Turbinoplasty is a procedure that aims to reduce the size of the inferior turbinate through exube... more Turbinoplasty is a procedure that aims to reduce the size of the inferior turbinate through exuberant bone removal with high mucosal preservation. The procedure is recommended for patients with or without allergic rhinitis and those showing irreversible hypertrophy of inferior turbinates. OBJECTIVE: To evaluate the efcacy of inferior turbinoplasty for obstructive and non-obstructive symptoms in patients with or without allergic rhinitis. METHODS: Prospective study with 57 patients who underwent inferior turbinoplasty. They were evaluated for nasal obstruction, snoring, facial pressure, smell alterations, sneezing, nasal itching and runny nose symptoms, surgery time, and intraoperative bleeding. The last evaluation took place three months after surgery. RESULTS:Thirty-nine patients with allergic rhinitis and 18 without were assessed. Ninety days after surgery, 94.7% of patients showed degrees IV and V of breathing improvement; 89.5% showed moderate or complete improvement in snoring; all patients showed smell improvement (only one showed moderate improvement; all the others had full improvement); 95.5% experienced complete facial pressure improvement; and 89.7% showed moderate to complete improvement in nasal itching and runny nose symptoms, as well as in sneezing. CONCLUSION: The efcacy of inferior turbinoplasty was conrmed not only for obstructive symptoms, but also for non-obstructive symptoms in patients with and without allergic rhinitis.

METHODS: Out of 50 patients 25 treated with conventional septoplasty and 25 Endoscopic septoplasty problems associated with various nasal DNS. Right DNS left DNS, Spur, hypertrophied IT; polypoid MT, concha bullosa nasal discharge were operated by both methods and RESULTS: post-surgical complications were Bleeding 0% in Endoscopic septoplasm (ES) and 1 (4%) in conventional septoplasty (CS), synechiae 1 (4%) in ES, 2 (8%) in CS, DNS, 2 (8%) in ES and 4 (16%) in CS Epistaxis was observed in both ES and CS 1 (4%) but crusting was observed only in CS 2 (8%). Majority of post-operative complication were observed in conventional septoplasty as comparative Endoplastic CONCLUSION: septoplasty because ES is more illuminative, least incision, precise cutting is an ideal for Nasal disease in all age groups of both sexes.

The achilles tendon is the strongest and largest tendon of the human body and can be exposed to f... more The achilles tendon is the strongest and largest tendon of the human body and can be exposed to forces six to eight times body weight. The spontaneous ruptures of the achilles tendon are relatively common and are frequently missed. We present a prospective case series of 11 patients of spontaneous tendoachilles rupture treated with speed bridge technique and augmented with flexor hallucis longus tendon transfer from a period of January 2019 to April 2020 operated by a single orthopaedic surgeon. The mean FAOS score before surgery was 56.5 ± 7.7 (42 -66) and improved to a mean score of 88.2 ± 5.7 (78-96) postoperatively. Speed bridge reconstruction technique combined with FHL transfer allows stable re-construction and early mobilisation of the patient with good to excellent results.

We report a case of a 41-years old male patient who presented with completely closed dislocation of navicular and was treated by open reduction and fixation of the naviculocuneiform and calcaneocuboid joints. At 12 months follow up, the patient had no deficits in activities of daily living.

The paper presents a study carried out in vasculotoxic and neurotoxic snakebite cases to find out... more The paper presents a study carried out in vasculotoxic and neurotoxic snakebite cases to find out the predictors of mortality in the state of Jharkhand. Materials and methods: An estimated 58 snakebite patients who fulfilled the inclusion criteria were enrolled into the study. The clinical parameters and epidemiological data were noted during admission. Patients were followed up during their stay in the hospital for the progress of the symptoms and the treatment effects. Data obtained were analyzed using SPSS and Microsoft Excel. Results: There was a significant positive association between the occurrence of GI bleed/epistaxis, DIC, or shock with an increased mortality in vasculotoxic snakebite cases. In the cases of neurotoxic snakebite, the occurrence of cardiac arrhythmias or respiratory failure was foretellers of an increased chance of mortality. Amongst the patients with vasculotoxic snakebite, local bleeding was present in all the patients and the infection was found in 94% of them. The complications included DIC in 36% of patients, GI bleed in 9%, epistaxis or gum bleed in 9%, shock in 21%, acute renal failure in 50%, and neurotoxic signs in 6% of the patients. In the neurotoxic subset, the most frequent symptoms were ptosis and blurring of vision, both in 100% of the cases, generalized paralysis in 79%, and local pain with swelling in 67%. Respiratory failure was seen in 50%, infection in 45%, cardiac arrhythmias in 33%, shock in 12%, and hepatotoxic features in 20%. Most of the patients required 30 vials or less of anti-snake venom serum (ASVS). Conclusion: GI bleed/epistaxis, DIC, or shock are positive predictors of mortality in vasculotoxic snakebite cases, while the occurrence of cardiac arrhythmias or respiratory failure are markers of a poor prognosis in neurotoxic snakebite cases.

To report outcome of 30 patients who underwent bipolar hemiarthroplasty for intracapsular femoral... more To report outcome of 30 patients who underwent bipolar hemiarthroplasty for intracapsular femoral neck fractures. 18 women and 12 men aged 56 to 86 (mean, 70) years with Garden type III (n=7) or IV (n=23) intracapsular femoral neck fractures underwent cemented (n=5) or uncemented (n=25) bipolar hemiarthroplasty. Pain was evaluated using the visual analogue score. Mobility status was classified as ambulation with or without aid (a cane or walker). Stability of the prosthesis was classified as stable or unstable. Functional outcome was evaluated using the Harris Hip Score. The mean length of hospital stay was 15.3 (range, 4-29) days. At the 6-month follow-up, 21 patients had no pain, 6 had mild pain, and 3 had moderate pain. The mean Harris Hip Score was 83.1; functional outcome was excellent in 10 patients, good in 13, fair in 5, and poor in 2. Sweat gland carcinoma is a rare malignant tumour, first described in 1865, with approximately 220... more Sweat gland carcinoma is a rare malignant tumour, first described in 1865, with approximately 220 cases reported in last 30 years. Lower limbs are the most common site of involvement, followed by the upper extremities, and the head. Trunk is rarely involved. Five histological types have been described, most common being porocarcinomas followed by ductal carcinomas, adenoid cystic carcinomas, syringomatous carcinomas and mucinous carcinomas. These are aggressive tumours with potential for distant metastasis. Wide surgical excision is the treatment of choice and the overall prognosis is poor. Here we report another case of sweat gland adenocarcinoma with no evidence of metastasis.
