Kidney injury (defined as the presence of albuminuria, proteinuria, glycosuria [without hyperglyc... more Kidney injury (defined as the presence of albuminuria, proteinuria, glycosuria [without hyperglycemia], hematuria, and/or renal hypophosphatemia) is an emerging problem in human immunodeficiency virus (HIV)-infected patients, although few data are available on the role of protease inhibitors (PIs) in this condition.To determine the time to kidney injury in a cohort of HIV-infected patients receiving a PI-containing regimen.We report the results of a subanalysis of a published cross-sectional study. The subanalysis included only patients receiving PI-containing regimens for more than 6 months (377 of the overall 970 patients). We determined associated factors and constructed receiver operating characteristic curves to estimate time to kidney injury depending on the PI used.The percentage of patients with kidney injury was 27.7% for darunavir, 27.9% for lopinavir, and 30% for atazanavir. A group of 146 patients with non-insulin- dependent diabetes mellitus (NIDDM) was studied. They w... more A group of 146 patients with non-insulin- dependent diabetes mellitus (NIDDM) was studied. They were divided into two groups of 73 age and sex matched patients, according to the presence or absence of hypertension. We recorded the presence of macrovascular and microvascular complications of NIDDM, family history, body mass index (BMI), glycemic control and lipidic profile. Renal parameters included plasma creatinine, urinary albumin excretion rate (UAER), glomerular filtration rate (GFR), effective renal plasma flow (ERPF) and filtration fraction. Hypertensive patients had a higher BMI (30 +/- 4.8 kg/m2 vs. 27.6 +/- 4.4 kg/m2, P < 0.005), total cholesterol (6.34 +/- 1.47 mmol/l vs. 5.72 +/- 1.14 mmol/liter, P < 0.01), creatinine (91.1 +/- 25.6 mumol/liter vs. 81.3 +/- 20.3 mumol/liter, P < 0.05) and UAER [63.7 (range 1 to 5160) mg/24 hr vs. 27.3 (3 to 5500) mg/24 hr, P < 0.001]. Fundamento Conocer la prevalencia de la microalbuminuria en la poblacion general de un area medit... more Fundamento Conocer la prevalencia de la microalbuminuria en la poblacion general de un area mediterranea espanola y su asociacion con otros factores de riesgo cardiovascular. Pacientes Y MeTodo Se estudiaron 399 pacientes consecutivos y se dividieron segun tuviesen una microalbuminuria superior o inferior a 30 mg/24 h en 2 de 3 muestras. Resultados En 37 pacientes se detecto microalbuminuria (9,3% de la poblacion general y 21% de los hipertensos). La microalbuminuria se correlaciono con la edad, el valor de presion sistolica, el indice de masa corporal, el indice cintura-cadera, presion de pulso, el colesterol ligado a lipoproteinas de alta densidad, los trigliceridos, la glucemia y la creatinina serica. La microalbuminuria en los no hipertensos ni diabeticos fue de 12,9 (11,2) mg/dia, en los diabeticos no hipertensos de 14,1 (16,5) mg/dia, en hipertensos no diabeticos de 22,5 (53,5) mg/dia y en los diabeticos e hipertensos de 27,7 (49,1) mg/dia. La presencia de hipertension arterial determino la aparicion de microalbuminuria y el riesgo (OR) de presentar microalbuminuria fue de 2,4 (IC del 95%, 1,1- 5,2), y si se asociaba a sexo varon, de 3,8 (IC del 95%, 1,8-8,1). Conclusiones La prevalencia de la microalbuminuria en esta area mediterranea es baja (9,3%) pero si se asocia a otros factores de riesgo cardiovascular aumenta a un 21%. Los valores de microalbuminuria mas elevados se encuentran en los pacientes hipertensos y varones, se asocie o no diabetes.

The aim of this study was to analyse IGF1 modifications in MO patients after WL and its relationship to ghrelin and to different types of surgeries.DesignRetrospective follow-up study at the University Medical Center.MethodsOne hundred and nine MO patients (age 44.1±9.3, BMI 51.74±8.75 kg/m2) were evaluated at baseline and 1 year after surgery: 28 sleeve gastrectomy (SG), 31 distal modified (m), and 50 ringed (r) Roux-en-Y gastric bypass (RYGBP) surgery. Changes in IGF1, IGFBP3, ratio IGF1:IGFBP3, and ghrelin were evaluated 1 year after surgery.ResultsBaseline prevalence of low IGF1 (defined bys.d.IGF1<−2) was 22%, and %WL 1 year after surgery was 34.9±8.9%. RESEARCH DESIGN AND METHODS A total of 26 patients were randomized in two homogeneous groups for clinical and analytical data in a 6-mo follow-up study. Group A received captopril (initial dose: 12.5 mg daily, increased according to tolerance); group B was untreated. RESULTS Microalbuminuria decreased only in the treated group at 6 mo (P = 0.044) and a significant (P = 0.027) mean percentage change on microalbuminuria excretion between the groups was observed.
