INTRODUCTION: The psychiatric care reform law foresees replacing psychiatric hospital beds by gen... more INTRODUCTION: The psychiatric care reform law foresees replacing psychiatric hospital beds by general hospital beds, and closing the Hospital Psiquiátrico São Pedro detoxification unit complies with these principles. OBJECTIVES: To evaluate whether the number of chemically-dependent patients admitted to the psychotics units at this hospital increased before and after the detoxification unit was closed, and whether the availability of hospital beds has decreased. METHOD: a cross-sectional study with a non-random sample was used. It was divided into: Group 1, with all patients admitted to units caring for adult males three months before closure and Group 2, all patients admitted to these units in the three months after that unit was closed. The number of places refused to chemical dependents was analyzed, and a retrospective assessment of records and medical files was performed. RESULTS: The sample in Group 1 was 135, while Group 2 was 126 (n=261). In Group 1, 15.6% were diagnosed as being chemically dependent and, in Group 2, 22.2%. There was an increase in the number of chemically-dependent patients admitted without psychiatric comorbidities (X2 =4,497; p=0,033) and in the number of those who could not be admitted (X2 =154,40; p=0,0000). DISCUSSION: Since the unit was closed, there has been an increase in the number of chemically addicted patients, without psychiatric comorbidities who were hospitalized in units for psychotics, and deprived from specialized treatment. It has also been noticed that there has been an increase in patients who do not receive treatment, which goes against the intention of the psychiatric care reform. CONCLUSION: Psychiatric Care Reform should be an objective to be pursued by all health professionals. However, we must neither overlook the patients' special needs nor neglect technical aspects.

This is a rather controversial concept among the researchers on the chemical dependence area. OBJECTIVE: The objective of this study was to carry out a theoretical review concerning craving at PsycInfo, Medline, ProQuest, Portal Capes and Science Direct Data Bases. METHODS: The keywords used were craving, dependence and drug, and the searching period was between 1995 and 2007. RESULTS: The results showed that many different meanings can be found for craving, some relate it only to desire, while others consider it not only as desire, but also as the anticipation of the positive result from the use of the substance, the relief of abstinence symptoms or negative affect and intention to smoke, which reflects a multidimensional view of this construct. The ethimology of craving can be explained by the following Models: Behavioral, Cognitive or Psychosocial and Neurobiological, being the option for one of them a factor that influences evaluation and usage. CONCLUSION: As a conclusion, we are in favor of craving multidimensionality, as well as the need to use a Biopsychsocial Model that integrates the various models for treating chemical dependent patients. It is also necessary to highlight the importance of carrying out more studies in oder to understand craving, for it is one of the principal risks of relapse.

Esta pesquisa tem por objetivo identificar a configuração dos estágios motivacionais em alcoolist... more Esta pesquisa tem por objetivo identificar a configuração dos estágios motivacionais em alcoolistas internados em unidades especializadas e verificar se há uma correlação entre estes estágios e o grau de dependência avaliado. A amostra foi composta por 120 alcoolistas, do sexo masculino com nível de dependência entre moderado e grave. Os instrumentos utilizados foram: Shortform Alcohol Dependence Data (SADD) para identificar a gravidade da dependência alcóolica e a University of Rhode Island Change Assessment Questionnaire (URICA) para avaliar os estágios motivacionais. Os resultados demonstraram uma correlação positiva e de intensidade baixa (r s = 0,197; p=0,031) entre a gravidade da dependência e o estágio de Manutenção, bem como uma preponderância de escores neste estágio. Discute-se a importância da aplicação das escalas utilizadas para um melhor delineamento das técnicas a serem adotadas durante o processo terapêutico. Palavras-chave: Dependência do álcool; estágios motivacionais; grau de dependência; escalas.

Smoking is one of the main causes of premature death worldwide. It is also an addiction that requ... more Smoking is one of the main causes of premature death worldwide. It is also an addiction that requires a difficult treatment. One approach that has been used to treat tobacco addiction is motivational interview, based on the transtheorical model proposed by Prochaska & Di Clemente. This study aims at performing a theoretical review concerning motivational stages of change and the use of brief motivational interview in smokers. The results obtained from some of the most important studies on the subject conducted in Europe and in the USA are presented here. These results indicate that, although brief motivational interview shows good outcomes concerning the treatment of addictive behaviors when compared with other categories of intervention, such success does not seem to apply to the results found in the treatment of smoking.
