El avance del conocimiento sobre el cometido celular preferente de osteocitos, osteoblastos y osteoclastos en el tejido oseo, de la actividad bioquimica y de la expresion genica, de la interaccion con la actividad fisica, de las medidas y dieta nutricional, asi como de los diversos medicamentos existentes en la actualidad, se exponen en interaccion con los estudios clinicos de etidronato y calcitonina, y paratohormona recombinante 1-84. Se concluye en relacion con su actividad a los dieciocho meses de la administracion y de su valoracion mediante densidad mineral osea, marcadores bioquimicos y produccion de fracturas

Una de las principales innovaciones son los sistemas de educación a distancia, que ofrecen formación continua a estudiantes que por diversas razones no pueden asistir a clases. Como en la enseñanza presencial, en la enseñanza virtual es necesario que los alumnos logren una asimilación efectiva del conocimiento. Sin embargo, frecuentemente tal asimilación no ocurre porque los cursos de e-learning se diseñan sin considerar las características particulares de cada estudiante. Actualmente, los procesos de aprendizaje centrados en el alumno requieren que estos sistemas sean capaces de personalizar la enseñanza a las características y necesidades individuales de cada estudiante.

The perceived pain on injection site caused by subcutaneous (SC) self-injection may negatively af... more The perceived pain on injection site caused by subcutaneous (SC) self-injection may negatively affect acceptance and adherence to treatment in patients with multiple sclerosis (MS). Pain on injection may be caused by inaccurate injection technique, inadequate needle length adjustment, or repeated use of the same injection body area. However, information is lacking concerning the optimal needle depth to minimize the injection pain. The purpose of this program was to characterize the perceived injection-site pain associated with the use of various injection depths of the autoinjector of glatiramer acetate (GA) based on SC tissue thickness (SCT) of the injection site. This was a pilot program performed by MS-specialized nurses in patients with MS new to GA. Patients were trained by MS nurses on the preparation and administration of SC injection and on an eight-site rotation (left and right arms, thighs, abdomen, and upper quadrant of the buttock). The clinical and hemodynamic effects of compassionate therapy with levosimendan were evaluated in... more The clinical and hemodynamic effects of compassionate therapy with levosimendan were evaluated in a 14-year-old patient with end-stage renal disease and arterial hypertension secondary to glycogenosis type Ia. The patient previously had normal heart function but experienced acute development of severe myocardial dysfunction resistant to diuretic therapy and inotropic support. Levosimendan administration was followed by a marked clinical and echocardiographic improvement. The authors believe that levosimendan may be useful for cases of resistant acute heart failure with arterial hypertension. Although administration of levosimendan is not recommended for patients with chronic renal disease, no adverse effect was observed. Therapeutic action for these patients seems to last longer than for patients with a normal kidney.

The normal developmental tasks and roles of adolescence are altered with a diagnosis of a reprodu... more The normal developmental tasks and roles of adolescence are altered with a diagnosis of a reproductive disorder. The crisis of impaired fertility affects both parent and child, stressing the family system. For the adolescent girl, a reproductive disorder impacts her developing sense of self, body-image and sexuality which, in turn, can affect her self-esteem and relationships with others. Because of the sexual nature of a reproductive disorder, feelings of embarrassment or protectiveness are often engendered that can make it difficult for families to discuss. Nonetheless, families do best with openness and honesty regarding the condition and should be discouraged from keeping the diagnosis a secret. Adolescence encompasses a broad spectrum of emotional maturity, which needs to be considered by parents and clinicians when communicating information. Understanding that the family is an emotional unit, a family systems approach to deal with health issues is most appropriate. In this context, parents need to first deal with their own feelings about the diagnosis, before they can help their child. Secondly, parents must be provided with tools to build an ongoing conversation with their child that will avoid stigmatizing her condition and handicapping her growth into healthy adulthood. The goal for parent and clinician is to help the adolescent girl formulate positive selfesteem and body image, despite impaired fertility. Adolescence is a transition time of profound physical, cognitive and psychosocial changes. The maturation from childhood to adulthood is marked by a transformation of mind and body as a girl develops into a young woman, capable of reproduction. For most young women, the metamorphosis of adolescence occurs in a gradual, continuous, and generally uneventful process, despite the normal ups and downs of life. However, when an adolescent and her family are faced with a health crisis impacting her future reproductive capacity, the norm is disrupted and the family must adapt to a new variety of normal with unknowns and uncertainty about the future. To understand the potential psychological impact of a medical condition affecting future fertility in a girl, it is important to first briefly consider the normal stages, goals, and tasks of

Introduction: Endermology is a mechanical massage therapy that enables fat mobilization and body ... more Introduction: Endermology is a mechanical massage therapy that enables fat mobilization and body contouring. The authors' aim was to assess the effect of endermology on indurations and panniculitis/lipoatrophy associated with subcutaneous administration of glatiramer acetate in patients with multiple sclerosis (MS). Methods: This was a multicenter pilot experience carried out in patients with MS treated with glatiramer acetate who showed indurations and/ or panniculitis/lipoatrophy at the injection site. Patients underwent endermology and glatiramer acetate treatment according to clinical practice. The primary endpoint was the change in indurations and/or panniculitis/lipoatrophy after 12 endermology sessions. Results: Between April and July 2011, a total of 13 evaluable patients were included (mean age, 40.7±3.1 years; female, 100%; white, 100%; mean MS duration, 10.1±2.3 years; previous MS treatment, 46.2%; mean glatiramer acetate treatment duration, 27.3±9.5 months). Eleven patients (84.6%) showed local indurations (mean diameter, 3.4±0.5 cm; mean number, 9.0±1.0) and six patients (46.2%) areas of panniculitis/ lipoatrophy (mean number, 5.0±1.1).
