Objectives: We evaluated the factors that have considerable significance on the outcome of open c... more Objectives: We evaluated the factors that have considerable significance on the outcome of open cavity procedures in the treatment of cholesteatoma. Patients and methods: The study included 66 patients (43 males, 23 females; mean age 47 years; range 21 to 69 years) who underwent surgery for cholesteatoma. Of these, 56 patients completed at least a five-year follow-up. Results: Mucosal infections and granulation tissue formation occurred in seven patients (10%) in the early postoperative period. Retraction pockets developed in three patients (4%); of these, only one patient required excision because of deep localization. Drum perforations that occurred in two patients (3%) were repaired by myringoplasty. Revision surgery was performed in four patients (7.1%) due to residual cholesteatoma within a five-year follow-up. Conclusion: The height of the facial ridge was found as the most important factor related to a successful outcome of open-technique procedures. Other factors included the creation of a smooth mastoid cavity with round edges, removal of all diseased mastoid cells, and an extensive conchameatoplasty.

There were three (10%) confirmed cases of secretory otitis media. One child had previous surgery for polyps. Ten children had chronic pansinusitis mildly responsive to medical therapy. Hypertrophy of the nasal conchae was detected in eight cases. The data showed that the risk of ear-nose-and-throat disease was increased in CF patients, but audiologic problems are not very common in CF patients. CF should be kept in mind in children who have nasal polyps, hypertrophied turbinates or sinusitis unresponsive to medical therapy.

Agenesis of the patient's left middle turbinate was detected. Coronal computed tomography images showed a septal spur replacing the absent left middle turbinate.Conclusion:The lateral nasal wall has complex anatomy, and several anatomical variations have been reported. The most common anatomical variation of the middle nasal turbinate is concha bullosa. Unilateral agenesis of the middle nasal turbinate has not previously been reported; the presented patient represents the first reported case.

This study was conducted on 34 thalassemic patients of which 27 of them had thalassaemia major and the remaining 7 had thalassaemia intermedia. Six (11.11%) of the 54 ears with thalassaemia major were found to have normal hearing. Thirty-two ears (59.26%) had conductive hearing loss, 8 ears (14.81%) had mixed type of hearing loss. Moreover, in this group, there was no ear which had pure sensorineural hearing loss. Of the majority of patients having thalassaemia major and thalassaemia intermedia, an air bone gap was found even though there was no negative pressure in the middle ear. In these patients a high degree of static compliance and normal shaped, stiff amplitude, normal pressure tympanograms were observed. Moreover, in most of these patients acoustic reflexes were not obtained. These findings are rather interesting since they show the pathological changes which may cause stiffness in the middle ear sound transmission system.

Thirty patients followed up because of kernicterus sequelae and 15 normal children serving as control group were examined to determine their hearing status and evoked response audiometry (ERA) results. Seventeen out of the 30 patients (57%) had high frequency sensorineural hearing loss. Sixteen of them had bilateral hearing loss. Ten had no response or prolonged bilateral latencies. The results showed that high neonatal serum bilirubin levels affect the hearing adversely.

The diagnosis is thus made too late, with resulting complications such as irreversible hearing loss and facial nerve paralysis. A case report and review of the literature are presented, emphasizing that tuberculosis should be considered in the differential diagnosis of chronic ear infection.

Among paranasal sinus infections associated with aspergillosis, the most common subtype is Aspergillus fumigatus. A 41-year-old woman presented with facial pain. She had a five-year history of root canal filling in the right upper molar teeth. Paranasal sinus tomography showed fungal sinusitis of radiopaque appearance, giving the impression of a foreign body in the maxillary sinus. The patient underwent endoscopic sinus surgery to purge infection of the maxillary sinus. Pathological examination revealed aspergillosis of the maxillary sinus.

Developmental hemi-thyroid anomalies can result from either an abnormal descent or an agenesis of one lobe of the thyroid gland. This study aimed at presenting a thyroid hemiagenesis case incidentally diagnosed by neck ultrasonography (USG), who had complaints of pain and swelling in the neck. USG examination revealed lack of left thyroid lobe and multiple nodules in the right lobe. Fine Needle Aspiration Biopsy (FNAB) showed follicular neoplasia, and right subtotal thyroidectomy was performed. We report the rarity of the condition and emphasize the role of imaging techniques in preoperative diagnosis and subsequent management.

The value of vestibular rehabilitation in patients with bilateral vestibular dysfunction was inve... more The value of vestibular rehabilitation in patients with bilateral vestibular dysfunction was investigated. MATERIALS and METHODS: This study assessed 17 patients (9 males, 8 females) with bilateral vestibular dysfunction. Vestibular rehabilitation continued for 1.5 months. Videonystagmography tests (including oculomotor testing, positional testing, and caloric tests), vestibular evoked myogenic potential (VEMP) testing, and computerized dynamic posturography were performed during the pre-, mid-, and post-treatment periods. The patients underwent cranial and internal acoustic canal MRI. Consultant physicians from the neurology and physical medicine and rehabilitation departments reviewed all patients. RESULTS: The post-treatment anteroposterior somatosensorial (APSO), anteroposterior global (APGLO), mediolateral visual (MLVI), and mediolateral global values and anteroposterior and mediolateral trials and conditions were significantly higher than those measured in the pre-treatment period. Similarly, mid-treatment values of the APSO, APGLO, and the anteroposterior sensory organization test (SOT) 2 were significantly higher than those measured in the pre-treatment period. CONCLUSION: Vestibular rehabilitation was effective in patients with bilateral vestibular dysfunction. As the vestibular rehabilitation duration increased, so did the efficacy of the treatment.

Akut ve sıklıkla yıkıcı bir komplikasyon ortaya çıkana kadar kolaylıkla saptanamazlar. Torasik aort anevrizmalı hastalar nonkardiyak bir cerrahi operasyon geçirmek zorunda kalabilirler ve bazen anevrizma, hastamızda olduğu gibi preoperatif değerlendirme sırasında tanınabilir. Bu olguda nonkardiyak cerrahi geçiren asendan aort anevrizmalı bir hastadaki anestezi uygulamamızı sunmayı amaçladık.

Stevens–Johnson Syndrome associated with Ramipril Dear Sir, An 80-year-old man was admitted to ou... more Stevens–Johnson Syndrome associated with Ramipril Dear Sir, An 80-year-old man was admitted to our hospital with a 10-day history of malaise and pruritic skin lesions. He complained of general fatigue, appetite loss, and oral lesions interfering with his oral nutrition. He had no known allergies to medication. He had a long history of diabetes mellitus, for which he was receiving therapy with gliclazide 150 mg once a day. He had also been suffering from hypertension for years, and one month before the eruption his antihypertensive drug regime was ceased and substituted with ramipril 5 mg daily. Dermatological examination revealed a macular and purpuric rash with erythematous, target-like lesions and multiple vesicles and bullae on the trunk, legs and arms (Fig. 1). There were also hemorrhagic bullous lesions symmetrically on his hands and soles, and blisters and erosions on his external genitalia. Oropharynx examination showed ulcers and vesicles on the hard and soft palate and epiglottis. His temperature was 38.3 ° C. The physical examination revealed no abnormality. On serial sections of the skin biopsy material, hyperkeratosis and slight acanthosis was observed. The epidermis showed subepidermal detachment from the underlying dermis to form a bullous cavity. The cavity contained fibrinoid material, erythrocytes, neutrophils, and a few eosinophils (Fig. 2). A mixed inflammatory infiltrate was also present in the dermis. Direct immunofluorescence testing was performed using IgA, IgG, IgM and C3, all of which were negative. Laboratory examinations, including complete blood count, liver and kidney function tests, electrolytes, erythrocyte sedimentation rate, urine analysis, and chest radiography, were all within normal limits. Serology for herpesvirus I and II and mycoplasma pneumonia was negative. On the basis of these findings, the diagnosis of ramiprilinduced Stevens–Johnson Syndrome (SJS) was established and ramipril was discontinued and replaced by another class of antihypertensive drug. His condition progressively resolved within 10 days after discontinuation of ramipril therapy. Appearance of the disease one month after the commencement of ramipril therapy and the lack of other etiologic factors associated with SJS indicated ramipril as the most suspicious cause of the disease. The oral rechallenge test was not clinically appropriate because of the severity of his eruptions. However, the patient developed similar eruptions following an accidental rechallenge by himself 2 weeks following cessation of ramipril, which confirmed the diagnosis of ramipril-induced SJS.

The independent existence of this entity is generally doubted. Recently, some radiological and histological clues to this disease have been reported. Although the exact cause of carotidynia is unknown it is thought that this entity is a self-limiting inflammatory process and may be a subset of vasculitis. Treatment is conservative and anti-inflammatory drugs are effective. In this paper, we report three carotidynia patients with similar clinical features but different radiological findings.

The disease is common throughout the world, and the most common clinical form is generalized or localized lymphadenopathy. However, in the literature, toxoplasmosis manifesting as a parotid mass is extremely rare. A 37-year-old female was admitted to our department with a painless mass in the parotid region, which had been present for one month. Upon physical examination, a hard, mobile, superficially located, and painless parotid mass with a size of 2x2 cm was detected. Following the diagnosis of a parotid neoplasm, a superficial parotidectomy was performed. Histopathological examination of the surgical specimen supported a diagnosis of toxoplasmosis lymphadenitis. Their ages ranged between 2 to 17 years and they were studied by means of complete audiologic evaluation and brainstem evoked response audiometry (BERA). Twenty of them had received some ototoxic drugs for variable periods. The results showed that there was no significant correlation between CF and BERA findings. The findings indicated that, except a few middle ear problems such as serous otitis media, CF did not affect hearing status of the CF children per se.

Developmental hemi-thyroid anomalies can result from either an abnormal descent or an agenesis of one lobe of the thyroid gland. This study aimed at presenting a thyroid hemiagenesis case incidentally diagnosed by neck ultrasonography (USG), who had complaints of pain and swelling in the neck. USG examination revealed lack of left thyroid lobe and multiple nodules in the right lobe. Fine Needle Aspiration Biopsy (FNAB) showed follicular neoplasia, and right subtotal thyroidectomy was performed. We report the rarity of the condition and emphasize the role of imaging techniques in preoperative diagnosis and subsequent management.

MATERIALS and METHODS: This study assessed 17 patients (9 males, 8 females) with bilateral vestibular dysfunction. Vestibular rehabilitation continued for 1.5 months. Videonystagmography tests (including oculomotor testing, positional testing, and caloric tests), vestibular evoked myogenic potential (VEMP) testing, and computerized dynamic posturography were performed during the pre-, mid-, and post-treatment periods. The patients underwent cranial and internal acoustic canal MRI. Consultant physicians from the neurology and physical medicine and rehabilitation departments reviewed all patients. RESULTS: The post-treatment anteroposterior somatosensorial (APSO), anteroposterior global (APGLO), mediolateral visual (MLVI), and mediolateral global values and anteroposterior and mediolateral trials and conditions were significantly higher than those measured in the pre-treatment period. Similarly, mid-treatment values of the APSO, APGLO, and the anteroposterior sensory organization test (SOT) 2 were significantly higher than those measured in the pre-treatment period. CONCLUSION: Vestibular rehabilitation was effective in patients with bilateral vestibular dysfunction. As the vestibular rehabilitation duration increased, so did the efficacy of the treatment.

Akut ve sıklıkla yıkıcı bir komplikasyon ortaya çıkana kadar kolaylıkla saptanamazlar. Torasik aort anevrizmalı hastalar nonkardiyak bir cerrahi operasyon geçirmek zorunda kalabilirler ve bazen anevrizma, hastamızda olduğu gibi preoperatif değerlendirme sırasında tanınabilir. Bu olguda nonkardiyak cerrahi geçiren asendan aort anevrizmalı bir hastadaki anestezi uygulamamızı sunmayı amaçladık.

Stevens–Johnson Syndrome associated with Ramipril Dear Sir, An 80-year-old man was admitted to ou... more Stevens–Johnson Syndrome associated with Ramipril Dear Sir, An 80-year-old man was admitted to our hospital with a 10-day history of malaise and pruritic skin lesions. He complained of general fatigue, appetite loss, and oral lesions interfering with his oral nutrition. He had no known allergies to medication. He had a long history of diabetes mellitus, for which he was receiving therapy with gliclazide 150 mg once a day. He had also been suffering from hypertension for years, and one month before the eruption his antihypertensive drug regime was ceased and substituted with ramipril 5 mg daily. Dermatological examination revealed a macular and purpuric rash with erythematous, target-like lesions and multiple vesicles and bullae on the trunk, legs and arms (Fig. 1). There were also hemorrhagic bullous lesions symmetrically on his hands and soles, and blisters and erosions on his external genitalia. Oropharynx examination showed ulcers and vesicles on the hard and soft palate and epiglottis. His temperature was 38.3 ° C. The physical examination revealed no abnormality. On serial sections of the skin biopsy material, hyperkeratosis and slight acanthosis was observed. The epidermis showed subepidermal detachment from the underlying dermis to form a bullous cavity. The cavity contained fibrinoid material, erythrocytes, neutrophils, and a few eosinophils (Fig. 2). A mixed inflammatory infiltrate was also present in the dermis. Direct immunofluorescence testing was performed using IgA, IgG, IgM and C3, all of which were negative. Laboratory examinations, including complete blood count, liver and kidney function tests, electrolytes, erythrocyte sedimentation rate, urine analysis, and chest radiography, were all within normal limits. Serology for herpesvirus I and II and mycoplasma pneumonia was negative. On the basis of these findings, the diagnosis of ramiprilinduced Stevens–Johnson Syndrome (SJS) was established and ramipril was discontinued and replaced by another class of antihypertensive drug. His condition progressively resolved within 10 days after discontinuation of ramipril therapy. Appearance of the disease one month after the commencement of ramipril therapy and the lack of other etiologic factors associated with SJS indicated ramipril as the most suspicious cause of the disease. The oral rechallenge test was not clinically appropriate because of the severity of his eruptions. However, the patient developed similar eruptions following an accidental rechallenge by himself 2 weeks following cessation of ramipril, which confirmed the diagnosis of ramipril-induced SJS.

The independent existence of this entity is generally doubted. Recently, some radiological and histological clues to this disease have been reported. Although the exact cause of carotidynia is unknown it is thought that this entity is a self-limiting inflammatory process and may be a subset of vasculitis. Treatment is conservative and anti-inflammatory drugs are effective. In this paper, we report three carotidynia patients with similar clinical features but different radiological findings.

A 37-year-old female was admitted to our department with a painless mass in the parotid region, which had been present for one month. Upon physical examination, a hard, mobile, superficially located, and painless parotid mass with a size of 2x2 cm was detected. Following the diagnosis of a parotid neoplasm, a superficial parotidectomy was performed. Histopathological examination of the surgical specimen supported a diagnosis of toxoplasmosis lymphadenitis. Their ages ranged between 2 to 17 years and they were studied by means of complete audiologic evaluation and brainstem evoked response audiometry (BERA). Twenty of them had received some ototoxic drugs for variable periods. The results showed that there was no significant correlation between CF and BERA findings. The findings indicated that, except a few middle ear problems such as serous otitis media, CF did not affect hearing status of the CF children per se.
