L'exenteration est une procedure chirurgicale defigurante qui se definit par l'exerese de... more L'exenteration est une procedure chirurgicale defigurante qui se definit par l'exerese de tout le contenu orbitaire avec le perioste, laissant ainsi a nu les parois osseuses de l'orbite. Il s'agit dans la plupart des cas d'une chirurgie carcinologique d'une tumeur maligne ayant secondairement envahi l'orbite. Le comblement de la cavite fait appel soit a l'epithelialisation spontanee, soit a la greffe de peau mince ou a un lambeau musculaire. Cette mise au point a pour but de retracer l'historique de l'exenteration orbitaire, de preciser les indications et de decrire la technique chirurgicale avec ses complications. Des progres significatifs en matiere d'implantologie ont permis d'ameliorer l'aspect esthetique apres cette chirurgie. Cependant les survies apres exenteration restent modestes et dependent du type histologique, de la qualite d'exerese, de la taille et de l'extension tumorale.

The authors report a series of 21 cases of spheno-orbital "en plaque" meningiomas opera... more The authors report a series of 21 cases of spheno-orbital "en plaque" meningiomas operated on between 1981 and 1993. All patients underwent surgery via fronto-temporal approach with orbito-zygomatic removal, followed by bone reconstruction. A complete tumor resection was performed in 15 cases. In the remaining patients, tumor resection remained subtotal, as the cavernous sinus was involved. Histological examination disclosed a low grade meningioma in 17 cases, high grade meningioma in the other patients. Radiation therapy was performed in 5 patients, when the tumoral resection was incomplete or for high grade meningiomas. The mean follow up was 7 years. One patient died in the post-operative course. Transient ophthalmoparesis occurred in 9 cases, 2 patients had pre-operative oculomotor palsy related to tumoral extension in the cavernous sinus and remained unchanged. Malignant tumors which invade the ethmoid are usually associated with a very reserved or poor pro... more Malignant tumors which invade the ethmoid are usually associated with a very reserved or poor prognosis. The severity of the prognosis depends on the extent of infiltration of the tumor into the base of the skull [1, 5, 7]. When operating on these patients, it is therefore logical to choose a surgical approach which provides maximum exposure of the anterior base of the skull for inspection [4, 6, 8, 9, 10]. This can be done by using a monoblock orbitofrontral bone flap approach in conjunction with the standard sublabial transmaxillary approach.

Exenteration of the orbit is a disfiguring and destructive procedure that is usually reserved for... more Exenteration of the orbit is a disfiguring and destructive procedure that is usually reserved for treatment of life-threatening orbital malignancy when a less radical operation such as local surgery, chemotherapy, or irradiation are deemed inadequate or have failed. Many methods have been published for managing the socket, but of primary importance is the need to remove all diseased tissue prior to considering any reconstructive efforts. Options include spontaneous granulation, skin grafting, or muscle flaps. This article will describe the development and the indications for this procedure and will outline the surgical techniques and its complications, the various reconstructive efforts, and survival. The success of orbital exenteration depends on recurrence, histological type, tumor size, and tumor-free margins.

A madreporic coral graft was used for orbital floor reconstruction following facial trauma. This ... more A madreporic coral graft was used for orbital floor reconstruction following facial trauma. This report presents a multicentric study of 83 patients with a follow-up period of 15 to 24 months. The results of this study indicate no significant rejection or infection opposed to so many synthetic implants outcome. The radiological follow-up demonstrates a partially resorption of the implant within about 2 years and its replacements by new bone. Coral implant was used to correct enophthalmos or diplopia due to enlarged orbital dimensions. It was technically easy to insert and its anatomic shape does not require to be fashioned before use. Its inflexibility allows to bridge large bone defects and this implant should be considered as an attractive alternative to autogenous grafts, avoiding a second surgical site, in reconstructing orbital floor fractures.

Usual surgical attitude is abstention and surgery is proposed only in order to treat sequel. But some particular situations require early surgery for life-threatening lesions or in case of functional impairment. Surgery consists in these cases in a simple volumetric diminution of the tumor with no aim for esthetic improvement. Other kinds of hemangiomas require an early surgical treatment before their complete involution. In some particular locations, deformation or growth delay can occur due to the lesion's development. A series of 19 children with congenital nasal dermoid cyst or fistulas admitted in our department... more A series of 19 children with congenital nasal dermoid cyst or fistulas admitted in our department between 1978 and 1992 was reviewed in a retrospective study. They were 8 females and 11 males, ages ranged from 9 months to 7 years (mean: 2 years and 4 months). At diagnosis 8 children presented with infection (superficial in 6 cases, neurological in 2 cases), and in 11 cases the malformation was discovered by the family physician. In 14 cases the cyst was located at the mid nasal ridge. Four children had associated malformations: neurodevelopmental delay (2 cases), hypertelorism (1 case) and external ear agenesia with facial palsy. The cyst was subcutaneous in 5 cases and was operated on through a nasal approach. The 14 other children were operated through associated intra and extracranial approaches. It is constructed from the two "vestibions", central points of the rings of lateral semi-circular canals. Reconstruction parallels this plane, passing through the vestibions provide vestibular orientation, with effective and constant coïncidence between right and left directions. That being obtained, it is easy to visualise orientation of the entire cephalic region in the three incidences of "vestibular space", and to work out sections parallels to the three vestibular planes, allowing analysis of the symmetry of the studied elements.

It is sometimes giant and responsible for complications. A newborn had a large infiltrated blue news located on buttocks, lombosacral and perianal areas. He was admitted at the age of 3 weeks for an intestinal obstruction which was due to an impassable anal stenosis secondary to the infiltrated nevus. The tumor was excised twice, completed by plastic surgery at the age of 10 months. This rare congenital giant blue nevus was responsible for an unreported intestinal obstruction. The risk of malignant transformation was another reason for complete excision.

It is sometimes giant and responsible for complications. A newborn had a large infiltrated blue news located on buttocks, lombosacral and perianal areas. He was admitted at the age of 3 weeks for an intestinal obstruction which was due to an impassable anal stenosis secondary to the infiltrated nevus. The tumor was excised twice, completed by plastic surgery at the age of 10 months. This rare congenital giant blue nevus was responsible for an unreported intestinal obstruction. The risk of malignant transformation was another reason for complete excision.
