Background and aims: This study aimed to clarify the incidence and severity of habitual sleep dis... more Background and aims: This study aimed to clarify the incidence and severity of habitual sleep disturbances in patients undergoing surgery. Methods: A survey was conducted to collect data in a cohort of patients awaiting abdominal surgery. The Insomnia Severity Index score was used as primary measurement, while the Epworth Sleepiness Scale and Pittsburgh Sleep Quality Index scores were used as secondary measurements. Findings: Overall, 119 patients were included in the analysis. The habitual sleep in the study cohort revealed high incidence of sleep difficulties and insomnia. Female sex and incidence of anxiety and depression were significantly related to all three scores, while age and employment were associated with Epworth Sleepiness Scale and Insomnia Severity Index scores, respectively. Conclusions: Female sex, experiencing depression or anxiety had severe negative impact on sleep in patients scheduled for elective or planned surgery. The prevalence of sleep problems in patients...

Anti-nociceptive effects of melatonin have been documented in a wide range of experimental animal... more Anti-nociceptive effects of melatonin have been documented in a wide range of experimental animal models. The aim of this study was to investigate the analgesic, anti-hyperalgesic and anti-inflammatory properties of melatonin employing a validated burn injury (BI) model in healthy male volunteers. The design was a randomized, double-blind, placebo-controlled, three-arm crossover study. Each volunteer participated in three identical study sessions with intravenous administration of either placebo, melatonin 10 mg or melatonin 100 mg. Sixty minutes after bolus injection of study medication, a BI was induced by a computerized contact thermode (47.0°C, 420 s, 5.0 x 2.5 cm). Pain ratings during the BI, and quantitative sensory testing (QST) at baseline, and, 1, 2, 4 and 6 hours after the BI, were performed. Amlapitta is a lifestyle disorder, kaphapittaja amashayaottavyadhi caused by the impairment of Ag... more Amlapitta is a lifestyle disorder, kaphapittaja amashayaottavyadhi caused by the impairment of Agni (pitta). Utkleshita Amlapitta is the avastha of a disease which produces symptoms like utklesha, amla tiktha udgara, shiroruja and kanta hrut kukshi daha which are taken as subjective parameter in current study. The other associated symptoms commonly seen during amlapitta are Vidbheda, Udara Aadmana, Angasadha Aantrakoojana, Romaharsha, Aruchi and Gurukoshta. Amlapitta can be corelated to Gastritis with prevelance rate around 10 millions in India. Prolong dependency on contemporary medicine showed side effects with 40% not responding to the treatment. In such situation, for quick relief one can adopt sadyovamana as Avastika Chikitsa for Samprapti Vighatana. To prove the same through clinical trial Sadyovamana using Lavanambu and Ksheera with group A and group B respectively are selected for comparative study. Results showed highly Statistical significance with P value <0.001 in both the groups in almost all the parameters. When compared using unpaired 't' test both group showed equally significance. Thus treatment is patient friendly, very effective, intervenes with minimal preparatory procedures, hospitalization, expenses and very easy procedure which can be adopted even at OPD level. So the study also strongly emphasizes that all the Ayurveda fraternity to adopt Sadyovamana in any diseases if Utklesha present in amashaya with due care, so that the pros & cons are highlighted through n number of trials in many diseases.

In this study, we hypothesised that patients intubated with the fiberoptic bronchoscope-guided intubation device differs in POST and hoarseness from other patients intubated with the oral Macintosh laryngoscope-guided intubation device. If this could be confirmed, the guideline of peri anaesthesia may need to be different. Objective: This study aimed to investigate whether fiberoptic intubation can provide increased patient safety and comfort compared to standard laryngoscope intubation performed by nurse anaesthetists. Methods: Within this single-blinded, consecutively controlled clinical trial two hundred and nine patients scheduled for elective neurosurgical treatment requiring intratracheal intubation, were randomly divided into intervention and control group. The intervention group received fiberoptic bronchoscopy-guided intubation during anaesthesia as well as nursing and postoperative treatment based on conventional drug therapy after neurosurgical treatment. The control group received intubation during anaesthesia, as usual, using Macintosh laryngoscope intubation, and postoperative treatment based on conventional drug therapy after neurosurgical treatment. Data regarding the patient safety were obtained during peri anaesthesia, and data concerning the patient comfort were collected on the first postoperative day. Statistical analyses were performed using SPSS version 22. Results: Seven patients were excluded (3.3%); thus, the final population consisted of two hundred and two patients: 62.4% women, the mean age of 52 years (20 to 86 years), 106 (52.5%) patients in the fiberoptic intubation group. A statistically significant difference was found between groups according to patient safety, measured by the number of anaesthetists involved P=0.024 in favour of the fiberoptic intubation device. A trend was pointing to increased pain in the mouth and throat P=0.053, when intubated with the laryngoscope. Other patient safety and comfort measurements showed no statistically significant differences between groups. Conclusions: The study directs attention to some possible benefits of using the fiberoptic intubation device, which may improve patient safety and comfort when compared to standard laryngoscope intubation.

Aims and objectives: To explore experiences and informational needs regarding sexual health in me... more Aims and objectives: To explore experiences and informational needs regarding sexual health in men and women with epilepsy or multiple sclerosis (MS). Background: Sexual health and well-being is dependent on access to comprehensive information about sexuality and good-quality sexual health care in an environment that affirms and promotes sexual health. Design: The study had a qualitative explorative design using focus group methodology and framework analysis to capture the patient perspective. Methods: We strategically selected 26 participants at a neurological clinic at a university hospital in Denmark and conducted four homogeneous focus group interviews: Women with epilepsy (n=8), Men with epilepsy (n=7), Women with MS (n=5), and Men with MS (n=6). We used the Framework Method for analysis and initially developed a matrix of disease versus gender before the authors triangulated their views and decided on the final themes. Results: We identified four themes and nine sub-themes. Our main findings included insights on the impact of ignorance regarding sexual rehabilitation, demonstrating the importance of information in patients with sexual dysfunction related to chronic neurologic disease. The study expanded our understanding of sexual activity in the unpredictable body of epilepsy and the progressively changing body of MS. The participants agreed that they would have taken a more constructive path to solve their sexual problems if they had been aware of common responses to their disease.
