To evaluate central opioid activity in women with prospectively documented premenstrual syndrome ... more To evaluate central opioid activity in women with prospectively documented premenstrual syndrome (PMS) and control women in the mid- and late luteal phases of the menstrual cycle. Blood was collected every 15 minutes 1 hour before (0800) and 2 hours after treatment (0900-1100). The treatment was administered in a randomized fashion and consisted of naloxone 1 or 4 mg or placebo, and blood was assayed for luteinizing hormone (LH). Baseline estradiol, progesterone, and prolactin were measured at 0800 and 0900 hours. There was a significant increase in LH area under the curve and mean LH in response to naloxone in the midluteal phase in the control (P < .001). The PMS subjects did not display a significant increase in LH concentration in response to naloxone in the midluteal phase. There were no significant LH responses to naloxone in either group in the late luteal phase. There were no significant differences in estradiol, progesterone, or prolactin concentrations or estrogen to progesterone ratios between groups. Control women have an enhanced central opioid tone during the midluteal phase that diminishes and becomes minimal in the late luteal phase of the menstrual cycle. In contrast, women with PMS have a loss of central opioid tone during the midluteal phase as indicated by the loss of LH response to naloxone. This attenuated central opioid tone in women with PMS as compared with asymptomatic control women may play a role in the pathophysiology of PMS.

Ninety-eight presumably normal London men, age 20-35 and sexually active in a stable relationship... more Ninety-eight presumably normal London men, age 20-35 and sexually active in a stable relationship, responded to an invitation at their work place to partieipate in a detailed sexuality interview. The interview was developed by a group of WHO collaborators to measure possible effects on sexuat functioning resulting from various medical regimens (for example, a male contraceptive pill). It assessed frequency of coitus and masturbation during the previous 4 weeks, a variety of subjective ratings of sexual interest, satisfaetion, and quality of relationships, as well as reports of the nature and incidence of various sexual problems. The study sample's responses displayed internal consistency and in general supported the inferenee that the sample was not atypical. The data appear to support Westoff's (1974) notion that the frequency of eoitus has inereased since the early normative reports by Kinsey et al. (1948). Most importantly, the results of principal components analyses point to the fact that overall sexual drive or "'libido" (e.g., frequency of sexual behavior) is independent of several other possibly significant dimensions, including latency to orgasm, quality of sexual experience, autoeroticism, and the incidenee of erectile diffieulties. It is concluded that the sexuality interview provides potentially useful baseline data against which to evaluate effeets of sexual therapy or drug regimens.

Episiotomy is one of the most commonly performed operative procedures and yet little information ... more Episiotomy is one of the most commonly performed operative procedures and yet little information is available on the subjective reactions in the puerperium to this procedure. The present study was designed to furnish information on the attitudes of patients, levels of pain, and of course recovery by studying a consecutive series of 101 Caucasian primiparea who received episiotomies at delivery. Women were interviewed within 24 hours of delivery and then, at three months after delivery, completed a questionnaire. The high level of pain experienced was noteworthy. Labour pain and episiotomy pain were uncorrelated, indicating the importance of distinguishing between them. Several women were experiencing problems at the three-month follow-up, with some attributing these to the episiotomy repair. The data are presented in the framework of providing women in the postpartum period with systematic information on the nature of postepisiotomy pain and subsequent recovery to facilitate their adjustment.
