There are remarkable geographical variations in the accessibility to emergency medical services (EMS). Moreover, transport delays have been documented to affect stroke outcomes. This study aimed to examine the spatial variations in in-hospital mortality among patients with symptoms of stroke transferred by EMS, and determine its related factors using the auto-logistic regression model. Methods In this historical cohort study, we included patients with symptoms of stroke transferred to Ghaem Hospital of Mashhad, as the referral center for stroke patients, from April 2018 to March 2019. The auto-logistic regression model was applied to examine the possible geographical variations of in-hospital mortality and its related factors. All analysis was performed using the Statistical Package for the Social Sciences (SPSS, v. 16) and R 4.0.0 software at the significance level of 0.05. Results In this study, a total of 1,170 patients with stroke symptoms were included. The overall mortality rate in the hospital was 14.2% and there was an uneven geographical distribution. The results of auto-logistic regression model showed that in-hospital stroke mortality was associated with age (OR = 1.03, 95% CI: 1.01-1.04), accessibility rate of ambulance vehicle (OR = 0.97, 95% CI: 0.94-0.99), final stroke diagnosis (OR = 1.60, 95% CI: 1.07-2.39), triage level (OR = 2.11, 95% CI: 1.31-3.54), and length of stay (LOS) in hospital (OR = 1.02, 95% CI: 1.01-1.04). Conclusion Our results showed considerable geographical variations in the odds of in-hospital stroke mortality in Mashhad neighborhoods. Also, the age-and sex-adjusted results highlighted the direct association between such variables as accessibility rate of an ambulance, screening time, and LOS in hospital with in-hospital stroke mortality. Thus, the prognosis of in-hospital stroke mortality could be improved by reducing delay time and increasing the EMS access rate.

Objectives: Hypertension is a global major health challenge and mechanisms related to the risk fa... more Objectives: Hypertension is a global major health challenge and mechanisms related to the risk factors associated with it are poorly understood. Therefore, we used structural modeling to test a hypothesized model to identify factors associated with hypertension. Methods: A cross-sectional population based survey, was performed and the data related to a random representative sample of 9704 subjects of MASHAD study were used. Then, we determined the relationship between risk factors for hypertension using structural equation modeling technique. The data were analyzed using Amos V. 22. Results: The conceptual model was validated by Goodness of Fit Indexes (CFI=0.939, TLI=0.908, NFI=0.935, RMSEA=0.04, SRMR=0.037). Obesity and lack of physical activity had the greatest impact on blood pressure. Discussion: Findings show evidences to confirm the conceptual model considered in the risk factors for hypertension that can be helpful in policies for preventing hypertension and consequently, the disabilities that arise from it. Highlights ● Fit indices indicate that the first proposed model fits poorly to the data. P values associated with SES, poor diet, and physical activity with blood pressure and also age and triglycerides were lower than 0.01, so their direct effects were removed. ● After modifications, the model showed a good fit with the sample data. CFI and TLI and NFI index values were greater than 0.9; RMSEA and SRMR were less than 0.05. ● Waist to hip ratio (WHR) appears to be more appropriate to represent obesity according to factor loadings. ● There are risk factors that do not directly increase blood pressure but indirectly affect it. For example, physical activity and diet may not directly increase blood pressure but they can be affected by other variables that cannot be ignored. ● Age, obesity, and physical activity were the most important factors on increasing blood pressure. Plain Language Summary Given the importance of hypertension in health sciences, we used structural equation modeling as a powerful statistical method to examine hypertension risk factors. A conceptual model was proposed about relationships between risk factors of hypertension. The model suitability was confirmed with available information of 9704 participants in MA-SHAD study. Findings highlighted the value of direct and indirect effects and indicated that age, obesity and physical activity were the most influential factors on increasing blood pressure but physical activity had indirect effect on blood pressure. It appears that choosing a healthy lifestyle, including sufficient physical activity and preventing obesity are essential for prevention and management of hypertension.

Materials and Methods:This cross-sectional study was performed on9761 cases within the age range of 35-65 years in Mashhad. The mean scores of variables, such as body mass index, waist circumference, systolic and diastolic blood pressure, fast blood sugar, total cholesterol, triglyceride, and high-density lipoprotein (HDL) were compared in two genders, and the correlations among them were calculated. In order to investigate the structure of these variables, exploratory factor analysis was applied. Results:The results of factor analysis showed a different factorial structure between the two genders. There were a three-facto...
