Even though the most widely prescribed drugs for the relief of acute pain have been in use for ma... more Even though the most widely prescribed drugs for the relief of acute pain have been in use for many years, their efficacy can still be enhanced by more precise assessment of pain and the judicial titration of doses. Some of the procedures applied within the Acute Pain Service of the Fundation Santa Fe de Bogota, as an example of the way in which an integrated approach can improve analgesic drug efficacy, will be discussed here.

Diversos factores intrínsecos y extrínsecos pueden provocar metatarsalgias por insuficiencia de l... more Diversos factores intrínsecos y extrínsecos pueden provocar metatarsalgias por insuficiencia de la almohadilla grasa plantar. La utilización de la lipoinfiltración autóloga como terapia de relleno del defecto tisular de la grasa plantar junto con el uso de la terapia biológica con plasma rico en plaquetas podría considerarse como una terapia alternativa a las terapias actuales con biomateriales, xenoinjerto y aloinjertos. El presente artículo presenta un caso clínico de un paciente tratado de forma satisfactoria mediante lipoinfiltración autóloga en un defecto de la grasa plantar en la zona del primer metatarsiano secundario a una cirugía previa por enfermedad de Ledderhose. Se trata de una opción terapéutica novedosa que podría ser de gran utilidad para el podólogo en el tratamiento de este tipo de casos de pérdida localizada de almohadilla grasa plantar.

Se determinaron los valores de gasimetría arterial en una muestra de 8 pacientes con in-suficienc... more Se determinaron los valores de gasimetría arterial en una muestra de 8 pacientes con in-suficiencia renal terminal en programa de hemodiálisis crónica a nivel de Bogotá; se compararon los valores hallados en arteria radial y fístula arteriovenosa sin encontrar diferencia significativa. Al comparar estos valores con los descritos previamente para pacientes normales a la misma altitud, se en-contró un aumento significante de la PaO2 en nuestra muestra (p < 0.01). Al estudiar un segundo grupo de pacientes transplantados estables, su gasimetría arterial no se diferenció en forma significativa de la de los pacientes normales, y al compararla con la del grupo en diálisis, las diferencias en la PaO2 no fueron estadísticamente signifi-cativas, a pesar de las consistentes diferencias en la PaCO2 (p<0.01) y HCO3 (p<0.05). The discovery that two cyclooxygenase (COX) isoenzymes, including inducible forms (COX-2), could ... more The discovery that two cyclooxygenase (COX) isoenzymes, including inducible forms (COX-2), could be selectively inhibited has triggered new research to develop selective inhibitors. During this process, pharmacotherapeutic concepts in inflammatory pain, and clinicians over-expectations, have created some degree of uncertainty for clinicians about the promise of COX-2 inhibitors and the real role they play. The purpose of this article is to critically review, within the context of NSAIDs, some of the old and new concepts in the management of inflammatory pain, as well as to examine the current evidence of the usefulness of COX-2 inhibitors today.

A basic knowledge update about the current status of concepts and techniques for Cancer pain management is of enormous importance for every pactioner caring for Cancer patients as a resource for appropiate information and solution to their pain problems. Acute and chronic pain occurs in a high frequency in Cancer patients. Inadequate assessment and treatment of pain and other distressing symptoms may interfere with primary antitumor therapy and markedly detract from their quality of life. While a strong focus on pain control is important independent of disease stage, it is a special priority in patients with advanced disease who are no longer candidates for potentially curative therapy under the scope of an integral perspective to palliative care.

The development of COX-2 selective inhibitors has opened a new era of clinical investigation in N... more The development of COX-2 selective inhibitors has opened a new era of clinical investigation in NSAIDs. Discussion of the established concepts of inflammation and therapeutical uses of these drugs has changed the rationale for its clinical use and therapeutic labeling of these drugs. A comprehensive discussion across basic science and clinical areas involved in each of these concepts is presented. This led to a remarkable re-evaluation of our insights on their traditionally proposed mechanisms of analgesia, their side-effects, and the clinical indication of NSAIDs as &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot;over the counter&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;quot; pain killers. This may shift physicians toward a more rational use of this drug class.
