Cervical stenosis can be a difficult clinical condition to treat effectively, and treatment may b... more Cervical stenosis can be a difficult clinical condition to treat effectively, and treatment may be followed by recurrences. Thirty-seven (37) women with this clinical diagnosis were evaluated and treated in our service over the last 5 years. Fifteen (15) women (43.2%) had previous cervical surgery such as conization, loop electrosurgical excision procedure (LEEP), or cervical laser vaporization and 11 had a history of diethylstilbestrol (DES) exposure. After treatment, 28 (75.5%) of these women obtained relief while in 24.5% no changes occurred. Four women (14.2%) had recurrence. Based on our experience, sequential progressive cervical dilatation under sonographic or laparoscopic control seems to add safety and effectiveness in the treatment of this condition. For those patients who develop recurrences, laser removal of a cervical central cylinder of tissue seems to provide the best results. Avoiding excessive trauma to the cervix with any surgical procedure should be paramount in decreasing chances of causing cervical stenosis, particularly in susceptible patients such as nulliparous and DES-exposed women. Experimental models suggest the use of different therapy protocols in rheumatoid arthritis (RA) a... more Experimental models suggest the use of different therapy protocols in rheumatoid arthritis (RA) as modulators on periodontal condition. This study evaluated the effects of conventional drug treatment and anti-TNF therapy in patients with RA on microbiological and periodontal condition, establishing the association of markers of periodontal infection with indexes of rheumatic activity. One hundred seventy nine individuals with RA were evaluated (62 with anti-TNF-α and 115 with only DMARDs). The periodontal evaluation included plaque and gingival indexes, bleeding on probing (BOP), clinical attachment loss (CAL), pocket depth (PD) and subgingival plaque samples for microbiological analysis. Rheumatologic evaluations included a clinical examination, rheumatoid factor (RF), antibodies against cyclic-citrullinated peptides (ACPAs), and activity markers (DAS28-ERS), high sentitive C-reactive protein (hs-CRP), erythrocyte sedimentation rate (ESR). Anti-TNF-α therapy influenced periodontal microbiota with a higher frequency of T. denticola (p=0.01). Methotrexate combined with leflunomide exhibited a higher extension of CAL (p=0.005), and anti-TNF-α therapy with methotrexate was associated with a lower extension of CAL (p=0.05). The use of corticosteroids exerted a protective effect on the number of teeth (p=0.027). The type of DMARD affected P. gingivalis, T. forsythia and E. nodatum presence. Elevated ACPAs titers were associated with the presence of red complex periodontal pathogens (p=0.025). Bleeding on probing was associated with elevated CPR levels (p=0.05), and ESR was associated with a greater PD (p=0.044) and presence of red complex (p=0.030). Different pharmacological treatments for RA affect the clinical condition and sub-gingival microbiota.

This retrospective study was undertaken to evaluate the degree of internal echogenicity in endometriomas preoperatively scanned by both techniques. Transvaginal ultrasound scanning should be added to the diagnostic armamentarium.

To review the experience with hysteroscopic treatment of uterine septa, particularly those involv... more To review the experience with hysteroscopic treatment of uterine septa, particularly those involving the cervix. One hundred twenty-four patients with septate uterus were treated hysteroscopically. Seven of these women had a uterine septum involving the uterus and cervix. All patients but nine who were infertile had demonstrated repetitive pregnancy losses with nothing to explain the reproductive failure but the uterine septation. The preoperative reproductive performance included 299 pregnancies, of which 258 were spontaneous abortions (86.6%) and 28 preterm pregnancies (9.6%) that resulted in viable infants. Following treatment, 101 patients achieved pregnancy (81.4%). There were 84 term pregnancies (83.1%) and 7 preterm viable pregnancies (6.9%). There were 12 spontaneous abortions, all in the first trimester (11.8%); 23 patients had not become pregnant as yet (18.5%). Two cases are presented describing late hemorrhage after this procedure. Approaches to avoiding this complication as well as management principles are described.

Ectopic pregnancy is a frequent clinic entity, with an incidence from 4.5 to 16.8 for 1000 pregna... more Ectopic pregnancy is a frequent clinic entity, with an incidence from 4.5 to 16.8 for 1000 pregnancies. The frequency of ectopic pregnancy has been triplicated in the last years, mainly owed to increase of sexual transmitted diseases, increase in salpinges' surgery and in assisted reproductive medicine. The ectopic pregnancy is also the most frequency cause of maternal death during the first trimester of pregnancy. The frequency of bilateral tubal ectopic pregnancy is extremely rare, it is reported from 1:125 to 1:1580 of all ectopic pregnancies. The first case of bilateral tubal ectopic pregnancy was reported n 1918 by Bledsoe. In Mexico, Molina described the first case in 1993, with conservative laparoscopic treatment. Two clinic cases are presented of bilateral ectopic pregnancy, treated conservatively by laparoscopy. The first one with background of sterility because of anovulation, receiving treatment with menotropins for ovarian hyperstimulation, the other one. In the second case, was a spontaneous pregnancy, in a patient with a history of several pelvic surgeries. In this case a bilateral salpingostomy was realized. In both cases was demonstrated chorionic villi by histopathology. These cases are a model of nature to evaluate the real utility of several diagnostic and therapeutic methods which are available nowadays for the treatment of ectopic pregnancy.
