Objective To evaluate arterial stiffness in TM receiving long-term gender-affirming hormone therapy by carotid-femoral pulse wave velocity (cf-PWV). Design A cross-sectional case-control study. Setting Gender Dysphoria Unit of the Division of Endocrinology, HC-FMUSP, Sao Paulo, Brazil. Patients Thirty-three TM receiving intramuscular testosterone (T) esters as regular treatment for an average time of 14 ± 8 years were compared to 111 healthy cisgender men and women controls matched for age and body mass index (BMI). Objectives: The pathophysiological mechanisms of increased arterial stiffness, currently an indep... more Objectives: The pathophysiological mechanisms of increased arterial stiffness, currently an independent risk factor for atrial fibrillation, remain poorly understood. This study investigated the association of arterial stiffness with the presence of nonsustained atrial tachycardia (NSAT) in hypertensive patients.Methods: We included sixty participants from a single centre without evident cardiovascular disease who underwent extensive cardiovascular examination. Arterial stiffness was assessed by pulse wave velocity (PWV) and Augmentation Index corrected for a heart rate of 75 bpm (Aix@75). Speckle-tracking echocardiography assessed left atrial (LA) function. A 24-hour ECG was used to identify patients with NSAT.Results: There was no statistically significant difference in arterial stiffness between groups. To compare augmentation index (AIx) between one Moderate-intensity continuous physical exercise (... more To compare augmentation index (AIx) between one Moderate-intensity continuous physical exercise (MICPE) and one High-intensity interval physical exercise (HIIPE) session in normal/high normal blood pressure (BP) (120-140 for systolic and 80-90 mmHg for diastolic). Additionally, to compare two AIx methods (SphygmoCor® and Arteriograph®) [1]. Methods: Exercise intensity and energy expenditure (equalizing) were according to the cardiopulmonary stress test. Individuals were randomized to exercise sessions, performed as cross-over. AIx were analyzed at baseline, immediately after and 24hours after MICPE and HIIPE session and compared among all times. ΔAIxHIIPE (AIxHIIPE-AIxBaseline) and ΔAIxMICPE were calculated. Correlation and agreement analysis was performed between AIx methods. Results: Individuals (n = 23; 78% women; 48 ± 1 years; systolic/diastolicBP = 125 ± 2/84 ± 1 mmHg) had lower AIxSphygmoCor® at MICPE compared to baseline and to 24 hours MICPE (27.2 ± 2.2 vs 32.8 ± 1 and 31.0 ± 2.5%; p < 0.01). AIxSphygmoCor® was lower in HIIPE than other times (23.2 ± 2.4 vs baseline 32.8 ± 1.9 p < 0.01; vs MICPE 27.2 ± 2.2; p = 0.039; vs 24 hours MICPE 31.0 ± 2.5; p < 0.01 and vs 24 hours HIIPE 32.2 ± 2.0%; p < 0.01). AIxArteriograph® was lower in HIIPE (16.0 ± 3.7%) than baseline (28.9 ± 3.4%; p = 0.001), 24 hours MICPE (25.7 ± 4.0%; p = 0.008) and 24 hours HIIPE (29.5 ± 3.9%; p = 0.005). ΔAIxHIIPE was greater than ΔAIxMICPE (−9.37 vs −5.15; p = 0.028). AIxArteriograph® showed a positive correlation with AIxSphygmoCor® (r = 0.793; p < 0.01) and showed agreement. Conclusion: Regardless of intensity, one exercise session improves AIx. The effect seems to be greater after HIIPE than MICPE.

Objective: To evaluate the predictive factors for mortality in adult patients undergoing heart tr... more Objective: To evaluate the predictive factors for mortality in adult patients undergoing heart transplantation. Method: a retrospective cohort study in a tertiary hospital, which is reference in cardiology and pneumology in Latin America. It was included 70 patients older than 18 years who underwent heart transplantation and excluded patients under 18 years as well as those diagnosed with congenital heart disease. Results: No differences were observed between the groups in relation to male gender (67.2% vs. 66.7%, p=0.969) and age (44 + 12 vs. 52 + 13, p=0.086). Regarding comorbidities the groups were not different, however, when comparing the score values of the Cleveland Clinic we observed a significant difference (5 [4-7] vs. 7 [5.25-8.75], p=0.010), being higher in the nonsurvivors group. In the multivariate analysis we observed that the Cleveland Clinic Score (OR: 1.573, p=0.007), baseline creatinine greater than 1.5 mg / dL (OR: 3.426, p=0.007), previous cardiac surgery (OR: 5.200, p = 0.032) and need for dialysis (OR: 6.750, p = 0.017) were the main predictive factors for death. Conclusion: The main predictive factors for death in this population were Cleveland Clinic Score increased, baseline creatinine greater than 1.5 mg / dL, having undergone heart surgery in advance and requiring dialysis.

Este estudo objetivou analisar concepções de graduandos de enfermagem sobre o cuidado coletivo na... more Este estudo objetivou analisar concepções de graduandos de enfermagem sobre o cuidado coletivo na atenção primária em saúde. Trata-se de um estudo de caso, com enfoque qualitativo e descritivo. A coleta de dados ocorreu em novembro de 2010, por meio de grupos focais com 11 graduandos de enfermagem de uma universidade pública do Estado de São Paulo. A análise dos resultados fundamentou-se na técnica de análise de conteúdo temático-categorial e destacou quatro categorias: o cuidado integral e a superação do modelo preventivista; a saúde da família é a grande sacada do Sistema Único de Saúde; a saúde da família possibilita a aproximação do contexto de vida e saúde da população; grupos educativos e vigilância em saúde caracterizam o cuidado coletivo. Concluiu-se que o processo de formação está propiciando a formulação de conceitos críticos sobre cuidado coletivo na atenção primária, porém, há necessidade de superação de perspectivas idealizadas. Palavras-chave: Enfermagem em saúde comunitária; atenção primária em saúde; educação em enfermagem; saúde da família.

No contexto das drogas, o álcool é um dos maiores problemas de saúde pública. Objetivo: identific... more No contexto das drogas, o álcool é um dos maiores problemas de saúde pública. Objetivo: identificar o padrão de consumo de bebida alcoólica dos usuários de uma Unidade de Saúde da Família do interior paulista, durante o acolhimento. Métodos: Estudo exploratório, transversal, de método quantitativo, com a utilização do Alcohol Use Disorders Identification Test (AUDIT) em 100 usuários adultos do serviço da rede de Atenção Primária à Saúde. Realizou-se análise estatística descritiva. Resultados: Da amostra, 74% são mulheres, 85% fazem uso de bebida alcoólica em baixo risco, 19% são adultos jovens, o beber em excesso foi relatado pela minoria dos entrevistados. Da amostra, 42% dos católicos nunca fizeram uso de álcool em binge, seguidos pelos evangélicos, com 22%. Conclusões: Os profissionais de saúde precisam ter conhecimento sobre o padrão de consumo de álcool dos moradores do território de abrangência da sua unidade, para que possam desenvolver ações de prevenção e promoção da saúde.
