Introducción: El paracetamol intravenoso fue autorizado en el año 2001 para Europa, en el 2016 se... more Introducción: El paracetamol intravenoso fue autorizado en el año 2001 para Europa, en el 2016 se autoriza en Guatemala. Esta presentación ha generado diferentes expectativas alrededor del mundo, por lo que se evaluó su administración al agregarse al tratamiento de analgesia postoperatoria multimodal. Objetivo: Evaluar el efecto analgésico en el tratamiento multimodal de dolor agudo postoperatorio con paracetamol IV en la población guatemalteca. Métodos: Estudio retrospectivo caso-control para la evaluación del dolor postoperatorio inmediato (24 horas) al agregar paracetamol IV al protocolo de analgesia estándar. En pacientes de ambos géneros, entre 18 y 70 años de edad, con peso mayor o igual a 50 Kg. con una cirugía electiva abierta o por vía laparoscópica, en una muestra de 110 pacientes dividida en dos grupos. Se comparó el número de pacientes que necesitaron dosis de rescate y el nivel de dolor según la Escala Visual Análoga (EVA) durante 4 evaluaciones en el postoperatorio agudo. Para determinar la correlación entre las variables se utilizó la prueba de independencia de Ji cuadrado. Resultados: Se demostró que agregar paracetamol IV al tratamiento estándar disminuye la cantidad de pacientes que requieren dosis de rescate y mejoran su analgesia durante el postoperatorio agudo. Conclusiones: El paracetamol IV es de beneficio al ser agregado al tratamiento de analgesia multimodal convencional de un hospital privado de Z.10.

SUMMARY. Three cases of stomal hernia are presented in this article; two females and one in male ... more SUMMARY. Three cases of stomal hernia are presented in this article; two females and one in male patient, the first two cases with colostomy and the remaining patient with ileostomy. One case was treated by replacing stoma and subsequent hernioplasty. The additional two were treated with Leslie technique. To date, there have been no recurrences. Patients were followed for a median of 2 years. Although this complication is infrequent we recommend careful planning prior to performing a stoma, as well as determining the best anatomic site and use good surgical technique.

Endoscopic retrograde cholangiopancreatography (ERCP) is technically more challenging in patients... more Endoscopic retrograde cholangiopancreatography (ERCP) is technically more challenging in patients with postsurgical anatomy, such as Roux-en-Y anastomosis, frequently mandating an operative intervention. Although limited, there is growing evidence that ERCP can be performed using the double balloon enteroscope (DBE) in patients with complex postoperative anatomy. We present the technical aspects of performing ERCP with the DBE in patients presenting with complex postsurgical anatomy having biliary problems. ERCP using the DBE is feasible in patients with complex postsurgical anatomy, permitting diagnostic and therapeutic interventions in 80% of patients.

The 2018 Edition of the Declaration of Istanbul on Organ Trafficking and Transplant Tourism (DoI)... more The 2018 Edition of the Declaration of Istanbul on Organ Trafficking and Transplant Tourism (DoI) provides an updated set of principles and definitions to guide policymakers and health professionals working in organ donation and transplantation. A draft of the new edition was circulated to the public and transplant professionals through an online consultation process, which also sought feedback on a draft explanatory article that explained the principles and discussed some of their practical implications. Both drafts were revised in response to feedback from participants in the consultation. We present here the discussion article, which is intended to assist stakeholders in applying the principles of the DoI by providing more detailed information about the meaning and potential implications of implementing the DoI in various contexts.

Organ procurement from executed prisoners in China is internationally condemned, yet this practic... more Organ procurement from executed prisoners in China is internationally condemned, yet this practice continues unabated in 2014. This is despite repeated announcements from Chinese authorities that constructive measures have been undertaken to conform to accepted ethical standards. While there is unanimous agreement on the unethical nature of using organs from executed prisoners, due to its limitations on voluntary and informed consent, there is insufficient coverage of forced organ procurement from prisoners of conscience without consent. Strategies to influence positive change in China over the last few decades have failed to bring this practice to an end. While organ donation and transplantation services in China have undergone considerable structural changes in the last few years, fundamental attempts to shift practice to ethically sourced organs have floundered. Although proton pump inhibitor treatment remains the first-line treatment for gastroesophageal re... more Although proton pump inhibitor treatment remains the first-line treatment for gastroesophageal reflux disease (GERD), the panorama of an indefinitely prolonged medical treatment faces patients and physicians with the need to look for an equivalent, more definitive therapy. Facing global results, after many series have demonstrated the safety of the laparoscopic antireflux surgery procedure in experienced hands, with a low incidence of intra- and postoperative morbidity and a short postoperative hospital stay (averaging 2 days), the immediate benefit as well as the long-term alleviation of symptoms (very good: 82%, good: 92%) seem to justify the place of surgical fundoplication in the therapeutic armamentarium for GERD as a valid alternative to medical treatment. Careful patient selection for surgery, a complete and thorough preoperative workup, an intervention in experienced hands, and a patient well informed about the pros and cons of both medical and surgical therapy are mandatory.
