Alveolar bone deficiency is a major postoperative complication in the treatment of traumatic inju... more Alveolar bone deficiency is a major postoperative complication in the treatment of traumatic injuries, periodontal diseases and likewise. Hence, alveolar bone repair remains a major hurdle in tissue engineering. Autogenous bone can be wellthought-of as benchmark for bone grafting sans its limitations and complications. In order to overcome these limitations, there is an increased demand of bone graft materials that led to numerous studies on different techniques and materials for bone regeneration over the years. Dentin and bone having same biochemical similarities led to the idea of using it as a bone regenerative material. Demineralized dentin matrix (DDM), an organic material obtained from dentin has been shown to possess osteogenic capacity. Demineralized dentin matrix may prosper in future endodontic world as an apexification material and as a permanent root canal filling material as well. Quick in bone forming as compared to conventional bone graft, this material is a boon to the dental world in this era. This manuscript reviews various studies on different types of DDM as a bone grafting material, and also summarizes the suggested pathway of bone regeneration.

Objective: To evaluate the feasibility of posterior maxillary teeth extraction buccal infiltratio... more Objective: To evaluate the feasibility of posterior maxillary teeth extraction buccal infiltration with or without the use of palatal injection. Material and Methods: A total of 70 patients underwent extraction of bilateral maxillary posterior teeth under 2% lignocaine hydrochloride with 1:2,00000 adrenaline infiltration in this single-centric split-mouth randomized trial. The test side was administered with a buccal infiltration of 2 mL of anesthetic alone. An extended waiting period of 10 minutes was given before the commencement of the procedure. A standard protocol was followed for the control side. A single operator performed all extractions. Results: A total of 140 posterior maxillary teeth were extracted. Patients marked pain perception on a visual analogue scale in three different instances. During the administration of injections for the test side, the pain score was less than that of the control side and was statistically significant. The overall pain during the extraction procedure was comparable and statistically insignificant. The overall success of the method was 90%. Conclusion: Extraction of posterior maxillary teeth was feasible with a single buccal infiltration without palatal injection in most cases using an extended waiting period. Dentists can attempt extraction without palatal injections with optimal success. However, the alternate technique could be used when there is a necessity for rescue palatal anesthesia.

Objective: To assess the diagnostic accuracy of Ultrasonography (USG) in diagnosing superficial f... more Objective: To assess the diagnostic accuracy of Ultrasonography (USG) in diagnosing superficial facial fractures. Material and Methods: Patients visiting our facility with facial trauma and suspected fracture of the facial skeleton, those who had undergone CT scans, and conventional radiographic examinations and those who were conscious and cooperative were included in the study. All conventional radiographs, CT scans and ultrasound examinations were done during 0-20 days after trauma in all the patients. Results: A total of 20 patients participated in our study, out of which 18 were male (90%) with a mean age of 34.4 years (range of 19-75 years). Eleven sites of the face were examined bilaterally in each patient, i.e., a total of 440 sites. Of these, 84 sites were found to be fractured according to the CT scan examination whereas conventional radiographs detected 59 and ultrasonography detected 74 fractures (of which 70 were true fractures, while 4 were false-positive results). The sensitivity and specificity of USG in all fracture sites were 83.33% and 98.88% respectively. The positive and negative predictive values were 94.59% and 96.17% respectively. Conclusion: Ultrasound examination had a better sensitivity when compared to conventional radiography in detecting superficial facial fractures.

A 75-year-old female with ischemic heart disease was heparinized for percutaneous transluminal coronary angioplasty of the left anterior descending artery leading to sublingual hematoma with difficulty in swallowing and breathing. This paper presents a case report and management of sublingual hematoma following heparin therapy.

Background: Palatal rugae are asymmetric and irregular ridges on the anterior palatal region, whi... more Background: Palatal rugae are asymmetric and irregular ridges on the anterior palatal region, which have proven to be useful in the field of forensic dentistry. This study aims to use morphological analysis of palatal rugae patterns to assess sex in a South Indian adult population. Materials and Methods: This cross-sectional study was undertaken to evaluate a total of 300 maxillary dental casts (150 males and 150 females). The rugae patterns were marked by one observer and classified as per Thomas and Kotze criteria by another observer. The rugae patterns were classified based on the total number of rugae, shape, predominant direction of rugae, and unification of rugae. Results: The palatal rugae in female participants showed a higher incidence of circular rugae and backward directed rugae, while male participants showed more forward-directed rugae. The logistic regression analysis showed a significant association of with circular (OR=1.48; 95% CI= 1.02-2.13), forward (OR=0.87; 95% CI= 0.78-0.97) and backward (OR=1.23; 95% CI= 1.08-1.41) palatal rugae with sex. Conclusion: Certain palatal rugae patterns can be used for the assessment of sex with limited accuracy.

The orocutaneous fistulous tract of odontogenic origin is often a diagnostic challenge, due to it... more The orocutaneous fistulous tract of odontogenic origin is often a diagnostic challenge, due to its rare manifestation and absence of dental signs and symptoms. The odontogenic cutaneous fistula is often misdiagnosed as a superficial skin lesion of non-odontogenic origin delaying the treatment. The diagnosis and treatment must be precise and swift to improve the clinical outcome and minimize the complications. This article presents a rare case of odontogenic keratocyst involving a linguoverted impacted third molar presenting as orocutaneous fistula. The patient was initially treated with empirical antibiotic therapy with no resolution of the cutaneous fistula and thickening of the skin around the sinus opening resulting in cosmetic deformity. Once the fistula was attributed to the underlying odontogenic cyst, treatment was done by cyst enucleation along with the extraction of tooth and fistula excision. The purpose of the paper is to emphasize the importance of early and accurate diagnosis and prompt management of the orocutaneous fistula due to the odontogenic origin.

The aim of this article was to review the efficacy of methylprednisolone versus dexamethasone in the management of postsurgical pain, swelling, and trismus after mandibular third molar surgery. Randomized, double-blinded studies from PubMed, CINAHL, Scopus, DOSS, Cochrane central, and Web of Science were identified by using a search strategy. Randomized controlled trials evaluating the efficacy of use of dexamethasone versus methylprednisolone for mandibular third molar extraction were only considered. The studies involving the use of any other corticosteroid agent were excluded. Outcomes assessed were postoperative pain, the number of rescue analgesics required, swelling, trismus, and adverse events. Orofacial granulomatosis (OFG) is an uncommon chronic inflammatory disorder with multifactorial e... more Orofacial granulomatosis (OFG) is an uncommon chronic inflammatory disorder with multifactorial etiology and pathogenesis affecting the orofacial region which appears usually as a persistent and/or recurrent swelling that may involve one or both the lips and/or recurrent ulcers along with other orofacial features. The diagnosis of OFG is challenging and it should be ruled out from other conditions characterized by granulomatous inflammation. This article describes a case of a 30-year-old female patient with upper lip swelling as the main manifestation without any systemic involvement and we have also laid down a proposal of a diagnostic algorithm for differential diagnoses and treatment modalities for OFG which could be helpful for the clinician and dentists alike to rule out other granulomatous disorders thus in effective management of OFG.

It is associated with significant co-morbidities affecting millions of people around the world. Many of these individuals remain undiagnosed while those who are diagnosed often exhibit poor compliance with the nightly use of Continuous Positive Air Pressure (CPAP), a very effective non-invasive modality. The growing failure and discomfort reported by the patients brought light into the possibility of other options such as oral appliance therapy and surgical therapy, with an absolute cure rate in moderate to severe OSAHS seen with Maxillo-Mandibular Advancement (MMA) surgery. The article reviews various Oral and Maxillofacial management options for the treatment of OSAHS with their success rates.

However, their inherent 2D limitations have ushered in an era of Cone Beam Computed Tomography (CBCT) for a volumetric holistic analysis. This case report depicts accidental displacement of implant into a mandibular medullary bone space with deficient bone density in an otherwise healthy individual and its subsequent surgical retrieval and prognosis. We wish to highlight the importance of CBCT as a mandatory pre-surgical tool for implant planning in all cases to prevent intraoperative adverse events, irrespective of their medical status.

AIM: To study the expression of embryonal markers by fetal cardiac mesenchymal stem cells (fC-MSC... more AIM: To study the expression of embryonal markers by fetal cardiac mesenchymal stem cells (fC-MSC) and their differentiation into cells of all the germ layers. METHODS: Ten independent cultures of rat fC-MSC were set up from cells derived from individual or pooled fetal hearts and studies given below were carried out at passages 3, 6, 15 and 21. The phenotypic markers CD29, CD31, CD34, CD45, CD73, CD90, CD105, CD166 and HLA-DR were analyzed by flow cytometry. The expression of embryonal markers Oct-4, Nanog, Sox-2, SSEA-1, SSEA-3, SSEA-4, TRA-1-60 and TRA 1-81 were studied by immunocytochemistry. The fC-MSC treated with specific induction medium were evaluated for their differentiation into (1) adipocytes and osteocytes (mesodermal cells) by Oil Red O and Alizarin Red staining, respectively, as well as by expression of lipoprotein lipase, PPARγ2 genes in adipocytes and osteopontin and RUNX2 genes in osteocytes by reverse-transcription polymerase chain reaction (RT-PCR); (2) neuronal (ectodermal) cells by expression of neuronal Filament-160 and Glial Fibrillar Acidic Protein by RT-PCR and immunocytochemistry; and (3) hepatocytic (endodermal) cells by expression of albumin by RT-PCR and immunocytochemistry, glycogen deposits by Periodic Acid Schiff staining and excretion of urea into the culture supernatant.

The fC-MSC treated with specific induction medium were evaluated for their differentiation into (1) adipocytes and osteocytes (mesodermal cells) by Oil Red O and Alizarin Red staining, respectively, as well as by expression of lipoprotein lipase, PPARγ2 genes in adipocytes and osteopontin and RUNX2 genes in osteocytes by reverse-transcription polymerase chain reaction (RT-PCR); (2) neuronal (ectodermal) cells by expression of neuronal Filament-160 and Glial Fibrillar Acidic Protein by RT-PCR and immunocytochemistry; and (3) hepatocytic (endodermal) cells by expression of albumin by RT-PCR and immunocytochemistry, glycogen deposits by Periodic Acid Schiff staining and excretion of urea into the culture supernatant.
