Prophylactic therapy response varies in migraine patients. The present study investigated the rel... more Prophylactic therapy response varies in migraine patients. The present study investigated the relationship between the resistance to the drugs commonly used in prophylactic therapy and the possible polymorphic variants of proteins involved in the metabolism of these drugs. Migraine patients with the MDR1 3435TT genotype exhibited a better treatment response to topiramate than migraine patients with the CC and CT genotypes (p=0.020). The MDR1 C3435T polymorphism was also found to be a higher risk factor for topiramate treatment failure in a comparison of the number of days with migraine (β2=1.152, p=0.015). However, there was no significant relationship between the treatment response to topiramate and either the CYP2D6 or CYP2C19 polymorphism, and there were no significant correlations between the treatment responses to amitriptyline, propranolol, and valproic acid and the MDR1, CYP2D6 and CYP2C19 gene polymorphisms. This is the first study to investigate the effect of the polymorphic variants on prophylactic therapy response in migraine patients.

Syphilis; which is a sexually transmitted disease that is caused by the agent Treponema Pallidum ... more Syphilis; which is a sexually transmitted disease that is caused by the agent Treponema Pallidum has been seen less frequently with penicillin therapy. In recent years, with the emergence of HIV infection and the increasing number of patients with immune deficiency, syphilis has started being seen frequently and gained importance. If the patients with primary and secondary syphilis are not treated, approximately 7-9% of them may develop neurosyphilis. Meningovasculary syphilis is characterized with conditions such as hemiparesis, aphasia, visual loss, confusion that are caused by cortical and subcortical infarcts. Thirty-six year old male and 35-year-old male patients presented to our hospital with status epilepticus. We decided to discuss neurosyphilis cases with literature that presented with generalized seizures, mental confusion, hemiparesis and had MRI findings compatible with herpes simplex encephalitis and lesions that disappeared with antibiotic therapy.

It is a rare entity that can occur with different clinics. Superior sagittal sinus is affected most (70-80%), then transverse, sigmoid, and in a lesser extent, cavernous sinus involvement is seen. Cortical and cerebellar veins may also be involved. A 22-year-old male patient presented to the emergency room with mental confusion and seizures. According to the information received from his relatives, he was actively involved in sports for 2 years. That he was using amino acid weighted and high-energy multi-vitamin complexes within a day, and monthly having an injection called Deca-Durabolin: (nandrolone decanoate), is learned. One week before his coming to the hospital, that he had cruciate ligament surgery on his left knee and that spinal anesthesia was administered to him during the operation is determined. Thereafter, it is learnt that he had generalized tonic-clonic seizure and impairment of consciousness was added when he came to the hospital. During the patient's neurologic examination in the emergency service, it is observed that his consciousness tended to fall asleep, and existing of bilateral papilledema and spontaneous movements in the right, and bilateral Babinski was positive. In brain computed tomography and magnetic resonance imaging (MRI) of the patient, in the left temporo-parieto-occipital hemorrhagic infarct and in venous MR angiography, in left transfer sinus, sigmoid sinus and jugular sinus complete thrombus, in right transfer and lateral sinus, reduction in blood stream and a view matched with thrombus were detected. On the 16th day of his hospitalization, brain death was determined in patient. At the arrival of the patient to the clinic, mental confusion, being male, to be accompanied by intracranial hemorrhage shows that poor prognosis.

To demonstrate the effect of glatiramer acetate (GA) in chick embryos on neural tube (NT) develop... more To demonstrate the effect of glatiramer acetate (GA) in chick embryos on neural tube (NT) development, and to explore its effects of Foxp1, apoptosis, and N-cadherin. One hundred fertile, specific pathogen free eggs were divided into 5 groups for this study. The eggshell was windowed specifically at 24 hours of incubation. The embryos in Group 1 (n=20) were treated with 10 μl physiological saline; in Group 2 the embryos (n=20) were given 10 μl GA (equal to daily human therapeutic dose); 20 μl GA (equal to twice daily human therapeutic dose) was injected to embryos in Group 3 (n=20); in Group 4 and 5, 30 μl and 40 μl GA were administered to the embryos (n=20) (equal to x3 and x4 daily human therapeutic dose, respectively). Each egg was re-incubated for 24 hours more. Then, histological and immunohistochemical evaluation of the subjects were done. The embryos with NT defect showed FOXP1 expression without N- cadherin or staining with N-cadherin in another location in our study. We interpreted this result as GA leading to an NT closure defect by increasing FOXP expression. Moreover, we also showed the reverse relation between FOXP1 and N-cadherin at the immunohistochemical level for the first time. GA affects the spinal cord development through FOXP in the chick embryo model at high doses.

Quality of life is the perception of an individual's position in life associated with his... more Quality of life is the perception of an individual's position in life associated with his objectives, expectations, interests, and standard's of life. Health-related quality of life, on the other hand, includes satisfaction with his health and emotional reaction to his state of health. Primary headaches are encountered commonly in adults during their most productive years like end of puberty and at the beginning of 50's. Migraine alone is responsible for 1.3% of years with disability in the world, all headaches together being responsible for twice of this load. Headaches both worsen quality of life of individuals and place a significant burden on the society. This review will focus on the effects of primary headaches, especially migraine, on quality of life and tools used to evaluate these effects.

Excessive yawning may be due to neurological, psychiatric, infectious, gastrointestinal or metabolic diseases. This reflex has also been associated with different selective serotonin reuptake inhibitors. We report a female patient, with excessive yawning, who was on selective serotonin reuptake inhibitor treatment with the diagnosis of generalised anxiety disorder. She was then found to have endometrial carcinoma. Her complaints of palpitation, shortness of breath and loss of energy might be explained by a psychiatric disease and/or anaemia. Previous anaemic periods and partial response of her psychiatric symptoms during last 3 years alerted us to think about an organic cause. Investigations for chronic disease anaemia resulted in diagnosis of endometrial carcinoma. This case is a good example showing misdiagnosis caused by medical stigmatisation.

We investigated the neuroprotective effects of pre-and postconditioning on infarct volume in the ... more We investigated the neuroprotective effects of pre-and postconditioning on infarct volume in the transient middle cerebral artery occlusion (MCAo) model in rats. Thirty-two male rats were divided into occlusion, preconditioning, postconditioning and both pre-and postconditioning groups. MCAo (120 minutes) was monitored with continuous cerebral tissue oxygen (O 2 ) pressure (PtiO 2 ). Pre-conditioning comprised 10 minutes of MCAo, 24 hours prior to the 120 minute MCAo. The postconditioning algorithm was 30 seconds of reperfusion followed by 30 seconds of MCAo. This cycle was repeated 3 times at the onset of reperfusion. Comparison of infarct volumes showed a significant difference between the conditioned groups and occlusion group. Although there was better protection in the preconditioning group compared with the other two conditioned groups, the results did not reach statistically significant levels. The results suggest that preconditioning, postconditioning and pre/post conditioning have protective effects on cerebral ischemia.
