Aim: To analyse the dental effects on digital dental casts in subjects with Class III malocclusio... more Aim: To analyse the dental effects on digital dental casts in subjects with Class III malocclusion treated with bonded Rapid Maxillary Expansion and Facial Mask (RME/FM). Materials and methods: A study group sample (SG) of 29 subjects (13 females and 16 males) and mean age of 7.4 years (SD 1.2 years) was selected. A bonded RME was placed and activated 1/4 of a turn per day until overcorrection of the transverse width. At the end of expansion, patients were given FMs and treated at least to a positive overjet. The SG was compared with a control group (CG) of 21 prepubertal subjects (9 females; 12 males) presenting with normal occlusion and mean age of 7.9 years (SD 1.6 years). For each subject of the SG and CG initial (pretreatment, T1) and final (post-treatment, T2) digital dental casts were available. Significant in between-group differences were tested with the Student's t-test. Results: The transverse dimension of the upper arch was significantly greater in SG vs. CG (IMAW: +2.6 mm; ICAW: +3.1 mm). Anterior arch length and arch depth were significantly smaller in SG when compared with CG (AAL: -3.2 mm, AD: -3.4 mm) at the end of therapy. Conclusion: The orthopaedic treatment with bonded RME/FM produced in subjects treated in the deciduous or early mixed dentition a significant expansion of the maxillary arch and mesialisation of the posterior teeth with a reduction of the arch depth.

Study design: 2 groups of subjects were selected: an experimental group (84 patients) with tooth agenesis and a control group (84 subjects) without tooth agenesis. Materials and methods: Skeletal, dental, morphological and aesthetic parameters were studied using lateral and posteroanterior cephalograms and dental casts. Results: Transversal and sagittal measurements showed a significant decrease in maxillary size associated with tooth agenesis in the agenesis group. No changes were observed in mandibular size. Conclusion: Orthodontists should consider that patients with agenesis present altered craniofacial dimensions.
The aim of this research was to determine the interrelationships among chronological, dental and ... more The aim of this research was to determine the interrelationships among chronological, dental and skeletal age in a group of developing subjects. A total of 117 Italian subjects, 54 males and 63 females, were analyzed. The age of the subjects measured ranged from 4.5 to 14 years. For each of them together with a radiographic and skeletal analysis, an accurate clinical-subjective evaluation in relation to the grade of body development was conducted. Dental age was calculated from panoramic radiographs according to methods described by Nolla (1960) and skeletal analysis was determinated by the Bjork-Grave-Brown classification (1972, 1976). It can be observed that there is a good interrelationships among the three parameters of age.

Polyunsaturated fatty acids have been reported to play a protective role in a wide range of disea... more Polyunsaturated fatty acids have been reported to play a protective role in a wide range of diseases characterized by an increased metalloproteinases (MMPs) activity. The recent finding that omega-3 and omega-6 fatty acids exert an anti-inflammatory effect in periodontal diseases has stimulated the present study, designed to determine whether such properties derive from a direct inhibitory action of these compounds on the activity of MMPs. To this issue, we investigated the effect exerted by omega-3 and omega-6 fatty acids on the activity of MMP-2 and MMP-9, two enzymes that actively participate to the destruction of the organic matrix of dentin following demineralization operated by bacteria acids. Data obtained (both in vitro and on ex-vivo teeth) reveal that omega-3 and omega-6 fatty acids inhibit the proteolytic activity of MMP-2 and MMP-9, two enzymes present in dentin. This observation is of interest since it assigns to these compounds a key role as MMPs inhibitors, and stimulates further study to better define their therapeutic potentialities in carious decay.

Purpose: The purpose of this study was to assess the effectiveness of rapid maxillary expansion (... more Purpose: The purpose of this study was to assess the effectiveness of rapid maxillary expansion (RME) vs simply monitoring the eruption of permanent maxillary incisors following the surgical removal of obstacles to their eruption (supernumerary teeth, odontomas). Methods: Following surgical removal of the obstacles to incisor eruption (T1), 62 patients were randomly assigned to either the group to undergo RME (34 subjects; mean age 8 years, 11 months ± 11 months) or the group that was monitored without further treatment (28 subjects; mean age=9 years, 1 month ± 1 year). At T2 (1 year after T1), the prevalence rate of erupted incisors was recorded. Also, the time of eruption of the incisors and the amount of space loss were analyzed. Results: At T2, eruption of impacted incisors occurred in approximately 82 percent of the RME group cases vs approximately 39 percent of the monitored group cases (chi-square=10.43, P<.001). Time of eruption was significantly faster in the RME group, and anterior space loss significantly smaller. Conclusions: Rapid maxillary expansion treatment following surgical removal of the obstacles to the eruption of permanent maxillary incisors appears to be an efficient interceptive approach leading to eruption of the incisors in four out of five cases within seven months.

Focalizzare l&#39;attenzione sull&#39;iperdon-zia, una tra le più frequenti anomalie dentali di numero, e sull&#39;associazione tra questa e il ritar-do di eruzione degli incisivi permanenti. Materiali e metodi . Attraverso l&#39;analisi della letteratura sono state esaminate le varie teorie eziologiche, le diverse classifi cazioni dei den-ti soprannumerari e le complicazioni cliniche associate a tale anomalia dentale. Nella mag-gior parte dei casi un dente soprannumerario nella regione della premaxilla è responsabile di un&#39;alterazione del processo eruttivo degli incisi-vi permanenti fi no a determinarne l&#39;inclusione. Sono stati analizzati i mezzi diagnostici e le op-zioni terapeutiche. Risultati e Conclusioni . Una diagnosi preco-ce, un approccio chirurgico conservativo e un trattamento ortodontico accurato permetto-no il corretto riposizionamento dell&#39;incisivo permanente. The paper calls attention to the phe-nomenon of supernumerary teeth, one of the most common anomalies involving the number of teeth, and examines its association with delayed eruption of the permanent incisors. Materials and methods . The literature on supernu-merary teeth was analyzed to identify hypotheses on the cause of the phenomenon, classifi cation of supernumerary teeth, and complications that can occur. In most cases, a supernumerary tooth in the anterior maxilla is responsible for altering the eruption of the permanent incisors, which may even become impacted. This article reviews diagnostic tools and options for the management of supernumerary teeth. Results and Conclusions . Early diagnosis and management with conservative surgery and appropriate orthodontic treatment can minimize the potential complications caused by super-numerary teeth and successfully reposition the permanent incisors. Treatment failure can be caused by insuffi cient bone removal during surgical exposure, application of excessive trac-tion, ankylosis of the impacted tooth, or negative prognostic factors.

ABSTRACT The aim of this study was to investigate pharyngeal size differences between pre- and po... more ABSTRACT The aim of this study was to investigate pharyngeal size differences between pre- and posttrials of a Mandibular Advancement Device (MAD), using a computed tomography (CT),in the treatment of Obstructive Sleep Apnea (OSA) adult patients. Eighteen patients with mild to moderate OSA (mean Apnea/Hypopnea Index, AHI, of 16.7) were treated with a MAD to wear at night only. After 3 months of treatment, three-dimensional changes in pharyngeal dimensions were measured on CT images performed with a sixteen detector-row CT scanner (Light Speed Plus; GE Medical Systems). Two consecutive axial sections from the hard palate to the epiglottis were obtained with and without the appliance. Measurements were made of the following airway areas (mm2) and lengths (mm): RF (nasopharynx); ROF (naso-oropharynx); OF (oropharynx); IPF (hypopharynx); SPL (soft palate length); SPT (soft palate thickness); Rgn (retrognation)-hyoid bone; hyoid bone-C2; Rgn-C2; PhL (oropharynx length); pharynx posterior wall thickness at three level. The angle between the hard and the soft palate (APDM) was also calculated. AHI improved significantly (from 16.7 to 11.2) when the appliance was used. Measurements from CT scans showed statistically significant expansion in the naso-oropharynx area (RF p<.014; ROF p<.050), in the Rgn-C2 length (p<.005) and in the angle between the hard and the soft palate (APDM p<.001). Our findings confirm the effectiveness of MAD in the treatment of patients with mild to moderate OSA. The use of MAD significantly expands the areas of the upper airway lumen most involved in the collapse.

Gli Autori esaminano il rapporto medico-paziente e tutte le problematiche ad esso correlate relat... more Gli Autori esaminano il rapporto medico-paziente e tutte le problematiche ad esso correlate relativamente all’ortodonzia. Tale rapporto ha subito negli anni un’importante evoluzione: i pazienti sono ormai ampiamente informati circa i loro diritti e i doveri dell’operatore. Proprio a ragione di cio’ e fondamentale che l’ortodontista imposti il proprio lavoro su solide basi di diligenza, prudenza e perizia: deve provvedere ad una attenta e completa documentazione che comprenda consenso informato firmato dal paziente, records clinici iniziali e finali e collaborazione attiva con altri specialisti. E’ importante inoltre che lo specialista conosca le cause piu comuni di malcontento denunciato dai pazienti in modo da evitare errori ed incomprensioni in un campo in cui si assiste ad un sempre piu crescente numero di contenziosi.

MAD is the gold standard therapy for mild/moderate OSA and is a second-choice treatment in patien... more MAD is the gold standard therapy for mild/moderate OSA and is a second-choice treatment in patients with severe OSA not responding to CPAP. The aim of this study was to analyze the upper airway characteristics of OSA patients and the morphological changes in the upper airway, inducted by four different types of mandibular advancement devices, using CBCT 3D imaging. Twenty-two patients were recruited after OSA diagnosis with polysomnography. Four different customized and titratable MAD were used and an initial CBCT scan was obtained for each patient. After 6 months to the end of the MAD titration phase, all the subjects showed an AHI of < 5 or a decrease of 50% in AHI with MAD when compared with initial AHI and a second CBCT scan with MAD in situ was performed. An important improvement of the airway was observed (+33.76%) and a significant difference in the enlargement ratio between the posterior soft palate (+32.41%) and the posterior tongue (+36.96%) region was also found.
