A 56-year-old man was admitted with a 3-month history of a painful lump on his left hand that had... more A 56-year-old man was admitted with a 3-month history of a painful lump on his left hand that had slowly increased in size. He also had a history of malaise and generalized abdominal pain. Clinical examination showed an expansile lesion on the dorsum of the left hand. An anteroposterior radiograph was obtained . Computed tomography of the left hand and isotope bone scanning of both hands were requested for further evaluation. A whole-body isotope bone scan showed that the lesion was solitary. On further questioning, the patient described neck swelling, and examination revealed a left-sided neck mass. Biochemical tests indicated hypercalcemia with a markedly elevated serum parathyroid hormone level. Radioisotope scanning of the neck was done.

Physical examination revealed no abnormality apart from minimal swelling. Plain radiographs were initially obtained to evaluate the cause of his pain . Because of the plain radiographic findings, magnetic resonance imaging (MRI) was performed. is a representative sagittal gradient-echo image, and is a representative transverse axial T 2 -weighted image.

A 36-year-old woman with a complaint of pain and weakness in her shoulder was referred to an orth... more A 36-year-old woman with a complaint of pain and weakness in her shoulder was referred to an orthopedic surgeon. She had no recent history of trauma, but had fallen some 9 months previously on her outstretched hand, injuring her shoulder. Plain radiographs taken at that time were normal.

There was no history of a specific inciting traumatic event.

Five patients with splanchnic arterial aneurysms and one patient with a tortuous splenic vessel p... more Five patients with splanchnic arterial aneurysms and one patient with a tortuous splenic vessel presented with peripancreatic masses that exhibited an anechoic portion in real-time ultrasonography (US) images; the masses were incorrectly diagnosed as pseudocysts. In two patients a vascular abnormality was suspected, and colour-flow Doppler and pulsed Doppler US were performed; these techniques showed the presence of pseudoaneurysms. In the other four patients the possibility of a pseudoaneurysm was not entertained until further studies had been performed. These results emphasize the need for colour-duplex US evaluation of all peripancreatic fluid collections during the initial US examination to avoid the misinterpretation of an aneurysm or a pseudoaneurysm as a simple or complex fluid collection and to prevent the potential disaster of performing biopsy, surgery or other interventions.

A 27-year-old male volleyball player was referred for an orthopedic consult with a complaint of l... more A 27-year-old male volleyball player was referred for an orthopedic consult with a complaint of left shoulder pain. His pain had been present for 3 months and was more prominent with overhead activities. Over the last 2 months, he had taken nonsteroidal anti-inflammatory agents, which helped reduce his pain. He had also undertaken a course of physiotherapy, which temporarily eased the symptoms. He had no history of trauma to the shoulder.

On examination, he had marked swelling over the medial aspect of the first metatarsal bone of his left foot. Plain films were normal (not shown). On the basis of the soft tissue swelling, we performed magnetic resonance imaging.
