Most reported cases of middle ear adenoma (MEA) have focused on histopathology because MEA is usu... more Most reported cases of middle ear adenoma (MEA) have focused on histopathology because MEA is usually diagnosed postoperatively, which is considered as a major setback. We focused on the surgical aspect of the disease to facilitate a preoperative diagnosis, resulting in prompt and proper treatment, without requiring a second stage of surgical treatment. In this report, we present the differential diagnoses in a 40-year-old man with MEA requiring surgical treatment. Preoperatively, the patient was suspected to have an MEA. An analysis of the surgical procedures in similar misdiagnosed tumors has enabled us to assess surgical procedures in cases wherein the preoperative diagnosis does not coincide with the postoperative histopathological results.

The result was the increased admission of patients with severe dyspnea and a need for tracheotomy due to extremely advanced HNC. The aim of this study wasevaluate the characteristics of two multi-center cohorts of HNC patients admitted for dyspnea.Methods..The therapeutic activity of four University Departments of Otolaryngology was studied over two time periods: September-February 2019/2020 and 2020/2021.Results. A group of 136 HNC patients who underwent tracheotomy in two time cohorts, pre-COVID-19 (N=59) and COVID-19 (N=77), were analyzed. Methods: The study is based on an analysis of two groups of children diagnosed and treated in a children’s ENT outpatient clinic and ENT department. Clonidine constricts peripheral blood vessels and reduces systemic blood pressure, which in combination decrease nasal mucosa blood flow. This dual effect can potentially reduce bleeding during FESS and stabilize the intraoperative hemodynamic profile of the patient. The aim of this prospective study was to assess if the quality of the surgical field visualization during FESS was improved when clonidine was used as a premedication agent. A group of 44 patients undergoing FESS for chronic sinusitis and polyp removal were enrolled and randomly assigned to receive either oral clonidine or midazolam as preoperative premedication. A 28-year-old man was hospitalized more than 15 timesfrom 2005 to 2008 at the Department of Otola... more A 28-year-old man was hospitalized more than 15 timesfrom 2005 to 2008 at the Department of Otolaryngology atour institutions because of chronic, recurrent osteomyelitisof the maxilla on the right side. His medical history began in1994 (when he was a healthy 16-year-old boy without anyallergies or medication intake). At that time, a simple dentalprocedure of excision of an epulis located between theupper molar teeth on the right side was performed. Theprocedure was complicated by opening of the maxillarysinus from the oral cavity, which was surgically closed atthat time. Since then, recurrent right-side sinusitis had de-veloped multiple times with several puncture proceduresand empiric antibiotic therapies. In 1995, a Caldwell-Lucprocedure was performed on the right side with bilateralinferior turbinate submucous resection. Histopathologic ex-amination showed a nonspecific chronic inflammation. Af-ter surgery, the patient felt worse and noted an ongoingsensation of pressure around the tooth, swelling of thecheek, purulent discharge from the nose, and recurrentfever. He was hospitalized at the Department of Otolaryn-gologyoftheMedicalUniversityofGdan´sk,Gdan´sk,Poland,from May to August 2001. He was admitted for facial edemaand subfebrile body temperature; the symptoms appeared afew weeks before hospitalization. Computed tomography(CT) scan showed mucous membrane edema in both max-illarysinusesandedemaoftherightcheekandlowereyelid.Bacteriologic investigations of the nose swabs were nega-tive (Table 1). The results of the other laboratory tests wereas follows: white blood cell count, 8,600 L; erythrocytesedimentation rate, 6 mm; and C-reactive protein level, 3.0mg/L. The patient was treated with intravenous empiricantibiotics (600 mg of clindamycin 3 times daily for 14 days,2.0 g of ciprofloxacin 2 times daily for 3 weeks, and 500 mgof metronidazole 3 times daily for 14 days); at the begin-ning, the symptoms ceased, but within the next 2 weeks,the symptoms returned. The aim of this case series was to present applications of rSLA pedicle flaps in the post-ablative oncologic reconstruction of the face. Patients and Methods: Using a retrospective case-series study design, the investigators enrolled a cohort of patients undergoing procedures involving rSLA flaps treated at a Polish Otolaryngology Department for facial reconstruction after tumour excision. The main outcomes were functional and aesthetic aspects. Descriptive statistics were computed as appropriate. In the field of otolaryngology it was used in 1992-1994. Shaver is a combination of suction and cutting round knife working together. Knives of different shapes are put into handpiece. They have outer sheath with window, which protect the inner rotating blade. The blade is connected to suction and cutting tissue is removed from the operation field. In otolaryngology surgery we mostly used the oscillation cutting mode which is the most sufficient. Shaver surgery is precise in soft tissue resection, so the most advantages to powered the system is in rhinosurgery, polypectomy with extension to pansinus surgery. Microdebridor can be also used for nasopharynx-adenoidectomy, chonanal atresia surgery, turbinoplasty, dacryocystorhinostomy, sella turcica tumor resection or laryngeal surgery. The most troublesome problem during endoscopic surgery is fouling of the endoscope lens (tip) wit... more The most troublesome problem during endoscopic surgery is fouling of the endoscope lens (tip) with blood. Irrigating systems are available on the market but they are relatively very expensive. We present our own simple irrigating system, very effective and inexpensive. The system consists of: polyethylene catheter No 6, sterile intravenous line with interposed stopcock, 500 ml saline solution, endoscope. The catheter is fixed to the endoscope with sterile sticks and connected to the bottle of saline solution with intravenous line. During the operation the irrigant solution flow is controlled with interposed stopcock. A simple irrigating system allows cleaning of blood from the endoscope obviating the need for frequent removal of the endoscope from the nose. This allows more rapid and safer endoscopic surgery. It is very simple and cheap.

Background: The purpose of this study is to analyse the long-term effects of a 12-week course of ... more Background: The purpose of this study is to analyse the long-term effects of a 12-week course of topical steroids on adenoid size and its mucus using endoscopy and on middle ear effusion measured by tympanometry. Methods: The study presents an endoscopic choanal assessment of the change in adenoid size (adenoid to choanae ratio, A/C ratio) and its mucus coverage in 165 children with Grade II and III adenoid hypertrophy three to six months after finishing a 12-week course of intranasal steroid treatment with mometasone furoate. Additionally, tympanometry was performed to measure middle ear effusion. Changes in the tympanograms were analysed. Results: The mean A/C ratio before treatment was 65.73%. Three to six months after finishing a 12-week course of intranasal steroid treatment, the mean A/C ratio decreased to 65.52%, although the change was not statistically significant (p = 0.743). Coronavirus disease (COVID-19) is caused by the SARS-CoV-2 virus and often presents with flu-like... more Coronavirus disease (COVID-19) is caused by the SARS-CoV-2 virus and often presents with flu-like symptoms that can have varying degrees, which may subsequently lead to acute respiratory distress (ARDS). The genetic material of the virus in samples of respiratory secretions is identified by way of basic diagnostic tests. Due to the altered course of the respiratory tract, patients after total laryngectomy require special attention in the diagnosis of SARS-CoV-2 infection. We present a case of a patient after laryngectomy who obtained different results of COVID-19 tests depending on the site of sampling.

Head and neck cancers, especially laryngeal carcinoma, are still an essential therapeutic problem... more Head and neck cancers, especially laryngeal carcinoma, are still an essential therapeutic problem worldwide. The most common risk factors which participate in initiation of cancer progression are tobacco smoking and alcohol abuse. The fundamental factor which confines effective therapy is tumor advanced stage at the moment of diagnosis. For proper determination of cancer progression there still is a need to identify new markers which can describe cancer cells and are also related in invasiveness and tumor matrix rebuild. The studies were performed on tissue samples divided in two groups: from patient who smoked less and above 20 cigarettes a day. We observed the expression of studied antigens as follows: MMP-2 in 100%, TGF-beta1 in 86% and TGF-betaRI in 79% cases. In tumor stroma we observed higher expression of MMP-2, TGF-beta1 in patients with higher exposure to tobacco smoke. Objectives: To analyze the role of factors that influence adenoid-related symptoms, and the influ... more Objectives: To analyze the role of factors that influence adenoid-related symptoms, and the influence of 3 months of isolation in preschool children who were qualified for adenoidectomy. Methods: This was a cohort study of the impact of 3 months of isolation on children aged 3 to 6 years with adenoid-related symptoms and endoscopically confirmed grade II and III adenoid hypertrophy. The children had previously qualified for adenoidectomy. After 3 months, 141 children were asked about their symptoms, 71 of whom were randomly chosen for medical examination, including endoscopic adenoid examination. Additionally, pre- and post-isolation tympanometry results were analyzed. Results: In our study, significant or mild improvements in health were observed in approximately 73% of the children; 92% of the surveyed parents reported that their children exhibited improved nasal patency, 63% of children discontinued snoring, and 30% of children showed a decrease in snoring. Recommendations of the Main Board of the Polish Society of Otorhinolaryngologists, Head and Neck ... more Recommendations of the Main Board of the Polish Society of Otorhinolaryngologists, Head and Neck Surgeons for providing services during the COVID-19 pandemic constitute the guidance to outpatient and hospital practices in all cases where contact with a patient whose status of COVID-19 is unknown. They have been created based on world publications and recommendations due to the current state of the COVID-19 pandemic. Justification for suspension of planned provision of services in the first phase of a pandemic was presented. The indication of the best medical practices for the time of stabilization, but with the persistence of the risk of COVID-19 infection in the population are discussed. The possibility of providing services in the following months of the pandemic is important. We provide the rationale for launching medical activities and indicate optimal practices until the consolidation of SARS COV-2 prevention and treatment methods.
On the base of retrospective analysis of 12,888 cases of carcinoma of larynx and hypopharynx, dia... more On the base of retrospective analysis of 12,888 cases of carcinoma of larynx and hypopharynx, diagnosed in 19 ENT Departments in Poland from 1991 to 2001, the assessment of basic epidemiological data, including the localization of tumor and stage of local and clinical advancement of the disease at the time of diagnosis has been conducted. In analyzed period of 11 years the trends to change of the mentioned above parameters has been examined. The significant increase of female patients in this period was observed, with average proportion M:F = 8:1. The glottis localization of carcinoma dominated (47.6%), followed by supraglottis (40.8%) and pyriform fossa (7.8%), with significant increase of pyriform fossa tumors in the analyzed period of 11 years. The aim of this article was to feature the use of giant bilobed flaps in surgical reconstruction ... more The aim of this article was to feature the use of giant bilobed flaps in surgical reconstruction after total or subtotal petrosectomy in oncology cases. The Esser technique was renewed and adopted for coverage huge skin loss. The authors reported 4 cases illustrated pre- and postoperative photography and intraoperative planning and schema. Good coverage and healing with no need for use of free tissue transfer or skin expanders or skin grafts was obtained. In authors’ opinion, the cosmetic effect was acceptable. Surgical times of operation were significantly shorter.

The work brings new information that, despite the presence of inflammation before surgery, patients with higher vitamin D levels had less inflammation, and this has been proven on three levels. We show that vitamin D levels clearly correlate with the severity of the disease in chronic sinusitis. These results have been confirmed in imaging studies (Lund MacKay scale), endoscopy (Lund-Kennedy scale) and in the SNOT 20 questionnaire. 40 patients suffering from chronic sinusitis were divided into two equal groups: group 1: with less severe radiological changes (10 or less points on the Lund-Mackay scale), group 2: with a more advanced form (>10 points). The aim of this case series was to present applications of rSLA pedicle flaps in the post-ablative oncologic reconstruction of the face. Patients and Methods: Using a retrospective case-series study design, the investigators enrolled a cohort of patients undergoing procedures involving rSLA flaps treated at a Polish Otolaryngology Department for facial reconstruction after tumour excision. The main outcomes were functional and aesthetic aspects. Descriptive statistics were computed as appropriate. In the field of otolaryngology it was used in 1992-1994. Shaver is a combination of suction and cutting round knife working together. Knives of different shapes are put into handpiece. They have outer sheath with window, which protect the inner rotating blade. The blade is connected to suction and cutting tissue is removed from the operation field. In otolaryngology surgery we mostly used the oscillation cutting mode which is the most sufficient. Shaver surgery is precise in soft tissue resection, so the most advantages to powered the system is in rhinosurgery, polypectomy with extension to pansinus surgery. Microdebridor can be also used for nasopharynx-adenoidectomy, chonanal atresia surgery, turbinoplasty, dacryocystorhinostomy, sella turcica tumor resection or laryngeal surgery. The most troublesome problem during endoscopic surgery is fouling of the endoscope lens (tip) wit... more The most troublesome problem during endoscopic surgery is fouling of the endoscope lens (tip) with blood. Irrigating systems are available on the market but they are relatively very expensive. We present our own simple irrigating system, very effective and inexpensive. The system consists of: polyethylene catheter No 6, sterile intravenous line with interposed stopcock, 500 ml saline solution, endoscope. The catheter is fixed to the endoscope with sterile sticks and connected to the bottle of saline solution with intravenous line. During the operation the irrigant solution flow is controlled with interposed stopcock. A simple irrigating system allows cleaning of blood from the endoscope obviating the need for frequent removal of the endoscope from the nose. This allows more rapid and safer endoscopic surgery. It is very simple and cheap.

Background: The purpose of this study is to analyse the long-term effects of a 12-week course of ... more Background: The purpose of this study is to analyse the long-term effects of a 12-week course of topical steroids on adenoid size and its mucus using endoscopy and on middle ear effusion measured by tympanometry. Methods: The study presents an endoscopic choanal assessment of the change in adenoid size (adenoid to choanae ratio, A/C ratio) and its mucus coverage in 165 children with Grade II and III adenoid hypertrophy three to six months after finishing a 12-week course of intranasal steroid treatment with mometasone furoate. Additionally, tympanometry was performed to measure middle ear effusion. Changes in the tympanograms were analysed. Results: The mean A/C ratio before treatment was 65.73%. Three to six months after finishing a 12-week course of intranasal steroid treatment, the mean A/C ratio decreased to 65.52%, although the change was not statistically significant (p = 0.743). Coronavirus disease (COVID-19) is caused by the SARS-CoV-2 virus and often presents with flu-like... more Coronavirus disease (COVID-19) is caused by the SARS-CoV-2 virus and often presents with flu-like symptoms that can have varying degrees, which may subsequently lead to acute respiratory distress (ARDS). The genetic material of the virus in samples of respiratory secretions is identified by way of basic diagnostic tests. Due to the altered course of the respiratory tract, patients after total laryngectomy require special attention in the diagnosis of SARS-CoV-2 infection. We present a case of a patient after laryngectomy who obtained different results of COVID-19 tests depending on the site of sampling.

Head and neck cancers, especially laryngeal carcinoma, are still an essential therapeutic problem... more Head and neck cancers, especially laryngeal carcinoma, are still an essential therapeutic problem worldwide. The most common risk factors which participate in initiation of cancer progression are tobacco smoking and alcohol abuse. The fundamental factor which confines effective therapy is tumor advanced stage at the moment of diagnosis. For proper determination of cancer progression there still is a need to identify new markers which can describe cancer cells and are also related in invasiveness and tumor matrix rebuild. The studies were performed on tissue samples divided in two groups: from patient who smoked less and above 20 cigarettes a day. We observed the expression of studied antigens as follows: MMP-2 in 100%, TGF-beta1 in 86% and TGF-betaRI in 79% cases. In tumor stroma we observed higher expression of MMP-2, TGF-beta1 in patients with higher exposure to tobacco smoke. Objectives: To analyze the role of factors that influence adenoid-related symptoms, and the influ... more Objectives: To analyze the role of factors that influence adenoid-related symptoms, and the influence of 3 months of isolation in preschool children who were qualified for adenoidectomy. Methods: This was a cohort study of the impact of 3 months of isolation on children aged 3 to 6 years with adenoid-related symptoms and endoscopically confirmed grade II and III adenoid hypertrophy. The children had previously qualified for adenoidectomy. After 3 months, 141 children were asked about their symptoms, 71 of whom were randomly chosen for medical examination, including endoscopic adenoid examination. Additionally, pre- and post-isolation tympanometry results were analyzed. Results: In our study, significant or mild improvements in health were observed in approximately 73% of the children; 92% of the surveyed parents reported that their children exhibited improved nasal patency, 63% of children discontinued snoring, and 30% of children showed a decrease in snoring. Recommendations of the Main Board of the Polish Society of Otorhinolaryngologists, Head and Neck ... more Recommendations of the Main Board of the Polish Society of Otorhinolaryngologists, Head and Neck Surgeons for providing services during the COVID-19 pandemic constitute the guidance to outpatient and hospital practices in all cases where contact with a patient whose status of COVID-19 is unknown. They have been created based on world publications and recommendations due to the current state of the COVID-19 pandemic. Justification for suspension of planned provision of services in the first phase of a pandemic was presented. The indication of the best medical practices for the time of stabilization, but with the persistence of the risk of COVID-19 infection in the population are discussed. The possibility of providing services in the following months of the pandemic is important. We provide the rationale for launching medical activities and indicate optimal practices until the consolidation of SARS COV-2 prevention and treatment methods.
On the base of retrospective analysis of 12,888 cases of carcinoma of larynx and hypopharynx, dia... more On the base of retrospective analysis of 12,888 cases of carcinoma of larynx and hypopharynx, diagnosed in 19 ENT Departments in Poland from 1991 to 2001, the assessment of basic epidemiological data, including the localization of tumor and stage of local and clinical advancement of the disease at the time of diagnosis has been conducted. In analyzed period of 11 years the trends to change of the mentioned above parameters has been examined. The significant increase of female patients in this period was observed, with average proportion M:F = 8:1. The glottis localization of carcinoma dominated (47.6%), followed by supraglottis (40.8%) and pyriform fossa (7.8%), with significant increase of pyriform fossa tumors in the analyzed period of 11 years. The aim of this article was to feature the use of giant bilobed flaps in surgical reconstruction ... more The aim of this article was to feature the use of giant bilobed flaps in surgical reconstruction after total or subtotal petrosectomy in oncology cases. The Esser technique was renewed and adopted for coverage huge skin loss. The authors reported 4 cases illustrated pre- and postoperative photography and intraoperative planning and schema. Good coverage and healing with no need for use of free tissue transfer or skin expanders or skin grafts was obtained. In authors’ opinion, the cosmetic effect was acceptable. Surgical times of operation were significantly shorter.

The work brings new information that, despite the presence of inflammation before surgery, patients with higher vitamin D levels had less inflammation, and this has been proven on three levels. We show that vitamin D levels clearly correlate with the severity of the disease in chronic sinusitis. These results have been confirmed in imaging studies (Lund MacKay scale), endoscopy (Lund-Kennedy scale) and in the SNOT 20 questionnaire. 40 patients suffering from chronic sinusitis were divided into two equal groups: group 1: with less severe radiological changes (10 or less points on the Lund-Mackay scale), group 2: with a more advanced form (>10 points). In this study, patients were divided into two groups: group 1—patients with CRSwNP (chronic rhinosinusitis with nasal polyps) taking a nasal steroid and an oral steroid in the preoperative period, and group 2—patients with CRSwNP taking only the nasal steroid in the preoperative period.
