Among the actions of early diagnosis of uterine cervix cancers, the sequence cytologic smear, col... more Among the actions of early diagnosis of uterine cervix cancers, the sequence cytologic smear, colposcopy, cervix biopsy is well-known. The authors draw the attention that in some situations the cytologic smear and colposcopy are not sufficient and it is only biopsy that defines the nature of the lesion and may give therapeutic indications. Out of a series of 55 women to whom a cervix biopsy was performed, there were 27 cases in which biopsy was justified, showing either a severe lesion or the presence of koilocytes. The authors plead for a more frequent use of cervix biopsy as a means of early diagnosis of uterine cervix cancer or of its dysplasias.

Based on the Doppler phenomenon, several tools were developed that are able to measure the blood ... more Based on the Doppler phenomenon, several tools were developed that are able to measure the blood flows in various vessels. The authors aimed at d etermining whether in some cases such an examination is sufficient to decide if a cesarean section is indicated. According to the literature and authors' own professional experience, the answer to this question is yes, as the most advanced method measures diastolic flows in the umbilical artery. Fetuses with intrauterine growth restriction are the most suited for this assessment. In less clear-cut cases, doctors may resort to middle cerebral artery, ductus venosus, uterine artery and umbilical vein assessments, or to biophysical score calculations. Any obstetrical approach should consider the fetal gestational age, condition and weight, and the corroborated results of the various flow and velocimetry measurements. The authors suggest a continuation of this research, the ultimate goal being the development of an overall score including several types of Doppler examinations, which would improve asse ssment accuracy.

Methods: The cases included represented patients of more than 45 years, with persistent bleeding, from 2 gynecological hospitals (one public and one private), during one year, that were submitted to hysteroscopy for clarifying the diagnostic. The hysteroscopic endometrium descriptions were categorized as: atrophy, normal, simple or suspect hypertrophy, polyp, and adenocarcinoma, and they were compared to the pathological exam results. Results: The descriptions were distributed among the categories: atrophy (8 cases), simple hypertrophy (17), suspect hypertrophy (3), polypoid (5), endometrial cancer (2) or normal (4). The correlation to pathological results was good, with only 14% of hysteroscopic underestimation of lesions. The sensitivity according to type of pathology, was 50 to 100%, specificity between 84% - 100%. Conclusion: Hysteroscopy is better than "blind" curettage in clarifying perimenopausal hemorrhagic endometrial pathology.

We analyzed selected well-known and less well-known serum markers that have been proposed for dia... more We analyzed selected well-known and less well-known serum markers that have been proposed for diagnosis and severity assessment of endometriosis, in a case-control study. Study design: This prospective study was carried out in a Clinical Department of Gynecology in Iasi, Romania. Study participants included endometriosis patients, and controls in whom laparoscopy had excluded endometriosis. Each case and control was investigated for serum levels of CA125, TNF, IL-1, IL-6 and IL-8. The data were correlated with clinical symptoms and revised American Fertility Society (rAFS) score and stage, and interpreted by Mann-Whitney U-test and ANOVA regression analysis. Results: Over the course of 1 year, 24 cases of endometriosis and 24 controls of matched age were selected. The rAFS stages were: stage I, 12.5%; stage II, 16.7%; stage III, 58.3%; and stage IV, 12.5%. CA125 levels were over the cutoff of 35 IU/l in 54% of patients (versus 8% of controls), averaging 67.5 (CI95: AE17.5). The sensitivity and specificity were 54% and 91%, respectively, with a p value of <0.001 (statistically significant). For IL-6, 71% of cases and 87% of controls were above the cutoff of 2 pg/ml, with an average of 11.83 AE 7. The sensitivity and specificity were 71% and 12%, respectively, but the difference was not statistically significant, p = 0.071. Other tested serum markers had no discrimination value. A correlation with severity of endometriosis was seen for CA125 (p = 0.03) but not for IL-6, by ANOVA. Conclusion: CA125 correlated with endometriosis screening and severity, indicating its superiority as a marker for further, larger studies.

Abordul endoscopic al sinechiilor uterine. Studiu clinic pe 78 cazuri Sinechiile uterine sunt o p... more Abordul endoscopic al sinechiilor uterine. Studiu clinic pe 78 cazuri Sinechiile uterine sunt o patologie usor de diagnosticat prin histeroscopie, care ofera si metoda cea mai buna de tratament. Acest studiu analizeaza valoarea metodei. Material si metode: Am selectat 78 cazuri diagnosticate cu sinechii intr-o perioada de 3 ani in serviciul nostru. In cazurile in care un gest operator a fost necesar, el a constat din metode mecanice (impingere, foarfece, pensa) sau rezectie electrica, pentru sinechii si patologii asociate (fibrom, polip). Urmarirea a fost de 3-6 luni pentru paciente cu amenoree, si de 6-12 luni pentru cele cu infertilitate. Rezultate: In cele 78 cazuri, diagnosticul de internare a fost de sinechie (44%), cicluri neregulate (19%), amenoree (16%), infertilitate (15%). Histeroscopia a apreciat extensia care a fost in majoritatea cazurilor in stadiu moderat. Din paciente, 60% au necesitat gesturi terapeutice, iar crteriile de succes au fost: vizualizarea ambelor orificii tubare (70%), cicluri normale clinic (91%), si sarcini (41% din pacientele internate pentru infertilitate). A fost o complicatie prin perforatie uterina, intr-un sindrom Ashermann, fara a necesita alte gesturi ulterioare. Principalul tratament post-histeroscopie a fost cel hormonal, in toate cazurile. In concluzie, desi histeroscopia permite diagnosticul precis al sinechiilor, tratamentul trebuie efectuat doar de specialisti cu experienta.

642 cases of mental disorder occurring or aggravating during the evolution of a pregnancy or post... more 642 cases of mental disorder occurring or aggravating during the evolution of a pregnancy or postpartum assisted at the "Socola" University Hospital of Psychiatry in the interval 1988-1993 were analyzed. In 91 (38.39%) of these cases the postpartum mental disorders occurred in the absence of a psychotic psychiatric history. Based on a computerized record, the psycho-social factors with a potential pathologic risk were analyzed dynamically. The influence of social dynamics on the incidence and psychopathologic profile of mental disturbances with onset in the postpartum was evidenced, a somewhat paradoxical correlation of the frequency of these disorders with the dynamics of abortions and not with that of deliveries being underlined. Objectives: To assess the reproducibility of hysteroscopic diagnosis in the various types of endo... more Objectives: To assess the reproducibility of hysteroscopic diagnosis in the various types of endometrial hyperplasias and to compare different classifications of endometrial hyperplasia from this point of view. Study Design: 109 endometrial hyperplasias, diagnosed in the last 5 years with hysteroscopy, were all biopsied and had a pathological examination. We tried to estimate the accuracy of hysteroscopy comparing the various classifications of endometrial hyperplasia to the final pathologic diagnosis. Results: Using older endometrial hyperplasia classification, the overall diagnostic accuracy of hysteroscopy was 76% as for the correct type of hyperplasia. Classifying them at hysteroscopy into two categories only, i.e. hyperplasias and neoplasias, gave a more accurate result (90.26%). Conclusions: The newer classification is better from the hysteroscopist's point of view.

Objectives: To assess the basic data of the patients who requested sterilisation reversal, togeth... more Objectives: To assess the basic data of the patients who requested sterilisation reversal, together with the success rate and the factors influencing the success rate of the operation. Study Methods: A total of 206 patients who underwent microsurgical sterilisation reversal in Srinagarind hospital Between January 1, 1984 to June 30, 1993 were recruited. The medical records of these patients were reviewed. The letters were sent to these patients to obtain the results of the procedure. Results: Mean age of the patients was 32.0 years, most of them were government officers and farmers. The main reason for reversal was remarriage (68.0 percent). Ampulla to ampulla was the most common type of reanastomosis. The mean duration of the operation was 135.8 minutes. Success rate of the operation as determined by delivery of the liveborn was 58.7 percent while abortion rate and ectopic pregnancy rate were 10.7 and 8.2 percent, respectively. This study revealed that the patients with age under 35 years, operation performed within 5 years after sterilisation, interval type of sterilisation, and isthmic-isthmic reanastomosis were the factors that resulted in the better success rate. Conclusions: Sterilization reversal in Srinagarind hospital had satisfactory success rate and factors that influenced the success were age of the patients, time interval after sterilization, type of sterilization and part of the uterine tubes being reanastomosed.

We compared the obstetric results for two groups of births: 344 vaginal deliveries in which nuchal cord was diagnosed during the second stage of labour, and 48 cases in which the diagnosis was made with an ultrasound scan ante partum and delivery was by caesarean section (CS). All were singleton term pregnancies with a cephalic presentation, and with no other significant associated pathology. Results. There were no significant obstetric differences between the group with a prenatal diagnosis of umbilical cord loop who delivered by CS and the group with a post partum diagnosis of umbilical cord loop who delivered vaginally. Conclusion. We concluded that ante partum ultrasound diagnosis of nuchal cord is not in itself an indication for CS delivery.

The endoscopic surgery, despite its very sophisticated aspect and appearance of advanced technolo... more The endoscopic surgery, despite its very sophisticated aspect and appearance of advanced technology, is still another way of doing surgery. The patients and sometimes even specialists are tempted to promote the technique as “safe” and “almost non-invasive”, disregarding the minor pitfalls and complications related to it. In gynaecology, this aspect is even more frequent as the pre-concept of “minor surgery” imposed to some interventions from this specialty find a favourable psychology in some women. Training and certification on gynaecological endoscopic surgery will play a pivotal role in the near future of medico-legal problems. Surgeons who underwent a recognizable education and testing, defending their position for a mistake have a strong argumentation that were eligible and able to perform a complicated operation. Whether Gynaecological Societies like the European Society for gynaecologic Endoscopy (ESGE) and the European Society for Human Reproduction and Embriology (ESHRE) will be called to the court to defend and justify their certified surgeon members’ abilities and performance is debatable. We analyze data of the literature, and make some considerations which unveil the miscommunication that should be focused by the gynaecologist in approaching this domain.

Research on tumor infiltrating lymphocytes at the primary tumor site could give us important information on how the immune cells are fighting against cancer. The aims of our study were to assess HPV status and to evaluate the significance of in situ cellular IR in CC. We performed a two-step retrospective analysis of IR in 18 CC: evaluation of HPV 16 and 18 infections by in situ hybridization and immune biomarkers (CD20, CD3, CD45) by immunohistochemistry. Intrauterine adhesions (1UA) are easily diagnosed by hysteroscopy, which also offers the main met... more Intrauterine adhesions (1UA) are easily diagnosed by hysteroscopy, which also offers the main method for treatment. In this study, we analyze the value of the method. We have selected 78 cases of IUA diagnosed during a 3 year period. In cases when interventions were needed, we used mechanical techniques (pushing, scissors, forceps) or electrical resection, addressed to synechiae and other concomitant pathologies (fibroids, polyps). The follow-up ranged between 3-6 months (for amenorrheic patients) and 6-12 months for infertility patients. In our 78 cases the presenting diagnostic was synechiae (44%), irregular cycles (19%), amenorthea (16%), infertility (15%). Hysteroscopy allowed the IUA extension evaluation, and the majority of cases had early stage disease. From them 60% had therapeutic gestures, with success criteria being: visualization of both tubal ostium- in 70%, clinical normal cycles (91%) and pregnancy in 5 cases (41% from patients addressed mainly for infertility). CA125 levels were over the cutoff of 35 IU/l in 54% of patients (versus 8% of controls), averaging 67.5 (CI95: AE17.5). The sensitivity and specificity were 54% and 91%, respectively, with a p value of <0.001 (statistically significant). For IL-6, 71% of cases and 87% of controls were above the cutoff of 2 pg/ml, with an average of 11.83 AE 7. The sensitivity and specificity were 71% and 12%, respectively, but the difference was not statistically significant, p = 0.071. Other tested serum markers had no discrimination value. A correlation with severity of endometriosis was seen for CA125 (p = 0.03) but not for IL-6, by ANOVA. Conclusion: CA125 correlated with endometriosis screening and severity, indicating its superiority as a marker for further, larger studies.

Studiu clinic pe 78 cazuri Sinechiile uterine sunt o patologie usor de diagnosticat prin histeroscopie, care ofera si metoda cea mai buna de tratament. Acest studiu analizeaza valoarea metodei. Material si metode: Am selectat 78 cazuri diagnosticate cu sinechii intr-o perioada de 3 ani in serviciul nostru. In cazurile in care un gest operator a fost necesar, el a constat din metode mecanice (impingere, foarfece, pensa) sau rezectie electrica, pentru sinechii si patologii asociate (fibrom, polip). Urmarirea a fost de 3-6 luni pentru paciente cu amenoree, si de 6-12 luni pentru cele cu infertilitate. Rezultate: In cele 78 cazuri, diagnosticul de internare a fost de sinechie (44%), cicluri neregulate (19%), amenoree (16%), infertilitate (15%). Histeroscopia a apreciat extensia care a fost in majoritatea cazurilor in stadiu moderat. Din paciente, 60% au necesitat gesturi terapeutice, iar crteriile de succes au fost: vizualizarea ambelor orificii tubare (70%), cicluri normale clinic (91%), si sarcini (41% din pacientele internate pentru infertilitate). A fost o complicatie prin perforatie uterina, intr-un sindrom Ashermann, fara a necesita alte gesturi ulterioare. Principalul tratament post-histeroscopie a fost cel hormonal, in toate cazurile. In concluzie, desi histeroscopia permite diagnosticul precis al sinechiilor, tratamentul trebuie efectuat doar de specialisti cu experienta.

642 cases of mental disorder occurring or aggravating during the evolution of a pregnancy or post... more 642 cases of mental disorder occurring or aggravating during the evolution of a pregnancy or postpartum assisted at the "Socola" University Hospital of Psychiatry in the interval 1988-1993 were analyzed. In 91 (38.39%) of these cases the postpartum mental disorders occurred in the absence of a psychotic psychiatric history. Based on a computerized record, the psycho-social factors with a potential pathologic risk were analyzed dynamically. The influence of social dynamics on the incidence and psychopathologic profile of mental disturbances with onset in the postpartum was evidenced, a somewhat paradoxical correlation of the frequency of these disorders with the dynamics of abortions and not with that of deliveries being underlined. Study Design: 109 endometrial hyperplasias, diagnosed in the last 5 years with hysteroscopy, were all biopsied and had a pathological examination. We tried to estimate the accuracy of hysteroscopy comparing the various classifications of endometrial hyperplasia to the final pathologic diagnosis. Results: Using older endometrial hyperplasia classification, the overall diagnostic accuracy of hysteroscopy was 76% as for the correct type of hyperplasia. Classifying them at hysteroscopy into two categories only, i.e. hyperplasias and neoplasias, gave a more accurate result (90.26%). Conclusions: The newer classification is better from the hysteroscopist's point of view.

Objectives: To assess the basic data of the patients who requested sterilisation reversal, togeth... more Objectives: To assess the basic data of the patients who requested sterilisation reversal, together with the success rate and the factors influencing the success rate of the operation. Study Methods: A total of 206 patients who underwent microsurgical sterilisation reversal in Srinagarind hospital Between January 1, 1984 to June 30, 1993 were recruited. The medical records of these patients were reviewed. The letters were sent to these patients to obtain the results of the procedure. Results: Mean age of the patients was 32.0 years, most of them were government officers and farmers. The main reason for reversal was remarriage (68.0 percent). Ampulla to ampulla was the most common type of reanastomosis. The mean duration of the operation was 135.8 minutes. Success rate of the operation as determined by delivery of the liveborn was 58.7 percent while abortion rate and ectopic pregnancy rate were 10.7 and 8.2 percent, respectively. This study revealed that the patients with age under 35 years, operation performed within 5 years after sterilisation, interval type of sterilisation, and isthmic-isthmic reanastomosis were the factors that resulted in the better success rate. Conclusions: Sterilization reversal in Srinagarind hospital had satisfactory success rate and factors that influenced the success were age of the patients, time interval after sterilization, type of sterilization and part of the uterine tubes being reanastomosed.

We compared the obstetric results for two groups of births: 344 vaginal deliveries in which nuchal cord was diagnosed during the second stage of labour, and 48 cases in which the diagnosis was made with an ultrasound scan ante partum and delivery was by caesarean section (CS). All were singleton term pregnancies with a cephalic presentation, and with no other significant associated pathology. Results. There were no significant obstetric differences between the group with a prenatal diagnosis of umbilical cord loop who delivered by CS and the group with a post partum diagnosis of umbilical cord loop who delivered vaginally. Conclusion. We concluded that ante partum ultrasound diagnosis of nuchal cord is not in itself an indication for CS delivery.

The endoscopic surgery, despite its very sophisticated aspect and appearance of advanced technolo... more The endoscopic surgery, despite its very sophisticated aspect and appearance of advanced technology, is still another way of doing surgery. The patients and sometimes even specialists are tempted to promote the technique as “safe” and “almost non-invasive”, disregarding the minor pitfalls and complications related to it. In gynaecology, this aspect is even more frequent as the pre-concept of “minor surgery” imposed to some interventions from this specialty find a favourable psychology in some women. Training and certification on gynaecological endoscopic surgery will play a pivotal role in the near future of medico-legal problems. Surgeons who underwent a recognizable education and testing, defending their position for a mistake have a strong argumentation that were eligible and able to perform a complicated operation. Whether Gynaecological Societies like the European Society for gynaecologic Endoscopy (ESGE) and the European Society for Human Reproduction and Embriology (ESHRE) will be called to the court to defend and justify their certified surgeon members’ abilities and performance is debatable. We analyze data of the literature, and make some considerations which unveil the miscommunication that should be focused by the gynaecologist in approaching this domain.

In this study, we analyze the value of the method. We have selected 78 cases of IUA diagnosed during a 3 year period. In cases when interventions were needed, we used mechanical techniques (pushing, scissors, forceps) or electrical resection, addressed to synechiae and other concomitant pathologies (fibroids, polyps). The follow-up ranged between 3-6 months (for amenorrheic patients) and 6-12 months for infertility patients. In our 78 cases the presenting diagnostic was synechiae (44%), irregular cycles (19%), amenorthea (16%), infertility (15%). Hysteroscopy allowed the IUA extension evaluation, and the majority of cases had early stage disease. Study design: This prospective study was carried out in a Clinical Department of Gynecology in Iasi, Romania. Study participants included endometriosis patients, and controls in whom laparoscopy had excluded endometriosis. Each case and control was investigated for serum levels of CA125, TNF, IL-1, IL-6 and IL-8. The data were correlated with clinical symptoms and revised American Fertility Society (rAFS) score and stage, and interpreted by Mann-Whitney U-test and ANOVA regression analysis. Results: Over the course of 1 year, 24 cases of endometriosis and 24 controls of matched age were selected. The rAFS stages were: stage I, 12.5%; stage II, 16.7%; stage III, 58.3%; and stage IV, 12.5%.
