Lipoma in hand of size more than 5 cm is classified as Giant Lipoma. Lipoma of larger sizes is rare in hand because of accessibility and lesser subcutaneous tissues. Large lipoma interferes in the hand functions. Lipoma in dominant hand is usually noted early and gets operated. This case is of 65 year old female patient who reported with the swelling in right hand for past 25 years. The size of the lipoma was big and asymptomatic till the presentation. The swelling was investigated and surgically removed which was diagnosed histopatholoically as Intermuscular Thenar Lipoma .

There were no complaints of pain or loss of function. It was slow growing and gradually increased to attain this size. There was no sensory neurological deficit in the thumb. She was able to do her personal work without any difficulty. On examination, there was a well-defined lobular mass of size 8×7 cm occupying entire right thenar eminence. It was firm and non-mobile. Thumb movements were full. She was able to oppose the thumb to other fingers with difficulty but a firm grip could not be made [Table/Fig-1]. Clinical diagnosis of Lipoma was made. Conclusions: Cosmetic appearance was good in patients who underwent McIndoe procedure as the genitalia were not distorted. Contracture rate was more in patients who underwent McIndoe procedure especially who were irregular in using stents. Flap procedure especially pudendal thigh flaps which has the least contraction rate is the preferable treatment option for such patients.

Such techniques have many disadvantages. FNAC of the intra abdominal malignancy is often a forgotten rather than
neglected investigation. But it can be done with confidence to arrive at the tissue diagnosis. Aim: To emphasize FNAC as a
sensitive and specific diagnostic tool for intra abdominal malignancies.
Material and methods: FNAC was done in 50 cases of palpable intraabdominal malignancies and compared with other
conventional biopsy techniques.
Results: FNAC gave positive report of malignancy in 80% of cases, specifying the type and differentiation in most cases.
FNAC of intra abdominal malignancies had a sensitivity of 86.96% and a specificity of 100%.The overall accuracy rate is 88%.
There were no false positive reports. It had no complications. It reduced cost and time for investigations.
Conclusion: FNAC was extremely helpful in arriving at a cytological diagnosis of palpable intraabdominal malignancy. This
was useful in the further management of the patients. FNAC can be safely used as first line investigation for intraabdominal
malignancies.

Duodenal ulcer perforation peritonitis is the commonest cause for emergency laparotomy in India. ... more Duodenal ulcer perforation peritonitis is the commonest cause for emergency laparotomy in India. Abdominal cavity is highly contaminated with bile, food particles and pathogens. It is customary to keep drainage tube on both flanks after laparotomy for duodenal perforation closure. Drainage fluid is minimal if the perforation is closed by live omental patch technique after thorough peritoneal lavage. Drainage tube itself may act as a track for skin pathogens to migrate from skin to the abdominal cavity. This comparative study was conducted between two groups of patients underwent laparotomy with and without drainage tube for duodenal ulcer perforation. Patients in our surgical unit were treated without drainage tube and compared with other surgical unit where patients were treated with drainage tube. The study reveals that the outcome was similar between the two groups. Drainage tube is unnecessary in laparotomy for duodenal ulcer perforation.
