The effect of electromagnetic field stimulation on corticosteroids-inhibited intestinal wound healing

Electromagnetic field (EMF) stimulation has been used successfully in the clinical setting to pro... more Electromagnetic field (EMF) stimulation has been used successfully in the clinical setting to promote healing of ununited fractures. In a few studies, EMF stimulation enhanced soft tissue healing. To investigate the effect of EMF stimulation on intestinal wound healing in normal rats and in those treated with corticosteroids, 80 Wistar rats received twice-daily injections of either saline dexamethasone (0.1 mg/kg/day for 2 weeks. Animals then underwent creation of single-layer, inverting small intestine anastomoses. All injections were continued postoperatively. Animals were grouped as intestinal anastomoses; intestinal anastomoses plus EMF stimulation; intestinal anastomoses plus dexamethasone; and intestinal anastomoses plus dexamethasone plus EMF. On postoperative days 7 and 14, the anastomosed intestines were removed and the tensile strength (TS) and hydroxyproline (OH-P) contents measured. This study was planned to investigate the extent of cellular damage of the contralateral ovary in... more This study was planned to investigate the extent of cellular damage of the contralateral ovary in unilateral ovarian torsion and to determine possible changes histopathologically by light and ultrastructurally by electron microscopy. Unilateral ovarian torsion was performed in 21 prepubertal, female New Zealand rabbits. One, 8 and 24 hours after the torsion, contralateral ovaries were biopsied. The control group consisted of rabbits without any torsion. Contralateral cellular damage was demonstrated after unilateral torsion. Oocytic nuclei, mitochondria, lysosomes and ovarian stromal cells were also affected when evaluated ultrastructurally.

Özet Objective: Although cognitive behavioral model of Internet addiction has been well described... more Özet Objective: Although cognitive behavioral model of Internet addiction has been well described, studies on metacognitions and ruminative response styles related with Internet addiction are very limited. The aim of the present study was to compare metacognitions and ruminative response style in Internet addicts with a healthy control group. Method: The study included 30 males who presented to our Internet Addiction Outpatient clinic, and diagnosed with Internet addiction, and a control group of 30 healthy males with similar sociodemographic characteristics. A sociodemographic data form, Internet Addiction Test (IAT), Metacognitions Questionnaire (MCQ-30), Ruminative Response Scale-short version (RRS-SV), and Beck Depression Inventory (BDI) were used for data collection. Results: The MCQ-30 total, MCQ-30 uncontrollability and danger score, MCQ-30 need to control thoughts score and RRS-SV scores statistically significantly higher in study group compared the control group. After correcting for BDI by ANCOVA, the difference between MCQ-30 total score and RRS-SV disappeared. Conclusion: Internet addicts show ruminative responses instead of having an effective problem-solving attitude and defining problems; and this self-focused rumination leads an individual to recall more reinforced memories about the Internet so that the problem of Internet addiction becomes deeper. As a result of this study, although Internet addiction is accompanied by depression primarily or secondarily, manifestation of Internet addiction is exacerbated by depression through ruminative responses and metacognitions.

Vigabatrin is an antiepileptic that functions by increasing ɣ-aminobutyric acid (GABA) levels in ... more Vigabatrin is an antiepileptic that functions by increasing ɣ-aminobutyric acid (GABA) levels in the brain. Like other antiepileptics, it also causes neuropsychiatric side effects. Among these side effects, psychosis draws particular attention. Drug-induced psychosis has been reported in previous publications at the beginning of the drug, while psychosis reports during the cessation period are limited. We discussed the possible mechanisms of psychosis that can be seen in the use of vigabatrin in the light of the assumptions that GABA, glutamate and dopamine are involved in the formation of positive symptoms in psychosis.

Objective: Although the Internet is used effectively in many areas of life, some users experience... more Objective: Although the Internet is used effectively in many areas of life, some users experience problems because of over-use due to a lack of control. The diagnostic criteria for Internet addiction include disruptions in family relationships, but adequate data on the attachment styles and family functioning associated with this condition are limited. This study aimed to investigate the attachment styles and family functioning of patients with Internet addiction. Method: The sample included 30 male patients consecutively admitted to the Bakırköy Mental Health and Research Hospital Internet Addiction Outpatient Clinic, who were diagnosed in clinical interviews as having Internet addiction according to Young's (1998) criteria. Thirty healthy males who were matched with the experimental group in terms of sociodemographic characteristics were included as control subjects. Both groups provided sociodemographic data and completed the Beck Depression Inventory (BDI), the Experiences in Close Relationships Questionnaire-r (ECR-r) and the Family Assessment Device (FAD). Results: Patients with Internet addiction had higher BDI scores (Pb.001) and higher attachment anxiety subscores on ECR-r (Pb .001) compared with those in the control group. Patients with Internet addiction evaluated their family functioning as more negative and reported problems in every aspect addressed by the FAD. Scores on the FAD behaviour control, affective responsiveness, and problem-solving subscales (Pb .05) and on the FAD communication, roles, and general functioning subscales (Pb.001) were significantly higher in the patient compared with the control group. Conclusion: Patients with Internet addiction have more anxious attachment styles as well as prominent disruptions in family functioning. Thus, it may be important to evaluate the attachment styles and family functioning of patients with Internet addiction. Indeed, comprehensive treatment approaches including other family members may make important contributions to treatment success.

A Bronchopleurocutaneous fistula caused by an unusual foreign body aspiration simulating acute abdomen

A 12-year-old boy was admitted to hospital for abdominal pain, vomiting and fever. On physical ex... more A 12-year-old boy was admitted to hospital for abdominal pain, vomiting and fever. On physical examination he had râles on the lower right hemithorax without any respiratory complaints. Chest X-ray revealed a condensation in the right lower chest. Abdominal findings were secondary to lobar pneumonia. Treatment of pneumonia with antibiotics showed no improvement over 2 days. On bronchoscopy no foreign body was seen, but pus was aspirated. Two days later a mass appeared on the right hemithorax and fistulized. An organic foreign body, Hordeum murinum, with 3-5 ml of pus was observed. Chest X-ray taken at the day of fistulization showed no pneumothorax or subcutaneous emphysema. Less than 11 cases of pneumocutaneous fistulas secondary to aspiration of grasses have been reported in literature. Why an ear of Hordeum murinum can migrate only in a forward direction and why a pneumothorax had not developed is discussed.

Objective: Our objective was to identify the validity, reliability and psychometric features of the Turkish adaptation of the Generalized Anxiety Disorder-7 (GAD-7) scale. Method: Patients who were diagnosed with 110 GAD patients and 112 healthy control subjects were included in this study. Three weeks after the initial administration, a certain group of the sample was administered the GAD-7 scale again. The principal components analysis for structural validity and confirmatory factor analysis were performed. Receiver operating characteristics (ROC) curve analysis was performed to identify the distinguishing feature. Positive predictive value, negative predictive value, positive and negative likelihood ratios were calculated at different cut-off levels. The total scores for the GAD-7 scale-Turkish version in the GAD group were found to be statistically significantly higher than those in the control group. A significant correlation was observed between all items of the GAD-7 scale -Turkish version that was found to have good internal consistency and intermittent administration. In the factor analysis performed for assessing the construct validity, the factor structure of the GAD-7 scale-Turkish version was found to be similar to the original form and the construct validity was found to be good. Good compliance to single factor structure was observed in the confirmatory factor analysis. The most acceptable cut-off point of the GAD-7 scale-Turkish version was found to be 8. The GAD-7 scale-Turkish version, similar to the original form, was found to have a high validity, reliability and good psychometric features in a clinical sample. The Turkish version of the GAD-7 scale was regarded as a useful screening tool.

Motivational interviewing is important in the treatment of Internet addiction as it is in all behavioral dependencies. This case study includes three different patients with specific internet addiction who presented to the Internet Addiction Clinic of Bakirkoy Training and Research Hospital for Psychiatry, Neurology and Neurosurgery. While the term "internet addiction" still remains controversial, disturbed family relationships are considered to be a diagnostic criterion. The use of the internet, even in non-excessive levels, is associated with disturbance in family and social life. As considering from systemic point of view; while family relationships may be disturbed with internet addiction, people who have problems with their family relationships also may use internet excessively. This case report is composed of both the cases with excessive internet usage and those who had problems in complying with the changes in their family systems following the decrease in duration of internet usage during the treatment process.

While the term "internet addiction" still remains controversial, disturbed family relationships are considered to be a diagnostic criterion. The use of the internet, even in non-excessive levels, is associated with disturbance in family and social life. As considering from systemic point of view; while family relationships may be disturbed with internet addiction, people who have problems with their family relationships also may use internet excessively. This case report is composed of both the cases with excessive internet usage and those who had problems in complying with the changes in their family systems following the decrease in duration of internet usage during the treatment process.
