Background: Insular glioma was considered a nonsurgical entity due to resection-associated morbid... more Background: Insular glioma was considered a nonsurgical entity due to resection-associated morbidities. The advancement in neurosurgical techniques and adjuncts used in the last two decades made the resection of insular gliomas simpler for neurosurgeons with the maximum extent of resection and acceptable morbidity rates. The complex anatomy of this region remains a challenge for neurosurgeons and requires expertise. The key factors to achieve complete resection in the insular region are the thorough knowledge of surgical anatomy and meticulous microsurgical techniques. Intraoperative adjuncts such as image guidance along with cortical and subcortical mapping assist in excellent outcomes. Objective: In this study, we describe the operative technique and application of trans-Sylvian approach to do the compartmental dissection done by the senior author, along with challenges faced, with the hope to highlight the efficacy of the approach to achieve maximal resection of this entity without compromising patient's safety. Materials and Methods: A 32-year-old right-handed gentleman presented with episodes of excessive salivation, tingling sensation on the right side of the body along with nausea that lasted for 15–20 s for the last 8 months and one episode of speech arrest without any loss of consciousness and motor or sensory deficit. CEMRI of the brain was suggestive of left insular glioma. The patient underwent left pterional craniotomy, and gross total resection of the tumor was done using compartmental dissection. Results: At follow-up after 1 month, the patient is seizure-free without any speech difficulty and motor or sensory deficit. Conclusion: Compartmental dissection of insular glioma is a safe and efficacious technique to achieve gross total resection of the tumor in this complex region without morbidity.

Objective: The aim of our study was to compare clinicoradiological outcome of the lumbar spinal fusion surgeries (PLF vs. PLIF/transforaminal lumbar interbody fusion [TLIF]), to assess the quality of life pre- and post-operatively in all patients, and to compare the results within the groups. Summary of Background Data: We retrieved the articles related to posterolateral fusion (PLF) and PLIF/TLIF through computer-assisted PubMed and Cochrane database search. Most of the studies in previous literature did not show any significant difference in the success of fusion between the two groups. Intracranial chondromas are extremely rare, benign slow-growing cartilaginous tumors mostly origi... more Intracranial chondromas are extremely rare, benign slow-growing cartilaginous tumors mostly originating from embryonic rests at sphenoethmoidal region and sometimes can originate from the falx, convexity dura, the tentorium, the choroid plexus, or the brain parenchyma. In this article, we present a 22-year-old woman with a chondroma of dural origin. The clinical, radiological, and histopathological findings along with the operative findings and postoperative course are described as well as the pertinent literature regarding intracranial chondromas is reviewed.

The role of early laparoscopic cholecystectomy for acute cholecystitis with cholelithiasis is not yet established. The aim of our prospective randomized study was to evaluate the safety and feasibility of early LC for acute cholecystitis and to compare the results with delayed LC.Methods. Between March 2007 to December 2008, 50 patients with diagnosis of acute cholecystitis were assigned randomly to early group,n=25(LC within 24 hrs of admission), and delayed group,n=25(initial conservative treatment followed by delayed LC, 6–8 weeks later).Results. We found in our study that the conversion rate in early LC and delayed LC was 16% and 8%, respectively, Operation time for early LC was 69.4 min versus 66.4 min for delayed LC, postoperative complications for early LC were 24% versus 8% for delayed LC, and blood loss was 159.6 mL early group versus 146.8 mL for delayed group. Two series of milbemycin analogues containing ester and methoxy oxime groups were designed accord... more Two series of milbemycin analogues containing ester and methoxy oxime groups were designed according to analog synthesis and sub-structure ligation from deglycosyl ivermectin, which is the analogue of milbemycin. The analogues reaction was carried out with the corresponding aromatic chloride, and the structures of the target compounds were confirmed by 1 H NMR and high resolution mass spectra (HRMS). The insecticidal activities of 18 analogues against Tetranychus cinnabarinus, Mythimna sepatara and Aphis fabae were tested. The results showed that all analogues had different degree of insecticidal activity, especially compounds 4IIa and 4IIb showed the highest insecticidal activities against Mythimna sepatara and Aphis fabae .

Matched filters are the optimal linear filters for signal detection under linear channel and whit... more Matched filters are the optimal linear filters for signal detection under linear channel and white noise conditions. Their optimality is guaranteed in the additive white Gaussian noise channel due to the sufficiency of second order statistics. In this paper, we introduce a nonlinear filter for signal detection based on the Cauchy-Schwartz quadratic mutual information (CS-QMI) criterion. This filter is still implementing correlation but now in a high dimensional transformed space defined by the kernel utilized in estimating the CS-QMI. Simulations show that the nonlinear filter significantly outperforms the traditional matched filter in nonlinear channels, as expected. In the linear channel case, the proposed filter outperforms the matched filter when the received signal is corrupted by impulsive noise such as Cauchy-distributed noise, but performs at the same level in Gaussian noise. A simple nonparametric sample-estimator for CS-QMI is derived for real-time implementation of the proposed filter.
