Methods: Patients with gastrectomy and D2 lymph node dissection for gastric cancer (GC) were retrospectively evaluated. Sarcopenia was diagnosed with computed tomography (CT) scan for preoperative staging. Post-operative complications were determined according to Clavien-Dindo (CD) classification. Results: Sarcopenia was seen in 59 (40.7%) of the 145 patients included in the study. The rate of overall postoperative complications in sarcopenic patients was higher than in non-sarcopenic patients. Surgical complications and medical complications were higher in sarcopenic patients. Severe surgical complications (CD Grade III and above) were significantly higher in the sarcopenia group. Sarcopenia (4.63, 95% confidence interval, 1.76-12.18; p=0.002) was a prognostic risk factor for postoperative complications. Conclusion: Sarcopenia is associated with early postoperative complications in many types of cancer. This study showed that sarcopenia was associated with complications after D2 lymph node dissection in GC surgery. CT use in pre-operative clinical staging of patients and taking the necessary precautions make it easier to address post-operative complications.

Introduction: Even though lifelong premature ejaculation (PE) is highly prevalent, few studies ha... more Introduction: Even though lifelong premature ejaculation (PE) is highly prevalent, few studies have investigated the neural mechanisms underlying PE. Aim: This study aimed to investigate whether patients with lifelong PE exhibit macrostructural or microstructural alterations of the parts of the brain involved in the male sexual response. Materials and Methods: We enrolled 42 healthy participants and 54 lifelong PE patients. Lifelong PE was diagnosed according to the Premature Ejaculation Diagnostic Tool (PEDT) and intravaginal ejaculation latency time (IELT). We compared measures of cortical morphology, such as volumes of gray matter, white matter, cerebellum volumes, and subcortical structures (ie, amygdala, caudate, hippocampus, globus pallidus, putamen, and thalamus) between the groups using a voxel-based morphometry method from whole-brain T1-weighted magnetic resonance imaging. Moreover, we evaluated the relationships between the relevant cerebral alterations and the severity of symptoms obtained from participants via self-reported questionnaires. Main Outcome Measures: Cerebral macrostructural and microstructural alterations were assessed in PE patients and controls, along with the correlation of caudate nucleus changes in PE patients with clinical data (including the PEDT and the IELT). Results: The mean volume of the caudate nucleus was significantly larger in the lifelong PE patients compared with healthy controls (P ¼ .048). Moreover, caudate nucleus volume was positively correlated with PEDT score (r ¼ 0.621; P ¼ .0179) and negatively correlated with the IELT (r ¼ À0.592; P ¼ .0101). However, cortex morphology and the other subcortical volumes were not significantly different between the 2 groups (P > .05). Clinical Implications: Microstructural alterations in deep gray matter nuclei might be a useful parameter for studying the mechanism of the neurobiology underlying PE. Strengths and Limitations: There are few studies examining microstructural changes in PE patients. This study furthers our understanding of the etiology of PE. Limitations include the small sample, which limits our ability to make an absolute determination as to whether such subcortical changes are the cause or the consequence of lifelong PE. Conclusions: We found a significant difference in caudate nucleus volume between patients with PE and healthy controls. In addition, the caudate nucleus volume was positively associated with the severity of PE symptoms. More extensive and possibly longitudinal studies are needed to improve our understanding of the mechanism of the neurobiology underlying PE.

Objective: We determined the temporal changes on thoracic computed tomography (CT) in patients wi... more Objective: We determined the temporal changes on thoracic computed tomography (CT) in patients with Coronavirus disease-2019 (COVID-19) pneumonia, during the disease. Methods: Our sample included 55 patients (29 men, 26 women) who were admitted to our hospital between March 10 and May 10 2020 with COVID-19, diagnosed by reverse-transcription polymerase chain reaction, who underwent at least two CT examinations in our hospital. From the onset of the symptoms, the disease process was divided into six stages, and CT patterns at each stage were analyzed. Additionally, we calculated the CT score by using a semiquantitative method to measure the involvement in all five lung lobes. Results: The most common CT patterns in all stages were ground-glass opacification, crazy-paving pattern, and consolidation. The total CT scores were highest between the 10 th and 21 st days. This pattern continued similarly for a while later. Residual signs of disease were found in 92% of the patients who underwent CT after the 28 th day. Conclusion: CT findings in COVID-19 pneumonia progress in a certain time-dependent pattern. CT findings of the disease is most severe between the 10 th and 21 st days. Residual disease findings are observed even after 28 days in most patients.

The chances of curative surgery are very low. It is usually unresectable. We aim to present a case of isolated adrenal metastasis that developed in a patient, who underwent a total gastrectomy with the diagnosis of gastric cancer approximately 26 months back. Left adrenalectomy was planned with curative intent. R0 resection was performed. The patient was followed up for one year after surgery. The option of surgical treatment is recommended for isolated metachronous adrenal metastases. Curative surgical resection may positively impact the prognosis of patients in selected cases.

To the best of our knowledge, this is the first case report of RS3PE associated with saxagliptin. The patient presented with acute onset of swelling and pain in multiple peripheral joints, swelling of the dorsum of the hands and feet occurred a month after the initiation of saxagliptin. Laboratory results were remarkable for elevated acute phase reactants. The diagnosis of RS3PE was made through exclusion and was based on acute onset of pitting edema, synovitis shown by ultrasound and magnetic resonance investigation, absence of joint erosions and negative rheumatoid factor. Patient showed a good response to low dose of steroids with a complete and sustained remission. The development of RS3PE following introduction of drug should suggest this drug as a possible trigger for the development of RS3PE.

METHODS We retrospectively analyzed out patient database and evaluated 27 patients who had percutaneous nephrolithotripsy. From the DICOM data of patient's preoperative CT images, stone structures were obtained using an image identification system. All DICOM files were saved in the stereolithography file format and a 3D printed model was created. True stone volume was found by water displacement method using this model and this volume compared with ellipsoid formula and 3D-reconstructed volume. A 25-year-old male patient presented with pulsatile swelling that occurred 1 year after the injury of the left underarm, and left-sided pain, weakness, and difficulty in controlling the left hand. He had left axillary region with 7 x 5 x 3 cm 3 pulsatile firm consistency, left hand 4/5 and left arm 3/5 power loss, left hand holding no objects, and left arm not lifting up to the shoulder level. Color Doppler ultrasonography revealed left axillary artery pseudoaneurysm with dimensions of 56 x 33 mm 2 and luminal thrombosis about 40%. Axillary artery, vein, and nerve suspension were taken after the left axillary artery was found and rotated. The organized thrombus within the sac was removed. Subsequently, aneurysmectomy and axillary artery primer repair were completed. This study aimed to discuss successful surgical repair and physiotherapy that started in the early period in the case that the arm and hand movement limitations were eliminated.

The purpose of this work was to evaluate the feasibility and clinical use of MR angiography (MRA)... more The purpose of this work was to evaluate the feasibility and clinical use of MR angiography (MRA) for examining the pelvic and lower limb arteries in patients with arterial occlusive disease. Seventy-six patients with clinical signs of peripheral arterial occlusive disease were included in the study. MRA was performed using a fast contrast-enhanced high-resolution 3D technique that covered the area from the distal abdominal aorta to the distal lower limbs in two examination steps. In all patients, diagnostic images comparable with those of conventional intraarterial digital subtraction angiography (DSA) could be obtained. No false-negative findings were seen in the iliac, femoral, or popliteal arteries. Ten to 16% of the mild stenoses and 6-14% of the severe stenoses, mainly in the crural vessels, were overgraded compared with intraarterial DSA. Particularly in patients with proximal severe obstructions or occlusions, the crural segments could be depicted more clearly due to decreased arterial runoff in conventional angiography. The consistency of the excellent depiction of the vascular territories of the distal aorta and the pelvic and lower limb arteries in a standardized setting suggests great potential for the use of MRA in the primary diagnosis of peripheral arterial occlusive disease.

A 25-year-old male patient presented with pulsatile swelling that occurred 1 year after the injury of the left underarm, and left-sided pain, weakness, and difficulty in controlling the left hand. He had left axillary region with 7 x 5 x 3 cm 3 pulsatile firm consistency, left hand 4/5 and left arm 3/5 power loss, left hand holding no objects, and left arm not lifting up to the shoulder level. Color Doppler ultrasonography revealed left axillary artery pseudoaneurysm with dimensions of 56 x 33 mm 2 and luminal thrombosis about 40%. Axillary artery, vein, and nerve suspension were taken after the left axillary artery was found and rotated. The organized thrombus within the sac was removed. Subsequently, aneurysmectomy and axillary artery primer repair were completed. This study aimed to discuss successful surgical repair and physiotherapy that started in the early period in the case that the arm and hand movement limitations were eliminated.

The purpose of this work was to evaluate the feasibility and clinical use of MR angiography (MRA)... more The purpose of this work was to evaluate the feasibility and clinical use of MR angiography (MRA) for examining the pelvic and lower limb arteries in patients with arterial occlusive disease. Seventy-six patients with clinical signs of peripheral arterial occlusive disease were included in the study. MRA was performed using a fast contrast-enhanced high-resolution 3D technique that covered the area from the distal abdominal aorta to the distal lower limbs in two examination steps. In all patients, diagnostic images comparable with those of conventional intraarterial digital subtraction angiography (DSA) could be obtained. No false-negative findings were seen in the iliac, femoral, or popliteal arteries. Ten to 16% of the mild stenoses and 6-14% of the severe stenoses, mainly in the crural vessels, were overgraded compared with intraarterial DSA. Particularly in patients with proximal severe obstructions or occlusions, the crural segments could be depicted more clearly due to decreased arterial runoff in conventional angiography. The consistency of the excellent depiction of the vascular territories of the distal aorta and the pelvic and lower limb arteries in a standardized setting suggests great potential for the use of MRA in the primary diagnosis of peripheral arterial occlusive disease.

The chances of curative surgery are very low. It is usually unresectable. We aim to present a case of isolated adrenal metastasis that developed in a patient, who underwent a total gastrectomy with the diagnosis of gastric cancer approximately 26 months back. Left adrenalectomy was planned with curative intent. R0 resection was performed. The patient was followed up for one year after surgery. The option of surgical treatment is recommended for isolated metachronous adrenal metastases. Curative surgical resection may positively impact the prognosis of patients in selected cases.
