Total parathyroidectomy (PTX) with parathyroid tissue autotransplantation (AT) is a treatment option in those individuals that do not respond to clinical management. To evaluate grafted parathyroid tissue response during induced hypocalcemia among CKD patients who underwent total PTX with AT. Eighteen patients with renal hyperparathyroidism were submitted to total PTX with parathyroid AT selected by stereomicroscopy between April and October 2008. Eleven (eight with successful kidney transplantation, 2 in peritoneal dialysis and 1 in hemodialysis) were clinically stable and eligible for testing. Hypocalcemia was induced using sodium bicarbonate infusion in 5 healthy controls and in patients 6-12 months after surgery. Among controls, hypocalcemia elicited a major rise in intact PTH (iPTH) levels 4 minutes after bicarbonate infusion. In patients, a significant decrease in ionized calcium concentration was observed [from 1.17 ± 0.12 to 1.09 ± 0.11 mean (± SE) mmol/L] in the 4th minute (p < 0.001) illustrating the nadir point. In the 10thminute, ionized calcium did not show a statistical increase compared to the 4th minute (p = 0.451). The iPTH levels ranged from 34.8 ± 18.6 to 34.1 ± 18.8 pg/mL (similar values between base line and 4thminute p = 0.087) and did not change in the 10th minute (33.3 ± 19,6 pg/ mL p = 0.693). Among CKD patients tested 6-12 months after surgery, grafted parathyroid tissue revealed a blunted secretory capacity during bicarbonate induced hypocalcemia with no changes in iPTH levels.

Ectopic and/or supernumerary parathyroid glands (PT) may be the cause of surgical failure in patients undergoing total parathyroidectomy (PTX). Aim: To define the locations of ectopic and supernumerary PT in patients with renal hyperparathyroidism and to correlate intraoperative findings with preoperative tests. Materials and methods: A retrospective study was conducted with 166 patients submitted to PTX. The location of PT during surgery was recorded and classified as eutopic or ectopic. The preoperative localizations of PT found by ultrasonography (USG) and Tc99m-Sestamibi scintigraphy (MIBI) were subsequently compared with intraoperative findings. Results: In the 166 patients studied, 664 PT were found. Five-hundred-seventy-seven (86.4%) glands were classified as eutopic and 91(13.6%) as ectopic. Eight supernumerary PT were found. The most common sites of ectopic PT were in the retroesophageal and thymic regions. Taken together, USG and MIBI did not identify 56 (61.5%) ectopic glands. MIBI was positive for 69,7% of all ectopic glands located in the mediastinal and thymic regions. Conclusion: The presence of ectopic and supernumerary PT in patients with renal hyperparathyroidism is significant. Although preoperative imaging tests did not locate most of ectopic glands, MIBI may be important for identifying ectopic PT in the mediastinal and thymic regions.

Para estudos de seleção de linhagens de inimigos naturais provenientes do campo, de comportamento... more Para estudos de seleção de linhagens de inimigos naturais provenientes do campo, de comportamento ou mesmo de taxonomia, a captura de insetos vivos é de grande importância. Uma das alternativas para obtenção de parasitoides (Hymenoptera) adultos é a utilização de armadilhas de emergência. Sendo assim, o objetivo deste trabalho foi propor dois modelos de armadilhas de emergência (armadilhas do tipo "caixa" e "garrafa"), para obtenção de parasitoides vivos de ovos endofíticos. Os dois modelos de armadilhas propostas foram eficientes na captura de parasitoides vivos. Entretanto, foi observada uma maior captura de espécimes na armadilha do tipo "caixa" em relação à do tipo "garrafa", decorrente de uma maior armazenagem de folíolos no interior desta. A utilização de cada um dos modelos dependerá do tipo de pesquisa a ser realizada, do custo/benefício da sua confecção e da quantidade de parasitoides necessários para realização do estudo.

The present study aimed to compare players' tactical behaviour in 3 vs. 3 and 6 vs. 6 soccer smal... more The present study aimed to compare players' tactical behaviour in 3 vs. 3 and 6 vs. 6 soccer small-sided games (SSGs). The sample comprised 3,482 tactical actions performed by 18 U-11 youth soccer players from a Portuguese club, in 3 vs. 3 and 6 vs. 6 SSGs. All participants played eight minutes in both situations and field size was adapted according to the number of players involved (30 m x 19.5 m for 3 vs. 3 and 60 m x 39 m for 6 vs. 6). The System of Tactical Assessment in Soccer (FUT-SAT) was used for data collection and analyses. Descriptive analysis was conducted to verify frequencies and percentages of the variables assessed. The chi-squared (χ2) test was performed to compare the frequencies of the variables between 3 vs. 3 and 6 vs. 6 SSGs and Standardized Residuals (e) were used to examine the influence of the frequency of one or more variables within 3 vs. 3 and 6 vs. 6 SSGs. Data treatment was performed through SPSS for Windows®, version 18.0. Results indicated that players displayed safer behaviours in 6 vs. 6 SSG and more aggressive behaviours in 3 vs. 3 SSG. Findings can aid coaches and teachers to develop different players' tactical skills according to the chosen SSG (3 vs. 3 or 6 vs. 6) form.

To evaluate stereomicroscopy in parathyroid tissue selection for total parathyroidectomy with autotransplantation in secondary (SHPT)/tertiary (THPT) hyperparathyroidism. 118 renal patients underwent surgery from April of 2000 to October 2009. They were divided into two groups: G1, 66 patients operated from April of 2000 to May of 2005, with tissue selection based on macroscopic observation; G2, 52 patients operated from March of 2008 to October 2009 with stereomicroscopy for tissue selection searching for the presence of adipose cells. All surgeries were performed by the same surgeon. Patients presented SHPT (dialysis treatment) or THPT (renal-grafted). Follow-up was 12-36 months. Intra-operative parathyroid hormone (PTH) was measured in 100/118 (84.7%) patients. Data are presented as means. G1 included 66 patients (38 SHPT, 24 females/14 males; 40.0 years of age; 28 THPT, 14 females/14 males; 44 years of age). G2 included 52 patients (29 SHPT, 11 females/18 males; 50.7 years of age; 23 THPT, 13 females/10 males, 44.4 years of age). SHPT patients from G2 presented preoperative serum calcium higher than those of SHPT patients in G1 (p<0.05), suggesting a more severe disease. Definitive hypoparathyroidism was found in seven of 118 patients (5.9%). Graft-dependent recurrence occurred in four patients, two in each group. All occurred in dialysis patients. Stereomicroscopy in SHPT/THPT surgical treatment may be a useful tool to standardize parathyroid tissue selection.

CONTEXT: Paranasal sinus cancer is considered rare, with an incidence of less than 1 per 100,000 ... more CONTEXT: Paranasal sinus cancer is considered rare, with an incidence of less than 1 per 100,000 per year, with the frontal sinus being the primary site in only 0.3%. We report a case of adenocarcinoma arising in the frontal sinus. DESIGN: Case report. CASE REPORT: A 59-year-old woman, secretary, came in February 1998 with a 4-month history of low intensity frontal headache. She denied contact with wood dust. On examination a non-tender swelling was noted over her right forehead next to the medial aspect of the right orbit. CT scan showed a soft-tissue mass involving frontal sinus with intracranial invasion through the posterior wall. The anterior ethmoid sinus and the medial aspect of the right orbit were also involved. MRI demonstrated dural thickening in communication with the frontal mass. She underwent an en-bloc tumor resection by craniotomy including orbital clearance. Histology revealed an adenocarcinoma. After surgery she had tumor recurrence, and chemotherapy and radiother...

Pulse wave velocity (PWV) is a simple method used for arterial distensibility evaluation. Few data are available concerning PWV in pre-dialysis patients. The aim of this study was to evaluate the association between PWV and cardiovascular disease in pre-dialysis. Methods. One hundred and four patients were submitted to PWV analysis, coronary artery calcium (CAC) determination with a multi-slice CT scan of the coronary arteries, echocardiogram and a carotid intima-media thickness (IMT) evaluation, with a high resolution ultrasound. The demographic characteristics and laboratory tests results were studied. Results. The mean age of those studied was 54.4 AE 11.5 years, 60% were males and the mean creatinine clearance was 40 ml/min/1.73 m 2 . The mean PWV was 12.2 AE 3.4 m/s and it was significantly higher in males, diabetics, those with creatinine clearance 560 ml/min and proteinuria !1 g/24 h. PWV was correlated with systolic blood pressure, age, triglycerides, total cholesterol and 24 h proteinuria. In the multiple regression analysis, PWV was significantly associated with diabetes, age, systolic blood pressure and cholesterol. Fifty-eight patients (56%) presented coronary calcification and PWV correlated with coronary calcium score (R ¼ 0.48; P50.001) and calcium volume (R ¼ 0.50; P50.001). Moreover, PWV was higher in patients with coronary calcification (13.4 AE 3.6 m/s vs 10.7 AE 2.4 m/s; P50.001). The mean left ventricular mass index (LVMI) was 106 AE 31 g/m 2 and 24% of patients had left ventricular hypertrophy, while 19 (18.3%) patients had left ventricular dysfunction. PVW was correlated with LVMI (R ¼ 0.25; P ¼ 0.01) while no association could be seen between PWV and the ejection fraction or left ventricular dysfunction. A correlation between the IMT and PWV was observed (R ¼ 0.27; P ¼ 0.005). In addition, those with a thicker IMT had a higher PWV (13.2 AE 3.4 m/s vs 11. 2 AE 3.2 m/s; P ¼ 0.003). Conclusion. PWV is associated with cardiovascular disease in pre-dialysis patients and can be a useful tool to identify patients with increased cardiovascular risk.

Taylor tetraspores are units of dispersal and are responsible for substrate attachment. This study aimed to examine evidence of direct interaction between germ tube formation and Golgi activity during tetraspore germination of G. floridanum. After release, the tetraspores were incubated with brefeldin A (BFA) in concentrations of 4 and 8 μM over a 6 h period. The controls and treatments were analyzed with light, fluorescence (FM4-64 dye) and transmission electron microscopy. In the control samples, the Golgi bodies were responsible for germ tube formation. In contrast, BFA-treated samples were observed to inhibit spore adhesion and germ tube formation. These tetraspores also showed an increase in volume (≥30 μm width). BFA treatment also resulted in the disassembly of Golgi cisternae and the formation of vesiculated areas of the cytoplasm, blocking the secretion of protein and amorphous matrix polysaccharides. When stained with FM4-64, the control samples showed fluorescence in the apical region of the germ tube, but the treated samples showed an intense fluorescence throughout the cytoplasm. From these results, we can conclude that the germ tube is formed by the incorporation of vesicles derived from Golgi. Thus, vesicle secretion and Golgi organization are basic processes and essential in adhesion and tube formation. By blocking the secretion of protein and amorphous matrix polysaccharides, BFA treatment precluded tetraspore germination.

Erbium-doped glasses with the molar composition 40GeO 2 ·10SiO 2 ·25Nb 2 O 5 ·25K 2 O, plus 0.1-4... more Erbium-doped glasses with the molar composition 40GeO 2 ·10SiO 2 ·25Nb 2 O 5 ·25K 2 O, plus 0.1-4 mol% of ErO 1.5 , have been prepared by conventional melt quenching technique and their density, refractive index and characteristic temperatures have been determined. Transparent glass ceramics containing 1 and 2 mol% of ErO 1.5 were also prepared and their XRD patterns and Raman spectra have been recorded and the crystalline phase K 3.8 Nb 5 Ge 3 O 20.4 has been identified. UV/visible absorption spectra of the glasses and glass ceramics have been recorded and the resulting data was used for the Judd-Ofelt analysis, from which the Ω 2 , Ω 4 and Ω 6 parameters were determined. Photoluminescence measurements of the glasses, at 1.54 μm, showed an increase in intensity with increasing Er 3+ concentration and a full width at half maximum ranging from ∼ 40 to 47 nm for 0.1% and 4% ErO 1.5 , respectively. Radiative lifetimes of 4.3 and 5.5 ms were obtained for the 0.4 and 2% ErO 1.5 doped glasses, respectively, while the measured lifetimes varied between 4.5 and 3.2 ms for the same compositions. Quantum efficiencies of 86% and 58% were obtained for the 1% and 2% Er-doped glasses, respectively. A quenching concentration of 6.2 × 10 20 ions/cm 3 was obtained for the glasses, corresponding to ∼ 3.8 mol% of ErO 1.5 . The crystallization of the Er-doped samples led to similar intensities of the 1.54 μm emission band as well as similar full width at half maximum, however a decrease was observed both in the radiative and the measured lifetimes, although quantum efficiencies remained around 85% and 54% for the 1% and 2% Er-doped glass ceramics, respectively.

ABSTRACT Objective: Obesity, arterial stiffness, cardiac hypertrophy, high aldosterone and endoth... more ABSTRACT Objective: Obesity, arterial stiffness, cardiac hypertrophy, high aldosterone and endothelial dysfunction may interact to cause resistant hypertension (RHTN). Lower adiponectin (APN) and aldosterone excess may be significantly associated with hypertension. However, the importance of hypoadiponectinemia as a complicating factor for the lack of blood pressure (BP) control in RHTN has not been demonstrated. Methods: Ninety-six RHTN patients were divided into uncontrolled (UCRHTN, n=44) and controlled (CRHTN, n=52) subgroups. APN and aldosterone, office BP and ambulatory BP measurement (ABPM), endothelium-dependent brachial artery responses [flow-mediated dilation (FMD)], left ventricular mass index (LVMI) and pulse wave velocity (PWV) were evaluated. Results: UCRHTN had increased aldosterone (12.6 +/- 1.4 vs. 8.9 +/- 0.8 ng/dL, p=0.02) as well as LVMI (142.2 +/- 6.0 vs. 122.9 +/- 4.3 g/m2, p=0.02) and PWV (12.0 +/- 0.3 vs. 9.2 +/- 0.2 m/s, p<0.0001). However, lower APN (6.9 +/- 0.7 vs. 9.5 +/- 0.8 [mu]g/mL, p=0.01) and impaired FMD response (6.6 +/- 0.3 vs. 7.5 +/- 0.3%, p=0.001) were found in this same subgroup. Brachial and ABPM pulse pressures were inversely associated with APN (r = - 0.45, p=0.002; r= - 0.33, p=0.03, respectively) as well as aldosterone and PWV (r = - 0.38, p=0.01; r = - 0.36, p=0.02, respectively) in UCRHTN. PWV was only significantly influenced by APN levels in UCRHTN ([beta]= - 0.16, SE= 0.05, p=0.01). APN did not correlate with the same parameters in CRHTN. Conclusion: Hypoadiponectinemia and high aldosterone may be implicated in antihypertensive therapy resistance in respect to arterial rigidity.

Purpose Left ventricular hypertrophy and diastolic dysfunction (LVDD) remain highly frequent mark... more Purpose Left ventricular hypertrophy and diastolic dysfunction (LVDD) remain highly frequent markers of cardiac damage and risk of progression to symptomatic heart failure, especially in resistant hypertension (RHTN). We have previously demonstrated that administration of sildenafil in hypertensive rats improves LVDD, restoring phosphodiesterase type 5 (PDE-5) inhibition in cardiac myocytes. Methods We hypothesized that the long-acting PDE-5 inhibitor tadalafil may be clinically useful in improving LVDD in RHTN independently of blood pressure (BP) reduction. A single blinded, placebo-controlled, crossover study enrolled 19 patients with both RHTN and LVDD. Firstly, subjects received tadalafil (20 mg) for 14 days and after a 2-week washout period, they received placebo orally for 14 days. Patients were evaluated by office BP and ambulatory BP monitoring (ABPM), endothelial function (FMD), echocardiography, plasma brain natriuretic peptide (BNP-32), cyclic guanosine monophosphate (cGMP) and nitrite levels.

In contrast to most developed countries, most patients with primary hyperparathyroidism in Brazil... more In contrast to most developed countries, most patients with primary hyperparathyroidism in Brazil are still symptomatic at diagnosis. However, we have been observing a change in this pattern, especially in the last few years. We evaluated 104 patients, 77 females and 27 males aged 11-79 years (mean: 54.4 years), diagnosed between 1985 and 2002 at a University Hospital. Diagnosis was made on the basis of clinical findings and of high total and/or ionized calcium levels, high or inappropriate levels of intact parathyroid hormone and of surgical findings in 80 patients. Patients were divided into three groups, i.e., patients diagnosed from 1985 to 1989, patients diagnosed from 1990 to 1994, and patients diagnosed from 1995 to 2002. The number of new cases diagnosed/year increased from 1.8/year in the first group to 6.0/year in the second group and 8.1/year in the third group. The first group comprised 9 patients (mean serum calcium +/- SD, 13.6 +/- 1.6 mg/dl), 8 of them (88.8%) defined as symptomatic. The second group comprised 30 patients (mean calcium +/- SD, 12.2 +/- 1.63 mg/dl), 22 of them defined as symptomatic (73.3%). The third group contained 65 patients (mean calcium 11.7 +/- 1.1 mg/dl), 34 of them symptomatic (52.3%). Patients from the first group tended to be younger (mean +/- SD, 43.0 +/- 15 vs 55.1 +/- 14.4 and 55.7 +/- 17.3 years, respectively) and their mean serum calcium was significantly higher (P < 0.05). All of symptomatic patients independent of group had higher serum calcium levels (12.4 +/- 1.53 mg/dl, N = 64) than asymptomatic patients (11.4 +/- 1.0 mg/dl, N = 40). Our data showed an increase in the percentage of asymptomatic patients over the years in the number of primary hyperparathyroidism cases diagnosed. This finding may be due to an increased availability of diagnostic methods and/or to an increased awareness about the disease.

INTRODUÇÃO: A medida de PTH intra-operatório (PTH-IO) foi inicialmente descrita em 1988, sendo po... more INTRODUÇÃO: A medida de PTH intra-operatório (PTH-IO) foi inicialmente descrita em 1988, sendo potencialmente útil na definição de sucesso após a paratiroidectomia. OBJETIVOS: Avaliar prospectivamente perfil de decaimento do PTH-IO e sua capacidade de prever sucesso cirúrgico no hiperparatiroidismo primário (HPP) e secundário à insuficiência renal (HPS). PACIENTES E MÉTODOS: 109 pacientes operados entre 06/2000 e 12/2004, sendo 33 HPP, 76 HPS (52 em diálise, 24 transplantados renais). PTH-IO: método imunométrico rápido (Elecsys-PTH/Immunoassay-Roche); tempo para resultado: 10 minutos. Coletas de sangue periférico nos tempos basal, 10 e 20 minutos pós-paratiroidectomia. RESULTADOS: HPP: queda média de PTH de 79,2% aos 10 minutos. HPS: queda média de PTH de 85,8% e 87,6% aos 10 minutos nos pacientes diálise e transplantados respectivamente. A cirurgia foi bem sucedida em todos, exceto em 2 pacientes (1 HPP, 1 HPS). Em ambos não houve queda PTH-IO, sendo constatado adenoma duplo no HPP...

The objective of this study was to investigate the spectrum of inflammatory gene expression in pa... more The objective of this study was to investigate the spectrum of inflammatory gene expression in patients with nasal polyposis. The cDNA microarray technique was used to identify gene expression in tissue samples from nasal polyps and adjacent inflammatory nasal mucosa of 21 patients with nonallergic nasal polyposis. To validate the microarray analysis, we compared the expression of five genes by reverse transcription-polymerase chain reaction (RT-PCR): tumor necrosis factor, IL-5, IL-9, fibroblast growth factor, and transforming growth factor (TGF)-beta1. We tested 96 different inflammatory genes in our samples. Thirty-six genes exhibited differences in expression between the two tissue types. In all 36 genes the level of expression was greater in the inflammatory mucosa than the polyps. The RT-PCR confirmed the cDNA results. We believe that the high expression of TGF-beta1 in inflammatory mucosa compared with the low expression in polyps may reflect an important role in the inhibitory mechanisms of nasal polyposis.

Ectopic and/or supernumerary parathyroid glands (PT) may be the cause of surgical failure in patients undergoing total parathyroidectomy (PTX). Aim: To define the locations of ectopic and supernumerary PT in patients with renal hyperparathyroidism and to correlate intraoperative findings with preoperative tests. Materials and methods: A retrospective study was conducted with 166 patients submitted to PTX. The location of PT during surgery was recorded and classified as eutopic or ectopic. The preoperative localizations of PT found by ultrasonography (USG) and Tc99m-Sestamibi scintigraphy (MIBI) were subsequently compared with intraoperative findings. Results: In the 166 patients studied, 664 PT were found. Five-hundred-seventy-seven (86.4%) glands were classified as eutopic and 91(13.6%) as ectopic. Eight supernumerary PT were found. The most common sites of ectopic PT were in the retroesophageal and thymic regions. Taken together, USG and MIBI did not identify 56 (61.5%) ectopic glands. MIBI was positive for 69,7% of all ectopic glands located in the mediastinal and thymic regions. Conclusion: The presence of ectopic and supernumerary PT in patients with renal hyperparathyroidism is significant. Although preoperative imaging tests did not locate most of ectopic glands, MIBI may be important for identifying ectopic PT in the mediastinal and thymic regions.

Para estudos de seleção de linhagens de inimigos naturais provenientes do campo, de comportamento... more Para estudos de seleção de linhagens de inimigos naturais provenientes do campo, de comportamento ou mesmo de taxonomia, a captura de insetos vivos é de grande importância. Uma das alternativas para obtenção de parasitoides (Hymenoptera) adultos é a utilização de armadilhas de emergência. Sendo assim, o objetivo deste trabalho foi propor dois modelos de armadilhas de emergência (armadilhas do tipo "caixa" e "garrafa"), para obtenção de parasitoides vivos de ovos endofíticos. Os dois modelos de armadilhas propostas foram eficientes na captura de parasitoides vivos. Entretanto, foi observada uma maior captura de espécimes na armadilha do tipo "caixa" em relação à do tipo "garrafa", decorrente de uma maior armazenagem de folíolos no interior desta. A utilização de cada um dos modelos dependerá do tipo de pesquisa a ser realizada, do custo/benefício da sua confecção e da quantidade de parasitoides necessários para realização do estudo.

The present study aimed to compare players' tactical behaviour in 3 vs. 3 and 6 vs. 6 soccer smal... more The present study aimed to compare players' tactical behaviour in 3 vs. 3 and 6 vs. 6 soccer small-sided games (SSGs). The sample comprised 3,482 tactical actions performed by 18 U-11 youth soccer players from a Portuguese club, in 3 vs. 3 and 6 vs. 6 SSGs. All participants played eight minutes in both situations and field size was adapted according to the number of players involved (30 m x 19.5 m for 3 vs. 3 and 60 m x 39 m for 6 vs. 6). The System of Tactical Assessment in Soccer (FUT-SAT) was used for data collection and analyses. Descriptive analysis was conducted to verify frequencies and percentages of the variables assessed. The chi-squared (χ2) test was performed to compare the frequencies of the variables between 3 vs. 3 and 6 vs. 6 SSGs and Standardized Residuals (e) were used to examine the influence of the frequency of one or more variables within 3 vs. 3 and 6 vs. 6 SSGs. Data treatment was performed through SPSS for Windows®, version 18.0. Results indicated that players displayed safer behaviours in 6 vs. 6 SSG and more aggressive behaviours in 3 vs. 3 SSG. Findings can aid coaches and teachers to develop different players' tactical skills according to the chosen SSG (3 vs. 3 or 6 vs. 6) form.

To evaluate stereomicroscopy in parathyroid tissue selection for total parathyroidectomy with autotransplantation in secondary (SHPT)/tertiary (THPT) hyperparathyroidism. 118 renal patients underwent surgery from April of 2000 to October 2009. They were divided into two groups: G1, 66 patients operated from April of 2000 to May of 2005, with tissue selection based on macroscopic observation; G2, 52 patients operated from March of 2008 to October 2009 with stereomicroscopy for tissue selection searching for the presence of adipose cells. All surgeries were performed by the same surgeon. Patients presented SHPT (dialysis treatment) or THPT (renal-grafted). Follow-up was 12-36 months. Intra-operative parathyroid hormone (PTH) was measured in 100/118 (84.7%) patients. Data are presented as means. G1 included 66 patients (38 SHPT, 24 females/14 males; 40.0 years of age; 28 THPT, 14 females/14 males; 44 years of age). G2 included 52 patients (29 SHPT, 11 females/18 males; 50.7 years of age; 23 THPT, 13 females/10 males, 44.4 years of age). Definitive hypoparathyroidism was found in seven of 118 patients (5.9%). Graft-dependent recurrence occurred in four patients, two in each group. All occurred in dialysis patients. Stereomicroscopy in SHPT/THPT surgical treatment may be a useful tool to standardize parathyroid tissue selection.

We report a case of adenocarcinoma arising in the frontal sinus. DESIGN: Case report. CASE REPORT: A 59-year-old woman, secretary, came in February 1998 with a 4-month history of low intensity frontal headache. She denied contact with wood dust. On examination a non-tender swelling was noted over her right forehead next to the medial aspect of the right orbit. CT scan showed a soft-tissue mass involving frontal sinus with intracranial invasion through the posterior wall. The anterior ethmoid sinus and the medial aspect of the right orbit were also involved. MRI demonstrated dural thickening in communication with the frontal mass. Pulse wave velocity (PWV) is a simple method used for arterial distensibility evaluation. Few data are available concerning PWV in pre-dialysis patients. The aim of this study was to evaluate the association between PWV and cardiovascular disease in pre-dialysis. Methods. One hundred and four patients were submitted to PWV analysis, coronary artery calcium (CAC) determination with a multi-slice CT scan of the coronary arteries, echocardiogram and a carotid intima-media thickness (IMT) evaluation, with a high resolution ultrasound. The demographic characteristics and laboratory tests results were studied. Results. The mean age of those studied was 54.4 AE 11.5 years, 60% were males and the mean creatinine clearance was 40 ml/min/1.73 m 2 . The mean PWV was 12.2 AE 3.4 m/s and it was significantly higher in males, diabetics, those with creatinine clearance 560 ml/min and proteinuria !1 g/24 h. PWV was correlated with systolic blood pressure, age, triglycerides, total cholesterol and 24 h proteinuria. In the multiple regression analysis, PWV was significantly associated with diabetes, age, systolic blood pressure and cholesterol. Fifty-eight patients (56%) presented coronary calcification and PWV correlated with coronary calcium score (R ¼ 0.48; P50.001) and calcium volume (R ¼ 0.50; P50.001). Moreover, PWV was higher in patients with coronary calcification (13.4 AE 3.6 m/s vs 10.7 AE 2.4 m/s; P50.001). The mean left ventricular mass index (LVMI) was 106 AE 31 g/m 2 and 24% of patients had left ventricular hypertrophy, while 19 (18.3%) patients had left ventricular dysfunction. PVW was correlated with LVMI (R ¼ 0.25; P ¼ 0.01) while no association could be seen between PWV and the ejection fraction or left ventricular dysfunction. A correlation between the IMT and PWV was observed (R ¼ 0.27; P ¼ 0.005). In addition, those with a thicker IMT had a higher PWV (13.2 AE 3.4 m/s vs 11. 2 AE 3.2 m/s; P ¼ 0.003). Conclusion. PWV is associated with cardiovascular disease in pre-dialysis patients and can be a useful tool to identify patients with increased cardiovascular risk.

Taylor tetraspores are units of dispersal and are responsible for substrate attachment. This study aimed to examine evidence of direct interaction between germ tube formation and Golgi activity during tetraspore germination of G. floridanum. After release, the tetraspores were incubated with brefeldin A (BFA) in concentrations of 4 and 8 μM over a 6 h period. The controls and treatments were analyzed with light, fluorescence (FM4-64 dye) and transmission electron microscopy. In the control samples, the Golgi bodies were responsible for germ tube formation. In contrast, BFA-treated samples were observed to inhibit spore adhesion and germ tube formation. These tetraspores also showed an increase in volume (≥30 μm width). BFA treatment also resulted in the disassembly of Golgi cisternae and the formation of vesiculated areas of the cytoplasm, blocking the secretion of protein and amorphous matrix polysaccharides. When stained with FM4-64, the control samples showed fluorescence in the apical region of the germ tube, but the treated samples showed an intense fluorescence throughout the cytoplasm. From these results, we can conclude that the germ tube is formed by the incorporation of vesicles derived from Golgi. Thus, vesicle secretion and Golgi organization are basic processes and essential in adhesion and tube formation. By blocking the secretion of protein and amorphous matrix polysaccharides, BFA treatment precluded tetraspore germination.

Erbium-doped glasses with the molar composition 40GeO 2 ·10SiO 2 ·25Nb 2 O 5 ·25K 2 O, plus 0.1-4... more Erbium-doped glasses with the molar composition 40GeO 2 ·10SiO 2 ·25Nb 2 O 5 ·25K 2 O, plus 0.1-4 mol% of ErO 1.5 , have been prepared by conventional melt quenching technique and their density, refractive index and characteristic temperatures have been determined. Transparent glass ceramics containing 1 and 2 mol% of ErO 1.5 were also prepared and their XRD patterns and Raman spectra have been recorded and the crystalline phase K 3.8 Nb 5 Ge 3 O 20.4 has been identified. UV/visible absorption spectra of the glasses and glass ceramics have been recorded and the resulting data was used for the Judd-Ofelt analysis, from which the Ω 2 , Ω 4 and Ω 6 parameters were determined. Photoluminescence measurements of the glasses, at 1.54 μm, showed an increase in intensity with increasing Er 3+ concentration and a full width at half maximum ranging from ∼ 40 to 47 nm for 0.1% and 4% ErO 1.5 , respectively. Radiative lifetimes of 4.3 and 5.5 ms were obtained for the 0.4 and 2% ErO 1.5 doped glasses, respectively, while the measured lifetimes varied between 4.5 and 3.2 ms for the same compositions. Quantum efficiencies of 86% and 58% were obtained for the 1% and 2% Er-doped glasses, respectively. A quenching concentration of 6.2 × 10 20 ions/cm 3 was obtained for the glasses, corresponding to ∼ 3.8 mol% of ErO 1.5 . The crystallization of the Er-doped samples led to similar intensities of the 1.54 μm emission band as well as similar full width at half maximum, however a decrease was observed both in the radiative and the measured lifetimes, although quantum efficiencies remained around 85% and 54% for the 1% and 2% Er-doped glass ceramics, respectively.

ABSTRACT Objective: Obesity, arterial stiffness, cardiac hypertrophy, high aldosterone and endoth... more ABSTRACT Objective: Obesity, arterial stiffness, cardiac hypertrophy, high aldosterone and endothelial dysfunction may interact to cause resistant hypertension (RHTN). Lower adiponectin (APN) and aldosterone excess may be significantly associated with hypertension. However, the importance of hypoadiponectinemia as a complicating factor for the lack of blood pressure (BP) control in RHTN has not been demonstrated. Methods: Ninety-six RHTN patients were divided into uncontrolled (UCRHTN, n=44) and controlled (CRHTN, n=52) subgroups. APN and aldosterone, office BP and ambulatory BP measurement (ABPM), endothelium-dependent brachial artery responses [flow-mediated dilation (FMD)], left ventricular mass index (LVMI) and pulse wave velocity (PWV) were evaluated. Results: UCRHTN had increased aldosterone (12.6 +/- 1.4 vs. 8.9 +/- 0.8 ng/dL, p=0.02) as well as LVMI (142.2 +/- 6.0 vs. 122.9 +/- 4.3 g/m2, p=0.02) and PWV (12.0 +/- 0.3 vs. 9.2 +/- 0.2 m/s, p<0.0001). However, lower APN (6.9 +/- 0.7 vs. 9.5 +/- 0.8 [mu]g/mL, p=0.01) and impaired FMD response (6.6 +/- 0.3 vs. 7.5 +/- 0.3%, p=0.001) were found in this same subgroup. Brachial and ABPM pulse pressures were inversely associated with APN (r = - 0.45, p=0.002; r= - 0.33, p=0.03, respectively) as well as aldosterone and PWV (r = - 0.38, p=0.01; r = - 0.36, p=0.02, respectively) in UCRHTN. PWV was only significantly influenced by APN levels in UCRHTN ([beta]= - 0.16, SE= 0.05, p=0.01). APN did not correlate with the same parameters in CRHTN. Conclusion: Hypoadiponectinemia and high aldosterone may be implicated in antihypertensive therapy resistance in respect to arterial rigidity.

Purpose Left ventricular hypertrophy and diastolic dysfunction (LVDD) remain highly frequent mark... more Purpose Left ventricular hypertrophy and diastolic dysfunction (LVDD) remain highly frequent markers of cardiac damage and risk of progression to symptomatic heart failure, especially in resistant hypertension (RHTN). We have previously demonstrated that administration of sildenafil in hypertensive rats improves LVDD, restoring phosphodiesterase type 5 (PDE-5) inhibition in cardiac myocytes. Methods We hypothesized that the long-acting PDE-5 inhibitor tadalafil may be clinically useful in improving LVDD in RHTN independently of blood pressure (BP) reduction. A single blinded, placebo-controlled, crossover study enrolled 19 patients with both RHTN and LVDD. Firstly, subjects received tadalafil (20 mg) for 14 days and after a 2-week washout period, they received placebo orally for 14 days. Patients were evaluated by office BP and ambulatory BP monitoring (ABPM), endothelial function (FMD), echocardiography, plasma brain natriuretic peptide (BNP-32), cyclic guanosine monophosphate (cGMP) and nitrite levels.

In contrast to most developed countries, most patients with primary hyperparathyroidism in Brazil... more In contrast to most developed countries, most patients with primary hyperparathyroidism in Brazil are still symptomatic at diagnosis. However, we have been observing a change in this pattern, especially in the last few years. We evaluated 104 patients, 77 females and 27 males aged 11-79 years (mean: 54.4 years), diagnosed between 1985 and 2002 at a University Hospital. Diagnosis was made on the basis of clinical findings and of high total and/or ionized calcium levels, high or inappropriate levels of intact parathyroid hormone and of surgical findings in 80 patients. Patients were divided into three groups, i.e., patients diagnosed from 1985 to 1989, patients diagnosed from 1990 to 1994, and patients diagnosed from 1995 to 2002. The number of new cases diagnosed/year increased from 1.8/year in the first group to 6.0/year in the second group and 8.1/year in the third group. The first group comprised 9 patients (mean serum calcium +/- SD, 13.6 +/- 1.6 mg/dl), 8 of them (88.8%) defined as symptomatic. The second group comprised 30 patients (mean calcium +/- SD, 12.2 +/- 1.63 mg/dl), 22 of them defined as symptomatic (73.3%). The third group contained 65 patients (mean calcium 11.7 +/- 1.1 mg/dl), 34 of them symptomatic (52.3%). Patients from the first group tended to be younger (mean +/- SD, 43.0 +/- 15 vs 55.1 +/- 14.4 and 55.7 +/- 17.3 years, respectively) and their mean serum calcium was significantly higher (P < 0.05). All of symptomatic patients independent of group had higher serum calcium levels (12.4 +/- 1.53 mg/dl, N = 64) than asymptomatic patients (11.4 +/- 1.0 mg/dl, N = 40). Our data showed an increase in the percentage of asymptomatic patients over the years in the number of primary hyperparathyroidism cases diagnosed. This finding may be due to an increased availability of diagnostic methods and/or to an increased awareness about the disease.

OBJETIVOS: Avaliar prospectivamente perfil de decaimento do PTH-IO e sua capacidade de prever sucesso cirúrgico no hiperparatiroidismo primário (HPP) e secundário à insuficiência renal (HPS). PACIENTES E MÉTODOS: 109 pacientes operados entre 06/2000 e 12/2004, sendo 33 HPP, 76 HPS (52 em diálise, 24 transplantados renais). PTH-IO: método imunométrico rápido (Elecsys-PTH/Immunoassay-Roche); tempo para resultado: 10 minutos. Coletas de sangue periférico nos tempos basal, 10 e 20 minutos pós-paratiroidectomia. RESULTADOS: HPP: queda média de PTH de 79,2% aos 10 minutos. HPS: queda média de PTH de 85,8% e 87,6% aos 10 minutos nos pacientes diálise e transplantados respectivamente. A cirurgia foi bem sucedida em todos, exceto em 2 pacientes (1 HPP, 1 HPS). Em ambos não houve queda PTH-IO, sendo constatado adenoma duplo no HPP...

The objective of this study was to investigate the spectrum of inflammatory gene expression in pa... more The objective of this study was to investigate the spectrum of inflammatory gene expression in patients with nasal polyposis. The cDNA microarray technique was used to identify gene expression in tissue samples from nasal polyps and adjacent inflammatory nasal mucosa of 21 patients with nonallergic nasal polyposis. To validate the microarray analysis, we compared the expression of five genes by reverse transcription-polymerase chain reaction (RT-PCR): tumor necrosis factor, IL-5, IL-9, fibroblast growth factor, and transforming growth factor (TGF)-beta1. We tested 96 different inflammatory genes in our samples. Thirty-six genes exhibited differences in expression between the two tissue types. In all 36 genes the level of expression was greater in the inflammatory mucosa than the polyps. The RT-PCR confirmed the cDNA results. We believe that the high expression of TGF-beta1 in inflammatory mucosa compared with the low expression in polyps may reflect an important role in the inhibitory mechanisms of nasal polyposis.
