To better understand the role of the BCL-3 locus at chromosome 17q22 in the pathogenesis and prog... more To better understand the role of the BCL-3 locus at chromosome 17q22 in the pathogenesis and progression of leukemias and lymphomas, we examined its genomic configuration in 264 B-cell malignancies and its expression in a smaller subset. Cases studied included 39 chronic lymphocytic leukemias, 58 low-grade follicular lymphomas, 20 mantle cell lymphomas, 30 small noncleaved cell lymphomas, 25 acute lymphoblastic leukemias, 10 acquired immunodeficiency syndrome--related non-Hodgkin's lymphomas, and 44 diffuse mixed- or diffuse large-cell lymphomas. In addition, 38 aggressive lymphomas (transformed follicular lymphomas) derived from previously indolent follicular lymphomas were examined. Southern blot analysis showed BCL-3 locus rearrangement in 4 cases (1.5%), ie, in 3 transformed follicular lymphomas and in 1 indolent follicular lymphoma. A 44-year-old woman was found to have a fistula between her gastric pouch and bypassed stomach 18 years after Roux-en-Y gastric bypass (RYGBP) for morbid obesity. She underwent an attempted open surgical repair, which was complicated by postoperative abdominal sepsis. An upper gastrointestinal series, abdominal CT scan and upper endoscopy confirmed the diagnosis of failed surgery with recurrent GGF. Under endoscopic and fluoroscopic guidance, two ports were inserted percutaneously into the stomach. The fistula was closed with a percutaneous, transgastric, totally extraperitoneal approach. She remains well 7 months after this intervention. This procedure appears to be a safe and effective minimally invasive approach for closure of GGF after RYGBP. This is the first description of an intragastric, percutaneous closure of a GGF after RYGBP in the medical literature. Further experience with this technique is needed to define the selection criteria, limitations, advantages, and disadvantages.

Several studies have suggested that positron emission tomography is more accurate than computed t... more Several studies have suggested that positron emission tomography is more accurate than computed tomography for the staging of non-small cell lung cancer and can reduce the rate of unnecessary thoracotomy in patients with potentially resectable disease. However, there are few data on the utility of positron emission tomography in the diagnosis of patients with tumors of 2 cm or less in size. Methods: Patients with cT1/cT2 tumors of 2 cm or less in size were retrospectively reviewed. All had a computed tomographic scan, as well as a positron emission tomographic scan on a dedicated scanner, with a standard uptake value reported. A standard uptake value of 2.5 g/mL or greater was considered positive. The results of computed tomography and positron emission tomography were correlated with pathologic results after either resection (n ϭ 60) or mediastinoscopy (n ϭ 4). Results: Sixty-four patients (38 women; mean age, 66 years) had a mean tumor size of 1.4 cm (range, 0.7-2.0 cm). Forty-three patients had adenocarcinoma, 13 had adenocarcinoma-bronchioloalveolar carcinoma, 5 had squamous cell carcinoma, and 3 had other tumor types. Twenty-nine (45%) tumors had negative positron emission tomographic results. Both tumor size (Ͼ1 cm vs Յ1 cm) and cell type (adenocarcinomaϪbronchioloalveolar carcinoma vs all other cell types) were significant predictors of positron emission tomography uptake in the primary tumor (P ϭ .05 and .01, respectively). Nodal metastases were detected pathologically in 11 (17%) patients (5 N1 and 6 N2). Positron emission tomographic sensitivity and specificity for nodal metastases were only 45% and 89%, respectively. There was no statistically demonstrable survival difference between positron emission tomography-positive and positron emission tomography-negative tumors (3-year survival of 87% vs 100%, respectively). Conclusion: Positron emission tomographic scanning has no demonstrable benefit in the diagnosis, staging, or prognosis of patients with tumors of 2 cm or less in size.

Foram coletados 665 espécimes de Argas miniatus em dois municípios dos estados da Bahia e Minas G... more Foram coletados 665 espécimes de Argas miniatus em dois municípios dos estados da Bahia e Minas Gerais. Destes, 596 (89,6%) tinham se alimentado, sendo que 489 (82%) reagiram para um único tipo de sangue, distribuídos entre aves (46,8%), roedores (30,9%), gambás (14,5%), bovinos (4,3%) e eqüinos (3,5%). Nesse tipo de reação, o sangue de mamíferos foi detectado em 53,2% (260/489) dos argasídios. As reações múltiplas foram observadas em 107 (17,9%) carrapatos, com sangue de aves presente em 84,1% (90/107), enquanto, o sangue de mamíferos reagiu em 100% (107/107). Os resultados apontam para a inespecificidade parasitária.

ABSTRACT The gold standard for mediastinal lymph node evaluation is mediastinoscopy, which is inv... more ABSTRACT The gold standard for mediastinal lymph node evaluation is mediastinoscopy, which is invasive and allows access to only a limited number of mediastinal lymph node (MLN) stations (1, 2, 3, 4, and 7). Endobronchial ultrasound-guided fine-needle aspiration (EBUS-FNA) is emerging as a useful, less invasive technique that offers access to a wider range of MLN stations (2, 3, 4, 7, 10, and 11). We report our initial experience with this procedure. Using our prospectively maintained database, we performed a single-institution retrospective chart review. Our study group consisted of all patients at the University of Minnesota who underwent EBUS-FNA for evaluation of mediastinal lymphadenopathy or for thoracic malignancy staging from September 1, 2006, through December 15, 2007. To assess our learning curve, we plotted the cumulative sensitivity (%) and accuracy (%) of our EBUS-FNA results as a function of the number of procedures we performed. During the study period, 100 patients underwent EBUS, 92 with FNA. Of these, 56 patients (34 women, 22 men; mean age, 60.4 +/- 13.7 years) met our inclusion criteria. We found no complications. After our first 10 procedures, the sensitivity of our EBUS-FNA results was 96.2%; accuracy was 97.8% (rates comparable with other large series in the literature). We conclude that the learning curve for EBUS-FNA for thoracic surgeons is about 10 procedures.

Diagnostic and therapeutic approaches to the disease often go unrewarded, partly because of our incomplete understanding of the underlying immunopathogenesis. To characterize the immune response to the flea bite in patients with papular urticaria. This study included 45 randomly selected patients and 17 controls. Cutaneous allergy tests were performed. The histopathologic and immunohistochemical characteristics of cellular infiltrate in skin lesions were established. Immunoblot analysis was used to describe the specific characteristics of flea proteins recognized by IgE and IgG in patients' serum samples. Cutaneous allergy test results were negative in 87% to 98% of patients and in 88% to 100% of controls. Histopathologic and immunohistochemical studies revealed a predominance of eosinophils and CD4+ T lymphocytes.
