Research paper thumbnail of Effects of Granulocyte Macrophage Colony Stimulating Factor on Random Flap Healing and Immune Profile in Rats With Impaired Wound Healing by Glucocorticoids

Annals of Plastic Surgery, 2004

Wound healing involves inflammation, cell proliferation, matrix deposition, and tissue remodeling... more Wound healing involves inflammation, cell proliferation, matrix deposition, and tissue remodeling. Priming the tissue to be incised with proinflammatory cytokines offers a new approach to wound healing. This has been studied mostly in incisional wounds till now. This study was designed to investigate the role of granulocyte macrophage colony stimulating factor (GM-CSF) priming of planned incision lines in random flaps in rats with impaired wound healing due to glucocorticoid administration, and immune profiles of rats were also evaluated. GM-CSF priming of incision lines in immunosuppressed rats was shown to improve the wound healing parameters, hydroxyproline levels, breaking strength, flap survival, and immune profile, although all of the parameters were somewhat lower than those of control group. The findings of this study suggest that wound healing can be accelerated and thus morbidity lowered by making use of the immunomodulator effects of GM-CSF in cases where wound healing is impaired due to systemic disease or drug use and flap reconstruction is necessary.

Immunologic pathways which were implicated in psoriasis induce a reduction in the number of melanocytic nevi. To investigate the number of melanocytic nevi in psoriatic patients compared with controls and its relationship with disease severity and type of treatment. We performed a prospective study in 100 psoriatic patients and 100 controls. Clinical data were recorded for all participants. As compared with controls, patients had overall fewer nevi congenital nevi. Among psoriatic patients, biologic agents and disease severity did not correlate with the number of nevi. Psoriatic patients have fewer nevi than controls. Frequency of nevi in psoriatic patients is not related to treatment and disease severity.

Its prevalence is however probably underestimated in dermatological literature. Early recognition of the signs can prevent misdiagnosis. We describe a Turkish patient who presented with palmoplantar pustulosis and involvement of the sternoclavicular joint and peripheral oligoarthritis.

However, confocal microscopes dedicated to eye examination are routinely adopted only in ophthalmology reference centres and do not allow an examination of periocular tissue, nor a fluorescence examination. Methods: We applied for the first time the two in vivo confocal microscopes commonly used in dermatology (VivaScope ® 1500 and 3000, CALIBER, distributed in Europe by Mavig GmbH, Munich, Germany) to observe the cornea, the bulbar and tarsal conjunctiva, the eyelid margin, the lacrimal punctum and the palpebral skin of healthy volunteers. Tumoral, inflammatory and infectious diseases of the ocular mucosa and periocular skin from more than 200 patients were observed under the same microscopes. Both microscopes have a reflectance mode. VivaScope ® 1500 allows an additional fluorescent examination and its placement on the ocular surface was made possible by the creation of a special interface between the microscope and the ocular apparatus. Results: Thanks to its compact and flexible configuration, the handheld camera VivaScope ® 3000 allowed to access more easily to the ocular and periocular tissues. Diagnosis of benign and malignant tumors (melanoma, squamous cell carcinoma and basal cell carcinoma), as well as infectious and genetic disease (storage diseases), could be evocated. The detection of parasites (Demodex folliculorum) on eyelids was possible. Confocal images correlated well with conventional histopathology. The fluorescence examination of corneal squamous cell carcinoma by VivaScope 1500 was characterized by extravasation of fluorescein after intravenous injection. Conclusions: Confocal microscopes dedicated to the skin offer new perspectives for the diagnosis, optimization of treatments, and follow-up of the ocular diseases. They will allow dermatologists to examine conjunctival and eyelid tumors, as it is for skin or genital mucosa. In addition, thanks to some adaptations of the dermatological device VivaScope ® 1500, it is possible for the first time to perform a fluorsecnte examination of the ocular and peri-ocular tissue, opening a new era in the clinical imaging of the ocular surface. A new semiology remains to be learned.

Synchronous or asynchronous trainings have been carried out in some of universities and colleges that have distance education technical infrastructure, during the Covid-19 pandemic. If every student has access to information technologies and the skills of the instructors who will prepare and deliver the training increase their ability to use information technologies, there is no problem in the implementation and maintenance of theoretical lessons. During the Covid-19 pandemic, we had to go to distance education, which we had not yet implemented at Bezmialem Vakıf University Faculty of Medicine. During this period, we applied asynchronous and synchronous education models (mixed model) for theoretical lessons. However, the fact that practical and internship applications were carried out by distance education-although videos about skills and practices were shot and uploaded to the system-it was not possible to replace the formal education. Distance education is inevitable for the continuity of education in epidemic and disaster situations. However, after the epidemic and disaster situations have passed, practical and internship practices should be carried out as much as possible in addition to distance education in medical education. In normal times, distance education can only be used to support formal education in medical education. As a result of all these evaluations and experiences we gained in the Covid 19 pandemic, we think that synchronous/synchronous distance education applications will improve over time and contribute to medical education.

Methods: Four hundred and seventy three women with breast malignancies stage I-III were included. Weight, height, the waist and hip circumferences (WC), body mass index (BMI), and the waist to hip ratio (WHR) were determined. The expression of estrogen (ER) and progesterone (PR) receptors was determined with immunohistochemistry. Results: Most women had central obesity (WC ! 88 and WHR ! 0.85 in 64.3 and 73.4%, respectively). The majority (78.1%) of the women had tumors that expressed at least one of the HR. BMI, WC and WHP were not related to the HR status in neither the pre-or postmenopausal women. Multivariate analysis confirmed these findings. Conclusions: BMI, WC or WHR may not be good predictors of HR status in breast malignancies in either pre-or postmenopausal women.
