Thyroid hormones exert their effect on all tissue and modulate the rate of metabolic activity. Al... more Thyroid hormones exert their effect on all tissue and modulate the rate of metabolic activity. Alterations in thyroid function can affect the various organ system of body and perturb measures like AST, ALT, GGT, ALP, CPK and LDH. Both hypothyroidism and hyperthyroidism have potentially fatal systemic manifestations. The main aim of study was to determine the relationship between thyroid alteration and serum enzymes level. The study included 110 subjects visiting Endocrinology Unit of Department of Biochemistry, MTH, Pokhara, with the request of TFT. Thyroid profile and serum enzymes were analyzed using standard kits. Each thirty hyperthyroid and hypothyroid cases show slight elevation of AST, ALT and GGT. However, the values were higher in hyperthyroid subjects. ALP, CPK and LDH were markedly elevated both in hyperthyroidism and hypothyroidism as compared to controls. User perceived quality is the most important aspect of Internet applications. After a single nega... more User perceived quality is the most important aspect of Internet applications. After a single negative experience, users tend to switch to one of the other myriad of alternatives available to them on the Internet. Two key components of Internet application quality are scalability and reliability. In this paper, we present the first general-purpose mechanism capable of maintaining reliability in the face of process, machine, and catastrophic failures. We define catastrophic failures as events that cause entire clusters of servers to become unavailable such as network partitioning, router failures, natural disasters, or even terrorist attacks. Our mechanism utilizes client-side tunneling, clientside redirection, and implicit redirection triggers to deliver reliable communication channels. We capitalize on previous work, Redirectable Sockets (RedSocks), that focuses on Internet application scalability. RedSocks are communication channels enhanced with a novel session layer aimed at modernizing network communication. We modify Red-Socks to create the first fault tolerant socket solution that can handle all server-side failures. Our mechanism is compatible with NATs and Firewalls, scalable, application independent, and backwards compatible.

Most implementations of HTTP servers do not distinguish among requests to differeut pages. This h... more Most implementations of HTTP servers do not distinguish among requests to differeut pages. This has the implicatiou that requests for popular pages have the tendency to overwhelm the requests for other pages. In addition, HTTP servers do not allow a site to specify policies for server resource allocation. This paper presents a notion of wality of service that enables a site to customize how an HTTP server should respond to external requests by setting priorities among page requests and allocating server resources. It also describes a design and an implementation of a distributed HTTP server, QoS Web Server, that enforces the quality of service constraints. The performance aualysis of the prototype server indicates that the server provides the desired quality of service with minimal overhead.

Middle East countries are the major destination for abroad job among Nepalese workers. This study carried out to identify the health problems among migrant workers and their access to health care in Saudi Arabia, United Arab Emirates (UAE) and Qatar of Middle East. Methods: This was a cross-sectional study; carried out among 480 returnee migrant workers who have given consent for the study. Data were collected at Tribhuvan International Airport (October 2018) using pretested structured interview schedule after taking approval from Institutional Review Committee of Pokhara University. Data were analyzed using SPSS 20 version. Percentage, mean/median, standard deviation, Chi square test and logistic regression performed. The main objective of this research is to assess the self-care practices, treatment satisfaction and quality of life among diabetes type2 patient in Pokhara. MATERIALS AND METHODS: A cross-sectional study was conducted among the diabetes type2 patients from June 2019 to December 2019 in Pokhara Metropolitan. The sample size for this study was 248. Consecutive sampling technique was used for the selection of participants. Interview schedule was used for doing face to face interview. A collection of questions adapted from Diabetes-39 and it was used as a tool for assessing the quality of life of the participants. The main objective of the study was to assess the nutritional status and quality of life in the geriatric population of Lahan municipality of Siraha district. Methods. A cross-sectional analytical study was conducted in Lahan municipality of Siraha district from June to December 2017. The Mini-Nutritional Assessment tool was used to investigate the nutritional status, and World Health Organization Quality of Life-OLD questionnaires were used to assess the quality of life among geriatric population. Result. Out of the total participants, one-third (45.7%) of the participants were at risk of malnutrition and 19.8% were malnourished while 34.5% had normal nutritional status. It was seen that 48.2% of participants had good quality of life whereas 51.8% of them had poor quality of life. Every year more than 9 million new cases are reported worldwide with death rate of around 2 million each year. Rapid and accurate diagnosis of tuberculosis in population is most supportive in proper medication and early recovery. The golden standard method for diagnosis of tuberculosis in patient is formerly culture method which is time consuming and cumbersome. Various other techniques used in the diagnosis of tuberculosis have poor sensitivity and specificity however serum adenosine deaminase (ADA) has emerged as a potent biochemical marker that can be used as tool for the diagnosis of tuberculosis for rapid, easy and better result. Increased level of ADA in blood generally indicates the presence of tuberculosis. In the present study, the serum ADA level was examined in 75 healthy control and 75 tuberculosis patients with positive sputum smear for acid fast bacilli (AFB), having clinical symptoms were diagnosed for pulmonary tuberculosis and radiological impression for extra pulmonary tuberculosis. The selection of cases and control were based on the inclusion and exclusion criteria. Samples were collected at the outpatient department of National Tuberculosis Centre, Nepal after asking simple questionnaires. Modified Guisti and Galanti method was adopted for estimation of serum ADA from pooled and processed blood samples. There was significant difference (p<0.001) in mean rank of level of serum ADA among the TB cases to controls. The P-TB had the highest mean rank (116.52) followed by EP-TB (99.48) and control had significantly less mean rank (40.16). At the cutoff point of 25 U/L; sensitivity, specificity, positive predictive value and negative predictive value were 90.7%, 100%, 90.66% and 100% respectively. It can be concluded from data that there were insignificant difference in mean rank among sex wise distribution with p=0.037 and sputum grading wise distribution with p=0.142.

The study aimed to determine the prevalence of multidrug resistant and extended spectrum beta-lactamase producing gram negative bacterial uropathogens among females in a tertiary level hospital. Methods: A hospital-based cross-sectional study was conducted in Pokhara Academy of Health Sciences, Pokhara from February to November, 2019. A total number of 301 midstream urine samples were collected and examined using MacConkey agar and blood agar medium. Antimicrobial susceptibility testing was done by Kirby Bauer disk diffusion method on Mueller Hinton agar using Clinical and Laboratory Standards Institute guidelines. To estimate the prevalence of smear-positive pulmonary tuberculosis (TB). In this cross-sectional study, adults aged >14 years were surveyed by home-to-home visit, and asked about cough of > or = 2 weeks with or without sputum, chest pain or difficulty in breathing. Symptomatic suspects submitted three sputum samples for standard smear microscopy. Of 16697 adults surveyed, 436 (2.6%) were symptomatic and submitted sputum samples. Thirteen (3%) were positive for acid-fast bacilli, and the prevalence of smear-positive TB was 78 per 100 000 population (95%CI 36-120). Twenty-four smear-positive cases identified through the existing health care delivery were on anti-tuberculosis medication at the time of the survey.
