We present the case of a young patient who sustained a cardiac arrest revealing an anomalous origin of the right coronary artery. Unroofing and pericardial enlargement of the coronary artery ostia was performed and the patient is actually asymptomatic.

Left ventricular assist devices are valuable options for mechanical circulatory support in patien... more Left ventricular assist devices are valuable options for mechanical circulatory support in patients with severe heart failure as a bridge to cardiac transplantation, to recovery, or as destination therapy. Transoesophageal echocardiography plays a key role in the management of left ventricular assist device patients; either in guiding the device implantation or in post-operative haemodynamic assessment. The aim of this observational study is to investigate the usefulness of a miniaturized transoesophageal echocardiography probe designed for continuous haemodynamic monitoring (haemodynamic transoesophageal echocardiography; ImaCor, Garden City, NY) in the haemodynamic assessment of patients undergoing left ventricular assist device implantation. Between June 2014 and September 2014, seven consecutive patients underwent left ventricular assist device implantation for severe heart failure. A haemodynamic transoesophageal echocardiography probe was used for continuous haemodynamic monitoring in the pre-, intra- and post-operative periods. All procedures were performed by haemodynamic transoesophageal echocardiography-trained physicians. All data were recorded and used without need of a further haemodynamic device. The average number of haemodynamic assessments in each patient was 5.3. The average number of haemodynamic assessments leading to therapeutic changes in each patient was 2.6. The mean duration of haemodynamic transoesophageal echocardiography probe insertion was approximately 20 hours (9 hours - 72 hours). No device complications were noticed during this period. This observation suggests that haemodynamic monitoring using the haemodynamic transoesophageal echocardiography system in left ventricular assist device patients is feasible after a brief training period. The overall impact of haemodynamic transoesophageal echocardiography use in these patients is yet to be determined and further studies are still required.

Heart failure with left ventricular dysfunction occurring during pregnancy or during the post-par... more Heart failure with left ventricular dysfunction occurring during pregnancy or during the post-partum period in patients without history of cardiovascular disease defines peripartum cardiomyopathy (PPCM). PPCM carries a high morbidity and mortality rate as well as the possibility of recovery ad integrum. Its incidence shows ethnic variations, with a greater prevalence of the disease among women with African descent. Pathogenesis of PPCM remains poorly understood. Both "oxidative stress-prolactin axis" and "anti-angiogenic-signaling excess" hypotheses are currently being investigated. Novel diagnostic strategies and biomarkers are currently being evaluated. Besides conventional treatment of heart failure, targeted therapies such as pharmacological prolactin blockade are under evaluation. The aim of this short review is to highlight current management as targeted therapy has far been disappointing.

Bone is the most common metastatic site of breast cancer (BC) and sternal metastasis usually occu... more Bone is the most common metastatic site of breast cancer (BC) and sternal metastasis usually occurs in an isolated setting. We performed sternal reconstruction using a new total titanium custom-made neosternum after subtotal sternectomy for an isolated BC metastasis. Postoperative care was uneventful during a 7-day in-hospital stay. After a 6-month follow-up, the patient denied any shortness of breath, chest pain or limitation on her daily activities. The chest was stable without any paradoxical motion. Chest X-ray did not show any material fracture, pleural effusion or lung abnormalities. As it entails lower postoperative risk than previously existing reconstructive materials, the new material used in our sternal reconstruction may extend the existing range of indications of sternectomy for cancer with curative intent.

To our knowledge a thymic metastasis from lung carcinoma has never been reported. We report the case of a metastatic lung cancer to the thymus, five years after a lobectomy for a stage I lung adenocarcinoma.

L'intervention s'est compliquée d'une médiastinite traitée premièrement à thorax fermé et secondairement par une épiplooplastie. Cette technique reste efficace dans les médiastinites graves et délabrantes. Tout chirurgien cardiaque et digestif doit la connaitre.

Thoracic endovascular aortic repair (TEVAR) is a possible therapeutic approach that needs close surveillance because its durability in such situations remains uncertain. We report a case of haemoptysis related to an aortobronchial fistula recurring 6 years after TEVAR, with an intraoperative finding of a midgraft hole. The patient underwent open repair with in situ aortic replacement with an aortic cryopreserved homograft.

When there exists an isolated metastasis to the heart, the melanoma tends to involve the right heart. Rarely does melanoma metastasize to the left ventricle. We report the first case of choroidal melanoma that had indeed metastasized to the left ventricle and was associated with a patent foramen ovale, which may explain its initially surprising location on this side of the heart.

A 7-year-old boy sustained blunt trauma after falling from a farm tractor. The child remained wel... more A 7-year-old boy sustained blunt trauma after falling from a farm tractor. The child remained well for 6 months before he began having recurrent cough. He was treated symptomatically for another 2 months before referral to our center. On chest radiograph, the trachea and heart were shifted to the left. The CT scan showed a collapsed left lung and hyper-inflated right lung with herniation to the opposite side . Bronchoscopy revealed normal trachea and bronchial take-off on both the sides, but the left main bronchus was completely occluded at 1 cm from the carina.
