Çocukluk döneminde pek sık rastlanmayan bir lezyondur. Bu nedenle boyunda kitle ile başvuran bir hastada ön planda dermoid kist, tiroglossal kist gibi başka nedenler düşünülür ve laringosel tanısına çoğunlukla cerrahi eksplorasyondaki bulgular ve histopatolojik inceleme ile ulaşılır. Bu yazıda, eksternal laringosel tanısı cerrahi girişim sonrasında histopatolojik inceleme ile konan üç yaşında bir erkek çocuk sunulmuş ve ayırıcı tanı yapılarak değerlendirilmiştir. Anahtar kelimeler: boyunda kitle, çocuk, laringosel Summary A laryngocele diagnosed at three years of age Laryngocele is a rare cervical lesion of childhood that may present either with a mass or respiratory distress. Therefore, other causes such as thyroglossal cyst or dermoid cyst are primarily suspected in a case with a midline cervical mass and the diagnosis of laryngocele is usually made after surgical exploration and histopathologic analysis. Herein a three-year-old boy, in whom the diagnosis of an external laryngocele was achieved after surgical intervention and histopathological analysis, is presented and the differential diagnosis is overviewed.

Purpose: To present the results of a two-stage technique used for the treatment of proximal hypos... more Purpose: To present the results of a two-stage technique used for the treatment of proximal hypospadias with severe curvature. Materials and methods: The medical records of children with proximal hypospadias and severe curvature were retrospectively analyzed. A 2-stage procedure was performed in 30 children. In the first stage, the release of chordee was performed, and a well-vascularized preputial island flap was created. The vascularized island flap was brought anteriorly and sutured over the ventral surface of the glans and degloved penile shaft. The second stage was performed 6-8 months later. A neourethra was reconstructed by the tubularization of the preputial-urethral plate utilizing the principles of Duplay technique. All surgical procedures were performed between 2005 and 2011. Results: The mean age of the patients was 4.4 years (1-17 years). The mean duration of urethral catheterization was 6 days after the first stage and 10 days following the second stage. The flaps were viable in all of the children. There was no residual chordee. Following the second stage (n = 30), complications developed in 11 children (36%), namely, a fistula in 7, a pinpoint fistula in 3, and a diverticulum formation in 1. The cosmetic outcome was satisfactory. Uroflowmetry measurements were evaluated, and only one patient had a diverticulum formation at the late follow-up. Conclusion: Vascularized preputial island flap is an alternative to free grafts for the reconstruction of the urethra. The main advantage of this flap technique is the creation of a thick, healthy and well-vascularized urethral plate. The advantages of this technique include better aesthetic appearance, an acceptable complication rate, and a very low rate of diverticula formation.

Extensive aganglionosis very close to the ligament of Treitz and total intestinal aganglionosis a... more Extensive aganglionosis very close to the ligament of Treitz and total intestinal aganglionosis are rare forms of Hirschsprung's disease. In these cases, nutrition, fluid, and electrolyte balance are a problem. Although the myectomy-myotomy technique of Ziegler appears hopeful, no effective surgical method has been put into the practice for the treatment of these rare forms of Hirschsprung's disease. We report four cases with extensive intestinal aganglionosis, one of which was total intestinal aganglionosis with involvement of the stomach.

: In this study, early and late results of heterotopic splenic autotransplantation in 18 children... more : In this study, early and late results of heterotopic splenic autotransplantation in 18 children with type IV splenic injuries (Upadhyaya-Simpson classification) are presented. Splenic scintigrams, quantitative analysis of Howell-Jolly inclusion bodies, immunoglobulin (IgG, IgM, IgA) and complement C3 levels and T and B lymphocyte counts were analysed in the postoperative evaluation of the autotransplantation group. The total follow-up period was 7 years. According to our results, splenic implants increased complement C3 levels and improved filtration function of the splenectomized children. This autotransplantation group has two important characteristics: a) it is one of the largest series of the literature (pediatric age group), with b) longest follow-up period.

Background/aim.-In this study we planned to reveal the problems and progress in the management of... more Background/aim.-In this study we planned to reveal the problems and progress in the management of the esophageal atresia patients. This study consists of 231 patients with esophageal atresia, treated during the last 22 years. Patients and methods: Patients are subgrouped according to the year of diagnosis: Group I (1978-1984), Group II (1985-1991) and Group III (1992-2000). All patients are also assessed by the prenatal history, delivery time and method, time of admission, diagnostic workup, preoperative management and timing of surgery, technique of surgery, associated anomaly, complication and mortality rate. Results: The mean weight was 2635 gr, 85% of deliveries had been in hospital and 14.7% at home, 91.1% of babies had been term and 8.9% were preterm. Prenatal polyhydramnios story had been detected in 18.8%. Among the babies with esophageal atresia 51.1% had already been diagnosed before the admission. The associated anomaly rate was 33%. Group I patients had been operated immediately, Group II patients had been stabilised and operated in semi-elective conditions, Group III patients are operated in elective conditions. Intraoperative most common complication was iatrogenic pleural opening. The most common early postoperative complication was pneumonia and atelectesia; late complication was gastroesophageal reflux. Overall mortality was 59.7% and had been found in Group I as 86.5%, in Group II 64.5% and 29.5% in Group III. Respiratory problems (respiratory stress, pneumonia) and septicemia have been the most common cause of mortality. Conclusion: This study is the largest study group published in Turkey. In this study we demonstrated, that in correlation with the development in surgical techniques, preoperative and postoperative management the complications and mortality rate decreased in esophageal atresia patients.
