Background’: India is ranked second in the world in terms of tobacco use and tops the list of eig... more Background’: India is ranked second in the world in terms of tobacco use and tops the list of eight high-burden countries that account for two-thirds of the new tuberculosis (TB) cases globally. As tobacco smoking among TB patients is associated with unsuccessful TB treatment outcomes, effective smoking cessation interventions for TB patients is essential to combat this double pandemic. Aim: The study determined the effectiveness of nicotine replacement therapy (NRT) on smoking reduction and cessation among pulmonary TB patients. Settings and design: The study included 300 pulmonary TB patients undergoing antitubercular treatment in government health care centers of Dakshina Kannada district in southern India. Materials and methods: The participants in this two-armed, single-blinded, placebo-controlled, block-randomized trial were divided equally (n = 150) for experimental arm (brief advice + NRT) and control arm (brief advice + placebo). The total duration of treatment and follow-up was 6 months. Unadjusted relative risk (URR) and adjusted relative risk (ARR) were calculated for treatment outcomes at 95% confidence interval (CI). Generalized linear regression analysis was performed upon variables with P value less than 0.2. Results: After 6 months of intervention including follow up, there was a significant reduction in self-reported smoking (ARR = 1.08, 95% CI 1.01-1.17, P = 0.03) in the experimental arm compared with the control arm. Biochemically verified smoking abstinence was not significantly different between the two arms (ARR = 1.01, 95% CI 0.72-1.38, P = 0.97). Conclusion: The study showed statistically significant effectiveness of NRT for self-reported smoking reduction.

The need of study was only because of there are many drugs in market to treat infection but most of the drugs are showing resistance because of the same it is difficult to treat the infection. In this study we chosen coumarin nucleus for study. Thus with the aim of developing novel molecule with improved potency for treating Mycobacterium tuberculosis H37Rv strain infections and with decreased probability of developing drug resistance. The synthesis of coumarin derivatives, starting from salicyaldehyde and ethyl acetoacetate, by conventional organic reaction and results of investigations of their anti-mycobacterial activity. The objective was to find out a co-relation between the SpO2 levels in a patient on admission and the severity of head injury. Methods: Prospective observational study was carried out after the approval of Institutional ethical committee. Patient with head injury coming to the emergency department of Pravara Rural hospital were assessed for the severity using Glasgow Coma scale (GCS) and hypoxic status by using pulse oximeter with bedside monitors. The SpO2 levels were monitored at 2hrs, 4hrs, 8hrs, 24hrs and 48hrs by pulse oximeter. At the time of discharge outcome was assessed again by Glasgow Outcome Scale (GOS) and the treatment that followed it. Results & Conclusions: Oxygenation status of patient on admission affects the prognosis of the patient in terms of residual neurological deficit as assessed by the Glasgow Outcome scale. There is significant co-relation between the Oxygen saturation at the time of admission and the outcome at the time of discharge. Therefore the SpO2 values are significant in patients with head injury as a prognostic factor.

Adverse drug reactions pose a major health problem worldwide. Drug reactions range from minor mac... more Adverse drug reactions pose a major health problem worldwide. Drug reactions range from minor maculopapular rash to erythema multiforme to severe Stevens Johnson Syndrome (SJS) or Toxic epidermal necrolysis (TEN). SJS is a life threatening vesicobullous disease characterised by an acute eruption that involves skin and mucous membranes. Here we present 4 cases of Stevens Johnson Syndrome which came to Pravara Rural Hospital, Loni, Ahmednagar, Maharashtra. First case is a 65 year old female presenting with severe mucocutaneous bleeding with exudative haemorrhagic ulcers on the back and chest next day after ingestion of ceftriaxone. Second case is a 74 year old elderly female presenting with peeling of the skin, blisters on neck, face, mouth, groin, chest region with ulcers in the oropharynx. The patient had history of consumption of ceftriaxone after which the skin lesions started. Third case is a 69 year old female presenting with dry scales with itching all over the body after taking ceftriaxone leading to Stevens Johnson Syndrome. The fourth case is a 22 year old female with history of ingestion of unknown medication from a local doctor and presented to our hospital with severe exudative ulcers on the mouth, lips and chest diagnosed to be Stevens Johnson syndrome. This case series highlights the importance of prescription of commonly used antibiotic like ceftriaxone with extreme caution. Early identification of severe adverse drug reactions can reduce the morbidity and mortality in these patients.

For authentication of of Achyranthes aspera A. aspera Herbarium of plant was made and sent to Biological Survey of India, Pune. The collected material was proceeded for air dry at 35-40º C and then it was pulverized in electric grinder. Extraction of obtained powder was completed in ethanol,Water-ethanol and Petroleum ether by using Soxhlet Apparatus.. The presence of Steroids, flavonides, phenolic compound, glycosides, tannins, saponins, alkaloids and carbohydrates were revealed by phytochemical screening. The extract obtained were of aerial part of screened for anti-asthmatic activity. The Petroleum ether extract of root part was result the right side shift of dose Achyranthes aspera response curve in isolated goat chain and isolated guinea pig so it is indicating antiasthmatic action of drug extract.

Background: The prevalence of hypertension is rapidly increasing among both rural and urban popul... more Background: The prevalence of hypertension is rapidly increasing among both rural and urban populations and is a matter of concern in India. L-Arginine serves as a principle substrate for nitric oxide production which is a potent vasodilator. Aims and Objectives: This study was designed to determine the effects of oral L-arginine on blood pressure in hypertensive patients on antihypertensive drugs. Materials and Methods: This was a randomized clinical trial, registration in Clinical Trial Registry of India (CTRI) Registration number: CTRI/2019/03/018026. All patients with hypertension visiting the medicine outpatient department of Dr. Vitthalrao Vikhe Patil Pravara Rural Hospital, Loni, were included in the study. Study participants were randomized into intervention group (I group) and control group (C group). The I group includes 74 participants and the C group includes 75 participants. The I group received antihypertensive therapy along with add-on L-arginine oral supplementation for 14 days. The C group received only standard antihypertensive therapy and had followed up similar to that of the participants of the I group. Results: The baseline systolic blood pressure (SBP) and diastolic blood pressure (DBP) of the I group and C group was statistically non-significant as analyzed by unpaired t-test, whereas in the I group, there was a significant decrease in SBP and DBP on the 1 st , 2 nd , and 3 rd follow-up visits compared to the C group (Friedman test). Conclusion: The findings of the present study showed that add-on L-arginine supplementation at a dose of 3 g/d (L-arginine 5 g sachet) for 2 weeks in patients of hypertension resulted in significant decrease in the SBP and DBP.

Background: The present study was done with an objective to evaluate the effect of melatonin as a... more Background: The present study was done with an objective to evaluate the effect of melatonin as an antioxidant in patients suffering from periodontitis. Methods: This study was conducted in the Department of Periodontics, Rural Dental College, Loni. Patients of chronic periodontitis, of age between 18 to 65 years of either gender ready to give informed consent to participate in the study were included. Postoperative patients, patients having night duties, drivers and those using heavy machinery, pregnant women, lactating mothers, patients with any clinically significant systemic disease and patients on any other drugs were excluded from the study. Patients were divided into three Groups. Group A included patients who underwent SRP (Scaling and Root Planning) alone, Group B included patients who underwent SRP & supplemented with Vitamin E 200 IU daily at night for 4 weeks. Group C included patients who underwent SRP & supplemented with tablet melatonin 3 mg daily at night for 4 weeks. Estimation of Malondialdehyde (MDA) for serum lipid peroxidation8, Superoxide dismutase (SOD) and Glutathione peroxidase (GPx) was done on day 0, day 30, day 60 and day 90. Results: A total 240 patients were enrolled in the study. It was demonstrated that there was considerable oxidative stress in periodontitis patients, as established by high serum MDA levels, which was reduced significantly by melatonin reflecting its antioxidant potential. Pretreatment levels of SOD and GPx also were low, which were improved with the treatment of melatonin far better than with vitamin E. Conclusions: Melatonin acts as an antioxidant in the patients of periodontitis which has positive effect on biochemical parameters of periodontitis, conferring a new facet to the management of periodontitis and an attempt to impede the disease progression.

In Phase I, the most commonly used tobacco cessation applications with the highest rating from Google Play Store were selected. The most relevant, positive and critical reviews of these apps were recorded. In Phase II, tobacco users with smart phone & internet connectivity and ready to give verbal consent were included in the study. The same apps were allocated to the study participants. At the end of the 2nd week of app use, the participants were subjected to a questionnaire for feedback on liked, disliked & most used features, frequency of app use, utility, expectations, and suggestions for app. The selected apps were QuitNow, Smoke Free, Quit Tracker, Stop Smoking – EasyQuit and Quit Smoking – SSC. The aim of study was to evaluate anti-asthmatic activity root part of. A. aspera. Collection of root part of is done from Rahata District Ahmednagar (Maharashtra). For authentication of of Achyranthes aspera A. aspera Herbarium of plant was made and sent to Biological Survey of India, Pune. The collected material was proceeded for air dry at 35-40º C and then it was pulverized in electric grinder. Extraction of obtained powder was completed in ethanol,Water-ethanol and Petroleum ether by using Soxhlet Apparatus.. The presence of Steroids, flavonides, phenolic compound, glycosides, tannins, saponins, alkaloids and carbohydrates were revealed by phytochemical screening. The extract obtained were of aerial part of screened for anti-asthmatic activity. The Petroleum ether extract of root part was result the right side shift of dose Achyranthes aspera response curve in isolated goat chain and isolated guinea pig so it is indicating antiasthmatic action of drug extract.

L-Arginine serves as a principle substrate for nitric oxide production which is a potent vasodilator. Aims and Objectives: This study was designed to determine the effects of oral L-arginine on blood pressure in hypertensive patients on antihypertensive drugs. Materials and Methods: This was a randomized clinical trial, registration in Clinical Trial Registry of India (CTRI) Registration number: CTRI/2019/03/018026. All patients with hypertension visiting the medicine outpatient department of Dr. Vitthalrao Vikhe Patil Pravara Rural Hospital, Loni, were included in the study. Study participants were randomized into intervention group (I group) and control group (C group). The I group includes 74 participants and the C group includes 75 participants. The I group received antihypertensive therapy along with add-on L-arginine oral supplementation for 14 days. The C group received only standard antihypertensive therapy and had followed up similar to that of the participants of the I group. Results: The baseline systolic blood pressure (SBP) and diastolic blood pressure (DBP) of the I group and C group was statistically non-significant as analyzed by unpaired t-test, whereas in the I group, there was a significant decrease in SBP and DBP on the 1 st , 2 nd , and 3 rd follow-up visits compared to the C group (Friedman test). Conclusion: The findings of the present study showed that add-on L-arginine supplementation at a dose of 3 g/d (L-arginine 5 g sachet) for 2 weeks in patients of hypertension resulted in significant decrease in the SBP and DBP.

Background: The present study was done with an objective to evaluate the effect of melatonin as a... more Background: The present study was done with an objective to evaluate the effect of melatonin as an antioxidant in patients suffering from periodontitis. Methods: This study was conducted in the Department of Periodontics, Rural Dental College, Loni. Patients of chronic periodontitis, of age between 18 to 65 years of either gender ready to give informed consent to participate in the study were included. Postoperative patients, patients having night duties, drivers and those using heavy machinery, pregnant women, lactating mothers, patients with any clinically significant systemic disease and patients on any other drugs were excluded from the study. Patients were divided into three Groups. Group A included patients who underwent SRP (Scaling and Root Planning) alone, Group B included patients who underwent SRP & supplemented with Vitamin E 200 IU daily at night for 4 weeks. Group C included patients who underwent SRP & supplemented with tablet melatonin 3 mg daily at night for 4 weeks. Estimation of Malondialdehyde (MDA) for serum lipid peroxidation8, Superoxide dismutase (SOD) and Glutathione peroxidase (GPx) was done on day 0, day 30, day 60 and day 90. Results: A total 240 patients were enrolled in the study. It was demonstrated that there was considerable oxidative stress in periodontitis patients, as established by high serum MDA levels, which was reduced significantly by melatonin reflecting its antioxidant potential. Pretreatment levels of SOD and GPx also were low, which were improved with the treatment of melatonin far better than with vitamin E. Conclusions: Melatonin acts as an antioxidant in the patients of periodontitis which has positive effect on biochemical parameters of periodontitis, conferring a new facet to the management of periodontitis and an attempt to impede the disease progression.
