Stevens-Johnson syndrome (SJS) is a cutaneous immunity reaction involving the skin and mucosa and... more Stevens-Johnson syndrome (SJS) is a cutaneous immunity reaction involving the skin and mucosa and is an emergency condition that can be fatal. The incidence of this disease is relatively rare in the range of 1-2 per 1.000.000 population. The pathogenesis of SJS involves the immune system response of antigenic drug to body tissues but it still cannot be fully explained to date. We reported a woman, 35 years old with systemic lupus erythematosus (SLE) who had been on steroid therapy and in the course of treatment developed into SJS after administration of anti-epileptic drug. Steroids have anti-inflammatory effects mainly due to decreased syntheses or suppression of inflammatory mediators. SJS still can develop in patient with SLE who had been on steroid therapy. Giving steroid that indicated for the treatment of a disease including SLE, cannot prevent the occurrence of an allergic event including SJS. The presence of steroid can extend the duration of starting the drug with the occurrence of SJS and reduce the severity of the disease. Steroid still have a role in treatment that can be used both in SJS and SLE.

Latar Belakang: Peripheral Neuropathy salah satunya adalah lesi nervus okulomotor merupakan salah... more Latar Belakang: Peripheral Neuropathy salah satunya adalah lesi nervus okulomotor merupakan salah satu manifestasi klinis pasien Systemic Lupus Erithematosus (SLE). Kasus: Perempuan 34 tahun mengeluh kelopak mata kiri turun disertai pandangan dobel bila kelopak mata kiri diangkat sejak 1 bulan sebelum pemeriksaan. Pasien dengan riwayat nyeri kepala dan SLE sejak tahun 2006. Pada pasien didapatkan pupil anisokor dengan ukuran pupil pada mata kanan 3 mm, dengan reflex langsung dan tidak langsung positif. Pada mata kiri didapatkan ukuran pupil 6 mm dengan reflex langsung dan tidak langsung negatif. Pada gambaran CT-Angiography didapatkan gambaran dilatasi Arteri Carotid Siphon Kiri. Diskusi: Lesi nervus okulomotor involved pupil paling sering disebabkan oleh karena adanya kompresi aneurisma pada posterior communicating artery dan internal carotid artery. Adanya SLE meningkatkan risiko terjadinya kelainan serebrovaskular. Simpulan: SLE with oculomotor nerve palsy will increase risk of cerebrovascular event.

Leprosy is a chronic granulomatous infection caused by Mycobacterium leprae, which mainly damages... more Leprosy is a chronic granulomatous infection caused by Mycobacterium leprae, which mainly damages the skin and peripheral nerves, and can also infect joints and bones. Clinical manifestations are complex, varied, and often underdiagnosed. The clinical manifestations vary and are called "the great imitator". musculoskeletal features are common in leprosy but infrequently reported. A case of borderline lepromatous (BL) type leprosy was initially suspected as rheumatoid arthritis in a 20-year-old Balinese male who complained of pain and swelling in small joints, accompanied by lesions in the form of multiple erythema macules, round-shaped geography, indistinct boundaries, with bilateral distribution is almost symmetrical in the face, thoracoabdominal, and extremity regions. The diagnosis of BL type leprosy is based on history taking, physical examination, and investigations in the form of Ziehl-Neelsen stain and blood tests.

Vaccination is a very important measure for the prevention of various infections worldwide includ... more Vaccination is a very important measure for the prevention of various infections worldwide including the recent COVID-19 disease. However, until now the COVID-19 vaccine with various platforms has not been clinically tested on autoimmune inflammatory rheumatic disease (AIIRD) patients, due to caution against possible side effects and unknown efficacy. Several recent studies proved that there is increased risk of SARS-CoV-2 infection in AIIRD patients and moreover, those patients also have worse COVID-19 outcomes. Thus, patients with AIIRD should be prioritized for vaccination because they have an increased burden of infections, including COVID-19. Many studies showed that inactivated/non-live vaccine is safe for AIIRD patients and do not cause disease exacerbations. We conclude that benefits of vaccination greatly outweigh the risks of infection and therefore, COVID-19 vaccines can also be administered safely in stable AIIRD patients.

The current meta-analysis systematically analyzed the effect of vitamin D supplementation on SLE disease activity with updated literature, also its effect toward other parameters. Material and Methods: Relevant literatures were obtained from PubMed database and Google Scholar. The obtained studies were analyzed using fixed effect model or random effect model. Results: Five eligible studies with a total of 318 participants were included. The inflammation process which occurred in patients with SLE also affect the differentiation of osteoclast and osteoblast. Interleukin-17 (IL-17) is a pro-inflammatory mediator which produced due to systemic inflammatory process. The increase of pro-inflammatory cytokine resulted in impairment of RANKL regulation, which affect osteoprotegerin (OPG). The increase of pro-inflammatory cytokine in patients with SLE may resulted in imbalance ratio of RANKL/OPG. Objective : Determine the correlation between serum IL-17 levels with ratio of RANKL/OPG in patients with SLE. Method : This was an observational analytical cross-sectional study conducted at outpatient clinic and inpatient ward of Internal Medicine of Sanglah Hospital Denpasar, Bali, Indonesia from January to March 2018. Sample were female SLE patients with age more than 18 years, not menopause and agree to participate. High sensitivity ELISA (Enzymelinked immunosorbent assay) performed to analyze serum IL-17 levels. RANKL measured with Human sRANKL (TOTAL) ELISA method, while OPG measured with Human Osteoprotegerin ELISA method. Ratio of RANKL/OPG obtained from comparation of RANKL and OPG. Result : 68 subjects were involved in this study with median of age was 31.32 (17-54). The IL-17 levels and ratio of RANKL/OPG in all subjects was 0.435 (0.23-30.65) and 70.18 (4.98-1060.46), respectively. Significant correlation was found between IL-17 with ratio of RANKL/OPG (p=0.010). The result of multivariate analysis obtained serum IL-17 correlated with ratio of RANKL/OPG (B=6,554, SE(B)=2,686, p=0,018). Conclusion : There was correlation between IL-17 serum with ratio of RANKL/OPG in patients with SLE.

The inflammation process which occurred in patients with SLE also affect the differentiation of osteoclast and osteoblast. Interleukin-17 (IL-17) is a pro-inflammatory mediator which produced due to systemic inflammatory process. The increase of pro-inflammatory cytokine resulted in impairment of RANKL regulation, which affect osteoprotegerin (OPG). The increase of pro-inflammatory cytokine in patients with SLE may resulted in imbalance ratio of RANKL/OPG. Objective : Determine the correlation between serum IL-17 levels with ratio of RANKL/OPG in patients with SLE. Method : This was an observational analytical cross-sectional study conducted at outpatient clinic and inpatient ward of Internal Medicine of Sanglah Hospital Denpasar, Bali, Indonesia from January to March 2018. Sample were female SLE patients with age more than 18 years, not menopause and agree to participate. High sensitivity ELISA (Enzymelinked immunosorbent assay) performed to analyze serum IL-17 levels. RANKL measured with Human sRANKL (TOTAL) ELISA method, while OPG measured with Human Osteoprotegerin ELISA method. Ratio of RANKL/OPG obtained from comparation of RANKL and OPG. Result : 68 subjects were involved in this study with median of age was 31.32 (17-54). The IL-17 levels and ratio of RANKL/OPG in all subjects was 0.435 (0.23-30.65) and 70.18 (4.98-1060.46), respectively. Significant correlation was found between IL-17 with ratio of RANKL/OPG (p=0.010). The result of multivariate analysis obtained serum IL-17 correlated with ratio of RANKL/OPG (B=6,554, SE(B)=2,686, p=0,018). Conclusion : There was correlation between IL-17 serum with ratio of RANKL/OPG in patients with SLE.
