Background In the wake of the recent Roe vs. Wade judgment, we performed a Google Trends analysis... more Background In the wake of the recent Roe vs. Wade judgment, we performed a Google Trends analysis to identify the impact of this decision on the interests regarding contraceptive choices in the United States. Methods A Google Trends search between April 6 and July 5, 2022, with the United States as the area of interest, was performed using the five most popular contraception choices. In addition, a second trend search was performed using oral and injectable hormonal birth control measures. Results Trends showed a spike in interest regarding various contraceptive methods immediately following the verdict. The highest increase in interest was noted for "vasectomy," followed by "tubal ligation." With respect to oral and injectable birth control measures, "morning after pill" showed a marked spike in interest. Conclusion This verdict has triggered increased interest in contraceptive practices, which can be translated into better reproductive health with proper public health initiatives.

Background and Aims: Bupivacaine has been the most frequently used local anaesthetic in brachial ... more Background and Aims: Bupivacaine has been the most frequently used local anaesthetic in brachial plexus block, but ropivacaine has also been successfully tried in the recent past. It is less cardiotoxic, less arrhythmogenic, less toxic to the central nervous system than bupivacaine, and it has intrinsic vasoconstrictor property. The effects of clonidine have been studied in peripheral nerve blockade. The purpose of this study was to evaluate the effects of clonidine on nerve blockade during brachial plexus block with ropivacaine using peripheral nerve stimulator. Methods: Sixty patients were randomly divided into two groups, Group A and B. Group A received 30 ml of 0.5% of ropivacaine with 0.5 ml normal saline while Group B received same amount of ropivacaine with 0.5 ml (equivalent to 75 µg) of clonidine for supraclavicular brachial plexus block. The groups were compared regarding quality of sensory and motor blockade, duration of post-operative analgesia and intra and post-operative complications. Results: There was a significant increase in duration of motor and sensory block and analgesia in Group B as compared to Group A patients (P < 0.0001). There was no significant difference in onset time in either group (P = 0.304). No significant side effects were noted. Conclusion: The addition of 75 µg of clonidine to ropivacaine for brachial plexus block prolongs motor and sensory block and analgesia without significant side effects.

Global healthcare system was caught off guard by a huge crisis in the beginning of 2020 when a pn... more Global healthcare system was caught off guard by a huge crisis in the beginning of 2020 when a pneumonia outbreak which originated from the city of Wuhan in Hubei province of China quickly spread across borders. The causative agent was identified as a novel coronavirus (SARS‐COV‐2) by the World Health Organization (WHO) and a pandemic was declared on March 11, 2020 affecting over 100 countries. By 4th May 2020, more than 34 million people were infected globally and about 2.39 million had died making it one of the worst global disease outbreaks in a century. The current pandemic situation can potentially overwhelm the health care resources in the coming times and may impair the delivery of optimal care to patients requiring surgical interventions. Therefore, planned and judicious utilization of manpower and resources is of prime importance.

This study was undertaken to compare the analgesic efficacy of ultrasound-guided single-shot caud... more This study was undertaken to compare the analgesic efficacy of ultrasound-guided single-shot caudal block with ultrasound-guided single-shot paravertebral block in children undergoing renal surgeries. Randomised, interventional, blinded clinical trial. Operating rooms of All India Institute of Medical Sciences, New Delhi, India. 50 children aged 2-10 years, of ASA status I/II, posted for elective renal surgeries. The children were randomised into two groups (Group C-caudal block, Group P-paravertebral block). After induction of general anesthesia, single-shot caudal or paravertebral block was performed under ultrasound guidance, with 0.2% ropivacaine with 1:200000 adrenaline. Time to first rescue analgesia, time to perform blocks, intraoperative and post-operative hemodynamics, post-operative FLACC scores, incidence of complications, parental satisfaction scores were recorded. Children in Group P had significantly longer duration of analgesia (p &lt; 0.0004) than Group C. Post-operative FLACC scores (p &lt; 0.005) and analgesic requirements (p &lt; 0.0004) were lower in Group P. The mean fentanyl requirement over 24 h in group P was 0.56 ± 0.82 μg/kg, compared to 1.8 ± 1.2 μg/kg in group C. Parents in Group P reported greater satisfaction (p &lt; 0.02). No complications were seen in either of the groups. This study showed superior analgesia and parental satisfaction with single-shot paravertebral block in comparison to single-shot caudal block for renal surgeries in children. However, the block performance in children requires adequate expertise and practice.

The postoperative pain management in open thoracotomy is very crucial as the effective analgesia ... more The postoperative pain management in open thoracotomy is very crucial as the effective analgesia can prevent respiratory and thrombotic complications and lead to early mobilization and discharge. The thoracic epidural analgesia is the gold standard in such surgeries; however, there are few adverse effects such as hypotension, dural puncture, and contralateral block that always warrants safer alternative. Recently, with the advent of ultrasound, the regional anesthetic techniques are getting more popular to avoid such complications. Erector spinae plane (ESP) block is one of the novel techniques that has been described as a safe thoracic paravertebral block. We are reporting here the continuous ESP block as a primary mode of postoperative analgesia which was continued for 48 h. The intraoperative opioid requirement was very less, and the maximum NRS score in postoperative period was 4 at 12 h, which was well managed with multimodal analgesic regimen along with rescue doses of opioid.
