT On s'intéresse ici au rôle des facteurs environnementaux et socio-économiques su... more ABSTRACT On s'intéresse ici au rôle des facteurs environnementaux et socio-économiques sur l'épidémie de cancer par des approches spatiales à l'aide des systèmes d'informations géographiques. Les objectifs sont d'analyser, au niveau cantonal, la relation entre cancer et exposition environnementale dans la région de Picardie, ainsi que son évolution dans le temps. Les données d'environnement sont celles qui ont déjà été acquises, lors de la 1ère étape du projet CIRCE (base PLAINE) qui a permis la transformation des principales bases de données environnementales(air, eau, sol, alimentation)en base de données spatialisées et agrégées pour 4 polluants métalliques (Pb, Cr, Ni, Co).Le risque relatif de mortalité par cancer par canton a pu être défini à partir des données du CépiDc de l'Inserm, pour deux périodes 1990-1999 et 2000-2009.Pour analyser l'association entre les indicateurs d'exposition et le risque de mortalité par cancer, un modèle de régression écologique(modèle hiérarchique bayésien) qui permet d'évaluer, sur des données agrégées à l'échelle des cantons, la part des variations spatiales de mortalité aux variations des indices d'exposition, a été utilisé. Le croisement de ces données spatialisées par canton nous a permis de mettre en évidence certaines corrélations par exemple, le risque de mortalité par cancer de la plèvre augmente avec le niveau d'exposition au nickel pour les deux périodes étudiées comme le montrent les résultats par quintiles:1.36[0.73-1.99],1.44[0.90-2.77],1.82[1.26-4.09],1.79[1.23-3.95] pour la période 1990-1999 et 1.09[0.86-1.72],1.57[1.08-3.06],1.55[0.94-3.18],1.83[1.03-4.09] pour la période 2000-2009.Des résultats complémentaires seront présentés, notamment ceux obtenus à l'aide de la base de données Corinne Land Cover. Bien que cette approche comporte des limites comme la non prise en compte de la mobilité à court et long terme des populations ou encore les temps de latence relativement longs pour le cancer, les études de corrélations permettent d'éclairer la complexité du lien environnement, social et santé.

A cross-sectional survey was distributed to 255 participants randomly selected from 1,021 second- to sixth-year medical students. Questionnaires were self-administered and included questions on sociodemographic characteristics, mental health, and alcohol (The Alcohol Use Disorders Identification Test [AUDIT test]), tobacco (Fagerstrom test), and illegal substance consumption (Cannabis Abuse Screening Test [CAST test]). The AUDIT scores indicated that 11% of the study participants were at risk for addiction and 21% were high-risk users. Tobacco dependence was strong or very strong for 12% of the participants. The CAST score showed that 5% of cannabis users needed health care services. Cannabis users were also more likely than non-users to fail their medical school examinations (89% vs. 39%, p < .01). We reviewed a series of 32 cases of recurrent hallux valgus treated by great toe metatarsophalang... more We reviewed a series of 32 cases of recurrent hallux valgus treated by great toe metatarsophalangeal arthrodesis with a minimal 5-year follow-up. Mean delay from the first surgical procedure and revision surgery was 11 years. All patients complained of forefoot pain. The average angle of the phalangeal valgus was 39 degrees. Sixteen patients had metatarsalgia. The first toe metatarsophalangeal joint was evaluated according to Regnauld's classification: two grade 1, eight grade 2a, six grade 2b, sixteen grade 3. Arthrodesis was fixed with an axial screw and associated with adductor hallux plasty. Outcome was assessed at a minimum 5-year follow-up according to Kitaoka's criteria. Seventy-eight percent of the patients were pain free at last follow-up. Valgus deviation of the great toe was corrected with an average angle of 19 degrees. Although the oral cavity is a potentially accessible site for examination, up to 50% of oral cancers are not detected until the disease is well advanced. In a region where incidence rate is particularly high, local teams involved in screening, in epidemiological survey, in diagnosis and treatment of oral cancer performed a pilot feasibility study to improve strategy of early detection of oral cancer and premalignant lesion. Tobacco venders were solicited to distribute a flyer, which invite smokers to a free examination by general practitioner. General practitioners were invited to examine smokers, and to fill a predeterminate systematic oral cavity examination record during 3 months. Breast cancer is the commonest female cancer in the Somme region and represents the leading cause... more Breast cancer is the commonest female cancer in the Somme region and represents the leading cause of cancer mortality in women. It therefore constitutes an important public health problem. From 1990 to 1993, 1,106 new cases of breast cancer were recorded by the Somme Cancer Registry. The incidence continued to increase over this period in the Somme region. The mortality rate for this period was 35.1/100,000 women, while the standardized mortality rate for the world population was 20.9/100,000 women. The most frequent histological types were invasive ductal carcinoma (64.3%) and invasive lobular carcinoma (5%). Carcinoma in situ represented 2.9% cases; 4% of patients presented metastases at the time of diagnosis. For the period 1990-1993, 44.3% cases were classified as T1, 37.9% as T2 and 11.5% as T3-T4. The male/female sex-ratio was 1 to 3. The average age was 13.2 years old. Hospitalisations were more frequent in the spring and were predominantly on Mondays (17%). More than 2/3 of the adolescents were accompanied by their parents and 20% were referred by their doctors or by the SAMU (mobile emergency medical service). Two percent arrive alone. The French hospital reform requires the development of different activities indicators which may ... more The French hospital reform requires the development of different activities indicators which may be utilized by hospitals in order to negotiate reorganizations and redistributions. The French-DRG's classification is a useful tool in this type of negotiation as it allows the description of an activity and the evaluation of its budget. Nevertheless, it is insufficient in taking into account high-cost patients. The clinical characterization of high-cost subgroups completes the DRG description. We studied the length of stays of 801 discharge reports of the 418 French DRG "diabetes, age > 36 years" of the Amiens University Hospital for 1996. It showed that this DRG is made up of 3 subgroups, of which the most costly gathers 14% of all patients. Criteria which were significantly linked with this third subgroup were: the number of comorbidities by RSS and the number of diabetes complications by RSS.

Randomized clinical trials have demonstrated that screening by mammography can reduce breast canc... more Randomized clinical trials have demonstrated that screening by mammography can reduce breast cancer mortality by 30% in women aged 50 to 69 years. In France, pilot screening programs were begun in 1989 and particularly in the Somme district. After 6 years and two rounds, the aim of this study was to assess the quality and the effectiveness of this screening program. The quality of the campaign was measured concerning effectiveness (recall rate, positive predictive value of screening test or of the screening procedure, intervention rate, breast cancer detection rate) or quality (percentage of in situ cancers, invasive cancers smaller than 10 mm, invasive cancers without lymph node invasion). These indicators were compared with European targets. Attendance rates at 6 years ranged from 33.6% to 37.1%. To test the hypothesis that osteoporosis, which results partly from long-term estrogen deficiency... more To test the hypothesis that osteoporosis, which results partly from long-term estrogen deficiency, is associated with a lowered risk of breast cancer, a population-based cohort study was performed in Amiens. To ascertain the incidence of breast cancer, 1300 women were followed through after a first hip fracture. Overall, 18 cases of cancer were observed cf. 21.8 expected (standardized incidence ratio (SIR): 0.82; 95% confidence interval (CI): 0.5-1.3). The results are consistent with previous studies which concluded that long-term estrogen deficiency is associated with a reduced risk of developing breast cancer.

We reviewed a series of 32 cases of recurrent hallux valgus treated by great toe metatarsophalang... more We reviewed a series of 32 cases of recurrent hallux valgus treated by great toe metatarsophalangeal arthrodesis with a minimal 5-year follow-up. Mean delay from the first surgical procedure and revision surgery was 11 years. All patients complained of forefoot pain. The average angle of the phalangeal valgus was 39 degrees. Sixteen patients had metatarsalgia. The first toe metatarsophalangeal joint was evaluated according to Regnauld's classification: two grade 1, eight grade 2a, six grade 2b, sixteen grade 3. Arthrodesis was fixed with an axial screw and associated with adductor hallux plasty. Outcome was assessed at a minimum 5-year follow-up according to Kitaoka's criteria. Seventy-eight percent of the patients were pain free at last follow-up. Valgus deviation of the great toe was corrected with an average angle of 19 degrees. Prognosis of this cancer is closely linked to the development. Although the oral cavity is a potentially accessible site for examination, up to 50% of oral cancers are not detected until the disease is well advanced. In a region where incidence rate is particularly high, local teams involved in screening, in epidemiological survey, in diagnosis and treatment of oral cancer performed a pilot feasibility study to improve strategy of early detection of oral cancer and premalignant lesion. Tobacco venders were solicited to distribute a flyer, which invite smokers to a free examination by general practitioner. General practitioners were invited to examine smokers, and to fill a predeterminate systematic oral cavity examination record during 3 months. Breast cancer is the commonest female cancer in the Somme region and represents the leading cause... more Breast cancer is the commonest female cancer in the Somme region and represents the leading cause of cancer mortality in women. It therefore constitutes an important public health problem. From 1990 to 1993, 1,106 new cases of breast cancer were recorded by the Somme Cancer Registry. The incidence continued to increase over this period in the Somme region. The mortality rate for this period was 35.1/100,000 women, while the standardized mortality rate for the world population was 20.9/100,000 women. The most frequent histological types were invasive ductal carcinoma (64.3%) and invasive lobular carcinoma (5%). Carcinoma in situ represented 2.9% cases; 4% of patients presented metastases at the time of diagnosis. For the period 1990-1993, 44.3% cases were classified as T1, 37.9% as T2 and 11.5% as T3-T4. A retrospective survey concerning all patients ages 12-19 who had registered themselves at the ad... more A retrospective survey concerning all patients ages 12-19 who had registered themselves at the admitting service of the paediatric emergency unit in the university hospital centre of Amiens was carried out in 1995. 2812 adolescents were received (18% of total admissions). The male/female sex-ratio was 1 to 3. The average age was 13.2 years old. Hospitalisations were more frequent in the spring and were predominantly on Mondays (17%). More than 2/3 of the adolescents were accompanied by their parents and 20% were referred by their doctors or by the SAMU (mobile emergency medical service). Two percent arrive alone. Traumatic pathology is the main reason for admission (1960 patients). Organic pathologies represent 24% of admissions and are essentially digestive pathologies (40%). The French hospital reform requires the development of different activities indicators which may ... more The French hospital reform requires the development of different activities indicators which may be utilized by hospitals in order to negotiate reorganizations and redistributions. The French-DRG's classification is a useful tool in this type of negotiation as it allows the description of an activity and the evaluation of its budget. Nevertheless, it is insufficient in taking into account high-cost patients. The clinical characterization of high-cost subgroups completes the DRG description. We studied the length of stays of 801 discharge reports of the 418 French DRG "diabetes, age > 36 years" of the Amiens University Hospital for 1996. It showed that this DRG is made up of 3 subgroups, of which the most costly gathers 14% of all patients. Criteria which were significantly linked with this third subgroup were: the number of comorbidities by RSS and the number of diabetes complications by RSS.

Randomized clinical trials have demonstrated that screening by mammography can reduce breast canc... more Randomized clinical trials have demonstrated that screening by mammography can reduce breast cancer mortality by 30% in women aged 50 to 69 years. In France, pilot screening programs were begun in 1989 and particularly in the Somme district. After 6 years and two rounds, the aim of this study was to assess the quality and the effectiveness of this screening program. The quality of the campaign was measured concerning effectiveness (recall rate, positive predictive value of screening test or of the screening procedure, intervention rate, breast cancer detection rate) or quality (percentage of in situ cancers, invasive cancers smaller than 10 mm, invasive cancers without lymph node invasion). These indicators were compared with European targets. Attendance rates at 6 years ranged from 33.6% to 37.1%. The recall rate was 14.3% in the first and 7.8% in the second round, 6.4 and 6 cancers were detected per 1,000 women screened during the first and second round, and 38.5% of invasive canc...

To test the hypothesis that osteoporosis, which results partly from long-term estrogen deficiency... more To test the hypothesis that osteoporosis, which results partly from long-term estrogen deficiency, is associated with a lowered risk of breast cancer, a population-based cohort study was performed in Amiens. To ascertain the incidence of breast cancer, 1300 women were followed through after a first hip fracture. Overall, 18 cases of cancer were observed cf. 21.8 expected (standardized incidence ratio (SIR): 0.82; 95% confidence interval (CI): 0.5-1.3). The results are consistent with previous studies which concluded that long-term estrogen deficiency is associated with a reduced risk of developing breast cancer.
