Apresenta-se o caso de criança do sexo masculino com síndrome de Russel devida a astrocitoma pilo... more Apresenta-se o caso de criança do sexo masculino com síndrome de Russel devida a astrocitoma pilocítico localizado na região diencefálica. O diagnóstico foi realizado aos 16 meses de idade, mas sua sintomatologia se iniciou aos 4 meses, com emagrecimento progressivo. À internação apresentava-se com peso de 6150g e 74cm de estatura, emaciada, sem panículo adiposo, irritadica e com sinais de hipertensão intracraniana. Existia estrabismo convergente, nistagmo vertical do olho esquerdo e papiledema bilateral. Os reflexos tendinosos eram exaltados e tinha tetraparesia espástica. A avaliação endócrina demonstrou aumento basal do GH (23 ng/ml), do TSH (6,2 mUI/l) e da prolactina (26 ng/ml). Os dois primeiros hormônios não responderam ao teste agudo com o TRH, enquanto a prolactina teve resposta pobre. The leakage of sensitive information is a pressing problem when information is processed digitall... more The leakage of sensitive information is a pressing problem when information is processed digitally due to the economic, political and social repercussions that it can cause to its owner. Despite the risks and possible threats, the information must always be kept available to users, therefore, alternatives must be available to protect, detect, and prevent the leakage of sensitive information. A particular case of this problem is the leakage of sensitive textual documents. However, the identification of unstructured sensitive information is a problem whose solution is not totally satisfactory despite the development of methods and applications with promising results. Thus, it is necessary to continue developing methods that contribute to the effective solution of the problem based on a critical analysis of existing techniques and their future projections. In this work we start from a taxonomy of the approaches with which this problem has been approached. From the taxonomy, the critical analysis of the techniques and above all considering the practical needs, a method of solution to the problem of determining the sensitivity of textual documents is proposed from the perspective of Logical Combinatorial Patterns Recognition. The problem is approached as a supervised classification problem with two classes: sensitive and non-sensitive textual documents. The proposal in this work is the STClass method to determine the sensitivity of documents, which consists of two phases: the training phase, where the parameters for classification are defined and the classification phase. With the datasets used, 96% of the well classified documents were reached.

Early diagnosis of persons infected with human immunodeficiency virus (HIV) through diagnostic te... more Early diagnosis of persons infected with human immunodeficiency virus (HIV) through diagnostic testing and screening is a critical priority for individual and public health. Emergency departments (EDs) have an important role in this effort. As EDs gain experience in HIV testing, it is increasingly apparent that implementing testing is conceptually and operationally complex. A wide variety of HIV testing practice and research models have emerged, each reflecting adaptations to site-specific factors and the needs of local populations. The diversity and complexity inherent in nascent ED HIV testing practice and research are associated with the risk that findings will not be described according to a common lexicon. This article presents a comprehensive set of terms and definitions that can be used to describe ED-based HIV testing programs, developed by consensus opinion from the inaugural meeting of the National ED HIV Testing Consortium. These definitions are designed to facilitate discussion, increase comparability of future reports, and potentially accelerate wider implementation of ED HIV testing.

Older cancer survivors may be a unique population. We evaluated whether older cancer survivors (age ≥ 65 years) had lifestyle behaviors, attitudes, and knowledge distinct from younger survivors. Adult cancer survivors with diverse cancer subtypes were recruited from Princess Margaret Cancer Centre (Toronto, Ontario, Canada). Multivariable models evaluated the effect of age on smoking, alcohol, and physical activity habits, attitudes toward and knowledge of these habits on cancer outcomes, and lifestyle information and recommendations received from health care providers, adjusted for sociodemographic and clinicopathologic covariates. Introduction: Biochemical recurrence (BCR) is a major clinical challenge in patients with Prostat... more Introduction: Biochemical recurrence (BCR) is a major clinical challenge in patients with Prostate Cancer (PCa), as rising PSA occurs in about 20-30% of patients treated with radical prostatectomy and up to 60% in patients treated primarily with radiotherapy. Patients may have local recurrence in the pelvis or in distant sites. The location and burden of recurrent disease is mandatory in guiding subsequent therapies. Standard practice for BCR in PCa includes computed tomography and bone scintigraphy, both known for limited diagnostic performance. The use of positron emission tomography (PET) with prostate-specific membrane antigen (PSMA) was introduced in clinical practice in the past few years due to its better accuracy. The present phase 3, prospective, multicentric, international study was planned to evaluate the diagnostic performance and clinical impact of [68Ga]Ga-PSMA-11 PET/CT in evaluating patients with biochemical recurrent PCa; the study was promoted and supported by the IAEA. Methods: Patients with PCa who have undergone primary definitive treatment and presented with rising PSA levels were recruited to the study. Overall 17 centers from 15 countries (Azerbaijan, Brazil, Colombia, India, Israel, Italy, Jordan, Lebanon, Malaysia, Mexico, Pakistan, Poland, South Africa, Turkey, and Uruguay) were involved. Images and data were centrally reviewed; data were collected and assessed for site of findings ([68Ga]Ga-PSMA-11 pathologic uptake), positivity rate (percentage of patients with a positive [68Ga]Ga-PSMA-11 PET/CT) and impact on patient management. Regarding PET findings, a composite reference standard was used, including pathology, correlative imaging, PSA response; regarding management, the reference included changes in the treating physician’s documented clinical plans before and after PET; all patients have at least 6 months clinical follow-up. Results: 1198 patients were prospectively enrolled between November 2017 and December 2019; 1004 patients had complete data for final evaluation. [68Ga]Ga-PSMA-11 PET/CT was positive in 654/1004 patients (65.1%); lesions were identified as: prostate/prostatic bed only in 13.7% of the cases; pelvic lymph nodes only in 20.5%, and with any metastatic disease in 27.0%. There was a correlation between [68Ga]Ga-PSMA-11 PET/CT positivity and Gleason score (p<0.001): detection rate was 371/613 (60.5%) in patients with Gleason 7, 130/196 (66.3%) in Gleason 8, 140/180 (77.8%) in Gleason 9 and 13/15 (86.7%) in Gleason 10. There was also statistically significant correlation between lesions identified by [68Ga]Ga-PSMA-11 PET/CT and PSA values (p<0.001): detection rate was 21/41 (51.2%) for PSA <0.2, 84/188 (44.7%) for PSA between 0.2-0.5, 124/232 (53.4%) for PSA 0.5-1.0, 158/235 (67.2%) for PSA ≥1 and <2, 171/206 (83.0%) for PSA ≥2 and <4, and 96/102 (94,1%) in PSA 4 to 10. Finally, treatment was modified based on PET PSMA results in 56.8% of patients. The [68Ga]Ga-PSMA-11 PET/CT positivity rate was consistent and not statistically different among the different countries. Conclusion: This is the largest multicenter international prospective trial on [68Ga]Ga-PSMA-11 PET/CT, and it confirmed the capability of [68Ga]Ga-PSMA-11 PET /CT to detect local and metastatic recurrence in most men with BCR. Based on imaging results, most men had a change in treatment approach. This study demonstrates the reliability of [68Ga]Ga-PSMA-11 PET/CT in PCa BCR, and the worldwide feasibility of such imaging approach. The high morbidity and mortality of patients with esophageal (E) and gastro-esophageal junction (... more The high morbidity and mortality of patients with esophageal (E) and gastro-esophageal junction (GEJ) cancers, warrants new pre-clinical models for drug testing. The utility of primary tumor xenografts (PTXGs) as pre-clinical models was assessed. Clinicopathological, immunohistochemical markers (p53, p16, Ki-67, Her-2/neu and EGFR), and global mRNA abundance profiles were evaluated to determine selection biases of samples implanted or engrafted, compared with the underlying population. Nine primary E/GEJ adenocarcinoma xenograft lines were further characterized for the spectrum and stability of gene/protein expression over passages. Seven primary esophageal adenocarcinoma xenograft lines were treated with individual or combination chemotherapy. Tumors that were implanted (n=55) in NOD/SCID mice had features suggestive of more aggressive biology than tumors that were never implanted (n=32).
