Journal of Family Medicine and Primary Care, 2022

Multidrug resistance/rifampicin resistance (MDR/RR) TB remains a public health crisis . India has the highest burden of tuberculosis and multidrug resistant TB (MDR TB) in the world. There is wide geographical variation in the epidemic and its trends that can be updated by regular reporting and sound surveillance systems. The current study tries to fill this gap by analyzing the data of TB patients from a Tuberculosis Unit, studying socio-demographic and clinical profile from December 2017 to November 2019 in Nadia district of West Bengal. Aims: The aims of this work were to study socio-demographic and clinical profile of TB patients attending Tuberculosis Unit of West Bengal, and to find out factors associated with drug-resistant TB. Settings and Design: Record-based study from Tuberculosis Unit. Methods: Records of all patients who undergo CBNAAT in TU are stored as monthly unit. We randomly selected 10 months from a period of December 2017 to November 2019 by using lot method. Data of all patients undergoing CBNAAT at Kalyani – Gayeshpur Tuberculosis Unit during randomly selected 10 months were accessed. Statistical Analysis Used: Monthly data was entered in Microsoft Excel and descriptive tests of significance, proportions and Chi-square were applied. Results: There was male preponderance for testing of tuberculosis. Seven percent of the TB suspects were HIV positive. The positivity rate of MTB by CBNAAT was 23%. Four percent of the samples were Rifampicin resistance. Tobacco consumption, contact with TB case and Diabetes were common risk factors of TB. Most of the information was missing in the records. Conclusions: Most of Rifampicin Resistant cases showed very low Ct value in CBNAAT. Previous history of TB treatment and positive HIV status was significantly associated with RR TB. There is a need to capture complete information on the records of presumptive TB cases.

Indeed, epidemiologic studies describe that about 60% of all malignancies occur in people aged 65 years or older , and if the current demographic trends continue, it can be estimated that by 2020 about 70% of all cancers will be diagnosed in those aged 65 years or older . One out of two cancer patients report psychiatric disorders, especially depression. At the same time, depression is a burdensome problem even in elderly people . Thus, depression is highly prevalent both in cancer and in the elderly. Consequently, depression appears to be a relevant problem in people with cancer. The diagnosis of cancer is associated with a lot of psychological distress. Untreated psychiatric morbidities among patients with cancer can significantly impact morbidity, lead to poor adherence to treatment, longer and more frequent hospitalizations, contribute to poor prognosis, poor quality of life, and lead to increased mortality. The prevalence of each risk factor for NCDs might differ among different age-groups, which will affect the implementation of intervention programs. Seeing the role of medical students as future physicians and role models in public health intervention programs, research related to the risk factors for NCDs among them is need of hour. A cross-sectional study was conducted using the WHO global STEPS- approach to know the prevalence of risk factors for NCDs among 166 undergraduate medical students of a medical college in Kolkata. STEPS questionnaire version 2.1 was used to collect information on behavioral risk factors, followed by physical measurements and blood sampling for biochemical profile. It was observed that most of students are at risk to develop NCDs .The cumulative effect of risk facto...

Thirty percent of all neonates are Low Birth Weight (LBW). LBW is highest in India. If we look at the causes of mortality in neonates, seventy five percent are related directly or indirectly to LBW. It is important that LBW babies are provided with scientifically sound, best, low cost, humane and comprehensive method of care.KMC meets the needs of warmth, nutrition and protection from infection with multimodal stimulation and humanizing high technology to achieve improvement in survival, morbidity and appropriate growth and development of LBW infants who are thermodynamically stable and are weighing between1800 gms to 2000 gms.

Medical undergraduate students need to develop one to one relationship with patients. The objective of the present study was to assess the sharing and caring dimensions of the doctor-patient relationship using the Patient-Practitioner Orientation Scale (PPOS) and to analyze any difference across the demographic characteristics, of the undergraduate's medical students. Materials & Methodology: We did an observational descriptive cross-sectional study in a tertiary medical college of West Bengal. We used one to one interview technique as a study technique using an interview schedule consisting of sociodemographic characteristics, patient-practitioner orientation scale (PPOS). Results: Male participants constituted 59%, that is 133, whereas females were 93 (41%). Mean PPOS score was 3+0.62, Mean sharing score was 3+0.63, mean caring score was 3+0.61. Results showed significant difference across gender where female scored higher in mean sharing (2.99+0.61 vs 3.35+0.63), caring (2.82+0.58 vs 3.02+.70) & total PPOS score compared to males (2.90+0.58 vs 3.1+0.67) (p=0.00). Students from the urban locality had a significantly higher score in sharing (3.16+0.64 vs 2.92+0.49), and the total score (3.04+0.66 vs2.87+0.51) compared to rural students. Conclusions: This study gave the idea about the perception of the doctor-patient relationship among medical students of a Medical college of West Bengal. Keeping in mind this baseline score, we suggested strategies to improve doctor-patient relationships. Various teaching leaning tools could be incorporated, and contextualised case-scenarios and role plays could be framed so that proper relationship could be built.
