A projected shortage of hematopoietic cell transplantation (HCT) health professionals was identif... more A projected shortage of hematopoietic cell transplantation (HCT) health professionals was identified as a major issue during the National Marrow Donor Program/Be The Match System Capacity Initiative. Work-related distress and work-life balance were noted to be potential barriers to recruitment/retention. This study examined these barriers and their association with career satisfaction across HCT disciplines. A cross-sectional, 90-item, web-based survey was administered to advanced practice providers, nurses, physicians, pharmacists, and social workers in 2015. Participants were recruited from membership lists of 6 professional groups. Burnout (measured with the Maslach Burnout Inventory subscales of emotional exhaustion and depersonalization) and moral distress (measured by Moral Distress Scale-Revised) were examined to identify work-related distress. Abstract Nurse practitioner (NP) and NP student perceptions of adult lesbian, gay, bisexual, and ... more Abstract Nurse practitioner (NP) and NP student perceptions of adult lesbian, gay, bisexual, and transgender (LGBT) health needs were qualitatively explored. One overreaching theme, “We need to do better. We have a lot to do,” and four key themes were identified: (1) same basic health needs as straight people; (2) a little more risk, a little more help, a little extra care; (3) breaking down that trusting, honest provider relationship; and (4) society sucks and social media and politics play a major role. Results support the need for future research exploring NP and NP student understanding of LGBT health needs.

This study aimed to ascertain unmet needs in autologous and allogeneic hematopoietic cell transpl... more This study aimed to ascertain unmet needs in autologous and allogeneic hematopoietic cell transplantation (HCT) recipients actively followed in an established long-term survivorship clinic at Roswell Park Cancer Institute from 2006 to 2012. The Survivor Unmet Needs Survey (SUNS) was mailed to 209 eligible patients and returned by 110 (53% participation rate). SUNS includes 89 items covering 5 domains: Emotional Health, Access and Continuity of Care, Relationships, Financial Concerns, and Information. The top 5 specific unmet needs for autologous HCT patients were inability to set future goals/long-term plans, changes in appearance, bad memory/lacking focus, losing confidence in abilities, and paying household or other bills. For allogeneic HCT patients these 5 unmet needs were tied at 21% of respondents: ability to earn money, pay bills, feeling tired, feeling depressed, and dealing with others' expectations of "returning to normal." The top 5 needs reported by females were all from the emotional health domain, whereas males reported financial domain unmet needs. Self-reported participation in health maintenance and screening tests varied greatly from 88% of patients having routine annual bloodwork to 13% of patients having an exercise stress test in the past year. Our findings demonstrate unmet needs in emotional health and financial burden in HCT survivors and variable compliance with survivorship screening.

Medline, CINAHL, PubMed. Variations continue to exist in educational curricula, program plans, and scholarly projects, leading to a lack of consistency in experiential learning. At this point in time it is too early in the DNP implementation process to determine ultimate impact. Continuous program self-assessment and evaluation of DNP education programs will be of paramount importance to assure program quality and optimization of health trajectories.

ABSTRACT Poor oral health and dental disease are major public health problems that are, in many c... more ABSTRACT Poor oral health and dental disease are major public health problems that are, in many cases, directly or indirectly linked with chronic conditions. Interprofessional collaborative teams are a way to address this problem. In this study we describe the development of an interprofessional education and interprofessional collaborative practice endeavor utilizing standardized patients to teach and evaluate clinical competencies in oral-systemic health and interprofessional collaborative practice settings. This pilot project paired family nurse practitioner students with student dentists to enhance the education of both groups. Standardized patients provided a safe and supervised environment to provide performance feedback and formatively evaluate student performance.

We report on the instructive case of a young, athletic patient who had 3 discrete episodes of documented cardiomyopathy in relation to treatment with imatinib, nilotinib, and dasatinib. Case Report Methods A previously healthy 27-year-old athletic female was diagnosed with Ph+ ALL in June 2004, when she presented with recurrent infections, weight loss, and a total white blood cell count of 56 × 10 9 /L. Lumbar puncture excluded central nervous system involvement. Induction therapy was per Cancer and Leukemia Group B (CALGB) protocol 9111. 3 The cumulative dose was 135 mg/m 2 for daunorubicin and 1,200 mg/m 2 for cyclophosphamide. The patient was then enrolled in CALGB 10001, a phase II trial of sequential chemotherapy and imatinib. 4 Because a fully matched donor was unavailable, the patient underwent an autologous stem cell transplant. Her left ventricular ejection fraction (LVEF) prior to the autologous transplant was 60%. The myeloablative conditioning regimen, administered in December 2004, included total body irradiation (1,320 cGy in 11 fractions), cyclophosphamide (100 mg/kg), and etoposide. This hospitalization was complicated by typhlitis as well as by Streptococcus viridans and Escherichia coli septicemia that required vasopressor support. The patient was restarted on imatinib maintenance therapy at 400 mg twice daily after count recovery. One year following her autologous transplant

We hypothesized that treatment of recipients peri-transplant with peg-mGM-CSF may be protective o... more We hypothesized that treatment of recipients peri-transplant with peg-mGM-CSF may be protective of GvHD and improve survival by increasing recipient Tregs. Balb/c mice (n 5 5/group) were treated with peg-mGM-CSF or PBS for 4 days prior to transplantation using myeloablative conditioning and transfer of B6 splenic T-cells and bone marrow. This was followed by continued treatment with either PBS or peg-mGM-CSF for the first week after transplant. There was a trend toward worse survival in the peg-mGM-CSF treated recipients. (Log-rank (mantel-cox) test, p 5 0.085) We next asked if treatment of T-cell donors might abrogate GvHD. B6 donors were treated with G-CSF, peg-mGM-CSF or PBS for 4 days, on the fifth day splenocytes were isolated and panT cell selected using an AutoMACS column. An equivalent dose of CD3 + T-cells along with congenic bone marrow was administered to recipient Balb/c mice (n 5 4-5/group) that had undergone myeloblative conditioning. Mice receiving GM and G treated donor T-cells had an improved survival compared to PBS alone (PBS vs. G p 5 0.0314, PBS vs. GM p 5 0.0467 (Log rank (Mantel cox) test)) but displayed similar GvHD scores. This effect may be explained by an increase in Tregs delivered in the G and GM group. Further studies will seek to validate these results.
