The goal of this paper is to present a critical review of the endoscopic procedures currently in use for the treatment of carpal tunnel syndrome. Endoscopic techniques and outcomes are discussed. Methods. An extensive review of published articles on the subject of endoscopic carpal tunnel release surgery is presented, encompassing six endoscopic techniques used to treat carpal tunnel syndrome. Since the first report in 1987, 7091 patients have undergone 8068 operations. The overall success rate has been 96.52%, with a complication rate of 2.67% and a failure rate of 2.61%. The mean time to return to work in patients not receiving Workers' Compensation was 17.8 days, ranging between 10.8 and 22.3 days. The most common complications were transient paresthesias of the ulnar and median nerves. Other complications included superficial palmar arch injuries, reflex sympathetic dystrophy, flexor tendon lacerations, and incomplete transverse carpal ligament division. In many stud...

During the procedure, engineered calcium phosphate compound (CPC) is injected. It is claimed by the manufacturer that during the healing process, the CPC is replaced with new bone. The purpose of this study was to verify the replacement of CPC with new bone after subchondroplasty for the first time in humans. A 76-year old woman was referred for resistant medial knee pain. Standing radiographs showed varus knee OA and magnetic resonance imaging (MRI) revealed BML. She was treated with subchondroplasty of medial femoral condyle. Excellent relief of pain was achieved after procedure. Afterwards, the pain worsened, the radiographs confirmed the OA progression and the patient was treated with a total knee arthroplasty (TKA) 4 years after primary procedure. The resected bone was examined histologically and with micro-computed tomography (CT). Histolo...

Depending on the severity, the bone loss may threaten the prosthesis survival. The current study investigated the effect of cyclic etidronate therapy on periprosthetic and contralateral bone mineral density (BMD) in an one-year, prospective, randomized, double-blind study on 46 patients after cemented hip arthroplasty. Etidronate was administered orally in a regimen repeated every 14 weeks and periprosthetic BMD was measured with dual energy X-ray absorptiometry (DXA) in the total periprosthetic area and in the seven Gruen Zones at 1 week (baseline), 6 weeks, 3 months, 6 months, and 12 months postoperatively. In the etidronate group there were significant temporal BMD decreases measured in Gruen Zones 2, 3, 6, and 7 as well as in the entire proximal femur; the greatest decrease was 11.1% and was measured in Zone 2 at 12 mont...

However, these events are highly preventable. For thromboprophylaxis we recommend combined mechanical (compression stockings, intermittent pneumatic compression or venous foot compression) and pharmacological (low molecular weight heparin, dabigatran etexilate or rivaroxaban) methods. In this article current guidelines for eff ective and safe thromboprophylaxis for hip and knee arthroplasty are presented.

Dear Colleagues, It is a great privilege for me to cordially invite you to always charming Paris ... more Dear Colleagues, It is a great privilege for me to cordially invite you to always charming Paris and the 17th Symposium of the European Association of Research Groups for Spinal Osteosynthesis (ArgoSpine), from January 31st to February 1st, 2013 at the elegant Maison des Arts & Métiers. We have faced tremendous progress in spine surgery during the last decades, which we, the spine specialists, have to follow very closely for the benefit of our patients. With this scope in mind, the Organizing Committee has developed an interesting programme to keep you updated with the newest knowledge about "Biomaterials, Hardware, Investigational Methods, Additional Key-points for Success", including infection and hardware, bone graft substitutes, stem cells, 3D imaging and navigation, intraoperative monitoring and neuromonitoring. Indeed, the latest in spinal research and technology will be presented, along with the newest concepts and breakthroughs in the field of spine surgery related basic knowledge. The highly interactive programme will include ArgoSpine's traditional controversial round tables, lectures, and symposia presented and moderated by well-known international experts, popular case and oral presentations, as well as posters and industry-sponsored workshops. However, only the most current and best practice, meeting the highest standards of our speciality, will be discussed. We look forward to warmly welcoming you to Paris for the 17th ArgoSpine Symposium. ¼

Material and Methods This study prospectively analyzed the success rates of 18 DLSS patients treated with decompressive surgery and implantation of soft IPD (DIAM) at one or two levels. We compared the results with a historic group of 20 patients which were treated with traditional surgical decompression with or without PL fusion at one or two levels. In the DIAM group, outcomes were measured with the Zurich Claudication Questionnaire (ZCQ) and the short form health survey 36 (SF-36), completed preoperatively and at 6, 12, 24, and 48 months postoperatively. In the historic group, outcomes were measured with the ZCQ preoperatively and at 2...

A percutaneous L5-S1 discectomy, interbody distraction, and fixation could be achieved while preserving the integrity of the muscles, ligaments, and disc anulus. Retroperitoneal viscera and dorsal neural elements are avoided via the presacral safe zone. Additional pedicle or facet screw fixation is recommended to provide stabilization and promote fusion. The shoulder function was assessed according to the Constant classification, each patient did strength measurements, and had electromyographic evaluation of the supraspinatus and infraspinatus muscles. The mean value of the Constant score for the surgically treated shoulder was 2.7 points. Forty-five patients (88.2%) had satisfactory scores and six patients (11.8%) had unsatisfactory scores. The result of treatment was significantly correlated to the size of the tear and the time elapsed from injury to surgery. However, no correlation was found between the result of the treatment and the type of surgery, mode of postoperative rehabilitation, and age. The strength measurement of abduction and of external rotation indicated significantly reduced power in surgically treated shoulders compared with contralateral shoulders. There was a significant difference of the infraspinatus motor unit potential interference pattern reduction between surgically treated and the contralateral shoulder. There was a positive correlation between electromyographic findings and the results of rotator cuff reconstruction.

Adolescence is a critical time for bone mineral acquisition and is as such the crucial period for the prevention of osteoporosis. The purpose of the study was to find whether adolescents’ calcium intake in Celje region is high enough to attain peak bone mass. Methods. 217 pupils attending primary school and 434 students attending secondary school participated in an anonymous survey regarding their calcium intake. The acquired data were statistically analysed. Results. Only 77 (11.8%) among 651 teenagers had an optimal daily intake of calcium (≥ 1300 mg). The average calcium intake was 828.9 mg per day. The average daily calcium intake for girls was 777.6 mg and for boys 910.0 mg. Calcium intake decreased with the age of the adolescents. The lowest calcium intake was found in those adolescents who were concerned that drinking milk would make them fat. Conclusions. Because of adolescents’ sub-optimal intake of calcium there is a probability that osteoporosis will break out...
