Cognitive Behavioural Therapy (CBT) has best evidence for treatment effect and planned exposure is considered one of the most effective elements of CBT in SAD. However, in vivo exposure can be difficult to access and control and is sometimes rejected by patients because they expect it to be too aversive. The use of virtual reality allows exposure to challenging situations in an immersive, but also protected, flexible and controlled environment. The objective of the study was to investigate variables, derived from both cognitive and emotion ... more The objective of the study was to investigate variables, derived from both cognitive and emotion regulation conceptualizations of social anxiety disorder (SAD), as possible change processes in cognitive behaviour therapy (CBT) for SAD. Several proposed change processes were investigated: estimated probability, estimated cost, safety behaviours, acceptance of emotions, cognitive reappraisal and expressive suppression. Participants were 50 patients with SAD, receiving a standard manualized CBT program, conducted in groups or individually. All variables were measured pre-therapy, mid-therapy and post-therapy. Lower level mediation models revealed that while a change in most process measures significantly predicted clinical improvement, only changes in estimated probability and cost and acceptance of emotions showed significant indirect effects of CBT for SAD. The results are in accordance with previous studies supporting the mediating role of changes in cognitive distortions in CBT for SAD. In addition, acceptance of emotions may also be a critical component to clinical improvement in SAD during CBT, although more research is needed on which elements of acceptance are most helpful for individuals with SAD. The study's lack of a control condition limits any conclusion regarding the specificity of the findings to CBT. Copyright © 2013 John Wiley & Sons, Ltd. Social anxiety disorder (SAD) has been associated with cluster A personality disorder (PD) traits... more Social anxiety disorder (SAD) has been associated with cluster A personality disorder (PD) traits, mainly paranoid and schizoid traits. The aim of the study was to further investigate cluster A personality pathology in patients with SAD. Self-reported PD traits were investigated in a clinical sample of 161 participants with SAD and in a clinical comparison group of 145 participants with panic disorder with or without agoraphobia (PAD). A diagnosis of SAD was associated with more paranoid and schizotypal PD traits, and an association between depression and personality pathology could indicate a state-effect of depression on PD traits. Patients with SAD had more cluster A personality pathology than patients with PAD, with the most solid indication for paranoid personality pathology.

We have translated and modified the "The Unified Protocol for Transdiagnostic Treatment of Emotional Disorders" (UP-CBT) for group delivery in Mental Health Service (MHS), and shown effects comparable to traditional CBT in a naturalistic study. As the use of one manual instead of several diagnosis-specific manuals could simplify logistics, reduce waiting time, and increase therapist expertise compared to diagnosis specific CBT, we aim to test the relative efficacy of group UP-CBT and diagnosis specific group CBT. The present study therefore examines negative autobiographical memories in participants with social anxiety disorder (SAD), compared to patients with panic disorder (PD), and healthy controls (HCs). A total of 107 participants retrieved four memories cued by verbal phrases associated with either social anxiety (SA) or panic anxiety (PA), with two memories for each cue category. PA-cued memories were experienced with stronger imagery and as more traumatic. They were also rated as more central to identity than SA-cued memories, but not among participants with SAD, who perceived SA-cued memories as equally central to their identity. Little is known about the impact of affect on the perception of body language. The aim of the present study was to investigate the relationship of implicit (versus explicit) affectivity with the recognition of briefly presented affective body expressions. Implicit affectivity, which can be measured using indirect assessment methods, has been found to be more predictive of spontaneous physiological reactions than explicit (self-reported) affect. Thirty-four healthy women had to label the expression of body postures (angry, fearful, happy, or neutral) presented for 66 ms and masked by a neutral body posture in a forced-choice format while undergoing functional magnetic resonance imaging (fMRI). Participants' implicit affectivity was assessed using the Implicit Positive and Negative Affect Test. Measures of explicit state and trait affectivity were also administered. Analysis of the fMRI data was focused on a subcortical network involved in the rapid perception of affective body expressions. Only implicit negative affect (but not explicit affect) was correlated with correct labeling performance for angry body posture. As expected, implicit negative affect was positively associated with activation of the subcortical network in response to fearful and angry expression (compared to neutral expression). Responses of the caudate nucleus to affective body expression were especially associated with its recognition. It appears that processes of rapid recognition of affects from body postures could be facilitated by an individual's implicit negative affect.

For instance, people with social phobia show increased attention to briefly presented threatening faces. However, when exposure times are increased, the direction of this attentional bias is more unclear. Studies investigating eye movements have found both increased as well as decreased attention to threatening faces in socially anxious participants. The current study investigated eye movements to emotional faces in eight patients with social phobia and 34 controls. Three different tasks with different exposure durations were used, which allowed for an investigation of the time course of attention. At the early time interval, patients showed a complex pattern of both vigilance and avoidance of threatening faces. At the longest time interval, patients avoided the eyes of sad, disgust, and neutral faces more than controls, whereas there were no group differences...

Present study aimed to describe details of social aspects of anxiety in dental situations, especially focusing on embarrassment phenomena. Subjects (Ss) were consecutive specialist clinic patients, 16 men, 14 women, 20-65 yr, who avoided treatment mean 12.7 yr due to anxiety. Electronic patient records and transcribed initial assessment and exit interviews were analyzed using QSR"N4" software to aid in exploring contexts related to social aspects of dental anxiety and embarrassment phenomena. Some examples are proposed. Distinctive structures and functions of immunological system in cutaneous, respiratory and digestive apparatus, give an explanation for the predominance of skin and respiratory immunological systems to promote hypersensitivity. For the prevention of food allergies, it is very important to avoid excessive skin and respiratory contaminations by allergens in the work and home environments.

This study is the first to investigate automatic brain reactivity to emotional stimuli as a function of trait emotional awareness. To assess emotional awareness the Levels of Emotional Awareness Scale (LEAS) was administered. During scanning, masked happy, angry, fearful and neutral facial expressions were presented to 46 healthy subjects, who had to rate the fit between artificial and emotional words. The rating procedure allowed assessment of shifts in implicit affectivity due to emotion faces. Trait emotional awareness was associated with increased activation in the primary somatosensory cortex, inferior parietal lobule, anterior cingulate gyrus, middle frontal and cerebellar areas, thalamus, putamen and amygdala in response to masked happy faces. Implicit affectivity, which can be measured using indirect assessment methods, has been found to be more predictive of spontaneous physiological reactions than explicit (self-reported) affect. Thirty-four healthy women had to label the expression of body postures (angry, fearful, happy, or neutral) presented for 66 ms and masked by a neutral body posture in a forced-choice format while undergoing functional magnetic resonance imaging (fMRI). Participants' implicit affectivity was assessed using the Implicit Positive and Negative Affect Test. Measures of explicit state and trait affectivity were also administered. Analysis of the fMRI data was focused on a subcortical network involved in the rapid perception of affective body expressions. Only implicit negative affect (but not explicit affect) was correlated with correct labeling performance for angry body posture. As expected, implicit negative affect was positively associated with activation of the subcortical network in response to fearful and angry expression (compared to neutral expression). Responses of the caudate nucleus to affective body expression were especially associated with its recognition. It appears that processes of rapid recognition of affects from body postures could be facilitated by an individual's implicit negative affect.

However, when exposure times are increased, the direction of this attentional bias is more unclear. Studies investigating eye movements have found both increased as well as decreased attention to threatening faces in socially anxious participants. The current study investigated eye movements to emotional faces in eight patients with social phobia and 34 controls. Three different tasks with different exposure durations were used, which allowed for an investigation of the time course of attention. At the early time interval, patients showed a complex pattern of both vigilance and avoidance of threatening faces. Isothiocyanates present in mustard spice is probably responsible for this cross-sensitivity. Frequently, food allergies result from skin or respiratory primary contact and sensitization. Some examples are proposed. Distinctive structures and functions of immunological system in cutaneous, respiratory and digestive apparatus, give an explanation for the predominance of skin and respiratory immunological systems to promote hypersensitivity. For the prevention of food allergies, it is very important to avoid excessive skin and respiratory contaminations by allergens in the work and home environments.

This study is the first to investigate automatic brain reactivity to emotional stimuli as a function of trait emotional awareness. To assess emotional awareness the Levels of Emotional Awareness Scale (LEAS) was administered. During scanning, masked happy, angry, fearful and neutral facial expressions were presented to 46 healthy subjects, who had to rate the fit between artificial and emotional words. The rating procedure allowed assessment of shifts in implicit affectivity due to emotion faces. Trait emotional awareness was associated with increased activation in the primary somatosensory cortex, inferior parietal lobule, anterior cingulate gyrus, middle frontal and cerebellar areas, thalamus, putamen and amygdala in response to masked happy faces.
